1.Early diagnosis and operative treatment of traumatic thoracic disc herniation

[Objective]To study the clinical manifestation,early diagnosis and surgical outcomes of traumatic thoracic disc herniation.[Method]Eleven patients of traumatic thoracic disc herniation were selected from June 2000 to June 2005.There were 8 males and 3 females with an average age of 23.96 years old(ranged,15-38 years old).The average time from traumatism to diagnosis were 4.1 months(range,2 days to 8 months).After early clinical diagnosis,discectomy through posterolateral transpedicular approach were carried out for all 11 patients.[Result]All 11 patients were followed up for an average time of 2.6 years(range,1.1~3.8 years).According to Otani's classification criterion,the results were excellent in 7cases,good in 3 cases,fair in I cases,no poor case and failure case.The excellent and good results were obtained in 90.91% of the patients.[Conclusion]The incidence of traumatic thoracic disc herniation is rather low,and this disease has no representative symptom and sign.Early diagnosis and operative treatment is a very effective guarantee to curative effect for traumatic thoracic disc herniation.
2.Effect of jujube on the expression of CYP2E1 and TNF-alpha in the hepatic tissue of mice with alcoholic liver disease.

OBJECTIVETo observe the effect of jujube pretreatment on serum levels of AST and ALT, liver pathology, and the expression of cytochrome P4502e1 (CYP2E1) and tumor necrosis factor-alpha (TNF-alpha) in the liver tissue of alcoholic liver disease (ALD) mice.
METHODSTotally 88 Kunming mice were randomly divided into the control group (n = 28), the model group (n = 32), and the jujube treatment group (n = 28). The animal model was prepared using intragastric alcoholism for mice in the model group and the jujube treatment group, while distilled water was intragastrically given to those in the control group. Extraction of jujube was intragastrically given to mice in the jujube treatment group at week 4, while equal volume of distilled water was intragastrically given to mice in the rest two groups. The therapeutic course lasted for 12 weeks. Serum levels of AST and ALT, liver pathology, and the expression of CYP2E1 and TNF-alpha in the liver tissue of ALD mice were observed after administration of jujube.
RESULTSCompared with the model group, serum levels of AST and ALT decreased, the liver pathology was improved, and the expression of CYP2E1 and TNF-alpha in the liver tissue decreased, showing statistical difference (P < 0.05).
CONCLUSIONJujube had certain effect in treating ALD.

3.Clinical Analysis of Intrauterine Occupational Disease in 71 Cases

Objective To diagnose the intrauterine occupational disease by vaginal ultrasonography or hysteroscopy, and to evaluate the diagnostic values of these two methods. Methods One hundred and fifty patients were detected by vaginal ultrasonography and hysteroscopy from Jan 2004 to Dec 2004, and a total of 71 cases were confirmed intrauterine occupational disease. Tissue hysteroscopy or bioposy were performed during the hysterosocopy in order to analyse the tissue pathology. Results In these 71 patients, 47 were no less than 46 years old. Most of them had abnormal uterine bleeding.Thirty-three cases (46.48%) were suggested by vaginal ultrasonography, and 49 (69.01%) by hysteroscopy. 4.Clinical Application Sheets for Special Antibiotics:Investigation and Analysis

OBJECTIVE To analyze the impact of clinical application sheets for special antibiotics on clinical medication and the existing questions in implementation and irrational phenomena caused by the use of antibiotics.METHODS All the applications sheets for the first month clinical use of special antibiotics in our hospital were analyzed retrospectively,and their used dosage before and after of was compared monthly.RESULTS It was indicated that the partial application sheets were filled out incompletely;the dosage of special antibiotics after clinical application was declined obviously,while the pathogen test rate for the sample was too low and the starting point of the preventive use of antibiotics in some departments was too high.CONCLUSIONS The applications sheets for special antibiotics promote the rational usage of antibiotics in some extent;however,there is still deficiency in operation.Meanwhile,there are some problems in the clinical medication in our hospital and need to take the comprehensive preventive measures to manage it.
5.Genetic cloning and sequence analysis of farnesyl pyrophosphate synthase in Panax notoginseng

