1.Drug Resistance and Distribution of Nosocomial Infection Pathogens

OBJECTIVE To investigate the drug resistance and distribution of nosocomial infection(NI) pathogens.METHODS A total of 1 163 strains of NI pathogens during Jan 2002-Dec 2005 were completely surveyed and analyzed.RESULTS From them Gram-positive cocci were 376 strains(32.3%),the main pathogen was Staphylococcus aureus(12.7%),the average isolation rate of MRSA was 82.5%.Gram-negative bacilli were 474 strains(40.8%),the predominant pathogens were Escherichia coli(9.9%),Klebsiella pneumoniae(7.1%),Pseudomonas aeruginosa(5.4%),and Enterobacter cloacae(5.2%).The drug resistance of NI pathogens was markedly increased.Especially,the rates of drug resistance of P.aeruginosa to ceftazidime and imipenem were from 0 to 48.9% in 2005.CONCLUSIONS The Gram-negatives of NI pathogens are markedly increase year by year.The drug resistance rate of pathogens is higher,the clinically selected anti-bacterial drugs should be based on the bacterial culture and drug susceptibility test.
3.The clinical application of vertebral artery CT angiography in fracture of transverse foramen of cervical vertebra

Objective:To evaluate the value of CT angiography (CTA) for vertebral artery (VA) injury in the transverse foramen region of cervical vertebra.Methods:The clinical and CTA imaging data of 56 patients with cervical spine trauma in People's Hospital of Yuxi City from January 2016 to December 2019 were retrospectively analyzed. To observe the VA were or not injuried and their degrees, and the relationship between fracture in transverse foramen region and injured of the VA by applying post processing techniques such as volume reconstruction (VR), image fusion, curved planar reconstruction (CPR), multi-planar reconstruction (MPR), vascular analysis and so on.Results:There were 35 cases in the positive group of transverse perforation stenosis among the 56 patients, and 21 cases in the negative group. There were 19 cases injuried the VA in the positive group of stenosis, but just 5 cases′VA were injuried in the negative group of stenosis, with statistically significant differences ( P<0.05). The risk of VA injuried caused by different degrees of transverse perforation stenosis in the positive stenosis group was statistically significant ( P<0.05). The VA damaged distribution were: 3 cases in V1 segment, 16 cases in V2 segment, 5 cases in V3 segment. The degree of luminal stenosis were: 9 cases of mild stenosi, 7 cases of moderate stenosis, 5 cases of severe stenosis, and 3 cases of luminal occlusion. Conclusions:The CTA of VA can clearly show its anatomical route, degree of injury, range and its adjacent relationship with the fracture end of the transverse foramina of cervical vertebra, which can provide evidences of objective and reliable imaging for spinal surgeons to assess their conditions and select the treatment plans.
4.Variations in the intracranial vertebrobasilar system:a CT angiography study

Objective To evaluate the valuations of the intracranial vertebrobasilar arteries using CT angiography (CTA) in order to improve the understanding of various variants in intracranial vertebrobasilar arteries.Methods The CTA images of 435 subjects were analyzed retrospectively.The data were collected by a 16-slice CT scanner.All the CTA images were processed technically by the maximum intensity projection and volume rendering.The intracranial vertebrobasilar arteries were measured and analyzed.Results Among the 435 subjects,bilateral intracranial vertebral arteries (V4) showed symmetry in 183 (42.07%) and dissymmetry in 252 (57.93%).Hypoplasia were found in 27 subjects (6.21%) on the left V4 segments and 36 (8.28%) on the right V4 segments,and a total of 21 (4.83%) were directly continued as the ipsilateral posterior inferior cerebellar arteries on the unilateral V4 segments.There were 2 subjects (0.46%) with vertebrobasilar dolichoectasia.There were 32 subjects (7.36%) had fenestration variation,3 of them (0.69%) were on the V4 segments,10 (2.30%) at the junctions,and 19 (4.37%) in the basilar arteries.Two subjects (0.46%) had persistent trigeminal arteries.Conclusions The variations of intracranial vertebrobasilar arteries mainly include angiodysplasia,vertebrobasilar dolichoectasia,fenestration variation,and persistent trigeminal artery.As a noninvasive vascular imaging method,CTA may accurately and visually assess the anatomic variation of intracranial vertebrobasilar artery,and provide imaging guidance for the preoperative evaluation of posterior fossa surgery.
5.Regulation of B-lymphocyte stimulator expression levels in response to IFN-γ and IL-6

