1.Therapeutic effect of different treatment progarams for epithelial ovarian cancer

Objective To explore different methods on the efficacy of epithelial ovarian cancer.Methods The patients with epithelial ovarian cancer,a total of 265 cases,who was treated were selected and divided into A,B groups.Its surgical,chemotherapy and efficacy were compared.It was basically the same between the two surgical.On chemotherapy,A group was given PAC program mainly,B group Was given TA program mainly.Results Diameter of residual tumor lesions>2 cm respectively accounted for 29.2%.14.2%.The difference of clinical remission rate,partial remission rate was not significant.Effective rates were respectively 51.7% and 81.4%(P<0.05).To group A,the survival rates of 1-year,2-years,3-years,4-years,5-years were respectively 90.2%,86.9%,62.7%,45.8%,32.7%:The median survival period and relapse-free survival time was respectively 37.9 months and 17.4 months.To group B,the survival rates of 1-year,2-years,3-years,4-years,5-years were respectively 94.3%、91.1%、68.7%、56.4%、44.2%;The median survival period and relapse-free survival time wag respectively 50.5 months and 32.2 months.The difference of 5-years survival rate,median survival,relapse-free survival of the two groups was statistically significant(P<0.01 or 0.05).Conclusions The size of postoperative residual disease and the chemotherapy im-paeted the efficacy of epithelial ovarian cancer.TC chemotherapy was superior to PAC chemotherapy.
2.Anticoagulants combined with calcium and vitamin E in the early intervention of hypertensive disorder complicating pregnancy patients

Objective To observe the clinical curative effect of anticoagulants combined with calcium,vitamin E in pregnancy induced hypertension patients at high risk for early intervention.Methods 113 patients with hypertensive disorder complicating pregnancy were selected as the observation subjects,according to the detection of prethrombotic state before pregnancy,they were divided into observation group one 38 cases,group two 43 cases,control group 32 cases.The observation group one was given intervention preconception,the group two was given intervention during pregnancy,the control group was given intervention when abnormal.The incidence rates of obstetric complication,intrauterine distress and neonatal asphyxia were compared among the three groups.Results The incidence rates of hypertension,obstetric complications,intrauterine distress and neonatal asphyxia in the group one were 5.26%,7.89%,5.26%,5.26%,those in the group two were 20.93 %,9.3 %,18.6%,20.93 %,those in the control group were 43.75%,28.13%,43.75%,48.88%,the differences between the observation group one and control group showed significant differences(x2 =14.59,5.01,14.59,16.36,all P < 0.05),the differences between the group two and the control group were statistically significant (x2 =4.49,4.54,5.60,5.68,all P < 0.05),the incidence rates of hypertension,neonatal asphyxia between the observation group one and group two had statistically significant differences (x2 =4.22,4.22,all P < 0.05).Conclusion Anticoagulants combined with calcium,vitamin E in the treatment of patients with hypertensive disorder complicating pregnancy,can effectively correct the adverse pregnancy outcomes,and improve the prognosis of perinatal infants,and early intervention is better.
3.Clinical observation of jiont analyesia in labor

Objective To evaluate the effect of nitrous oxide-inhale combined with lidocaine-injection by Ciliao and Uterine points and single therapy in labor analgesia,and the impact on maternal and child.Methods 400 cases were randomly divided into four groups.Observation group:100 cases take the therapy of Nitrous oxide-inhale combined with lidocaine-injection in Ciliao and Uterine points in the time of active period.Control group 1:100 cases take the therapy of gas mixture-inhale( 50% Nitrous oxide and 50% Oxygen) in the time of active period.Control group 2:100 cases take the therapy of lidocaine-injection in Ciliao and Uterine points in the time of active period.Control group 3:100 cases do not take any analgesia.Analgesic effects,labor time,mode of delivery,neonatal conditions,the amount of bleeding in every group were observed.Results There was statistically significant differences between the observation group with the control group 1,2,3 on analgesic effects ( x2 =48.513,P < 0.05 ) labor time( F =782.88,1120.81,1104.57,all P < 0.05 ),mode of delivery ( x2 =85.715,P < 0.05 ),neonatal conditions,the amount of bleeding( F =422.66,P < 0.05 ).Conclusion Combination therapy was more effective than the single treatment.
4.Study on correlation between the joint detection of CHE,AFU and AFP and primary liver cancer

Objective To explore the correlation between the joint detection of cholinesterase(CHE),alpha-L-fucosidase(AFU) and alpha-fetoprotein(AFP)and primary hepatic cancer(PHC).Methods 56 cases of patients with PHC(PHC group),52 cases of patients with liver cirrhosis(LC group)and 60 cases of healthy individuals(control group)were enrolled in this study,and serum levels of CHE,AFU and AFP were detected and statistically analysed.Results Serum levels of CHE in patients with PHC were negatively correlated with levels of AFU and AFP(correlation coefficient was -0.889 and -0.797 respectively,P <0.05),the ser-um levels of CHE in patients with LC were also negatively correlated with levels of AFU and AFP (correlation coefficient was-0.598 and -0.653 respectively,P <0.05).The positive rates of joint detection of CHE,AFU and AFP in PHC group and LC group were higher than that in the control group,had statistically significant differences(P <0.05 ).Conclusion CHE,AFU and AFP are sensitive indicators of liver lesions,and the joint detection could provide reference value for diagnosis and monitoring pro-gression of PHC.
5.Analysis of risk factors for carotid atherosclerosis in patients with renal glomerular diseases

