1.Clinical results of early operative intervention for lower cervical vertebral degenerative instability

Objective To investigate the results of early decompression and internal fixation for lower cervical vertebral degenerative instability. Methods Eighty-two patients of low cervical vertebral degenerative instability were reviewed. The patients comprised of 51 males and 31 females, with an average age of 54.3 years. There were 13 patients with neck and shoulder pain, 28 with cervical spondylotic radiculopathy (CSR), 25 with cervical spondylotic myelopathy (CSM), 16 with combined CSR and CSM. Magnetic resonance imaging(MRI) showed that the cervical spinal cords were compressed. All patients had cervical vertebral angular displacement (AD) exceeding 10? or horizontal displacement(HD) surpassing 3 mm at the same level on hyperflexion/hyperextension X-ray films. According to the course of cervical degenerative instability and Kirkaldy-Willis criterion, all patients were diagnosed as cervical vertebral degenerative instability, and recieved anterior decompression, bone grafting and internal fixation. Results All the patients were followed for 9 to 30 months (average 15.4 months). Objective The reliability of the RUS-CHN of The Standards of Skeletal Maturity of Hand and Wrist for Chinese-China 05 and the TW_3-C Carpal methods was studied and the effects of readers' experience on the measured skeletal age were determined.Methods Under the conditions that readers were blinded to the age and sex of the children,the hand and wrist radiographs of 75 children(skeletal age from 3 to 18 years) were independently read by 11 readers using RUS-CHN method and the radiographs of 46 children(skeletal age from 3 to 13.5 years) were analyzed by 6 readers using TW_3-C Carpal method.The radiographs were re-read by the same readers 20 days later.The percentage agreement in the ratings and the differences of skeletal age in the rereading were used to evaluate the reliability.Results It was found that(a) intraobserver percentage agreement of ratings were 63.4% to 82.2% for RUS-CHN and 65.6% to 83.2% for TW_3-C Carpal respectively,the intraobserver 95% confidence limits for a single reading were ?0.40 year to ?0.76 year for RUS-CHN and ?0.38 year to(?0.71) year for TW_3-C Carpal respectively;(b) interobserver percentage agreement of ratings were 61.3% to 77.3% for RUS-CHN and 77.4% to 88.0% for TW_3-C Carpal respectively;interobserver 95% confidence limits for a single reading were ?0.42 year to ?0.96 year for RUS-CHN and ?0.42 year to ?0.78 year for TW_3-C Carpal respectively.Conclusion The present findings suggest that the reliability of skeletal age assessment between readers is significantly different and that the reliability levels of the experienced and relative experienced readers is higher,which can be obtained for few nonexperienced readers through training of assessment of skeletal age.The assessments of skeletal age are unreliability for the majority of non-experienced readers,so that the further training for them is needed.
3.Protective Effects of Total Flavones of Herba Epimedii on Focal Cerebral Ischemia in Rats

Objective To study the protective effects of total flavones of Herba Epimedii(TFE)on focal cerebral ischemia of rats.Methods Rat model of middle cerebral artery occlusion was established.With Jinnaduo as a positive control drug,the effect of TFE on the neurological behavior of cerebral ischemia rat,the cerebral water volume,the cerebral infarction area,the cerebral biochemistry and the pathologic change of brain were evaluated.Results TFE improved rats neurological behavior,decreased the cerebral water volume,reduced cerebral infarction area,increased ATPase and SOD activity,decreased MDA content,and also improved the pathological changes of ischemic brain.Conclusion TFE have the protective effect on cerebral ischemia in rats,its mechanism may be concerned about improving energy metabolism and anti-oxidation.
4.Determination of Notoginsenoside R_1,Ginsenoside Rg_1 and Rb_1 in Kangliu Pills by HPLC

