1.Application of Liechtenstein hernioplasty in the therapy of oblique or direct hernia

Objective To inquire into the value of Liechtenstein Hernioplasty in the therapy of oblique or direct hernia.Method Liechtenstein was used in the therapy of 163 cases of oblique hernia (154 cases) and direct hernia (9 cases) from Feb.1998 to Jun.2001 and the clinical data was retrospectively studied.Most of them are of onset hernia except 8 cases of recurrent hernia.In 148 cases,the diameter of hernia ring is less than 2.5 cm and 30.1% of patients are elder than 65.The emergent operation was adopted in 12 cases.Results Off-bed exercises can be admitted at early stage of postoperation with slight pain and no local tension.Total recovery was obtained in 5-10 days after operation.After 6-36 months' follow-up,no mortality and less recurrence was detected.Conclusions There are so many advantages to treat oblique or direct hernia with Liechtenstein, such as minimized trauma, accelerated recovery,low recurrence rate and low cost. The indication of this operation involved all kinds of hernia (onset or recurrence of oblique or direct hernia,emergent operation of incarcerated oblique hernia, etc).
2.Comparison of the effect of nifedipine controlled-release tablet and nifedipine sustained-release tablet in the treatment of hypertension

Objective To investigate the clinical efficacy of nifedipine sustained-release formulation(SR)and nifedipine controlled-release(GITS)in the treatment of essential hypertension.Methods 160 patients with essential hypertension were randomly divided into two groups.The SR group was treated with SR 20 mg twice a day;GITS group was treated with GITS 30 mg once a day.All patients were given ambulatory blood pressure monitoring(ABPM)and observed the 24-hour average blood pressure,heart rate,changes in the trough/peak(T/P)values,and adverse reactions before and after treatment in two groups.Results The T/P value of systolic blood pressure(SBP)and diastolic(DBP)in the GITS group was 68.5%,74.6%,respectively,which were higher than those of SR group(57.2%,61.1%)(x2=6.32,7.05,all P<0.05);GITS group was significantly lower than the SR group in heart rate(t=4.17,P<0.05);Adverse reactions between two groups had no significant difference(x2=0.16,P>0.05).Conclusion SR and GITS can effectively lower blood pressure,but GITS is more efficient,stable,safe,small impact on the heart rate.
3.The CT Features and the Prognosis of Hypoxic Ischemic Encephalopathy (HIE) with Intracranial Hemorrhages in Neonates

Objective To discuss the CT features and the prognosis of hypoxic ischemic encephalopathy (HIE) with intracranial hemorrhages in neonates.Methods The CT features of the HIE with intracranial hemorrhage were analyzed and follow-up studied in 64 cases.Results Of all the 64 cases with HIE, 53 cases were associated with subarachnoid hemorrhage (SAH), and all cases were normal in the follow-up study after 1 month. 1 case was associated with intraventricle hemorrhage (IVH), 10 cases were associated with mixed bleeding (SAH+IVH in 3 cases, IVH+IPH in 1 case, SAH+IPH in 2 cases, SAH+SHE in 2 cases, SAH+SDH in 2 cases). Of the follow-up studies on 10 mixed bleeding cases, 1 was normal, 1 was dead, and the others were cerebromalacia, cerebral atrophy, porencephaly and calcium.Conclusion Different intracranial hemorrhages results in different prognosis.
4.Advances of nocoding RNA in the mechanism of epilepsy

This review focused on the recent research progress in the relationship between microRNA, long-nocoding RNA and circular RNA with epilepsy.
5.Association of rs2282679 A/C polymorphism in vitamin D binding protein gene with vitamin D deficiency

