3.Inhibitory effect of transgiutaminase 2 on apoptosis of osteosarcoma cell line MG-63 and its mechanism

Objective To investigate the anti-apoptosis effect of transgiutaminase 2 (TG2)in osteosarcoma cell line MG-63 and to explore the mechanism of inhibiting apoptosis of tumor cells.Methods The TG2-tgrgeted siRNA was designed,and the hypoxia culture model of MG-63 cells was established by a hypoxia incubator and the cells were divided into four groups:normal oxygen group,the cells were cultured under normal oxygen;hypoxia group, the cells were cultured in hypoxic incubators;control siRNA hypoxia group,the cells were cultured in hypoxic incubators after transfection of control siRNA;TG2 siRNA hypoxia group , the cells were cultured in hypoxic incubators after transfection of TG2 siRNA.The expressions of Bax and cytochrome C (Cyt C)and the apoptotic rates were observed at different time (6,12,24,48,and 72 h)after hypoxia culture.Microtiter plate assay was performed to monitor the intracellular TG2 activity.RT-PCR was used to detect the mRNA expressions of TG2 and Bax.The expressions levels of TG2,Bax and Cyt C were observed by immunohistochemical staining and Western blotting method.The apoptotic rates were analyzed using flow cytometry.Results Compared with normal oxygen group,the activity of TG2,the mRNA and protein expression levels of TG2 in hypoxia group and control siRNA hypoxia group were increased remarkably with the prolongation of the hypoxia time (P<0.01);the expression level of Bax protein was decreased significantly (P<0.01 ), but the expression level of Bax mRNA had no significant change (P>0.05);the expression levels of Cyt C protein in cytoplasm and mitochondria and the apoptotic rates had no markedly changes (P>0.05).Compared with hypoxia group and control siRNA hypoxia group,the expression levels of mRNA and protein of TG2 in TG2 siRNA hypoxia group were decreased significantly at different time points (P<0.01);the protein expression levels of Bax and Cyt C in cytoplasm and the apoptotic rates were increased markedly (P<0.01 );the expression level of Cyt C in mitochondria was decreased (P<0.01).Conclusion TG2 can inhibit the apoptosis of tumor cells through down-regulating the Bax expression and preventing Cyt C releasing into the cytoplasm.
4.Treatment of post operative complications of the procedure for prolapse and hemorrhoids

Objective To evaluate and analyze the postoperative complications induced from the procedure for prolapse and hemorrhoids(PPH).Methods The data of the third or fourth degree hemorrhoids operated by pro- cedure for prolapse and hemorrhoids were analyzed.Results Total 224 patients with hemorrhoids were operated on by PPH,and the mean operative time was 25 minutes and mean postoperative hospital stay were 3.5 days.Postoper- ative bleeding requiring haemostatic procedures occurred in two patients.Urine retention rate was 30.36%,there were 56 eases pain scores>5,there of re-prolapse,16 cases of residual skin tags.One case of mucocele.Conclusion The rule of PPH is individual management.
5.Surgical treatment of upper cervical vertebra disease from high anterior cervical retropharyngeal approach

Objective To study the clinical result of the surgical treatment of upper cervical vertebra disease from high anterior cervical retropharyngeal approach.Methods Among 14 patients,there were 7 C_(2.3)disc hernia- rions combined with C_(2.3)dislocations,3 Hangman fractures,2 odontoid process fractures and 2 C_3 tumors.All patients underwent the high anterior cervical retropharyngeal approach.10 cases were given discectomy of C_(2.3)or focus clear- ance of C_3,iliac bone autograft or titanium cage filled and internal fixation with titanium plate.2 cases of odontoid process fractures were given anterior screw fixation.Results 14 cases got successful exposure.The decompression, reduction,lesion clearance and fusion with internal fixation were done in all patients.There were no important vascu- lar or nerve injuries.The surgical treatment could improve the spinal cord nerve function to a certain degree for the cases with compression of spinal cord.Conclusion Good exposure of upper cervical spine and effective surgical oper- ation could be achieved from high anterior cervical retropharyngeal approach,with minimal trauma and complication, and it was a favorable approach.

7.Analysis of the growth and development of HIV-infected children on initiated ART in Guangxi, 2004-2019