Objective To clone and sequence the cDNA encoding farnesyl pyrophosphate synthase(FPS) from Panax notoginseng.Methods The cDNA,encoding FPS in P.notoginseng,was amplified by RACE strategy with the total RNA of root as the template.The fragment of FPS was cloned and sequenced.Results The analysis results revealed that the full-length cDNA had(1 409) bp with an open reading frame encoding 343 amino acids of protein.The FPS sequence had 95%,87%,and 86% amino acid sequence homology to the FPS sequence of Centella asiatica,Parthenium argentatum,and Artemisia annua,respectively. Conclusion The cDNA encoding FPS from P. notoginseng is cloned and reported.This works provide a foundation for exploring the mechanism of saponins biosynthesis and application to the other medical plants.
6.The study of cause of early death and a matched study for the risk factors in patients undergoing maintenance hemodialysis

ObjectiveTo explore the cause of early death (death within 3-12 months after hemodialysis) and the related influencing factors patients undergoing maintenance hemodialysis (MHD) as to provide a scientific basis for the prevention of early death.Methods A retrospective matched controlled study was conducted. Fifty-one patients who underwent MHD from January 2004 to April 2014 and died within 3-12 months after hemodialysis in hemodialysis center of the 174th Chinese People's Liberation Army Hospital were included in the case group by retrospective analysis method. According to 1∶2 matched controls, 102 patients underwent hemodialysis in the same period (±2 months) and survived over 12 months were selected as control group. All patients received regular hemodialysis (dialysis 2-3 times per week), with conventional limitation of water and sodium intake, routine treatments such as control of blood pressure, treatment of anemia and disorders of calcium and phosphorus contents. Causes of short-term death were analyzed. Clinical and biochemical parameters of two groups were collected when dialysis was started, and the single factor and multiple factors logistic regression was used to analyze the related risk factors when dialysis was started. Receiver operating characteristic curve (ROC) was plotted to evaluate the value of above parameters in predicting the early death in patents with MHD.Results The main causes of early death of 51 patients with MHD were mainly cardiovascular and cerebrovascular diseases (27 cases, 52.9%), and infections (15 cases, 29.4%). It was shown by single factor analysis that the age [odds ratio (OR) = 6.625, 95% confidence interval (95%CI) = 3.232-13.580,P = 0.000], diabetes (OR = 3.875, 95%CI = 0.654 - 10.622,P = 0.031), specialist intervention time before dialysis (OR = 0.349, 95%CI =0.287 - 0.572,P = 0.004), the emergence of cardiovascular and cerebrovascular events before dialysis (OR = 9.667, 95%CI = 4.632 - 20.174,P = 0.000), the first dialysis for emergency dialysis (OR = 3.875, 95%CI = 1.713 - 8.765, P = 0.005), blood albumin level (OR = 0.294, 95%CI = 0.068 - 0.550,P = 0.008), leukocyte count (OR = 6.286, 95%CI = 1.648 - 23.982,P = 0.026), neutrophil count (OR = 2.833, 95%CI = 1.630 - 4.923,P = 0.001) might be the factors correlating with early death. Eight independent factors were statistically significant, and their effect on the MHD patients was analyzed by logistic regression analysis inα = 0.05 level. The results showed that patients with old age (OR = 1.054, 95%CI = 1.019-1.090,P = 0.002), and the emergence of cardio-cerebrovascular events (OR = 7.469, 95%CI = 2.474 - 22.545,P = 0.000)were early death risk factors of MHD patients, and early specialist intervention before dialysis was a protective factor (OR = 0.286, 95%CI = 0.113-0.722,P = 0.008). ROC curve showed that age had moderate diagnostic value for early death of MHD [area under ROC curve (AUC) = 0.756], the cut-off value was 59.0 years old, the sensitivity was 66.7%, and the specificity was 77.5%. The diagnostic value of early specialist intervention before dialysis was relatively low (AUC = 0.367), the cut-off value was 0.875 years, the sensitivity was 39.2%, and the specificity was 33.3%.Conclusion Old age, the emergency of cardiovascular and cerebrovascular events before dialysis is associated with early death, and specialist intervention ahead of dialysis can reduce the risk of early death.
7.Investigation of metabolic kinetics and reaction phenotyping of ligustrazin by using liver microsomes and recombinant human enzymes.

