Objective To explore the treatment and nursing intervention in heart failure, the use of cedilanid therapy. Methods In 40 cases of heart failure patients using conventional methods of intervention, and classified as the control group, the other 40 patients in the control group using Western intervention on LAN, and classified as the observation group, two groups the patients in our hospital from January 2017 to March 2015. Results The two groups of patients have found efficiency, patients in the observation group were significantly higher, the observation of patients was 92.5%, the control group was 72.5%; the two groups of patients with heart rate, blood pressure were not significantly different in the treatment before and after intervention in observation group were improved after grouping obviously better than that shows obvious differences (P<0.05). The two groups had no obvious adverse reactions. Conclusion Cediland analysis shows that in the treatment of heart failure and intervention effect, can improve the clinical situation of patients, improve the clinical treatment effect, and no significant adverse reactions, worthy of clinical reference.

Objective To investigate the application value of urine excrement shunt surgery for the treatment of severe metabolic disorders after Sigma rectum pouch of bladder cancer.Methods From July 2011 to December 2013,4 male patients (mean age 72±10 years old,ranged from 58-82 years old),who received radical cystectomy and Sigma rectum pouch due to bladder cancer,were admitted in our hospital.Within four years after the operation,all patients complained about the recurrent fatigue,anorexia and even unconsciousness.The hyperchloremic metabolic acidosis,hypokalemia and renal dysfunction were diagnosed by blood gas analysis and biochemical tests.One week before urine excrement shunt operation,the arterial blood pH,HCO3-,serum potassium,chloride and creatinine were (7.15±0.08),(7.8±4.7)mmol/L,(3.1 ±0.2) mmol/L,(110.3±4.7) mmol/L,(314.8±66.4) μmol/L,respectively.They received urine excrement shunt surgery.The lower abdominal mid-line incision was made and the pouch was isolated from intestinal tract.The abdomen ostomy was made and the sigmoid colon and rectum were re-coincided in order to recover the intestinal continuity.The results of blood gas analysis,electrolytes and renal function after surgery were compared with those before surgery.Results The average operation time was (256.3±26.9) min and the mean volume of bleeding was (147.5±111.2) ml.There was no surgery related complication.Blood bicarbonate was (19.1±4.8) mmol/L one week after surgery,which was significantly improved than that before operation (P＜0.05).One month later,blood pH level descended to (7.38±0.05),potassium level was (3.9±0.3) mmol/L,creatinine was (208.8±50.8) umol/L.All of them were significantly improved,compared with those results before surgery (P＜0.05).Blood chloride was significantly reduced to (102.4±5.8) mmol/L (P＜0.05) after 6 months of operation.Nobody developed severe acidosis and electrolyte disturbances during 6 months to 3 years' follow-up.Conclusions Urine excrement shunt surgery can effectively corrected the acidosis and electrolyte disturbances due to the radical cystectomy and Sigma rectum pouch.It might be a alternative method to treat such kind of severe complications.

OBJECTIVETo report a case of simple congenital bilateral ejaculatory duct absence (EDA) complicated with seminal vesicle cyst and review the relevant literature in order to improve the diagnosis and treatment of the disease.

METHODSWe retrospectively reviewed the clinical data of a case of bilateral congenital EDA complicated with seminal vesicle cyst, reviewed the relevant literature at home and abroad, and comprehensively analyzed the embryonic development, diagnosis, and treatment of congenital EDA.

RESULTSThe patient was a 23-year-old man, present at the clinic for infertility after married for a year. Vasography and other imaging examinations confirmed simple congenital bilateral EDA complicated with seminal vesicle cyst. Pathologic biopsy showed normal spermatogenic function of the testes.

CONCLUSIONCongenital EDA originates from embryonic developmental defect of the mesonephric duct, and it can be confirmed by vasography. Transurethral incision of the ejaculatory duct and intracytoplasmic sperm injection can be employed for the treatment of bilateral EDA.

Objective To clone and sequence the cDNA encoding farnesyl pyrophosphate synthase(FPS) from Panax notoginseng.Methods The cDNA,encoding FPS in P.notoginseng,was amplified by RACE strategy with the total RNA of root as the template.The fragment of FPS was cloned and sequenced.Results The analysis results revealed that the full-length cDNA had(1 409) bp with an open reading frame encoding 343 amino acids of protein.The FPS sequence had 95%,87%,and 86% amino acid sequence homology to the FPS sequence of Centella asiatica,Parthenium argentatum,and Artemisia annua,respectively. Conclusion The cDNA encoding FPS from P. notoginseng is cloned and reported.This works provide a foundation for exploring the mechanism of saponins biosynthesis and application to the other medical plants.

