1.Investigation in sleep problems of infants and analysis of its influencing factors

ObJective To acknowledge the sleep status of infants and analyze the influencing factors of sleep problems in them. Methods The sleep status of 136 infants and its influencing factors were investigat-ed using questionnaires, the results underwent descriptive analysis, single-factor analysis and multifactor anal-ysis. Results Difficulty in falling asleep was the major sleep problems, then was frequent night awakening. The influencing factors included: family type, education degree of the parents, inhabited environment, the feed-ing mode after birth, the period of supplementary protein dietary addition, the times of taking food through the night, and the sleep position, the bed, the emotion when falling asleep, etc. Conclusions Sleep problems is ubiquity in infants,and the unproper child rearing behaviors is the major influencing factor. In order to prevent sleep problems in infants and improve the quality of sleep in them, the parents need health guidance.
2.Effect of relaxation therapy on rehabilitation patients with functional dyspepsia

Objective To explore the effect of relaxation therapy on rehabilitation patients with functional dyspepsia.Methods 112 cases with functional dyspepsia were randomly divided into test group(n=56) and control group(n=56).Relaxation trains was adopted in test group,usual nursing care was adopted to the patients in both groups.The therapeutic effects and relapse rate were observed in two groups. Results There were no significant differences of clinical curative efficacy in two groups first-visit and less than six-month pathogenesis patients,but compared with the control group,the cure efficacy of recurrent-attack patients and over six-month-pathogenesis patients were significantly lower in test group,and the relapse rate in control group is higher than that in test group. Conclusion The relaxation therapy can promote the rehabilitation in functional dyspepsia patients of recurrent attack and over-six-month pathogenesis,and decrease the relapse rate.
3.Research in influence of patient-controlled analgesia on incidence of postpartum depression after cesarean section

Objective To explore the influence of patient-controlled epidural analgesia(PCEA) on incidence of postpartum depression after cesarean section. Methods 256 cases of singleton term primary cesarean delivery were randomly divided into the study group( 130 cases)and the control group( 126 cases).The study group retained analgesia pump connected with epidural catheter,analgesic liquid was injected.In the control group,epidural catheter was removed,and was given intramuscular injection of pethidine hydrochloride 100 mg 4~6 h after operation,repeated 6 h later,total dosage was 200 mg.Postoperative observation time was 48h.Plasma prolactin(PRL) concentration was determined by radioimmunoassay in prenatal,postnatal 24 h and 48 h.Edinburgh postnatal depression scale (EPDS) was used to assess depression state 6 weeks postpartum in the two groups.The incidence of postpartum depression was compared between two groups. Results The plasma PRL level in the study group was significantly higher than the control group 24 h,48 h postpartum.The incidence rate of depression in the study group was 6.15%,while was 16.67% in the control group,the differences were statistically significant. Conclusions The application of PCEA after cesarean section can significantly reduce the incidence of postpartum depression,and has very important practical significance in preventing the incidence of postpartum depression.
4.Application of Roy adaptation mode in post-operative self-image disorder and reformation of patients suffered from breast cancer

Objective To explore the application value of Roy adaptation mode in self-image disorder and reformation of patients suffered from breast cancer after operation.Methods 178 cases of postoperative patients suffered from breast cancer were separately divided into the experimental group (90 cases)and the control group(88 cases) at random according to the order of hospital admission.The experimental group adopted nursing of the Roy adaptation mode based upon operation and normal treatment,while the control group received general nursing besides operation and routine treatment.The investigation was carried out using the form of questionnaires.The occurrence rate of self-image disorder,life quality by using GQOLI-74 questionnaire,psychological quality by using emotion scale were compared between the two groups.Results The occurrence rate of the self-image disorder of the patients of the experimental group was evidently lower than that of the control group after the nursing measure was taken.Scores of all items of life quality and emotional scale of the experimental group were evidently higher than those of the control group.Conclusions Applying Roy adaptation mode to nurse post-operative patients suffered from breast cancer is a very good nursing mode,it can facilitate psychological suitability,improve psychological quality and promote their effective adaptability.It has pivotal significance to lower the occurrence rate of the self-image disorder of the patients,reform the self-image and make them return to society early.
5.TLR4/NF-κB signal pathway mediate HBD-2 in patients with preterm premature rupture of membrane associated with histologic chorioamnionitis

