1.Study of Factors Affecting Mental Health of Military Officers and Soldiers in Tibet

Objective: To explore the possible factors that affect the mental health of military officers and soldiers in Tibet and provide the basis for the prevention of mental disorder and the practice of mental intervention. Methods: 90 Symptoms Check List (SCL-90),Life Events Scale(LES) and self-made questionnaire for general condition were used to investigate 299 army officers and soldiers who stationed in the frontier post of Tibet. The subjects were chosen in the principle of cluster sampling in squad or platoon. Results: The factors of environment satisfaction, negative live events, ways of entering Tibet, total score of life events, army years and age were positively related with SCL-90, while the hometown living condition had significantly negative relation with SCL-90. 2.Gamma knife radiosurgery for hypothalamic hamartomas

Objective To investigate the treatment effect of gamma knife radiosurgery for hypothalamic hamartomas. Methods Gamma knife radiosurgery was performed in 9 patients with gelastic seizures. Collimator helmets (4 and 8 mm) were used and a radiosurgery plan was designed to cover 90% of the hamartoma at the 40%~55% isodose curve with the center dose of 28~40 Gy and the margin dose of 12~18 Gy. The clinical outcome was evaluated with the Tan’s scale. Results No complications due to gamma knife radiosurgery occurred. A follow-up from 24 to 54 months showed effectiveness in 7 patients, disappearance of seizure in 1 patient, rare occurrence in 3 patients, significantly reduced occurrence in 2 patients, improvement in seizure control in 1 patient, but ineffectiveness in 2 patients. Conclusion As an approach to the treatment of hypothalamic hamartomas, gamma knife is a safe and cost-effective therapy with rare side effects for hypothalamic harmatomas.
3.The clinical choice in the method of interventional treatment to portal hypertension

Objective To explore the selection of reasonable interventional treatment to portal hypertension under different conditions.Methods The data of 76 patients with portal hypertension and interventional treatment from 1997 to 2002 were retrospectively analyzed. 26 patients were treated with transjugular intrahepatic porto-systemic stent shunt (TIPSS). Of these 26 patients, 11 patients suffered from hemorrhage after surgery treatment by devascularization, 6 patients with hemorrhage after stomach mirror treatment, and 9 patients with ineffective medicine treatment. 50 patients were treated with percutaneous transhepatic variceal embolization (PTVE) combined with partial splenic embolization (PSE), and they all showed inefficacy by medicine treatment. Free portal pressure (FPP) in pre- and post-treatment was measured in all 76 cases. 5.Biological markers of ischemic stroke subtypes according to TOAST classification

The identification of the specific cause in every patient has important clinical implications, because ischemic stroke is an etiologically heterogeneous disease. The Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification can be used to define the etiology of stroke. However, TOAST classification can not be completed timely on admission of patients with acute stroke, which has impacted early guidance of clinical treatment. This article reviews the biological markers of early differential-diagnostic significance of the TOAST classification.

Objective: To investigate the effects of Kangguzhe Formula(KGZF) on promoting union of experimental rat fracture.Methods: The experimental rat fracture models were reproduced by cannon born injure,continually treated with KGZF 21d by oral administration.The X-ray,pathological histology and anti-fracture intension of fracture union tissue were detected to investigate the effects of KGZF.Results: KGZF can significantly improve the osteotylus rebuild,trabeculation of bone and concentrated osteoblasts of fracture tissue;X-ray detection also showed that it can increase the grade of X-ray and anti-fracture intension of fracture union tissue;Compared with model group,it had significant difference.Conclusion: KGZF can obviously improve the union of experimental rat fracture.

Objective To report the modified surgical technique currently used for the construction of an orthotopic ileal neobladder.So as to improve the vesical function of urinary storage and urination after total cystectomy. Methods Radical cystectomy and ileal neobladder with double “U” shape were performed in 20 patients with transitional cell carcinoma. After the detubularized ileal segment approximately 40 cm in length was arranged in a U configuration,the two medial borders of the U-shaped open segment were oversewn with a single layer seromuscular continuous 2-0 polyglycolic acid suture.The bottom of the U was folded over between the two ends of the U.Thus a spherical pouch, the neobladder, was fashioned. During that period, the neobladder anastomosed to the urethra.The ureters were implanted according to the improved stitching method. Results The operative time to form a neobladder was (90?15)min.Follow-up turned out that no perioperative or early postoperative mortality was observed, and no severe early complications occurred. The mean postoperative follow-up time was 25 (range,6 to 48) months.All the 20 cases achieved excellent continence except for 6 cases with temporary incontinence at night.Dysuria occurred in 1 case,and hydronephrosis on the left was found in 1 case.Six months later, the mean bladder capacity was 340 ml (range,250 to 450 ml). Conclusions The construction of an ileal neobladder using a smaller part of ileum that has been completely detubularized and fashioned by double U shape is easy and agreeable to perform.This mode of operation has low complication rates, achieves adequate capacity at low pressure, and provides satisfactory continence rates.
9.Research advances in uterine papillary serous carcinoma

Uterine papillary serous carcinoma is a special pathological type of endometrial cancer. It is similar to ovarian papillary serous carcinoma in histology and can have distant metastasis when it is still confined to the endometrium. As it is very aggressive and has a rather poor prognosis,complete surgical staging is necessary in the treatment,assisted by radiotherapy and chemotherapy. In this review,the author discussed the recent research advances in uterine papillary serous carcinoma.
10.Effect of percutaneous kyphoplasty in treatment of acute and chronic osteoporotic vertebral compression fractures

[Objective]To analyze the therapeutic effects of percutaneous kyphoplasty for acute versus chronic osteoporotic vertebral compressive fracture.[Method]Percutaneous kyphoplasty was performed in 56 patients(137 vertebraes) with osteoporotic vertebral compression fracture (VCF).Based on the changes on MRI,all compression fractures were divided into 3 types:acute VCF,repairing VCF and chronic VCF.Types were examined for differences in treatment efficacy,according to the anterior/middle vertebral body height,Cobb's angle and visual analogue scale(VAS) at pre-and post-operation.[Result]All patients with the VCFs had rapid and significant improvement in back pain following percutaneous kyphoplasty.VAS was from 8.09?1.12 preoperatively to 2.31?0.91 postoperatively,and 2.26?0.88 at final follow-up.The results showed that 41 cases were Type Ⅰ on MRI,69 were Type Ⅱ and 27 were Type Ⅲ.Significant improvement was observed at the post-operative versus pre-operative assessments in Type Ⅰ and Type Ⅱ.No significant differences were observed between post-and pre-operative assessments in Type Ⅲ.[Conclusion]The curative effect of percutaneous kyphoplasty for the treatment of acute VCFs appears superior to that of chronic VCFs.After a definite diagnosis of VCFs,percutaneous kyphoplasty should be performed as early as possible in order to obtain a better therapeutic effect.