[Summary] To investigate the association of rs2282679 A/C polymorphism in vitamin D binding protein gene with vitamin D deficiency in 1 216 participants.A standardized questionnaire was applied to collect information on alcohol consumption,smoking habits,and use of medications.Serum 25-hydroxyvitamin D [25 (OH) D] was determined by radioimmunoassay.Vitamin D binding protein genotypes were determined by SNaPshot method.1216participants included 457 women(37.6%),478 (39.3%)with vitamin D deficiency [25 (OH) D<50 nmol/L],and 386 (31.7%) overweight/obese participants.The frequencies of rs2282679 CC,AC,and AA genotypes were 8.7%,41.0%,and 50.3%,respectively.The distributions of genotypes and alleles differed significantly between participants with sufficient vitamin D and those with vitamin D deficiency (P < 0.05).There were significant differences in the serum levels of 25 (OH) D between three genotypes before and after being adjusted for covariates (P<0.01).AA had a 22% (OR =0.78,95% CI 0.65-0.93,P =0.006) lowered risk of vitamin D deficiency compared with the whole studied population.Compared with CC genotype,AA had a 36% lowered risk of vitamin D deficiency(OR =0.64,95 % CI 0.42-0.98,P =0.006),while there was no significant difference between AC and CC genotypes(OR=0.87,95% CI0.57-1.34,P=0.53).In conclusion,the vitamin D binding protein rs2282679A/C polymorphism was significantly associated with serum level of 25 (OH) D and risk of vitamin D deficiency.
6.Effect of fengliao changweikang granule on SOD and MDA in blood of rats with cellular immunoreactive colitis

AIM To investigate the influence on change of SOD and MDA in the blood of rats which had cellular immunoreactive colitis by Fengliao Changweikang Granule (CWKG). METHODS Male Wistar rats were divided into six groups. Numbers of each group were 32. Animal model of rat cellular iumynoreactive colitis was made by TNBS and ethanol mixed liguid enema. Rats were killed at the 1st, 3rd, 7th and 21st day respectively. Gross morphologic observation of intestinal wall was made, SOD vitality and MDA content in blood were determined. Objective To observe the morphological changes of corneal nerve in experimental herpes simplex keratitis (HSK) in rabbit eyes. Methods Inoculation of the scarified cornea with herpes simplex virus-Ⅰ(HSV-Ⅰ)leads to herpetic infection of cornea. The HSK was detected by slit lamp and polymerase chain reaction(PCR), and the changes of corneal nerves in various periods with methylene blue vital staining and ultrastructure of corneal nerve were studied. Results Under light microscope, fewer corneal nerve bundles were observed, scattered with lower density of nerve fiber at center of cornea on the 4th day after the infection. At days 7 and 14, the seriously damaged nerve fiber was intermittent. The neuraxon became shorter with little polarity and the density of nerve fiber was extremely low. At day 30, the density was still low while the nerve fiber was approximately normal. Under electromicroscope, at day 4, the lamellar sheath of nerve fiber in the epitheliums appeared intermittent, and the neuroplasm of endings was partly lysed. During day 7 to day 14,neuroplasm was damaged and became vacuolar. The mitochondria swelled with vacuolar crest, and then were destroyed and lysed. The nerves in stroma were also injured. On day 30, neurilemma sheath was still intermittent with the decrease of microfilament and racuole. Conclusion HSK leads to the damage of nerve and the decrease of nerval density, and the damage of corneal nerve repairs very slowly.

Objective To explore the efficacy of simultaneous reconstruction of composite tissue defects in two adjacent metacarpophalangeal joints of hand by split the 2nd toes.Methods From December 2006 to December 2011,the second toe composite flap pedicled with the dorsalis pedis artery-first,second metatarsal dorsal artery-dorsal artery were designed,and split it into the second metatarsophalangeal joint composite flap pedicled with the dorsalis pedis artery-first dorsal metatarsal artery-tibial artery and the proximal interphalangeal joint composite flap pedicled with dorsalis pedis artery-second dorsal metatarsal arteryfibular dorsal artery.Then they were used to repair composite tissue defects in two adjacent metacarpophalangeal joints of hand at the same time after reconstruction.Results This group includes 14 metacarpophalangeal joints of 7 cases,twelve joints was successful,two joints of 1 case to give up because the first dorsal metatarsal artery variation.At 6-24 months follow-up study in 12 out of 14 cases showed that there was no joint degeneration in X-ray manifestation and the recovering of joint function was satisfying.According to the function evaluation standard of reconstraction of thrumb and finger issued by Hand Surgery Society Chinese Medical Association,the excellent and good rate was 83.3%.Conclusion It is feasible to reconstruct of composite tissue defects in two adjacent metacarpophalangeal joints of hand at the same time by split the 2nd toes,and can effectively improve the functions of injury metacarpophalangeal joint.
9.Interventional embolization of congenital arteriovenous fistula in the common femoral profounda artery in children