Objective To investigate the regulation of B-lymphocyte stimulator(BLyS) levels in response to IFN-γand IL-6. Methods Flow cytometry, quantitative polymerase chain reaction, ELISA and Western blot were applied to examine the expression level of BLyS in response to IFN-γ and IL-6 . Results IFN-γand IL-6 induced BLyS expression in KM3 cells. After treated with BAY11-7082, an IkB-α phospho- rylation inhibitor, the up regulation of BI,yS induced by IFN-γ was completely inhibited. Inhibiting the nu-clear faetor-kB (NF-kB) and mitogen activated protein kinase(MAPK) activation in KM3 cells reduced BLyS protein and gene expression. Conclusion MAPK and NF-kB pathways are involved in the regulation of BLyS expression, which suggests that MAPK and NF-kB might be used for the treatment of multiple mye- loma.
6.Evaluation of diffusion weighted imaging after contrast in characterizing breast tumors

Objective To evaluate the effect of intravenous administration of gadolinium-DTPA on diffusion weighted imaging (DWI)of breast lesions and apparent diffusion coefficient(ADC)values,and to evaluate the diagnostic performance of post-contrast ADC for differentiating benign from malignant lesions in breast.Methods Thirty-eight histopathologically-confirmed breast lesions were analyzed.We examined the signal to noise ratio(SNR)of normal breast tissue and lesions and evaluated the contrast to noise ratio(CNR)of each lesion.We also assessed the relation between pre-and post-contrast ADC values.Results SNRs and CNRs of DWI before and after contrast were not significantly different.The mean post-contrast ADC values for malignant lesions significantly decreased after contrast injected (mean of -1 1.6%),while ADC values in the benign lesions were not significantly different before and after contrast.Conclusion Post-contrast ADC values may be a better indicator of potential breast tumors than pre-contrast ADC values.
7.Value of real-time fluorescent quantitative polymerase chain reaction in detecting expression of miR-100 in patients with esophageal cancer

Objective To compare the expression of serum miR-100 in patients with esophageal cancer and healthy person ,and explore the value of miR-100 in diagnosis for esophageal cancer .Methods Real-time fluorescent quantitative polymerase chain reac-tion was used to detecting miR-100 in 40 esophageal cancer patients(study group) and 50 healthy person(control group) .Results The expression of miR-100 in the study group and control group were 6 .399 ± 3 .541 ,2 .625 ± 1 .515 respective ,the expression in the study group was significant higher than that of the control group(t= 9 .07 ,P< 0 .05) .The under area of receiver operating char-acteristic curve of miR-100 in diagnosis for esophageal cancer was 0 .832(95% confidence interval was 0 .731 - 0 .934) ,when the Cut off value was 5 .285 ,the sensitivity and specificity of miR-100 in diagnosis for esophageal cancer were 65% and 95% . Conclusion Serum miR-100 in esophageal cancer patients is higher than that in healthy person ,which might be a new molecular markers in diagnosis for esophageal caner .
8.Change of Whole Brain Degree in Primary Insomnia

[Objective] Based on the resting state functional magnetic resonance imaging to investigate the abnormal features of the functional connectivity of resting brain neural network in the patients with primary insomnia,by using voxel-wise whole-brain finctional networks analysis of degree centrality (DC) for imaging evidence of neural mechanisms underlying primary insomnia.[Methods] The resting state fMRI were performed in 59 PI patients and 47 age,education,and sex-matched normal healthy subjects.Analysis of DC map changes between the two patient groups and the control group were performed by two sample t test.(threshold at P < 0.05).[Results] Compared with the control group,the patients with PI showed significantly reduced DC value in bilateral medial frontal gyrus (MFG),bilateral anterior cingulate gyrus (ACG),and right insula;and increased DC value in right middle temporal gyrus (MTG),and left cuneus,(CUN),P < 0.05.[Conclusion]Changes of DC value occurred in some region of brain in the P[patient groups when compared with the control group.It was indicated that DC,as a novel resting-state fMRI parameter in the voxel-wise whole-brain functional networks,might be an appealing alternative approach for further study on pathologic and neuropsychological states of PI.
9.Contents Determination of Nine Components in Guizhi Decoction and Comparison of Different Decoction Methods

