1.Nosocomial Infection of Inpatients:Investigation and Analysis of Hospitalized Cases from 2000 to 2006

OBJECTIVE To investigate the clinical feature of nosocomial infection and the type of pathogens in general hospital, and provide the scientific theory basis in order to prevent and control the nosocomial infection. METHODS An analysis was made on the basis of the survey, the data came from hospitalized cases from 2000 to 2006. RESULTS Their prevalence rate was 3.8%. The highest infected rate was in ICU (16.9%). The main infection site was respiratory tract. Among pathogens isolated from nosocomial infection cases, 45.65% were Gram-negatives, 31.9% were fungi, and 20.58% were Gram-positives. The formation rate of the Enterococcus gradually increasesd from year to year. CONCLUSIONS There are many effective actions to reduce the nosocomial infection, such as strengthening the medical training, carrying out the aseptic technology strictly, reducing the invasive operation, raising the executable rate of hand hygiene, and using the antibacterial drug reasonably.
2.Sterilizing Effect and Evaluation of Avagard Surgical Washing Liquid and Orthodox Surgical Hand Antisepsis Method

OBJECTIVE To evaluate the clinical application of the two surgical hand antisepsis methods on hand sterilizing effect. METHODS Totally 123 medical staff of operating theater were divided into Avagard surgical washing liquid (60) and orthodox surgical hand antisepsis (63) groups at random. The samples were derived from fingers and cultured before,just after and 2h after sterilization. RESULTS The colony counts were 4.64?4.87,0.05?0.16 and 0.02?0.10 CFU/cm2,respectively in avagard surgical washing liquid group;vs 4.10?4.03,0.08?0.26 and 0.49?1.31 CFU/cm2,respectively,in orthodox surgical hand antisepsis group. 4.Comparison of Transcervical Resection and Loop Electrical Excision Procedure for the Treatment of Moderate-to-severe Chronic Cervicitis

Objective To compare the therapeutic effects of transcervical resection(TCRC) and loop electrical excision procedure(LEEP) for the treatment of moderate-to-severe chronic cervicitis.MethodsFrom January 2003 to July 2006,totally 520 patients with moderate-to-severe chronic cervicitis were randomly divided into two groups to receive TCRC or LEEP.ResultsThe intraoperative blood loss in the TCRC group was significantly less than that in the LEEP group [(4.2?1.3) ml vs(10.1?4.5) ml,t=-20.310,P=0.000].Whereas,no significantly differences were found in the drainage and bleeding time and cure rates between the two groups [(15.9? 3.7) d vs(16.2? 3.3) d,t=-0.976,P=0.330;and 95.2%(236/248) vs 93.2%(235/252),?2=0.832,P=0.362].ConclusionsBoth TCRC and LEEP are effective for chronic cervicitis.TCRC is superior to LEEP in the surgical outcomes;however the latter is easier to perform than TCRC.
5.Design and application of a new neonatal hand ring

Objective In order to reduce neonatal missing and obstetric malpractice,design and application of a new neonatal hand ring was proposed in this paper.Methods Positive stop button,positive stop knot,positive stop cannula,rope,double sandwich tag set were included in the structure of new neonatal hand ring.Results The new neonatal hand ring has been experimentally applied by some departments in Nanjing Maternity and Child Health Care Hospital.The result showed the new neonatal hand ring effectively reduced the incidence of hand ring falling off,arm erythema and beds error.Conclusions Application of the new neonatal hand ring can guarantee the legal rights of newnates,avoid the disputes caused by misunderstanding and increase the automatic management level of hospitals.
6.Comparison of Transcervical Resection and Loop Electrosurgical Excision for the Treatment of Grade Ⅰ Cervical Intraepithelial Neoplasia

Objective To compare the efficacy of transcervical resection(TCRC)and loop electrosurgical excision procedure(LEEP)for the treatment of cervical intraepithelial neoplasia grade I(CINⅠ).Methods A total of 231 CIN I patients were divided into two groups according to their patient number to receive TCRC or LEEP.The resected specimens were sent for pathological diagnosis and human papilloma virus(HPV)-16/18 test.Results No significant difference was found in the operation time[(14.1?2.2)min vs.(13.8?2.1)min],rates of wound infection and cervical stricture[1.7%(2/115)vs.1.7%(2/116)and 0.9%(1/115)vs.1.7%(2/116)],and rates of cure and recurrence [99.1%(107/108)vs.99.1%(108/109)and 0.9%(1/108)vs.0.9%(1/109)] between the TCRC and LEEP groups(t=1.060,P=0.290;?2=0.000,P=1.000;?2=0.000,P=1.000;?2=0.000,P=1.000;?2=0.000,P=1.000).In the TCRC group,the surgical wound was healed in(5.0?0.6)weeks,which was significantly shorter than that in the LEEP group[(5.2?0.7)weeks,t=-2.331,P=0.021].The patients who had severe endocervicitis in both the groups had similar rate of residual endocervicitis[16.7%(2/12)vs.53.8%(7/13),Fisher's test:P=0.097].In both the groups,one patient respectively showed residual lesion after the procedure,in whom HPV-16 and/or HPV-18 were detected.Conclusions Both TCRC and LEEP are effective for CINI.LEEP is easier to master than TCRC.Closed follow-up is essential of the with patients positive HPV-16 or-18.

