1.Evaluation of diagnostic ability of CT and diffusion-weighted imaging in extremity soft tissue tumors

Objective To compare the diagnostic value of CT and diffusion-weighted imaging in extremity soft tissue tumors. Methods A total of 104 cases of extremity soft tissue tumors were examined with CT scanning and MRI. All cases were histologically proven. Then we compared the CT value of various types of tumors. The b values of diffusion were 0 and 500 s/mm2. The apparent diffusion coefficient (ADC) values of a large region with no hemorrhage, necrosis, scar tissue, or calcification representing the lesion were measured. ADC values of benign tumors, malignant tumors and normal muscles were compared. Results There were 68 cases of benign tumors and 36 cases of malignant tumors. The CT findings of 45 cases and the MRI findings of 87cases were in accordance with pathological examination. The diagnosis of 59 cases by CT and 17 cases by MRI were wrong. The CT features of soft tissue tumors showed the low density masses. The features of lipoma or cyst were typical on CT. There were large differences among the different types of tumors performance on T1WI and T2WI. The ADC values of the malignant tumors were significantly lower than those of benign lesion sand muscles (P < 0.01). There was no significant difference in ADC values between benign lesions and muscles. there was significant difference between the detection level of CT and MRI (P < 0.01). Conclusion CT can clearly show soft tissue tumor lesions and to clarify their relationship and the surrounding tissue, but can not accurately characterize. MRI diffusion-weighted imaging can better differentiate benign and malignant, and speculate the histological lesions sources. MRI detection level is significantly higher than CT and more consistent with a higher degree of pathology. Thus in the preoperative diagnosis of soft tissue tumors, diffusion-weighted imaging MRI should be preferred.
2.Posterior-anterior approach surgical treatment for severe cervical spondylotic myelopathy

[Objective]To explore the therapeutic effect of the posterior-anterior approach surgical treatment for severe cervical spondylotic myelopathy.[Method]Totally 32 patients with severe cervical spondylotic myelopathy operated by posterior-anterior approach surgical treatment from September in 1998 to January in 2005 were retrospectively analyzed.In the 32 patients,20 males and 12 females.Their age averaged 53.8 years(ranged,41~78 years),invasional segments:C3、42 cases,C4、5 9 cases,C5、611 cases,C6、7 10 cases.Four cases with lesions involving one intervenebral space,the other 12 and 16 cases involving two and three intervertebral spaces respectively.All cases had the posterior cervical open-door laminoplasty decompression in first,and had the anterior cervical decompression and bone grafting or fusion with titanium Cage internal fixation after a week.To follow up all the postoperative cases,neural function was recorded.According to JOA score,preoperative score and postoperative follow-up score were compared,improvement rates were caleulated respectively.[Result]All cases were regularly followed up.The duration of follow-up averaged 13.6 months(ranged,12~18 months).The JOA of before operation was(6.51?1.10),the JOA of first period of time after operation was(9.47?0.5),and follow-up JOA was(12.42?0.79).T-test was used to compare pre-operative and follow-up JOA scores.The statistical results were significantly different(P
3.Expression of MMP-11 in serum of gastric cancer and its significance

Background and purpose:Matrix metalloproteinase11(MMP-11)is an interesting member of the MMP family,and its overexpression has been demonstrated in human cancers such as non-small cell lung cancer and oesophageal adenocarcinoma.Furthermore,strong MMP11 gene expression is correlated with both increased aggressiveness of tumors and a poor clinical outcome,but there was no related reports on gastric cancer.We focussed our study on the significance of MMP-11 expression in gastric cancer.Methods:MMP-11 was selected and confirmed at both mRNA and protein level in 130 cases of gastric cancer serum specimen by RT-PCR,enzyme-linked-immuno-sorbent-assay(ELISA)techniques,respectively.Results:The total positive rates of serum MMP-11 mRNA in gastric cancer and healthy people were 68.4% and 5%,respectively.Our data demonstrated that MMP-11 protein levels in gastric cancer were(44?27)ng/ml,significantly higher than those in control subjects(P=0.001).MMP-11 protein levels in 30 cases with recurrence were(51?27)ng/ml and(27?28)ng/ml in 30 patients without recurrence.MMP-11mRNA positive expression was consistent with MMP-11 protein levels in gastric cancer patients.Conclusions:Serum MMP-11 may play an important role as a biomarker in diagnosis and prediction of recurrent gastric cancer.