Objective To study the relationship between intrarenal arterial lesions(IAL)and carotid atherosclerosis(AS)in patients with renal glomerular disease.Methods 251 cases with IAL were selected and 129 age-,pressure-and eGFR-matched renal glomerular disease cases without IAL were randomly selected.The carotid intima-media thickness(IMT)was detected.Clinical and laboratory examination and renal histological characteristics were compared the two guoups.Results ①The detection rate of carotid AS was higher in patients with IAL than those without IAL(38.2%vs.20.2%),and higher in patients≥40 years than in patients<40 years(51.3%vs.13.1%)(P<0.05 for each). ②The carotid AS group was older and had higher frequencey of fasting blood glucose,body mass index,smoking,and family history of hypertension,longer duration of established hypertension and renal glomerular disease,higher prevalence of hypertension,IAL and renal lesions,and had lower eGFR than the carotid normal group(all P<0.05).Binary logistic regression analysis revealed that IAL and age emerged as an independent risk factor for carotid AS(OR=1.826 and 1.129,P=0.001 and 0.003).Conclusion The intrarenal arterial lesion is an independent risk factor for carotid atherosclerosis in patients with renal glomerular disease.Controlling blood glucose,blood pressure,weight,smoking quit and delaying progression of kidney disease have important significance in relieving or preventing atherosclerosis and intrarenal arterial lesions of patients with renal glomerular diseases.
6.The effectiveness of the cytokines and underglycosylation IgA_1 in mesangial deposition with IgA nephropathy

Objective To investigate the effects of TNF-?,IL-12,IL-15 and underglycosylation IgA_1 in mesangial deposition in the clinicopathological study of IgA nephropathy(IgAN).Methods Seventy-four patients with IgAN were enrolled in the study,with 10 patients randomly selected after remission,10 MCD and 10 healthy volunteers as control groups.The levels of TNF-? in serum and urine were examined by radioimmunoassay,and the levels of IL-12 and IL-15 were examined by ELISA.Immunohistochemistry was used to detect the expression of TNF-? in renal tissue,and the directed immunofluorescence was used to detect the underglycosylation of IgA_1(UGIgA_1)in mesangial deposition.Results The levels of TNF-?,IL-12 and IL-15 in serum of patients with IgAN were higher than those of MCD group,healthy control group and remission group.Compared with the negative group,33 patients with UGIgA_1 positive in renal tissues had lower levels of TNF-? in serum,longer course and less class of renal tissue injury.The positive areas of TNF-? in renal tissue and levels of TNF-? in serum were positively correlated with urinary protein(r=0.249,0.702,P=0.000).There was negative correlation between the levels of TNF-? in serum and Ccr(r=-0.231,P=0.048).The levels of IL-15 was positively correlated with the course of disease(r=0.266,P=0.022) and negatively with Scr(r=-0.320,P=0.005).Conclusions The TNF-?,IL-12 and IL-15 are all involved in the onset of proteinuria in IgAN patients,and IL-15 acts as a factor against renal injury.The patients with UGIgA_1 positive in mesangial have longer course and less pathological changes damaged than those of the UGIgA_1 negative patients.The TNF-? may serve as an important factor in the aggravation of IgAN.
7.Expression and significance of M-type phospholipase A2 receptor and thrombospondin type-1 domain-containing 7A in adult idiopathic membranous nephropathy

Objective To detect the M-type phospholipase A2 receptor (PLA2R),and thrombospondin type-1 domain-containing 7A (THSD7A) expression in renal tissue and the levels of their antibodies in adult idiopathic membranous nephropathy (IMN).Also to determine the value of the two markers in the diagnosis of IMN.Methods One hundred and sixteen patients with biopsyproven MN at the Second Hospital of Hebei Medical University from December 2014 to August 2015 were enrolled,including 86 patients with IMN,10 patients with HBV-MN and 10 patients with stage Ⅴ lupus nephritis (LN-Ⅴ).Twenty patients with minimal change disease (MCD) were regarded as control group.We conducted immunohistochemical analysis of the presence of THSD7A and PLA2R the Paraffin section and enzyme linked immunosorbent assay (ELISA) detecting serum PLA2R-AB and THSD7A-AB concentration to investigate whether there was a correlation between them and clinical indicators.Results Compared with the SMN and MCD groups,the positive rates of PLA2R and PLA2R-AB were significantly higher in IMN groups.Expression PLA2R was detected in 88.4%,47.4%,10% and 0% and PLA2R-AB in 82.6%,15%,10%,0%,respectively,of the patients with IMN,HBV-MN,LN-Ⅴ and MCD.Expression THSD7A was detected in 2.3% of the patients with IMN while not detected in SMN and MCD.THSD7A-AB antibody was negative in all patients.Compared with serum PLA2R-Ab negative individuals,patients with serum PLA2R-Ab positive had lower serum albumin (P < 0.001),higher urine protein excretion (P=0.01).The sensitivity of PLA2R-AB,PLA2R,THSD7A and PLA2R+THSD7A in the diagnosis of IMN were 82.6%,88.4%,2.3%,88.6%,and the specificity was 92%,66.7%,100%,66.7%,respectively.Conclusions PLA2R in renal tissue and serum PLA2R-AB are specific markers for the diagnosis of IMN,which are closely related with the severity of IMN.Expression of THSD7A is only positive in some of IMN patients with negative PLA2R,which can be used as a supplementary examination of IMN patients with negative PLA2R.
8.Effectiveness of ADDIE instruction model in on-the-job training of neonatal transfer nurses

