2.Retrospective analysis of 58 patients with Takayasu' s arteritis

Objective To evaluate the clinical features and curative effect of patients with Takayasu's arteritis.Methods The clinical data of fifty-eight patients,which were divided into different groups according to gender,clinical classifications,disease activity,and so on,were retrospectively analyzed,to compare whether the results of clinical features,laboratory tests or prognosis had significant differences.Results The data suggested the fact that stroke as first manifestation(2/9 vs 0/49,P =0.022) and incidence of hypertension (9/9 vs 28/49,P =0.037),as well as critical hypertension(8/9 vs 18/49,P =0.011) were more common in male patients than in female patients.The active Takayasu's arteritis patients showed that the level of erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),platelet and fibrinogen was elevated,while the level of Albumin/Globulin ratio was decreased.Electrophoresis showed that the elevation of globulin mainly based on globulin α1 and globulin α2 (P < 0.05).The reduction of complement C3 is more common in inactive group than in active group.Conclusion Stroke and hypertension are more common in male Takayasu's arteritis patients than in female patients.The active Takayasu' s arteritis patients showed elevated level of ESR,CRP,platelet and fibrinogen,while the level of Albumin/Globulin ratio was reduced.
3.Digital renovation of S2001 field X-ray vehicle

S2001 field X-ray vehicle is used to perform X-ray examination in the field hospital, while the disadvantages of the traditional film-screen radiography restrict its role of field examination in this digital era. By analyzing the detail of modern digital X-ray radiography, and comparing three different digital X-ray flat detectors, a solution is put forward to update S2001 field X-ray vehicle to a field digital X- ray radiography vehicle by using Canon CXDI digital X- ray detector.
4.Application of picture archiving and communication system based case-based learning mode in orthopedic clinical teaching

With the rapid development of China's higher education and the intensified reforms in medical and health services,the problems and contradictions are increasingly prominent in ortho-pedic clinical teaching,especially how to improve medical interns' ability to read images. Picture archiving and communication system (PACS)plays an important role in academic education of medi-cal imaging. Case-based learning (CBL)is a team seminar teaching mode that is guided by cases, based on problems,targeted at students,and dominated by teachers. PACS and CBL were perfectly combined in orthopedic clinical teaching to improve teaching efficiency and the outcome was evaluat-ed through a questionnaire. Results showed that this teaching method inspired students' interests in learning,cultivated the orthopedic clinical way of thinking thus achieved better teaching effects.

6.Reclassification of obesity and diagnosis of obesity based on individuals′ metabolic status

Obesity has been widely accepted as a metabolic disease and its occurrence is closely related to the genetics, environment, and inflammation. Up to now, body mass index is still regarded as the standard diagnostic criterion for obesity. It has not been revised for decades and could not meet the needs of clinical diagnosis and demands for specific treatment at present. In 2013, the concept of metabolic obesity was introduced at the American Diabetes Association ( ADA) annual conference, and it was further proposed that obesity should be classified according to the metabolic status and its related complications at the 2014 American Association of Clinical Endocrinologists ( AACE) annual conference. This means that scientists and clinicians have realized that the etiology of obesity may vary with different outcomes, the treatment should be focused on the metabolic regulation, not merely on weight loss. With years of clinical practice and research in obesity, we have observed and treated numerous obese patients, and we have found that obesity has a lot of phenotypes and clinical features which are related to the metabolic status. Based on our clinical findings, combined with the experience of Chinese traditional medicine, we now propose a new clinical classification and diagnosis of obesity based on individuals′ metabolic status, which, we believe, can facilitate clinicians′practice. Based on the metabolic status and skin features of obese patients, obesity is divided into metabolic healthy obesity (‘white obesity’) and metabolic unhealthy obesity. Then, the latter is further divided into three groups including high metabolic obesity (‘red obesity’ ) , low metabolic obesity (‘yellow obesity’ ) , and severe metabolic disorder with inflammation obesity (‘black obesity’ ) . If we also consider to add normal weight metabolic obesity to this classification, there should be five types of obesity to be classified as presented. We wish this proposed classification of obesity can play a valuable role in enabling clinicians to have a better understanding of obesity in relation to its metabolism, and to develop individualized treatment according to the metabolic status of the patient. As a result, we may finally achieve the desired outcomes through making appropriate diagnosis and treatments.
7.The research progress of protein lysine succinylation