Objective:To clone the rat cardiac myosin ? heavy chain cDNA fragment encoding aa736 960 and construct its recombinant retrovirus vector Methods:The 681 bp target gene was amplified from heart tissue of young rats with RT PCR Fusion gene of hIL 2/myosin was constructed by splicing with preserved region of hIL 2 cDNA using ligation methods and subsequently the plasmid pLNC hIL 2 myosin was constructed NIH3T3 cells and PA317 cells were transfected with plasmid pLNC hIL 2 myosin using Lipofectamine After screening with medium containing G418, the positive clone was chosen and was detected using RT PCR, immunohistochemistry, immune electron microscope and dot blot Results:The determination of nucleotide sequence showed that the nucleotide and amino acid sequence of the gene cloned was the same as the reported sequence, and its open reading frame was correct RT PCR analysis indicated that mRNA of the fused gene was present in the positive clone Immunohistochemistry, immune electron microscope and dot blot showed that the fused gene IL 2 myosin was successfully expressed Conclusion:The fused gene of rat cardiac ? heavy chain fragment and the preserved region of human IL 2 was constructed and expressed successfully
5.Clinical observation of effect of modified vaginal paravaginal repair on pelvic floor reconstruction in 52 cases

Objective To explore the efficacy and safety of modified vaginal paravaginal repair (VPVR) in treatment of symptomatic paravaginal defect cystocele(PDCC). Methods This study was an observational case series of 52 consecutive women in Beijing hospital from January 2007 to August 2009, with symptomatic paravaginal defect cystocele. Among them, 6 patients had anterior wall colporrhaphy. Preoperative and postoperative pelvic evaluations were performed with the POP-Q system. All the cases underwent vaginal approach to paravaginal repair, 49 cases underwent vaginal hysterectomy simultaneously. Patients were followed up for 1 to 24 months. Criteria for objective cure were defined as the anterior vaginal walls at grade 0 and firmly apposing to the lateral pelvic sidewalls.Results Of the 52 patients, 30 had grade Ⅱ , 17 had grade Ⅲ, and 5 had grade Ⅳ paravaginal defect cystocele. In all patients the VPVR was performed successfully. No complication happened during and after operation. The average operation time was (65 ±18) min (45-110 min) and estimated blood loss was (95 ± 27) ml (50-200 ml). Patients were followed up postoperatively for 3 to 24months, with a mean of (10. 1±3.1) months. The PDCC recurred in 6 cases, 5 patients had grade Ⅰcystocele and 1 had grade Ⅲ cystocele. Conclusions The vaginal approach to the correction of paravaginal defect cystocele is highly effective in our population.
6.Healing effect of limited fluid resuscitation treatments on hemorrhagic traumatic shock