The metabolic characteristics of ligustrazin (TMPz) in liver microsomes were investigated in the present study. The reaction phenotyping of TMPz metabolism was also identified by in vitro assessment using recombinant human cytochrome P450 enzymes (CYP) and UDP glucuronosyltransferases (UGT). TMPz was incubated at 37 degrees C with human (HLM) and rat liver microsomes (RLM) in the presence of different co-factors. The metabolic stability and enzyme kinetics of TMPz were studied by determining its remaining concentrations with a LC-MS/MS method. TMPz was only metabolically eliminated in the microsomes with NADPH or NADPH+UDPGA. In the HLM and RLM with NADPH+UDPGA, t1/2, K(m) and V(max) of TMPz were 94.24 +/- 4.53 and 105.07 +/- 9.44 min, 22.74 +/- 1.89 and 33.09 +/- 2.74 micromol x L(-1), 253.50 +/- 10.06 and 190.40 +/- 8.35 nmol x min(-1) x mg(-1) (protein), respectively. TMPz showed a slightly higher metabolic rate in HLM than that in RLM. Its primary oxidative metabolites, 2-hydroxymethyl-3, 5, 6-trimethylpyrazine (HTMP), could undergo glucuronide conjugation. The CYP reaction phenotyping of TMPz metabolism was identified using a panel of recombinant CYP isoforms (rCYP) and specific CYP inhibitors in HLM. CYP1A2, 2C9 and 3A4 were found to be the major CYP isoforms involved in TMPz metabolism. Their individual contributions were assessed b) using the method of the total normalized rate to be 19.32%, 27.79% and 52.90%, respectively. It was observed that these CYP isoforms mediated the formation of HTMP in rCYP incubation. The UGT reaction phenotyping of HTMP glucuronidation was also investigated preliminarily by using a panel of 6 UGT isoforms (rUGT). UGT1A1, 1A4 and 1A6 were the predominant isoforms mediated the HTMP glucuronidation. The results above indicate that the metabolism of TMPz involves multiple enzymes mediated phase I and phase II reactions.
8.Antigen Analysis of Angiostrongylus cantonensis in Different Developmental Stages

Objective To analyze the difference among antigens of Angiostrongylus cantonensis in different developmental stages and identify dominant diagnostic antigen for angiostrongyliasis. Methods Antigens of A.cantonensis in different developmental stages were analyzed by SDS-PAGE and immunoblot. Results The protein bands of all developmental stages were similar on SDS-PAGE. The Mr 40 000 , 50 000 , 66 000 and 80 000 antigens reacted not only with the sera of rats infected by A.cantonensis but also with the sera of normal rats. The Mr 104 000 antigen could be discerned by sera of rats infected with A.cantonensis for 2 weeks. The Mr 32 000 antigen could be recognized by sera of rats 2 weeks after infection, and the reaction became stronger with the infection continued. Conclusion The Mr 40 000 , 50 000 , 66 000 and 80 000 antigens might result in the unspecific reaction in the immunodiagnosis of angiostrongyliasis using the crude antigen of A.cantonensis. The Mr 104 000 of larva, Mr 33 000 of adult females and Mr 32 000 of the worms might be used as candidate antigens in early diagnosis and epidemiological survey of angiostrongyliasis.
9.The effects of recombinant human endostatin on adjuvant arthritis in rats

AIMTo investigate the effects of the recombina nt human endostatin on adjuvant arthritis (AA) in rats and its mechanisms. METHODSThe model of rat AA was induced by injection of intradermal CFA. ConA and LPS induce d splencytes proliferation was examined by MTT asssy and the activities of inter leukin-1 (IL-1) and IL-2 were measured by the method of thymocytes prolifera tion. Synoviocytes were dispersed with incubation of collagenase and trypsin, an d IL-1, TNF production of synoviocytes was estimated with radio-immunity assay . RESULTSThe secondary inflammation of AA rats appeared on the 1 0th day after injection of CFA. The therapeutic administration of endostatin (0 1, 0 5, 2 5 mg?kg -1 ?d -1 , sc, ?7 d) was given at that time(d 10). It was found that endostatin significantly inhibited the secondary paw swel ling. The increased ConA-induced splenic lymphocyte proliferation reaction and the activated IL-1 and IL-2 activity of AA rats was reversed by the treatment with endostatin. Meanwhile,IL-1 produced by PM? was increased. Endostatin inh ibited IL-1 production from PM? and IL-1 and reduced TNF level of Synoviocyte s in AA rats. CONCLUSIONThe recombinant human endostatin has the rapeutical effect on AA rats, its mechanisms is related to its immunoregulative function.