Objective To set up a HPLC method for the determination of the contents of notoginsenoside R1,ginsenoside Rg1 and Rb1 in Kangliu pills.Methods It was performed on Kromasil C18(4.6 mm?150 mm,5 ?m),and the mobile phase was acetonitrile-0.05% phosphoric acid with gradient elution system.The flow rate was 1.0 mL/min,the detective wavelength was 203 nm,injection volume was 10 ?L and the column temperature was 30 ℃.Results The liner range of notoginsenoside R1 was 82.8~621 ?g(r=0.999 9) and the average recovery was 100.057%(RSD=0.399 6%).The liner range of ginsenoside Rg1 was 109.8~ 823.5 ?g(r =0.999 6) and the average recovery was 99.248%(RSD=1.004 6%).The liner range of ginsenoside Rb1 was 112.6~844.5 ?g(r=0.999 9) and the average recovery was 99.812%(RSD=1.474 4%).Conclusion The method is simple,accurate and reproducible,and suitable for the content determination of notoginsenoside R1,ginsenoside Rg1 and Rb1 in Kangliu pills.
5.Phenotype profiles and u-PA activity of bone marrow micrometastasis in breast cancer patients

Objective To study the phenotype profiles of bone marrow micrometastatic tumor cells in patients with breast cancer. Methods Immunocytochemical staining was used to study the biologic markers of disseminated tumor cells, and U-PA activity of the plasma in the bone marrow was assayed in breast cancer patients.Results Judged by positive immunocytochemical staining of EMA and CK19, bone marrow micrometastasis was found in 30 out of 72 (41 67%) breast cancer patients. The primary tumor size (? 2=6 417, P=0 040) and p53 protein expression (? 2 =5 930, P=0 025) were significantly correlated with the incidence of micrometastasis. Disseminated tumor cells expressed low protein cyclinD1, p53,Ki-67, EGFR, and high protein p21. Tumor size and axillary lymph node status were found to be significantly correlated with the u-PA activity level. Conclusion Disseminated tumor cells in the bone marrow was in a status of low-cycling and low-proliferation.
6.Optimal therapy for patients with HBeAg positive chronic hepatitis B who have poor response to interferon α

Objective To investigate the efficacy of interferon alpha (IFNα) plus adefovir dipivoxil (ADV) and tebivudine (LdT) monotherapy for patients with HBeAg positive chronic hepatitis B (CHB) who have poor response to IFNα treatment.Methods A total of 86 HBeAg positive CHB patients admitted to the Sixth People' s Hospital of Shaoxing during February 2010 and April 2013 were enrolled in the study.All the patients received IFNα monotherapy for 24 weeks and had poor responses.The patients were voluntarily divided into three groups:IFNα monotherapy group (n =21),IFNα plus ADV group (n =30) and LdT monotherapy group (n =35).Chi-square test was used to compare ALT normalization rates,rate of HBV DNA load < 500 copies/mL,HBeAg seroconversion and HBsAg seroconversion rates among three groups.Results After 48 weeks of treatment,the ALT normalization rate in IFNα monotherapy group was 52.6% (10/19),which was lower than those in IFNα plus ADV group (86.7%,26/30) and LdT monotherapy group (84.8%,28/33) (x2 =6.913 and 6.361,P < 0.05).The rate of HBV DNA load <500 copies/mL in IFNα monotherapy group was 26.3% (5/19),which was lower than those in IFNα plus ADV group (60.0%,18/30) and LdT monotherapy group (54.5%,18/33) (x2 =11.33 and 3.895,P <0.05).No HBeAg negative conversion or seroconversion was observed in IFNα monotherapy group,but it was observed in 6 (20.0%,6/30) patients in IFNαt plus ADV group and 7 (21.2%,7/33) patients in LdT monotherapy group (x2 =4.330 and 4.657,P < 0.05).No HBsAg seroconversion was observed in three groups.There were no statistical significant differences in ALT normalization rates,rate of HBV DNA load < 500 copies/mL,HBeAg seroconversion and HBsAg seroconversion rates between IFNαt plus ADV group and LdT monotherapy (x2 =0.042,0019 and 0.064,P > 0.05).Conclusion For patients with HBeAg positive CHB who had poor response to IFNα treatment,IFNα plus ADV therapy and LdT monotherapy have the same efficacy in improvement of both liver function and virological response.
7.Clinical significance of the detection of serum iNOS and SOD in patients with chronic hepatitis B

