1.Efficacy of different dose of Botulinum toxin A in treatment of patients with neurogenic detrusor overactivity

Objective To research the efficacy of different dose botulinum toxin A in the treatment of patients with neurogenic detrusor overactivity due to spinal cord injury or multiple sclerosis.Methods The datas of 43 patients with neurogenic detrusor overactivity caused by spinal cord injury or multiple sclerosis which accepted treatment in our hostipal were analyzed.And 38 patients were followed up for 12 weeks,of which 20 cases were treated with 200 U botulinum toxin A,and 18 cases received 100 U botulinum toxin A.The average age of 38 patients was 45.3 years old,and the ratio of famale was higher.The incontinence quality of life(I-QOL)changed from baselin after 6 weeks and 12 weeks was recorded.Resluts The final outcomes showed that the efficacy of botulinum toxin A with 200U was better than that with 100 U according to I-QOL score,and the difference was statistical significance(P <0.05).Conclusion The botulinum toxin A has positive effect on neurogenic detrusor overactivity,and the efficacy of 200 U injection is better than that of 100 U.
2.The Research Progress on Anti-inflammatory Effects and the Therapy to Acute Lung Injury of Emodin

[Objective] To explore the anti-inflammatory effects of emodin and its therapeutic effect on acute lung injury. [Methods] By means of analyzing the relevant literatures in PUBMED,CNKI,CBM,Wanfang and Vip Da-tabase from 1998 to 2012,summarized the anti-inflammatory effects and mecha-nisms of emodin, as wel as its therapeutic effect on acute lung injury. [Results] A variety of studies have confirmed the anti-inflammatory effects of emodin, whose mechanism is related to emodin inhibiting the activation of NF-kappa B,modulaing various inflammatory factors, etc. In addition, the effect of emodin in the treatment of acute lung injury has been confirmed on celland animal levels. [Conclusion] Emodin has good prospect of clinical application.
3.Oct-4 expression in epidermal tumors

Objective The aim of this study was to understand the role of specific markers of stem cells Oct-4 expression in the development of human epidermal non-melanoma cutaneous tumors.Methods The paraffin-embedded samples were retrieved from files of pathology department at our hospital,including 20 cases of skin squamous cell carcinomas (SCC),20 cases of basal cell carcinomas (BCC),20 cases of seborrhoeic keratosis (SK) and 20 cases of normal skin (from head,face,trunk,extremities).The expression of Oct-4 and PCNA were observed by immunohistochemical staining technique.Results Oct-4 protein was abnormally increased in SCC and BCC comparel to normal skin and SK ( P <0.05 ).However there were no significant difference of Oct-4 protein expression between SCC and BCC ( P >0.05).There were also no significantly different Oct-4 protein expression between Sk and the normal skin ( P > 0.05 ),and no significantly different Oct-4 protein expression between SK and BCC( P >0.05 ).PCNA protein was abnormally increased in SCC and BCC compared to normal skin and SK ( P <0.01 ).There were significantly different PCNA protein expression between SCC and BCC( P <0.05).There were also significantly different PCNA protein expression between SK and the normal skin ( P < 0.05 ).However there were no significant difference of PCNA protein expression between SK and BCC ( P > 0.05 ).There were positive correlation between the expression intensity of Oct-4 and PCNA in SCC and BCC.Conclusions The abnormal expression of Oct-4 may have an important role in the development of BCC and SCC.Positive Oct-4 expression cells may be the tumor stem cell in SCC and BCC.There were positive correlation between the expression intensity of Oct-4 and PCNA in SCC and BCC.The over expression Oct-4 in BCC and SCC may play an important role in proliferation of tumor.
4.Digital renovation of S2001 field X-ray vehicle

S2001 field X-ray vehicle is used to perform X-ray examination in the field hospital, while the disadvantages of the traditional film-screen radiography restrict its role of field examination in this digital era. By analyzing the detail of modern digital X-ray radiography, and comparing three different digital X-ray flat detectors, a solution is put forward to update S2001 field X-ray vehicle to a field digital X- ray radiography vehicle by using Canon CXDI digital X- ray detector.
5.Diagnosis of acute anuria caused by upper urinary tract obstruction

Objective To study the diagnostic techniques of acute anuria caused by upper urinary tract obstruction. Methods Fifty-eight patients with acute anuria caused by upper urinary tract obstruction were analyzed on clinical manifestations,etiology of obstruction and imageologic findings. Results The main clinical manifestations consisted of anuria in 58 cases,pain in renal region in 41,percussion tenderness over kidney region in 33,hypertension in 20,high levels of BUN and creatinine in 57 and hyperkaliemia in 8.The main adjuvant examinations included MRU,B-ultrasound,X-ray (KUB,IVU and retrograde pyelography),CT and MRI.The detection rates of upper urinary tract obstruction by above-mentioned examinations were as follows:100% (25/25) with MRU,88% (50/57) with B-ultrasound,60% (6/10) with CT/MRI,49% (19/39) with X-ray. 7.THE EFFECTS OF PROPAGANDA AND EDUCATION ON ACCEPTANCE OF BALANCED DIET IN THE ELDERLY