The growth-propagation and decolorization-degradation systems for the model dy e Biebrich Scarlet by Trametes hirsuta were established preliminar ily. It was showed that 30℃ and the static culture were better; the effects of compo nents in culture media on the efficiencies of decolorization and degradation wer e not significant; considering the convenience of observing the changes of dye c o lors and shortenig the culture period, the potato liquid medium had the advantag e of others as a preferable medium for reaction systems of Trametes hirsuta . The decolorization and degradation of all dyes Biebrich Scarlet and Direct Deep Blu e L-3RB, Reactive Blue X-BR, Basic Violet 5BN, and Methylene Blue by Tramete s hirsuta were better.
8.Pathogens and Antibiotic Resistance Analysis in Urinary Tract Infection

OBJECTIVE To investigate the bacterial distribution and antibiotic resistance situation with urinary tract infection(UTI) for the guidance of rational use of antibiotics.METHODS The antibiotic resistance of clinical isolates from urinary tract infection from Mar 2005 to Jul 2006 was analyzed. RESULTS The most common pathogens in urinary tract infection were Escherichia coli(50.2%),Enterococcus(14.4%),Staphyloccus aureus(8.7%),Klebsiella pneumoniae(7.3%),and Proteus mirabilis(3.9%).E.coli,K.pneumoniae,and P.mirabilis were found to be highly resistant to ampicillin,quinolones and SMZ(70.6-100.0%).Enterococcus were highly resistant to penicillin and quinolones(81.0-96.8%).41.4% of E.coli and 31.3% of K.pneumoniae isolates produced ESBLs.HLGR-Enterococcus were 79.4%.78.9% S.aureus isolates were resistant to oxacillin.CONCLUSIONS The high antibiotic resistance of commonly encountered pathogens is a serious problem and much attention should be paid to detect pathogens and their antibiotic resistance.
9.The clinical features of acute coronary syndrome combined with low T3 syndrome

Objective To investigate the clinical features of acute coronary syndrome (ACS) combined with low T3 syndrome and its prognosis.Method Eighty-seven ACS patients with low free triiodothyronine (FT3) level (low FT3 group) and additional 64 ACS patients with normal FT3 level(control group) were selected in parallel; all the patients underwent coronary angiography.The contents of FT3,N-terminal pro-brain natriuretic peptide (NT-proBNP),cardiac troponin I (cTnI) and high sensitivity-C reactive protein (hs-CRP) were recorded,and the related factors were evaluated.The number of stenotic vessels was calculated.Results The contents of NT-proBNP,cTnI and hs-CRP in low FT3 group were elevated significantly compared with those in control group [(1242.14 ± 226.32) ng/L vs.(1126.36 ±195.55) ng/L,(0.92 ± 0.23) ng/L vs.(0.84 ± 0.19) ng/L,(6.46 ± 2.25) mg/L vs.(5.71 ± 1.78) mg/L],and there were significant differences (P < 0.01 or < 0.05).The contents of FT3 were negatively correlated with NT-proBNP,cTnI and hs-CRP in two groups (P < 0.05).There was no significant difference in the number of stenotic vessels between two groups (x2 =4.512,P =0.105).Conclusion In patients with ACS,there is a negative correlation between contents of FT3 and the prognosis,but there is no correlation between contents of FT3 and the number of stenotic vessels.
10.Effects of Fluvastatin on microalbuminuria of type 2 diabetes mellitus patients

Objective To observe the effect of fluvastatin on urinary albumin excretion rate(UAER)of microalbuminuria in type 2 diabetes patients.Methods Type 2 diabetes patients with mieroalbuminuria were randomly divided into fluvastatin goup(n=126)and control group(n=129).Fluvastatin group was given fluvastatin 40 mg each night.Follow up was conducted for 1.5 years.The UAER and glomerular filtration rate(GFR)as well as blood lipid level before and after therapy were compared.Results UAER in fluvastatin group was significantly lower than that in control group[(59.6±10.5)vs(87.5±12.3)mg/min,P<0.05]before therapy[(104.4±25.2)vs(110.6±19.7)mg/min,P<0.05],which was independent of its lowering-lipid effect.But GFR had no signifimicroalbuminuria which is independent of lowering-lipid effect in type 2 diabetic nephropathy patients and delay the progress of diabetic nephropathy.
