1.Research and progress of diffusion-weighted magnetic resonance imaging in the diagnosis of renal tumor

With the improvement of MRI equipment performance and the usage of multi-channel high sensitivity coil, the application of the diffusion-weighted imaging (DWI) in the abdomen has been achieved. And the DWI has great significance in the diagnosis and differential diagnosis of renal tumors. In this article we reviewed advantages and limitations of magnetic resonance diffusion weighted imaging technology diagnosis in renal cell carcinoma, and also reviewed the latest research progress of DWI technology in the use of kidney.
2.Effects of serum containing Yiqi Xiaoji Recipe on cell line NKN-28 of gastric cancer

OBJECTIVE: To explore the effects of serum containing Yiqi Xiaoji Recipe (YQXJR), a compound traditional Chinese herbal medicine for benefiting qi and removing stagnation, on cell line NKN-28 of gastric cancer. METHODS: The experimental SD rats were taken as the provider of the animal serum, and the serum was inactivated before the experiment. The serum was divided into high-dose, medium-dose, low-dose and blank serum groups based on whether the rats were given YQXJR and administration dosage. The inhibition rate was regarded as the observational index. RESULTS: The four groups of serum all had inhibitory effect on the growth of NKN-28 cells depending on the drug concentration. And there were significant differences among the experimental groups. High-dose, medium-dose and low-dose concentrations of serum all could inhibit the growth of NKN-28 cells with positive relations with the concentration and function time. CONCLUSION: YQXJR serum can inhibit the growth of NKN-28 cells depending on the drug concentration and function time.
3.Vitamin D requirements and supplementation in pregnant women

Large amounts of calcium are transferred from mother to fetus to meet the fetus's demand for calcium during its bone development.Pregnant women should increase the intake of calcium and vitamin D.However,the exact maternal vitamin D requirements and supplementation remain controversial.This article reviews the recent updates in this field with an attempt to provide evidences for nutritional decision-making.
4.Clinical observation on propofol combined with fentanyl in painless gastrointestinal endoscopy

Objective To explore the effect, safety and value of propofol combined with fentanyl in painless gastrointestinal endoscopy.Methods 1 200 patients were collected and divided into normal group and painless group,600 cases in each group.Normal gastrointestinal endoscopy was performed on unanesthetic patients,while painless gastrointestinal endoscopy was operated on anesthetic patients with Propofol and Fentanyl.The safety and clinical effect were compared between the two groups.Results 1 200 cases of patients were able to complete the gastrointestinal endoscopy examination successfully with no serious side effects.Compared with normal gastrointestinal endoscopy,the patients undergoing painless gastrointestinal endoscopy examination guaranteed more comfortable experiences,suffering less pain and side effects.The differences were significant between the two groups (all P < 0.05 ).Conclusion Propofol combined with fentanyl in painless gastrointestinal endoscopy was a safe,effective and painless process and was worthy of promoting widely.
5.Effect of penehyclidine hydrochloride combined with mechanical ventilation on inflammation in rats with acute respiratory distress syndrome induced by hydrochloric acid inhalation

Objective To observe the effect of penehyclidine hydrochloride (PHCD) combined with mechanical ventilation on inflammatory response in rats with acute respiratory distress syndrome(ARDS)induced by hydrochloric acid(HCl)inhalation. Methods 32 adult healthy male Sprague-Dawley(SD)rats were randomly divided into four groups:sham operation group,ARDS model group,PHCD group(intraperitoneal injection of 0.5 mg/kg PHCD at 30 minutes before HCl instillation)and mechanical ventilation group〔mechanical ventilation was given after PHCD treatment,ventilator settings were as follows:tidal volume(VT)=4 mL/kg,respiratory rate(RR)= 70 beats/min,the time ratio of inspiration to expiration(I:E)=1:2,fraction of inspired oxygen concentration(FiO2)=0.21〕,8 rats in each group. The rat ARDS model was created by HCl(0.1 mol/L,1.2 mL/kg) intratracheal instillation. After mechanical ventilation for 4 hours,the experiment was stopped. Arterial partial pressure of oxygen(PaO2)was measured,and the oxygenation index as well as lung wet/dry weight ratio(W/D)were calculated respectively. The pathological changes in lung tissues were observed under light microscope,and the pathological score was evaluated. The levels of myeloperoxidase(MPO),interlukin-8(IL-8)and nuclear factor-κB (NF-κB)in homogenate of lung tissues were all measured respectively by enzyme linked immunosorbent assay (ELISA). Results In rats of ARDS model group, there were significant infiltration of inflammatory cells, collapse of extensive pulmonary alveoli and hyaline membrane formation in lung tissues,but the edema in pulmonary mesenchymal tissue and congestion of micro vessels were not obvious. Compared with sham operation group,the lung tissue pathological score(9.88±2.03 vs. 1.38±0.92),lung W/D ratio(7.67±0.96 vs. 4.08±0.65)were significantly increased(both P<0.01),while oxygenation index〔mmHg(1 mmHg=0.133 kPa):213±29 vs. 428±28〕was markedly decreased in model group(P<0.01);and the concentrations of MPO(μg/L:34.18±3.99 vs. 20.92±1.40), IL-8(ng/L:864±53 vs. 583±91)and NF-κB(ng/L:1 229±133 vs. 803±130)in homogenate of lung tissues were significantly higher in model group(all P<0.01). The pathological changes in lung tissues including inflammatory cell infiltration,collapse of alveoli and pulmonary edema,etc. were obviously improved when treated by PHCD or mechanical ventilation and the lung pathological score and W/D ratio were significantly decreased compared with those of model group,oxygenation index in these two groups were both significantly increased,meanwhile, the concentrations of MPO,IL-8 and NF-κB were sharply decreased when compared with those of model group (all P<0.05). The degree of improvement in all above indexes were greater in mechanical ventilation group than those in PHCD group(lung injury score:5.63±1.85 vs. 7.63±1.67,lung W/D ratio:5.35±1.05 vs. 6.46±1.17, oxygenation index:325±57 vs. 272±44,MPO:25.80±3.36 vs. 29.88±3.93,IL-8:736±53 vs. 802±53, NF-κB:984±75 vs. 1 109±118,all P<0.05),however they did not reach the levels of the sham operation group (P<0.05 or P<0.01). Conclusion PHCD can inhibit the inflammatory response in ARDS rats induced by HCl inhalation,thus it may protect the lung tissue from injury induced by HCl,and the protective role of PHCD plus menchanical ventilation is superior to that of PHCD only.
6.The detection and clinical application of serologic biomarkers for liver fibrosis, the present and future perspectives