Objective To explore the effect and safty of interventional embolization of congenital arteriovenous fistula (CAVF) in the common femoral profounda artery in children.Methods Eight children (aged 6 to 14 years ;6 males and 2 females) with CAVF underwent interventional embolization From January 2006 and December 2010.Color Doppler sonography and DSA were performed on all patients.Arterial duplex imaging revealed the arteriovenous fistula in the common femoral profounda artery and DSA further confirmed the presence of an AVF over the common femoral profounda artery via the other vein.All children were treated with interventional embolization therapy by the injection ofcoils or ethanol through a microcather.It was necessary to performed repeated interventional embolization if the lesion was not obliterated in 4 weeks.All patients were followed-up from 6 months to 2 years and the clinical symptoms were observed.Results Arteriograpy can clearly demonstrate the femoral profounda artery and its branches as well as the fistula.Ten interventional embolization procedures were performed.Forty seven coils were deployedduring the procedure and seven ethanol embolization procedures were performed.Post-procedural angiography revealed complete occlusion in 8 patients.No major complications such as ectopic coil embolization,tissue necrosis,peripheral nerve palsy or cardio-pulmonary collapse were found.The abnormal AVFs were embolized completely in 7 cases after only one therapy and there was no recurrent disease in the 6 months to 2 years follow-up.Some tiny AVFs were still found in another case which underwent three additional treatments.However,the clinical symptoms were under control.Conclusion Interventional embolization is a safe and effective therapeutic method for CAVF of the common femoral profounda artery in children and it might become the primary treatment option.
10.Clinical efficiency of monoclonal antibody induction therapy in intestinal transplantation

Objective To evaluate the clinical efficiency of humanized anti-CD52 monoclonal antibody (Campath-1H) and anti-CD25 monoclonal antibody (Zenapax) induction therapy in intestinal transplantation patients.Method The data of 6 patients receiving Campath-1H and 5 patients receving Zenapax induction therapy in intestinal transplantation between 2007 and 2012 were analyzed retrospectively.The counts of peripheral blood lymphocytes and monocytes,incidence of rejection and infention,and liver and kidney toxicity of recipients were recorded before and 3 months after transplantation.Results Of 6 intestinal transplantation patients receiving Campath-1H induction therapy,1 died of acute heart failure on the postoperative day 3,and the rest 5 patients had a powerful depletion of lymphocytes and monocytes in 8 weeks,followed by gradual increases after 8 weeks.The percentage of peripheral blood CD3 + T cells,CD4 + T cells,and CD8 + T cells was dropped to 5% before administration,and remained at a steady low level in the first 8 weeks after induction.Of 5 patients receiving Zenapax induction therapy,1 died of Aspergillus infection on the postoperative day 25,and the rest 4 patients had an obeivous increase of lymphocytes and monocytes on the postoperative day 1.Counts of lymphocytes and monocytes kept steady at normal levels from the 1st to 12th week.One case of mild rejection was found in Campath-1H group.One case of mild,one moderate and one severe rejection were detected in Zenapax group.All rejections were successfully cured by prompt anti-rejection therapy.There were no significant difference in serum creatimine,urea nitrogen,alanine aminotransferase or total bilirubin after 3 months in comparison to preoperation.Conclusion Both Campath-1H induction therapy and Zenapax induction therapy successfully induce immune tolerance in patients with intestinal transplantation.Campath-1H seems to offer better immunosuppression against Zenapax during the first 3 months posttransplantation.
