Objective To evaluate the efficacy of metal stent placement for the treatment of malignant gastroduodenal obstruction. Methods Medical records of 24 patients with gastroduodenal obstruction secondary to malignant tumors (gastric cancer, 19 patients; pancreatic cancer, 4 patients; postsurgical biliary duct cancer, 1 patient) from October 2002 to November 2004 were reviewed. Under radioscopy, a self-expanding metal stent was placed endoscopically into the stenosis site of the stomach or the duodenum. Results All the metal stents were placed at the expected position. After operation the patients were given fluid diet at 1 day and soft diet at 3 days. No stent-related gastrointestinal perforation or hemorrhage was observed. Follow-up for 1~24 months in 21 patients found re-obstruction in only 2 patients and no migration of metal stents. Conclusions Self-expanding metal stent placement for malignant gastroduodenal obstruction is safe and effective.

Chinese Journal of Immunology 1985;0(05):-
Objective:To produce a fusion protein(ICOS-Ig)between extracellular portion of human ICOS and Fc portion of mouse IgG 2a and study on its biological activity. Methods:cDNA encoding the extracellular domain of human ICOS was prepared by RT-PCR from the RNA of the stimulated human peripherial blood mononuclear cells. The Fc portion of mouse IgG2a was cloned by PCR from the vector that contains the sequence-encoding Fc portion of mouse IgG2a.Two resulting amplified PCR products were ligated into a secrection mammalian expression vector, pSecTag2/Hygro A. The recombined vector was transfected into CHO cells by lipofectamine2000.Results:RT-PCR demonstrated the integration and mRNA synthesis of fusion gene. ELISA and Western blot analysis showed the expression of ICOS-Ig and its molecular weight was between 43-66 kD, its concentration was 5-25 ?g/ml. FACS analysis assured that ICOS-Ig had ligand specific binding activity. Addition of ICOS-Ig to MLR resulted in inhibition of T-cell proliferation and IL-2,IFN-? secretion. Conclusion:The fused gene ICOS-Ig was constructed and expressed successfully. It had the bioactivity of inhibition of T cell proliferation and cytokine secretion.

Objective To evaluate the comprehensive experiment in fundamental nursing by using the indicator system and analyze the teaching effect. Methods 450 nursing graduates of grade 2011 were divided into 56 groups and comprehensive experiments were graded by using the indicator system in each group. Results The four indexes in level one scored (18.08±1.50),(19.99±2.31),(28.18±3.35),(18.16±1.73) points and accounted for 90% 、80% 、81%、91% respectively. The total points' scorings and proportions were (84.41±5.84) points and 84% respectively. Some deficiencies were found to be improved during evaluation. Conclusions Nursing comprehensive experiment evaluation indicator system can assess comprehensive experiments well, and the related competencies of nursing graduates need to be improved.

Objective To investigate the influence of molecular targeted therapy (thalidomide, lenalidomide or bortezomib) on survival and curative effect of patients with multiple myeloma (MM).Methods The treatment of 217 patients diagnosised as MM from 1998 to 2013 were reviewed. The survival of patients who were diagnosed during different periods were compared, and the impact of improved treatment on the survival and curative effect of patients were analyzed. Results Patients who were recently diagnosed as MM after 1998 had better survival.75 patients with targeted therapy (thalidomide or bortezomib) for their early treatment had better survival compared to the patients with conventional treatments of MP/VAD.49 patients with targeted therapy after relapse or progression had a better curative effect than those with conventional treatments.16 patients had auto-ASCT or allo-SCT,and 4 of them had both auto-ASCT and allo-SCT,who had better survival than patients without transplantation. Conclusion Patients treated with molecular targeted therapy (thalidomide, lenalidomide, bortezomib) had a better curative effect and longer survival.

?AIM:To investigate the causes and characteristics of retinal detachment after vitrectomy.?METHODS: Retrospective analysis of patients in our hospital in recent 4a who underwent primary, standard, three -port pars plana vitrectomy ( PPV ) . Retinal detachment occurred in 24 eyes ( 23 cases ) , 24-67 years old, average 49.0±12.0 years old. There were male in 17 cases, female in 6 cases, 6 right eyes, 18 left eyes.?RESULTS:Eighteen (75%) eyes without any symptoms were found during a routine check, 6 eyes ( 25%) with decreased visual acuity or in front of shadow. It took 1-223d from PPV to retinal detachment, average 40.7 ± 54.2d, of which retinal detachment occurred in 10 (42%) eyes within 1wk, 14 (58%) eyes within 1mo.The direct cause of retinal detachment was new retinal break ( a total of 27 holes ) in 19 eyes, proliferative vitreoretinopathy ( PVR) in 4 eyes, 1 eyes with primary hole not closed. All retinal detachment were ultimately completely reset.?CONCLUSION: The occurrence of new retinal breaks is the main cause of retinal detachment after vitrectomy, attention should be paid to the early postoperative peripheral retinal examination;after a timely surgery, the retina can be reset.

Objective:To study the bioactivity of inducible costimulator Ig(ICOS-Ig) as an inhibitor of ICOS-B7RP-1 costimulatory pathway in vitro. Methods: cDNA encoding the extracellular domain of human ICOS was prepared by RT-PCR from the RNA of the stimulated human peripheral blood mononuclear cells. The Fc portion of mouse IgG2a was cloned by PCR from the vector containing the sequence-encoding Fc portion of mouse IgG2a.The above 2 PCR products were ligated into a clone vector: pGL-3-Basic. The fusion gene was then cloned and ligated into a mammalian expression vector: pcDNA4/HisMAX A. The recombined vector was transfected into CHO cells by Lipofectamine2000 and the expression of the fusion protein was identified by Western blot. The mixture lymphoproliferation reaction(MLR) of the lymphocytes derived from BALB/c and C57BL mice was used to detect the fusion protein function in vitro. Results: Western blot analysis showed the expression of fusion protein, with the molecular weight being 43 000-66 000. FACS analysis assured that expression products had ligand specific binding activity. MLR was inhibited by the fusion protein. Conclusion: The constructed recombinant fusion protein has ligand specific binding activity and can inhibit the lymphoproliferation.