Abstract: Objective To investigate the growth and development of HIV-infected children initiating antiretroviral therapy (ART) and its influencing factors. Methods This retrospective cohort study was conducted in Guangxi. HIV-infected children initiating free antiretroviral therapy (ART) from 2004 to 2019 were included. Z-score was calculated according to the Chinese Child Growth and Development Standard, the Cox-Stuart test was used for trend analysis, and the generalized estimating equation was used to analyze HAZ (Height-for-age Z-score)≥-2 and WAZ (Weight-for-age Z-score)≥-2 influencing factors. Results A total of 943 children with HIV infection were enrolled in the cohort. The median HAZ at baseline and 1, 2, 5, and 10 years after treatment was -2.47, -2.14, -1.94, -1.55, -1.23, respectively, and the median WAZ was -1.85, -1.40, -1.30, -1.21, -1.09, respectively. By Cox-Stuart trend test, HAZ and WAZ showed an upward trend with the treatment time (P<0.05). The proportions of HAZ≥-2 at baseline and at 1, 2, 5, and 10 years after treatment were 38.1%, 46.5%, 51.6%, 66.8%, and 74.6%, respectively, and the proportions of WAZ≥-2 were 57.1%, 76.9%, 81.1%, 85.8% and 89.2%, respectively. According to Cox-Stuart trend test, the proportions of HAZ≥-2 and WAZ≥-2 increased with the treatment time (P<0.05). The results of multivariate generalized estimating equation analysis showed that the associated factors with HAZ≥-2 were that age at ART initiation was 3-7 years old (aOR=0.71, 95%CI: 0.53-0.94), age at ART initiation was >7 years old (aOR=0.66, 95%CI: 0.47-0.93), CD4 cell counts before ART was <200/μL (aOR=0.64, 95%CI: 0.47-0.87), WHO clinical stage before ART was Stage Ⅲ/Ⅳ (aOR=0.74, 95%CI: 0.56-0.97) and time on ART (aOR=1.01, 95%CI: 1.01-1.01); the associated factors with WAZ≥-2 were male (aOR=0.72, 95%CI: 0.53-0.97), WHO clinical stage before ART was Stage Ⅲ/Ⅳ (aOR=0.63, 95%CI: 0.45-0.86) and time on ART (aOR=1.01, 95%CI: 1.01-1.01). Conclusion Antiretroviral therapy can effectively improve the growth and development status of children with HIV infection, but a large proportion of children still have stunted growth and development in the 10th year after treatment. It is necessary to strengthen the training of antiretroviral treatment staff and the publicity and education of parents of children infected with HIV. Improve the effect of antiviral treatment and guide children's nutrition reasonably.
9.Calcitriol pulse therapy combined with resin adsorption in treatment of severe secondary hyperparathyroidism

Objective To investigate the effects of oral calcitriol pulse therapy combined with resin adsorption in treatment of severe secondary hyperparathyroidism.Methods Thirty-two hemodialysis patinents were randomly divided into three groups:oral calcitriol pulse therapy(CP group,12 cases),oral calcitriol pulse therapy combined with resin adsorption group(CP+RA group,10 cases),and resin adsorption group(RA group,10 cases).Serum immunoreactive parathyroid hormone(iPTH),albumin,urea,creatinine,hemoglobin levels were detected before treatment and 1,2,3 months after treatment respectively.Serum calcium and phosphorus levels were also detected every half a month.Results The clearance rate of resin adsorption Was decreased significantly every time(P<0.05).Serum iPTH in CP+RA group decreased significantly 2 months after treatment.Serum iPTH3 months after treatment in CP+RA group[(598.50 ±317.45)ng/L]was less than that in CP group[(1008.67±436.85)ng/L]and RA group[(1464.60±411.27)ns/L](P<0.05).Conclusion Oral calcitriol pulse therapy combined with resin adsorption can clear iPTH safely and effectively.
10.Computer-assisted navigation technique in the spinal pedicle screw internal fixation

BACKGROUND: Recently, trans-pedicle screw internal fixation has markedly improved the rigidity of spinal fixation and hence the fusion rate. But when placed incorrectly, the pedicle screw can injure the spinal cord and/or nerve roots, resulting in serious complications.OBJECTIVE: To evaluate the application value of preoperative CT scans-based navigation technique in the spinal pedicle screw internal fixation surgery.DESIGN, TINE AND SETTING: A prospective, randomized, and controlled observation was performed at the Department of Orthopedics, Peking Union Medical College Hospital, Chinese Academy of Medical Sciences between January 2006 and December 2008.PARTICIPANTS: A total of 95 patients who underwent pedicle screw internal fixation due to spine diseases were randomly assigned to a navigation group (n = 45) and a conventional group (n = 50).METHODS: In the navigation group, patients were subjected to pedicle screw insertion with the assistance of computer navigation technique and while in the conventional group, patients underwent pedicle screw insertion using the conventional anatomic landmark combined with fluoroscopy.MAIN OUTCOME MEASURES: Screw channel preparation time, excellent and good rate of screw position, and postoperative complications.RESULTS: In the navigation group, totally 206 pedicle screws were inserted under navigation guidance, with an excellent and good rate of 96.1%; and navigation could not be continued in 9 patients for a three-dimensional registration error. In the conventional group, altogether 285 pedicle screws were inserted, and the excellent and good rate was 100%. No significant difference was found between the two groups (P > 0.05). The navigation group exhibited longer screw channel preparation time than the conventional group [(360±22) seconds vs. (56+8) seconds, P < 0.01]. No postoperative complications were found in each group.CONCLUSION: The preoperative CT scans-based navigation technique produces similar accuracy of pedicle screw insertion, but markedly prolonged operation time, compared with the conventional anatomic landmarks, exhibiting limited application value in the spinal pedicle screw internal fixation.