To carry out propaganda and education regarding nutrition in the elderly is to improve their knowledge about balanced diets, appropriate nutrition in relation to health, to rectify their original irrational diet hapbits, and to select scientifically their nutritious diets. 797 retired military cadres were enrolled for the study of the effects of education. The results showed that proper education significantly changed their irrational diet habits, sach as over nutrition and under nutrition, and improper constitution of fatty acid in the food, and obesity and over-weight were corrected after the propaganda and education regarding nutrition were carried out. This study suggested that it was very important to edacate elderly people in regard to proper diet habit to help them give up their bad habits on selecting foods, and to avoid obecity and constipation.
9.Preparation and characterization of magnetic poly D, L- lactide -co-glycolic acid phenylarsine oxide nanoparticles

BACKGROUND: With the development of nanotechnology, a new system for the delivery of drugs by magnetic nanovectors has been proposed. Within a magnetic field, the system can implement site-specific drug administration, thereby raising drug concentration at the lesion focus, elevate therapeutic effects, and reduce side effects.OBJECTIVE: To study the preparation of magnetic poly D, L-lactide-co-glycolic acid phenylarsine oxide nanoparticles (M-PLGA-PAO-NPs) and to evaluate characteristics of the prepared nanoparticles.DESIGN: Several factors influencing nanoparticle characteristics were selected for single-factor tests. Then, according to experimental results, and in conjunction with orthogonally designed statistics, the optimized prescription was obtained. SETTING: Department of Special Diagnosis, Changhai Hospital, Second Military Medical University of Chinese PLA.MATERIALS: The study was performed at the Department of Pharmaceutics, School of Pharmacy, Second Military Medical University of Chinese PLA from January 2005 to March 2006. The reagents used were as follows: phenylarsine oxide (Sigma, USA), poly D, L-lactic-co-glycolic acid (Shandong Medical Apparatus Institute, China), ferroso-ferric oxide (nanometer, Sigma, USA), polyvinyl alcohol (PVA1788, Beijing Organic Chemical Industry Plant, China). Methylene dichloride and other agents were all analytical grade and purchased from Shanghai Sinopharm Chemical Reagent Co., Ltd, China.METHODS: M-PLGA-PAO-NPs were prepared through an emulsion-evaporation process. Nanoparticle shape was observed by transmission electron microscopy. Magnetism was determined by a vibrating sample magnetometer. The size and diametral distribution of nanoparticles were determined by a laser particle size analyzer. The encapsulation ratio and drug loading of phenylarsine were measured by high performance liquid chromatography (HPLC). The percentage of phenylarsine oxide release in vitro was calculated [the percentage of phenylarsine oxide release in vitro =(total dose of phenylarsine oxide-residual dose of phenylarsine oxide)/ total dose of phenylarsine oxide].MAIN OUTCOME MEASURES: The shape, size, drug loading, encapsulation ratio and release in vitro of M-PLGA-PAO-NPs.RESULTS: The prepared nanoparticles had an average encapsulation ratio of 34.2%. Drug loading of 5 batches of nanoparticles was 3.06%, 3.15%, 3.18%, 3.21%, and 3.41%, respectively, with an average drug loading of 3.20%. Drug loading difference was small between batches, indicating good stability and reproducibility of the technology. M-PLGA-PAO-NPs were spherical, smooth, evenly distributed and non-adhesive. Ferrosoferric oxide microparticles, which exhibited unevenly dispersed black opacities, were found in the magnetic microparticles. Nanoparticles were in a narrow size range, with an average diameter of 290 nm (range 140-500 nm). When the magnitude and the direction of the outside magnetic field were changed, nanoparticles showed different intensities of magnetization. This indicated that M-PLGA-PAO-NPs had a certain magnetic response. The in vitro nanoparticle-release curve indicated that drug release was initially fast followed by a slow controlled release, and on day 8, it was basically stable.CONCLUSION:The experiment acquires a satisfactory technique for preparation of M-PLGA-PAO-NPs. The prepared M-PLGA-PAO-NPs were well targeted and exhibited slowly controlled drug release effects.
10.Preoperative predictors for nature of duodenal papillary tumors

Objective:To establish a preoperative forecasting model for the nature of duodenal papillary tumors and to discuss its main predictors.Methods:A case-control study was conducted;the case group included patients with malignant duodenal papillary tumors and the control group included patients with benign duodenal papillary tumors.All the patients were from Changhai Hospital. Data of patients,including the demographic characteristics,clinical symptoms during onset,laboratory findings,and radiological data, were collected by face-to-face interviews or by reviewing the medical history.Chi-square,t-test or ANOVA were employed to performed univariate analysis.All factors with P values less than or equal to 0.25 in the univariate analysis were used as independent variables for multivariate analysis,and a Logistic regression forecasting model for the nature of duodenal papillary tumors was established.Results: Totally 199 patients with pathologically-confirmed duodenal papillary tumors were included in the present study,with 166 in the case group and 33 in the control group.Multivariate analysis showed that hemoglobin(Hb),total bilirubin(Tbil),direct bilirubin(Dbil), aspartate transferase(AST),alkaline phosphatase(AKP),gamma glutamyl transpeptidase(GGT),and carcinoembryonic antigen(CEA) were independent predictors for nature of duodenal papillary tumors,with their odds ratios(95% confidence interval)being 0.981 (0.959,1.003),0.867(0.794,0.948),1.207(1.075,1.355),1.028(1.008,1.048),0.996(0.992,1.000),1.002(1.000,1.004),and 0.974(0.953,0.994),respectively.Conclusion:The Logistic regression model,which takes into consideration of Hb,Tbil,Dbil,AST, AKP,GGT,and CEA,can be used to predict the nature of duodenal papillary tumors,and its clinical value need to be further studied.
