1.Effects of neural retina on development of the structure of outer blood-retinal barrier in embryogenesis

Objective To investigate effects of neural retina on development of the structure of outer blood retinal barrier in embryogenesis. Methods The retinal neural epithelium (RNE) and pigment epithelium (RPE) layers of 150, 120 and 90 embryonic chicken eyes incubated for 7, 10, and 14 days were peeled off. RNE was used to prepare the culture medium with different conditions (7drcSF3, 10drcSF3, 14drcSF3). RPE cells of 7- and 14-incubated chicken embryos were cultured on laminin-coated transwell filter. The SF3, 7drcSF3, 10drcSF3, 14drcSF3 medium were used respectively in the apical chamber and SF2 was used in basolateral chamber. After the formation of monolayer, the transepithelial electrical resistance of the RPE was detected. After the fixation of RPE cells, the condition of the tight junction among the cells was observed by immunohistochemistry and transmission electron microscopy. Objective To investigate the feasibility of labeling iris pigment epithelial(IPE)cells of rabbits with 5-(and 6-)carboxyfluorescein diacetate succinimidyl ester(CFSE). Methods Enzyme-assisted microdissection was used to isolate the cultured rabbit′s IPE cells.The third or forth subcultured IPE cells were incubated with 2.5,5,10,20,and 40 ?mol/L of CFSE for 1,5,and10min respectively.The fluorescence intensity was detected by flow cytometry,and the leakage of CFSE and its dyeing were observed by fluorescence antibody labeling. Results Incubation with 20 ?mol/L CFSE under 37℃for1minute was the most optimal condition for IPE cells labeling.The coloration of IPE cells stained by CFSE lasted 4 weeks.There was no leakage of dye from labeled rabbit IPE cells to non-labeled human IPE cells in mixed culture process. Conclusion With the advantages of high rate of dyeing,long time of tracing,safety and convenience,CFSE can be used as a new method to label the rabbit′s IPE cells.
3.The tention-free repair of abdominal incision hernia with polypropylene mesh

Objective To evaluate the effect of the tention free repair on patient with abdominal insicion hernia. Methods 15 cases of abdominal insicion hernia were reviewed, all of them were repaired with polypropylene mesh. The mesh was overlaid on the defect, continuously monofilament nonabsorbable suture was used to fix the mesh on the edge of the defect and the mesh must be overlapped 2 cm on fascia abdominal layer and fixed by suture. Prophylactic antibiotics was used routinely in single dose. Result No relapse was found in follow up(rang 15~38 month). Conclusion The tension free hernia repair with polypropylene mesh in patient with abdominal incision hernia is a safe and simple operation with minimal postoperative pain.
4.The distant occult peritoneal metastasis in patients with operable gastric cancer

Objective To evaluate the existence of occult peritoneal metastasis,Douglas′ pouch biopsy was undertaken intraoperatively in patients with gastric cancer. Methods From April 1998 to August 1999, Douglas′ pouch biopsy was performed in 16 cases of gastric cancer during laparotomy in which obvious distant peritoneal metastasis was not found by inspection or palpation.[WT5”HZ] Results Occult peritoneal metastasis in Douglas′s pouch was established in 5 out of the 16 cases,while the invasion depth of primary tumor were all under S stage category and the numbers of regional lymph node metastasis were 7 and up.Conclusion[WT5”BZ] At stages when gastric cancer invaded the visceral serosa, peritoneal biopsy in Douglas′ pouch must be performed as routine procedure in order to detect occult distant peritoneal metastasis.
5.Side effect analysis and nursing for childhood malignant patients who received in-vitro amplification HLA-haploidentical donor immune cell infusions