OBJECTIVE:To establish a method for the contents determination of 9 components in Guizhi decoction,and com-pare the effects of traditional decoction method and the extracting machine decoction method on these contents in Guizhi decoction. METHODS:HPLC was performed on the column of Agilent Zorbax SB-C18 with mobile phaseof acetonitrile- 0.1% phosphoric ac-id(gradient elution)at a flow rate of 1.0 ml/min,the detection wavelength was 230 nm,254 nm and 280 nm,the column tempera-ture was 25℃,and the injection volume was 10μl. RESULTS:The linear range was 0.410 2-210.0μg/ml for gallic acid(r=0.999 9), 0.994 0-254.5μg/ml for albiflorin(r=0.999 9),1.636 0-1 675.0μg/ml for paeoniflorin(r=0.999 9),0.988 3-506.0μg/ml for liquiri-tin(r=0.999 6),0.987 3-31.59 μg/ml for coumarin(r=0.999 5),0.486 8-124.6 μg/ml for cinnamic acid(r=0.999 5),2.458 0-314.6μg/ml for cinnamaldehyde(r=0.999 5),0.034 3-1.096 μg/ml for 2-methoxy cinnamaldehyde(r=0.999 8),and 1.711 0-219.0 μg/ml for glycyrrdhizic acid (r=0.999 7);RSDs of precision,stability and reproducibility tests were lower than 5%,recoveries were 93.56%-103.19%(RSD=4.00%,n=9)、101.51%-107.32%(RSD=2.21%,n=9)、95.08%-103.76%(RSD=2.87%,n=9)、100.82%-105.73%(RSD=1.85%,n=9)、85.08%-89.12%(RSD=1.40%,n=9)、92.31%-99.12%(RSD=2.71%,n=9)、99.17%-102.32%(RSD=1.24%,n=9)、100.15%-103.98%(RSD=1.18%,n=9)、99.93%-102.61%(RSD=1.03%,n=9). The content of total effective components from the extracting machine decoction method was 4 565μg/g,that from the traditional decoc-tion method was 2 742 μg/g.CONCLUSIONS:The method is simple,stable and reproducible,and can be used for the simultaneous determination of 9 componentsin Guizhi decoction. The contents of gallic acid,albiflorin and 2-methoxy cinnamaldehyde are first re-ported. The total effective components from the extracting machine decoction method are higher than that from the traditional decoc-tion method.
10.Initial experience of using improved motion sensitized driven equilibrium prepared balanced steady-state free precession

Objective To assess the application value of NCE-MRA using iMSDE prepared bSSFP sequence in lower limb in patients with diabetes. Methods This prospective study included a total of 35 patients with type II diabetes who underwent CE-MRA on the 1.5T MR scanner after the NCE-MRA. The obtained MIP images were independently rated by two radiologist with a four score table and using CE-MRA as a reference standard to evaluate the diagnostic accuracy of NCE-MRA for the narrowed arteries. The difference of the percent age of diagnostic arterial segments between NCE-MRA and CE-MRA on diabetic patients was evaluated by the χ2 test. Results Compared with CE-MRA, the diagnostic-value arterial segment ratio of pelvic arteries on NCE-MRA is decreased significantly. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of two readers in NCE-MRA were 95%/87%, 96%/95%, 57%/72%, 99%/99%, and 96%/95%(k=0.76), respectively. Conclusions The NCE-MRA using iMSDE prepared bSSFP sequence is capable of depicting vascu-lar lesions for the lower extremities in diabetic patients with the advantages of contrast agent free, short scan times and good diagnostic value in the thigh and calves.