Hepatic fibrosis is the final common pathway for liver injury caused by various chronic liver diseases.Noninvasive assessments of liver fibrosis are valuable in clinical use since they are easy to apply and monitor disease progression.Serologic biomarkers for liver fibrosis consist of direct markers which reflect ECM turnover and indirect markers which show liver function of synthesis,metabolism and reservation.A number of assessment models can identify significant fibrosis and cirrhosis,and avoid unnecessary liver biopsy.They can be used for disease staging,prognosis prediction,and treatment surveillance.Variation of analytical performances exists among diverse analyzers and assays.The standardization of these tests will enhance their comparability.Large scale and multicenter clinical verification studies are needed for the clinical utility of these assessment models based on Chinese population of chronic hepatic diseases.
7.Research on Molecular Biological Characteristics of Proto-oncogene pim-2.

The purpose of this paper is to present the research on the molecular biological characteristics of proto-oncogene pim-2 and to analyze the related mechanism. Proto-oncogene pim-2 was studied and analyzed by the bioinformatics method and technology. With an online server, the chromosomal localization of pim-2 gene was analyzed, and the exon, open reading frame, CpG island and miRNAs complementary fragments and the like were predicted. With bioinformatics software, the physicochemical property of transcription protein of proto-oncogene pim-2 and various modification sites of protein sequence, such as ubiquitination and glycosylation, were predicted, the antigenic index was calculated, and the spatial structural was modeled. The research findings showed that the proto-oncogene pim-2 comprised six exons, the CDS (coding sequence) transcribed a section of peptide chain including 311 amino acids, a gene promoter has a CpG island, and the 3'UTR region contains an miRNA gene. The molecular weight of the Pim-2 protein was 34,188. 47, the isoelectric point was 5.78, the instability index was 45.87, and the extinction coefficient was 279nm. A plurality of covalent modification sites, two ubiquitination sites, four glycosylation sites, an SUMO sumoylation site, a nitrosation site, two palmitoylation sites and sixteen regions with higher antigenic index were distributed in the protein sequence. This research showed that the related regions and modification sites distributed on the sequence of proto-oncogene pim-2 were closely related to the carcinogenic effect thereof.

8.Anesthetic Management of Non-cardiac Surgery In Patients With Cardiac Disease

13) underwent non-cardiac surgery were divided into three groups depend on anesthesia choice. Epidural anesthesia (group E n=26), General anesthesia (group G n=19), Combinative anesthesia means epidural anesthesia plus general anesthesia(group C n=25).Three anesthesia manner were analysed to hemodynamic effects. Results Mean Glodman score: group C was 16?3.8, group G was 18?7.0 , group E was 17?3.3. Cases of using vasoactive drug in group E (69.23%) were more than group C (56%) and group G (47.36%.).Conclusions It is important for patients with severe cardiac disease to maintain organs oxygen supply during non-cardiac surgery. Steady hemodynamic might be gainned by using vasoactive drugs carefully.
9.Application of Oral Hypoglycemic Agents in Our Hospital During 2004~2006

OBJECTIVE:To probe into the status quo and the developmental trend of oral hypoglycemic agents used in our hospital.METHODS:Using the defined daily dose as the study unit to make statistic analysis of oral hypoglycemic agents used in our hospital during 2004~2006 in respect of DDDs,consumption sum and average daily cost etc.RESULTS:Both the categories and consumption quantity of oral hypoglycemic agents increased year by year while the proportion of consumption sum was descending.Over the 3 years,dominating the first 5 places on the list of DDDs were glipizide,metformin,gliquidone,gliclazide and glibenclamide,respectively.CONCLUSION:The clinical use of oral hypoglycemic agents was basically reasonable in our hospital.
10.Analysis of the Use of Narcotic Analgetics in Our Hospital During 2005~2007

OBJECTIVE: To evaluate the utilization and its trend of narcotic analgetics in this hospital.METHODS: The use of narcotic analgetics during the period 2005-2007 in our hospital was analyzed retrospectively.RESULTS: The consumption quantity of narcotic analgetics in our hospital increased year-on-year,much as in the consumption of morphine preparations,with oral morphine preparations dominating the 3 first places over the 3 years,however,the increase of pethidine was slowed down in consumption quantity.CONCLUSION: The use of narcotic analgetics in our hospital is rational on the whole,but measures should be taken to tighten the control on the use of narcotic analgetics.