Objective To analyze the side effects of in-vitro amplification human leukocyte antigen (HLA)-haploidentical donor immune cell infusion(HDICI) in childhood malignant patients,and to provide the basis for how to nurse these patients.Methods From September 2011 to September 2012,twelve hospitalized childhood malignant patients were recruited in Cancer Center of Sun Yat-sen University,and they received a total of 92 times of HDICIs for immunotherapy.Side effects were carefully observed both during and 2 hours and 24 hours after each infusion,and we also provided psychological nursing,pre-infusion nursing,side effect analysis and corresponding care during infusion,post-infusion education and follow-up for every childhood patient.Results Among 92 times of HDICIs,fever occurred in 20 cases,of whom 6 cases were with chills,1 case with febrile convulsion,all achieved remission after receiving symptomatic treatment.5 cases were also recorded with a slight change of mood.Vital signs were all stable both during and after every HDICIs; and no nausea,vomiting,abdominal pain,diarrhea,rash,swelling or allergic reaction was observed.Neither change of the hemogram nor biochemical indexes was recorded before or after every course of HDICI.Conclusions Our study showed that HDICIs for immunotherapy in childhood malignant patients were safe,and during the whole infusions,much attention should be paid both for psychological nursing,pre-infusion anti-anaphylactic treatment,temperature monitoring,discovery of the precursor of febrile convulsion promptly,symptomatic treatment against corresponding side effects,and post-infusion education and follow-up,all those above are of great importance for childhood malignant patients who received HDICIs for immunotherapy successfully.
6.Postoperative complications of Lichtenstein herniorrhaphy in 334 adult inguinal hernia cases

ObjectiveTo analyze the cause of and the prevention for the postoperative complications in adult patients undergoing Lichtenstein herniorrhaphy for inguinal hernia. Methods Retrospective review was made on 334 inguinal hernia cases receiving Lichtenstein repair in our hospital. Results Recurrence was found in 1 case(0 3%). Significant postoperative pain occurred in 5 patients. Four cases (1 2%) suffered from superficial wound infection, and 5 cases(1 5%) were complicated with subcutaneous seroma. Conclusions Lichtenstein′s tension free repair for the treatment of adult inguinal hernia has the advantage of less postoperative pain and low recurrence.

9.The relationship between the resection extent and number of lymph node dissection and the effect of metastatic lymph node number on the prognosis of proximal gastric cancer

Objective To study the relation between the surgical extent and number of lymph node dissection and the effect of metastatic lymph node number on the prognosis of proximal gastric cancer. Method Thirty-one patients with proximal gastric cancer underwent D_2 or D_3 operation, and lymph node were harvested from specimen. The number of lymph node dissection in proximal gastrectomy was compared with that in total gastrectomy. Results A total of 1971 lymph nodes were obtained from the 31 specimens, the mean was 63 per case. There were 57 lymph nodes per case for patients with proximal gastrectomy and 71 per case with total gastrectomy,64 per case with left half pancreaticosplenectomy and 63 per case with splenectomy. According to the new 5th N stage system, the 5 year survival of N_1,N_2,N_3 were 36%, 11%, and 0 respectively. Conclusion Along with the wide invasion of the carcinoma wide resection with extended lymph node dissection is mandatory, reserved left half of the pancreas has no influence on the number of harvested lymphnode, the new quantitative N staging is superior to the old in predicting the prognosis.

OBJECTIVE:To determine the retative bioavailability of Diclofenac sodium eye-ointment in rabbit eyes and to evaluate its pharmacokinetics following topical application METHODS:48 rabbits were divided into two groups 0 1% Diclofenac sodium eye-ointment and Diclofenac sodium eyedrops were applied topically to rabbit eyes and aqueous humor and iridic tissue were collected at given times The concentration of Diclofenac sodium in tissue were measured by HPLC RESULTS:Diclofenac sodium eye-ointment group had higher concentration and longer half-time than Diclofenac sodium eyedrops group,however,the peak time were the same in two groups The relative bioavailabilities of Diclofenac sodium ointment in aqueous humor and iridic tissue were 183 33% and 205 68% respectively,compared with those of Diclofenac sodium eyedrops CONCLUSION:Diclofenac sodium eye-ointment can be absorbed well and released slowly,which can reduce the frequency of application of drug It is especially suitable for operation patient or application at night