Objective To explore the relationship between the HBV‐DNA ,ALT ,AST and NO ,iNOS ,SOD levels in serum of patients with chronic hepatitis B(CHB) .Methods 24 patients with mild CHB ,30 patients with moderate CHB ,18 patients with se‐vere CHB and 30 healthy individuals were selected and set in group A ,B ,C and D ,respectively .The serum levels of HBV‐DNA , ALT ,AST ,NO ,iNOS ,and SOD were detected by FQ‐PCR and chemical analysis respectively .Results There were significant difference in the levels of ALT ,AST ,NO ,iNOS and SOD between group D and group A ,B and C (P<0 .05) .The serum level of ALT was positive relative to the levels of NO and iNOS(r=0 .487 ,0 .521 ,P<0 .05) ,and was negative relative to the level of SOD (r= -0 .574 ,P<0 .05) .The serum level of AST was positive relative to the levels of NO and iNOS(r=0 .453 ,0 .545 ,P<0 .05) , and was negative relative to the level of SOD(r= -0 .484 ,P<0 .05) .Conclusion With the increase of ALT and AST levels ,the levels of NO and iNOS increase ,and the level of SOD decreases simultaneously in CHB patients .It is suggested hepatocellular inju‐ry .

The present study was designed to evaluate the effect of anti intercellular adhesion molecule 1(ICAM 1) monoclonal antibody in a rat model of myocardial ischemia reperfusion injury (I/R). Anaesthetized rats were subjected to total occlusion (45 min) of the left main coronary artery followed by 24 h reperfusion. The area of myocardial necrosis, myocardial function (such as LVSP, +P′max, LVEDP and -P′max) and PMN infiltration were measured. 9.Roles of P53 and P21 in the differentiation of PC12 cells

Objective To investigate the roles of P53/P21 during neuronal differentiation with a differentiated model of PC12 cells. Methods A new cell line PC12(P53/m175) was created by stable transfection of a retrovirus plasmid pBabe-P53/m175, which contains a dominant-negative P53 gene mutant. After NGF treatment, observing with phase-contrast microscopy, flow cytometric analysis and western blotting of P53 and P21 were performed. Results Expression of P53 and P21 was obviously increased in NGF-induced PC12 cells. The appearance of cell cycle G1 phase arrest paralleled the increased expressions of P53 and P21. The level of P21 protein did not change after treatment with NGF in PC12(P53/m175) cells and the extent of G1 phase arrest markedly decreased. However, we did find the normal neurite outgrowth in NGF-treated PC12(P53/m175) cells. Conclusion NGF-induced an increased protein levels of P53 and its transcriptional element, P21 is essential for cell cycle G1 phase arrest, but does not necessarily correlate with the neurite outgrowth of PC12 cells.
10.Electro-acupuncture on the iNOS of Lumbar Intervertebral Disk Tissues in Rats

Objective: The present study is to investigate the mechanism of electro-acupuncture in preventing and treating lumbar intervertebral disk displacement. Methods: Seventy-five Sprauge-Dawley rats were used to establish the models of lumbar intervertebral disk displacement by operation, and divided into five groups including electro-acupuncture group, western medicine group, acupuncture plus medicine group, model group and normal control group, then they were treated with corresponding therapeutic method. The expression of inducible nitric oxide synthase (iNOS) mRNA in the lumbar intervertebral disk tissues of rats was detected by RT-PCR. Results: The expression of iNOS mRNA in the lumbar intervertebral disk tissues of rats in model group was compared with that in normal control group, the significant difference was found (P<0.01); While the expressions of iNOS mRNA in the lumbar intervertebral disk tissues of rats in electro-acupuncture group, western drug group and acupuncture plus medicine group were all inhibited, and compared with the model group, the significant differences were also found (P<0.01). Conclusion: Electro-acupuncture could inhibit the expression of iNOS mRNA in the lumbar intervertebral disk tissue.
