1.Medical management of cerebral vasospasm following aneurysmal subarachnoid hemorrhage

The occurrence of cerebral vasospasm (CVS) following aneurysmal subarachnoid hemorrhage is a multi-factor participatory process.Its pathogenic mechanism is complex.The clinical presentation and imaging findings are not consistent.Theoretically,by the measures of blocking calcium channels,inhibiting Rho kinase,removing oxygen free radicals and antagonizing endothelin receptors and other measures can prevent and treat CVS,but the actual clinical effects of various drugs are not satisfactory,especially in the aspects of improving clinical outcomes.The clinical application of the 3H therapy has been limited because of having many potential complications,and the intra-arterial interventional treatment is limited by the technology,equipment,indications,and other factors.Therefore,the prevention and treatment of CVS is still facing many challenges.
2.Homocysteine and ischemic stroke

Homocystine (Hcy) is an important intermediate product of amino acid metabolism.It is dual-regulated by both gene and environmental factors.The key enzyme of metabolic reaction is dependent on vitamin B class.The studies of in vitro experiment and animal models have demonstrated that Hcy may aggravate atherosclerosis by reducing the nitric oxide activity,enhancing intracellular oxidative stress,and promoting thrombosis,etc.Epidemiological studies have also demonstrated that hyperhomocystinemia (HHcy) is an independent risk factor for atherosclerosis.However,large randomized controlled trials have failed to confirm the close correlation between HHcy and ischemic stroke.By supplementing vitamin B class to decrease the Hcy level is still controversial in the primary and secondary prevention.The therapeutic dose of vitamin B class may be unable to obtain additional benefits in patients with ischemic stroke.
3.Value of Brain Natriurectic Peptide and Cardiac Troponin I in Diagnosis and Prognosis on Heart Failure in Children

Objective To explore diagnosis and prognosis of heart failure in children by measuring brain natriurettic peptide(BNP) and cardiac troponin I(CTnI).Methods Forty-one children subjects with heart failure were chosen,and then they were divided into three stages,early stage of heart failure,stage of heart failure and stage of recovery according to the ROSS cardiac function score method.In contrast with the former group,41 healthy children were chosen as control group.BNP and CTnI levels of blood plasma were analyzed by radioimmunoassay(RIA) and chemiluminescent immunoassay(CLIA) respectively,the relationship between the BNP level of blood plasma and prognosis of CHF group was analyzed.Results At early stage of heart failure,BNP and CTnI levels began to rise.They amount to highest peaks at stage of heart failure and went down at stage of recovery.But they were still higher than those of control group(P
4.Heat shock protein 27 and its role in retinal ganglion cell apoptosis in rat high intraocular pressure model

Background Glaucoma is common blinding eye diseases characterized by chronic loss of retinal ganglion cells(RGCs).Currently glaucoma pathogenesis is not completely understood,heat shock protein 27 ( HSP27 )may be associated with the pathogenesis of glaucomatous optic neuropathy. Objective Through the establishment of a rat model of high intraocular pressure,detection of the expression of HSP27 antibody in serum and RGCs apoptosis to investigate the role of HSP27 in RGCs apoptosis. Methods Fifty-one clean Wistar rats were divided into high intraocular pressure group (34 rats)and sham operation group( 17 rats)using a random number table.An animal model of high intraocular pressure was established in the right eye by the application of bipolar underwater electrocoagulation on vein of sclera surface in the experimental group,and rats with conjunctiva incision only without electric coagulation were served as sham operation (control).The intraocular pressure of rats of the both groups including experimental and control rats was measured 1,2,4,6,8 weeks after operation and then the rats were sacrificed.1 ml serum was collected from these rats to determine the concentration of HSP27 antibody.The retinas of the rats were isolated and homogenated for the extraction and analysis of the retinal protein by Western blot.Apoptosis of RGCs were assayed by TUNEL.The use of the experimental animals followed the Regulations for the Administration of Affair Concerning Experimental Animals by Kunming Medical Collegc. Results Intraocular pressure was elevated significantly after modeling and remained a high value during the expcrimental duration,showing a significant difference among the different groups ( F =318.502,P<0.01 ).However,no significant change in intraocular pressure before and after operation in the sham-operation group ( F =2.076,P > 0.05 ).The serum concentration of HSP27 antibody was increased significantly overtime from 1,2,4,6 to 8 weeks after the surgery applied,with a significant difference among various time points( F =154.221,P<0.01 ),but no obvious difference was seen in the shamoperation group( F =0.422,P>0.05 ).Importantly,Apoptosis rate of RGCs in the experimental eyes was significant higher as the time prolong after intraocular elevated(x2=856.12,P<0.05 ).The positive rate of RGCs apoptosis in the rats with high intraocular pressure group was elevated significantly compared to the rats in the sham operation group(P<0.05). Conclusions High intraocular pressure induces the expressions of HSP27 in retina and the accumulation of HSP27 antibody in rats serum;the elevated HSP27 is associated with the increased cell death in RGCs.
5.Medicinal vesiculation combined with quick cupping at Shenque (CV 8) for allergic rhinitis with syndrome of yang deficiency: a randomized controlled trial.

OBJECTIVETo compare the clinical efficacy differences between medicinal vesiculation combined with quick cupping at Shenque (CV 8) and regular medication for allergic rhinitis with syndrome of yang deficiency.
METHODSEighty-two cases were randomly divided into an observation group (42 cases) and a control group (40 cases). The observation group was treated with medicinal vesiculation combined with quick cupping at Shenque (CV 8). The medicinal vesiculation was applied at Feishu (BL 13), Gaohuang (BL 43), Fengmen (BL 12), Mingmen (GV 4) on the dog days in the summer, one treatment on the 1st dog-day, 2nd dog-day and last dog-day respectively with an interval of 10 days between two treatments. Three treatments were taken as one course, and totally one course was given. The quick cupping was applied at Shenque (CV 8), once a day, ten treatments were taken as one course, and totally three courses was given. The control group was treated with oral administration of loratadine and nasal spray of budesonide. The loratadine was given 10 mg per time, once a day for continuous 14 days; budesonide was given once a day, ten treatments were taken as one course, and totally three courses was given. The clinical efficacy in two groups after treatment was observed, and the contents of immune globulin E (IgE), interleukin-4 (IL-4), interleukin-5 (IL-5) and tumor necrosis factor-α (TNF-α) in peripheral serum were measured before and after the treatments.
RESULTSOf all the 82 patients, 79 cases completed the treatment, and 1 patient in the observation group and 2 patients in the control group dropped out. The effective rate was 87.8% (36/41) in the observation group, which was superior to 78.9% (30/38) in the control group (P<0.05). After treatment, both groups effectively reduced the contents of IgE, IL-4, IL-5 and TNF-α, and the observation group had superior effect on reducing IgE and IL-4 to the control group (P<0.05).
CONCLUSIONThe medicinal vesiculation combined with quick cupping at Shenque (CV 8) have better effect for allergic rhinitis with syndrome of yang deficiency than oral administration of loratadine and nasal spray of budesonide.

6.A new triterpene from Vernonia chunii

Objective To study the chemical constituents from Vernonia chunii of Hainan Province.Methods The constituents were isolated and purified by repeated column chromatography.Their structures were identified on the basis of physiochemical properties and spectral data.Results Four compounds were isolated and identified as:lupenyl propionate(Ⅰ),lupenyl(Ⅱ),oleanolic acid(Ⅲ),and daucosterol(Ⅳ).Conclusion Compound Ⅰ is a new compound,the others are isolated from this plant for the first time.