Objective To observe the effect of human β-defensins-2(HBD-2) for chorioamnionitis(HCA) pregnant women before term premature rupture of fetal membrane(PROM) process,and explore toll-like receptor 4 / nuclear factor-κ B (TLR4 / NF-κB) predominate role in the process of signal transduction pathway in the mechanism.Methods Fifty five women with PROM were enrolled in the study.According to the Results of pathological diagnosis of membranes,pregnant women with PROM divided into histological chorioamnionitis,HCA and non-HCA.The same sample without PROM pregnancies matching the same gestational ages were recruited as control group.We examined the messenger RNA(mRNA) of TLR4,NF-κB p65 and HBD-2 in placenta and fetal membrane real-time reverse transcription polymerase chain reactions by dsDNA-binding dyes of SYBR Green.Results (1)In the placenta,the level of TLR-4(17.15±4.52),NF-κB p65(47.11±14.23),HBD-2mRNA(27.35±2.67) in PROM group were significantly higher than the level of TLR-4(7.21±3.25),NF-κB p65(30.51±13.05),HBD-2mRNA(13.55±0.8) in control group(t=-1.966,-1.474,-1.754,P<0.05).In the fetal membrane,the level of TLR-4(18.54±3.35),NF-κB p65(49.29±16.18),HBD-2mRNA(25.19±3.39) in PROM group were significantly higher than the level of TLR-4(6.88±2.75),NF-κB p65(32.41±11.45),HBD-2mRNA(13.80±2.21) in control group(t=-2.378,-1.487,-1.847,P<0.05).In the placenta,the level of TLR-4(19.35±4.12),NF-κB p65(49.3±13.36),HBD-2mRNA(31.54±2.43) in HCA group were significantly higher than the level of TLR-4(6.71±2.25),NF-κB p65(32±12.25),HBD-2mRNA(7.41±0.45) in non-HCA group(t=-3.456,-1.547,-5.324,P<0.05).The level of TLR-4(7.78±2.65),NF-κB p65(8.48±1.01),HBD-2mRNA(30.45±11.42) in HCA group were significantly higher than the level of TLR-4(7.78±2.65),NF-κB p65(30.45±11.42),HBD-2mRNA(8.48±1.01) in non-HCA group(t=-3.654,-1.654,-6.352,P<0.0).(3)HCA group women:there were positive correlations between TLR4mRNA and NF-κB p65 mRNA,TLR4mRNAand HBD2 mRNA,TLR4mRNA and HBD2 mRNA(r=0.334,0.427,0.432,0.562,0.471,0.392,P<0.01)(4;)Non-HCA group women:there were positive correlations between HBD-2 and NF-κB p56(r=0.492,0.298,P<0.01),there had no correlation betweenTLR4mRNA and NF-κB p65mRNA,TLR4mRNAand HBD2 mRNA(P>0.05).Conclusion Linear positive correlation of TLR4,NF-κB and HBD2 indicated that TLR4/NF-κB/HBD2 signal transduction pathway may be involved in the development of preterm premature rupture of membrane associated with histologic chorioamnionitis.
6.Investigation in correlation between social factors and postpartum depression

Objective To investigate the correlation between social factors and postpartum depression.Methods A total of 182 parturients were investigated with Edinburgh Postnatal Depression Scale (EPDS)and self-designed questionnaire.The social factors and postpartum depression of parturients were analyzed using univariate and multivariable logistic regression analysis.Results Family income,relation of family,relation of couples,sex of newborns and negative life events were associated with the risk of postpartum depression.Conclusions Low family income,bad relation of family,bad relation of couples,female newborns,and negative life events during pregnancy may increase the risk of postpartum depression.
7.Study on the causes and countermeasures of medical disputes between nursing students and patients in hospitals

Objective To investigate the causes and countermeasures of medical disputes between nursing students and patients in hospitals, which will prevent the occurrence of the disputes and promote the comprehensive development of the hospitals. Methods The causes of 107 cases of medical disputes between nursing students and patients during the last three years in hospitals was analyzed. Results In recent years, there was an upward trend in medical disputes be-tween nursing students and patients, in which 57%were caused by poor communication and service attitude. And some preventive measures were put forward. Conclusion To strengthen the communication between nursing students and pa-tients, improve service awareness, better service attitude, enhance the legal safety education, regulate the profession practice and improve the comprehensive quality and nursing quality of nursing students are the key factors in reducing the nurse-patient disputes.