Objective To evaluate the effect of limited fluid resuscitation treatments on the hemorrhagic traumatic shock in order to improve the cure rate. Methods Forty cases of the hemorrhagic traumatic shock patient were randomly divided into the regular fluid resuscitation group (n=21, regular group) and the limited fluid resuscitation group (n=19, limited group). In regular group, fast infusion of the full quantity fluid underwent in the early stage before hemostasia, keeping systolic blood pressure ≥ 90 mm Hg 1mm Hg=0.133 kPa. In limited group, infusion of fluid was limited before hemostasia, keeping average MAP at 6.65 kPa, then blood transfuse and fluid infusion underwent after hemostasia. After hemostasia, two groups kept MAP at 9.31~10.64 kPa. Results Three cases died in regular group, and the cure rate was 85.7%. No death in limit group, and the cure rate was 100%. AIM:To investigate whether nimesulide a selective cyclooxygenase 2 (COX-2) inhibitor and piroxicam (an inhibitor of COX-1) protect the rat hearts against oxidative stress induced by hydrogen peroxide, superoxide anion or hydroxyl free radical. METHODS: Cardiac contractility, lactate dehydrogenase (LDH) and malondialdehyde (MDA) were analyzed by the Langendorff method in isolated rat hearts. Production of 6-Keto-PGF1?, a marker of COX activity, was measured in isolated rat hearts. RESULTS: Rat hearts were exposed to hydrogen peroxide (H2O2), pyrogallol (which produced superoxide anion) or Vit C+Fe2+ (which produced hydroxyl free radical) for 10 min followed by reperfusion for 30 min. 8.Clinical observation of fluorouracil implanting to abdominal cavity during operation of advanced colonic cancer patients

Objective To explore the safety of fluoreuracil implanting to abdominal cavity during operation of advanced colonic cancer patients, search for new approach for preventing from local relapse of colonic cancer. Methods Divide 60 patients accepting radical colonic cancer surgery into 2 groups, treating group and control group. Treating group are implanted of fluorouracil along the wounded surface of tumor and draining path of lymph nodes. To observe the curing of wounded surface, stomal leak ,wound infection, intestinal obstruction and hospital day. Results There is no significant differences between treating group and control group in stomal leak,wound infection, intestinal obstruction and hospital day (P>0.05). Conclusion It is safe to implant fluoruuracil during radical colonic surgery, which is an effective path of regional chemotherapy after operation.
9.Peritoneal tuberculosis mimicking advanced ovarian carcinoma in elderly women:three case reports and literature review

Objective To document peritoneal tuberculosis mimicking ovarian malignancy in elderly post-menopausal women and to review pertinent literature.Methods The records of 3 women with peritoneal tuberculosis who were managed at Beijing Hospital from January 2003 to September 2009 were reviewed.Results Three patients with peritoneal tuberculosis mimicking ovarian malignancy all presented with the classical symptoms of advanced-stage ovarian carcinoma,including ascites,abdominopelvic masses,elevated serum CA125,bloating and progressive emaciation.Two patients received laparotomy revealing peritoneal tuberculosis but no malignancy.All the patients were treated with anti-tuberculosis chemotherapy.Conclusions Medical awareness of peritoneal tuberculosis is still lacking and many women with this disease are initially thought to have ovarian malignancy and undergo unnecessary extended surgery.Laparoscopy including biopsies seems to be a sufficient and safe method to provide diagnosis of peritoneal tuberculosis.If laparoscopy is not feasible,laparotomy should be performed.Ascites and high level of CA125 do not necessarily indicate that the clinical picture is malignant in post-menopausal women.
10.Impact of uric acid on peripheral blood glucose tested by portable blood glucose meter

Objective To investigate the impact of uric acid on peripheral blood glucose measured by the portable blood glu‐cose meter .Methods A total of 60 inpatients with 2 diabetic mellitus(T2DM ) complicating hyperuricemia were chosen as the re‐search group and contemporaneous 60 cases of T2DM without complicating hyperuricemia as the control group .Blood glucose in both groups was measured by the portable blood glucose meter ,meanwhile plasma glucose and serum uric acid were assessed by the biochemical analyzer in order to explore the impact of uric acid on peripheral blood glucose .Results (1)The difference between pe‐ripheral blood glucose and venous blood glucose was correlated with serum uric acid .(2)There was statistically significant differ‐ence in the blood glucose between the two groups .(3 ) The bias of blood glucose value were within the acceptable range . Conclusion Serum uric acid has certain influence on the peripheral blood glucose ,but which is within the acceptable range .The de‐tected blood glucose value of the portable blood glucose meter can be trusted .