Objective To explore the effectiveness of ADDIE instruction model(Analyse,Design,Develop,Implement,Evaluate)in the on-the-job training for neonatal transfer nurses to improve the comprehensive ability in life-saving as well as to ensure the safety of neonates during the transfer.Methods The pre-and post-control study in self-trained comparison was conducted with 35 neonatal nurses in our hospital.Thirty-five neonatal nurses in our hospital were included in a special training group for neonatal transport.According to the ADDIE instruction model,the training needs of nurses were analysed,special training protocols were designed and developed,training rules were implemented and the effects of training on the nurses were evaluated.Scores in professional theoretical knowledge,practical and various skills and teamwork and communication abilities in neonatal transfer were compared before and after the implementation of the ADDIE instruction model.Results All of the 35 nurses had completed the training and responded to the survey.The scores in professional theoretical knowledge,practical skills and teamwork and communication abilities in the neonatal transfer nurses were all significantly increased after the training with ADDIE instruction model(P<0.001).Conclusions On-the-job training of ADDIE instruction model among the neonatal transfer nurses can effectively enhance the professional knowledge and practice skills,enable the nurses to play a leading role in the process of treatment and improve abilities in teamwork and communication of the neonatal transfer nurses.The ADDIE instruction model provides valuable insights in the training of neonatal transfer nurses.
9.Trastuzumab administered concurrently with anthracycline-containing adjuvant regimen for breast cancer.

OBJECTIVETo investigate the safety and efficacy of trastuzumab administered concurrently with anthracycline-containing adjuvant regimen for breast cancer.
METHODSIt is a prospective, randomized and controlled trial. Participants were randomized to receive trastuzumab administered concurrently or sequentially with anthracycline-containing adjuvant regimen. The primary endpoint was cardiac safety. The second endpoints were disease-free survival (DFS) and overall survival (OS).
RESULTSOne hundred and nine breast cancer patients were enrolled and randomized in this trial. Fifty-five participants received trastuzumab administered concurrently with anthracycline-containing adjuvant regimen and 54 patients received trastuzumab administered sequentially with anthracycline. The primary cardiac event was asymptomatic decrease in the left ventricular ejection fraction (LVEF). There was no significant difference between concurrent and sequential groups in cardiac event rates (9.1% vs13.0%, P = 0.556), neither of LVEF values at basline or at 3, 6, 9 and 12 months during trastuzumab treatment (P > 0.05). Four patients (7.3%) in the concurrent group suffered local recurrences or distant metastases, and 6 participants (11.1%) in the sequential group had distant metastases. There was no significant difference between the two groups in DFS (P = 0.724). There was no death in both groups.
CONCLUSIONSTrastuzumab administered concurrently with anthracycline is a safe adjuvant regimen for breast cancer and does not increase cardiac events. Further research is needed to determine the efficacy of this treatment regimen.

10.Construction and functional analysis of EGFRvIII CAR-T cells co-expressing IL-15 and CCL19.

The aim of this study was to investigate the functional characteristics and in vitro specific killing effect of EGFRvIII CAR-T cells co-expressing interleukin-15 and chemokine CCL19, in order to optimize the multiple functions of CAR-T cells and improve the therapeutic effect of CAR-T cells targeting EGFRvIII on glioblastoma (GBM). The recombinant lentivirus plasmid was obtained by genetic engineering, transfected into 293T cells to obtain lentivirus and infected T cells to obtain the fourth generation CAR-T cells targeting EGFRvIII (EGFRvIII-IL-15-CCL19 CAR-T). The expression rate of CAR molecules, proliferation, chemotactic ability, in vitro specific killing ability and anti-apoptotic ability of the fourth and second generation CAR-T cells (EGFRvIII CAR-T) were detected by flow cytometry, cell counter, chemotaxis chamber and apoptosis kit. The results showed that compared with EGFRvIII CAR-T cells, EGFRvIII-IL-15-CCL19 CAR-T cells successfully secreted IL-15 and CCL19, and had stronger proliferation, chemotactic ability and anti-apoptosis ability in vitro (all P < 0.05), while there was no significant difference in killing ability in vitro. Therefore, CAR-T cells targeting EGFRvIII and secreting IL-15 and CCL19 are expected to improve the therapeutic effect of glioblastoma and provide an experimental basis for clinical trials.