Recently the lysine succinylation was discovered in vivo, and it was demonstrated to be widely involved in cell dif-ferentiation, cell metabolism and other important physiological activities.Lysine succinylation has become the forefront of life science research.Scientists have provided a lot of evidences that proteins in prokaryotes and eukaryotes are widely succinylated, without which the central metabolism and intermediary metabolism of many metabolic enzymes are disrupted.To better understand the importance of Succinylation in vivo, protein-succinyl modification of the current research and the latest developments are reviewed, which summarize succinylated protein lysine sites in the center of important physiological metabolic pathways, diseases and other pathological conditions.
8.Effects of Shengmai Injection on Myocardial Function of Patients with General Anesthesia of Hysterectomy

OBJECTIVE:To investigate the effect and safety of Shengmai injection on myocardial function of patients with gen-eral anesthesia of hysterectomy. METHODS:40 patients underwent hysterectomy were randomly divided into observation group and control group with 20 cases in each group. Both groups received general anesthesia induction of fentanyl citrate and rocuroni-um,and inhalation anesthesia of propofol. Observation group was given Shengmai injection 1 ml/kg,ivgtt;control group was given same dose of 0.9%Sodium chloride injection,ivgtt,before anesthesia. Mean arterial pressure(MAP),heart rate(HR),central ve-nous pressure(CVP),blood oxygen saturation(SpO2),the levels of MDA,SOD and NOS were compared between 2 groups be-fore operation(T0),before anesthesia induction(T1),reaction during(T2),after operation(T3). The occurrence of ADR was also observed. RESULTS:There was no statistical significance in MAP and HR between 2 groups at T0 and T1 (P>0.05). MAP and HR of observation group was significantly higher than those of control group at T2 and T3,with statistical significance(P<0.05). There was no statistical significance in CVP and SpO2 between 2 groups at different time points(P>0.05). There was no statistical significance in MDA,SOD and NOS levels between 2 groups at T0(P>0.05). MDA and NOS levels of 2 groups increased signifi-cantly at T1,T2 and T3,while SOD level decreased significantly;MDA and NOS level of observation group were lower than the control group while SOD level was higher than control group at T1,T2,with statistical significance (P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Shengmai injection can maintain stable HR and MAP,and reduce the oxide in patients underwent hysterectomy surgery so as to protect myocardial function with good safety.
9.Risk factors of delayed bleeding after endoscopic submucosal dissection treatment for duodenal mass lesions

Data of 55 patients with duodenal mass lesions treated by endoscopic submucosal dissection (ESD) were retrospectively analyzed. Risk factors of delayed bleeding after ESD were explored by univariate analysis and multivariate unconditional logistic regression analysis. Duodenal delayed bleeding occurred in 5 patients (9.09%). No closure treatment under endoscopy( P=0.035) was significantly different between the delayed bleeding group and the non-delayed bleeding group. Multivariate non-conditional logistic regression showed no closure treatment under endoscopy was an independent risk factor for delayed bleeding after ESD in duodenal mass lesions ( P=0.029, OR=0.079, 95% CI: 0.008-0.776). Patients older than 60 years and who did not take endoscopic closure treatment have higher incidences of delayed bleeding after ESD. No closure treatment under endoscopy is directly related to postoperative delayed bleeding.
10.New perspectives on medical treatment of gastric cancer.

Perioperative chemotherapy for gastric cancer has made steady progress with the application of chemotherapeutic agents and conduct of relevant clinical trials. However, palliative chemotherapy has only made marginal progress, and the median survival remains between 8-10 months. An emerging understanding of tumor biology, cellular and molecular mechanisms has revealed novel targets in gastric cancer therapy, such as trastuzumab, bevacizumab, cetuximab, etc. There are still some problems in clinical practice due to the high heterogeneity of gastric cancer including optimization of perioperative chemotherapy regimen, determination of right strategy to personalize chemotherapy or target therapy for advanced gastric cancer. In order to practically achieve individualized therapy, multicenter, prospective, randomized clinical trials according specific matters of gastric cancer should be performed in China. A deeper understanding of the biological characteristics of gastric cancer, and more translational medicine research and multi-disciplinary team collaboration should be carried out to achieve personalized medicine.