A new ionizing-radiation resistant strain WGR702 was isolated from arid soils which had been ra-diated. The strain WGR702 was Gram-positive and coccus, the diameter of the cell was 1.5 ?m～2.5?m.The strain WGR702 was pink-pigmented, motile, facultative anaerobe and non-spore forming. The range tem-perature and pH for strain WGR702 growth were 10℃～35℃ and pH 5.0～10.0 respectively. The strain WGR702 had a G+C content of 60.5 mol%. UV and gamma radiation survival curves showed the strain WGR702 had highly ionizing-radiation resistant. Phylogenetic analysis of the 16S rDNA gene sequences (EU315117) showed 94.79%～98.53% similarities with other recognized Serratia species. Primary charac-teristics that distinguish isolate WGR702 from the species of genus Serratia include the cells are spherical and Gram-positive. Based on the phenotypic, biochemical and physiological characteristics differences it is proposed that the new isolated strain WGR702 might be classified as a novel species of Serratia.

The essential medium of B115 composed of 1% tryptone, 0.25% yeast extract and 0.5% sodium chloride was determined by using an orthogonal design. The orthogonal design was also employed in testing the optimum additions. It was composed of 0.1%(NH_(4))_(2)SO_(4),1.4%K_(2)HPO_(4), 0.6% KH_(2)PO_(4) and 0.1% (Na_(3)C_(6)H_(5)O_(7)). The yield of B115 cultured in optimum medium was compared with the one in essential medium. Statistic analysis showed that the growth of B115 was most significantly improved by adding K~(+)、NH~+_(4) and (Na_(3)C_(6)H_(5)O_(7)) to essential medium. The antibacterial effect of Bacillus subtilis strain B115 on pathogenic Aeromonas was studied. The results showed different antibacterial effects of B115 on different aeromonads. There were obvious antibacterial effects on BSK-10 and CL990920, while no effect on the growth of TL970424.

OBJECTIVETo identify the correlation of the volume of residual urine (VRU) with the severity of bladder outlet obstruction (BOO) and detrusor contractility in patients with benign prostatic hyperplasia (BPH).

METHODSA total of 152 patients with clinically diagnosed BPH underwent ultrasonography for measurement of the prostate volume and RVU, free uroflowmetry, and urodynamic examination for the severity of BOO and detrusor contractility. Using the software SPSS20. 0, we analyzed the correlation between the ultrasonographic results and urodynamic parameters and compared the two sample means by the t-test.

RESULTSThe prostate volume was correlated positively with BOO severity (r = 0.432, P < 0.01) and detrusor contractility (r = 0.343 , P < 0.01) while Qmax negatively with BOO severity (r = 0.327, P < 0.01) but not significantly with detrusor contractility (r = 0.123, P > 0.05). VRU showed a significantly negative correlation with detrusor contractility when > 150 ml (r = -0.490, P < 0.01), even more significantly when > 300 ml (r = -0.717, P < 0.01), but exhibited no significant correlation with it when ≤ 150 ml (r = 0.041, P > 0.05).

CONCLUSIONVRU can somehow predict the detrusor function. For patients with VRU > 150 ml, especially for those with VRU > 300 ml, the detrusor function should be evaluated and urodynamic examination is recommended for exact assessment of BOO severity and detrusor contractility.

Objective To evaluate the effectiveness of comprehensive lifestyle modification in the primary health care system on status of pre-chronic diseases in a conventional healthcare program.Methods Two hundred and twenty-nine adult volunteers in 5 conventional healthcare centers were divided into 4 groups:group of prehypertension(n=67),group of prediabetes(n=38),group of abnormity serum cholesterol(n=71)and group of osteopenia(n=53).A muhicomponent behavioral intervention including education,physical activity,dietary practice,cease smoke,calcium supplement and exposure to sunlight was conducted for 12 months.The main outcome of each group was blood pressure,blood glucose,serum cholesterol and bone mineral density.Results Over 12 months,compared with the status before intervention,participants in each group were statistically significantly improved,the mean net reduction in systolic BP/diastolic BP was 9.4 mm Hg/6.6 mm Hg(1 mm Hg=0.133 kPa,t=5.93/8.29.P＜0.05)in group of prehypertension;the reductions of blood glucose,serum cholesterol were 0.9 mmol/L(t=3.69,P＜0.05)and 0.5 mmol/L(t=4.09,P＜0.05)in group of prediabetes and group of abnormity serum cholesterol,and the increase of bone mineral density on lumbar spine was 0.015 g/cm2(t=2.03,P＜0.05)in group of osteopenia,Conclusions For chronic disease prevention,ater one year the comprehensive lifestyle intervention produced beneficial changes in diet,physical activity,and biochemical parameters.This type of intervention is a feasible option to prevent chronic disease and should be implemented in the primary health care system.
