1.Study on effects of Fentanyl transdermal patches and pethidine in postoperative analgesia of malignant tumor and nursing measures

Objective To study and compare the effects of Fentanyl transdermal pain patches and Pethidine in postoperative analgesia of malignant tumor and its nursing methods.Methods 100 patients with malignant tumor in our hospital from January 2008 to November 2010 were selected as research object,all the patients were randomly divided into group A (Fentanyl transdermal pain patches group) and group B (Pethidine group)with 50 cases in each group,then the VAS score 2,4,8,24 and 48h after the surgery,incidence of adverse reactions,satisfaction rate and serum catecholamine,cortisol,blood glucose before and after the surgery were analyzed and compared,and all the patients were given corresponding nursing measures.Results The VAS score of group A at 2,4,8,24 and 48h after the surgery were all better than those of group B,incidence of adverse reactions was lower than that of group B,satisfaction rate was higher than that of group B,serum catecholamine,cortisol,blood glucose after the surgery were all lower than those of group B,there were significant differences.Conclusions The effects of Fentanyl transdermal pain patches in postoperative analgesia of malignant tumor is stable,and it gets good reputation from the patients,the effects is obvious combined with nursing.
2.Application of Kolb experience learning theory in Surgical Nursing teaching

Objective To investigate the effect of the Kolb experience learning theory applied in Surgical Nursing teaching.Methods 120 students were selected randomly and were divided into 2 groups,the observation group operated the teaching mode based on the Kolb experience learning theory,the control group operated traditional teaching.After the experiment,using the theory test,the skill test,the questionnaire survey to evaluate the teaching effectiveness.Results The two groups showed statistical difference in theory exam results,learning effect of each dimension and evaluation of teaching effect.Conclusions The teaching mode based on the Kolb experience learning theory can improve the students' interest in learning and study effect,and push forward the reform of teaching implementatiou.
3.Modulation of macrophage function stimulated with oxidized low density lipoprotein by CD4+CD25+regulatory T cells

Objective To investigate whether and how CD4 + CD25 + regulatory T cells(Tr) affect oxidized low density lipoprotein(oxLDL) induced proinflammatory response in macrophages.Methods Tr were isolated from lymphocyte suspensions by magnetic cell sorting-column and analyzed by flow cytometry.Macrophages were cultured alone,with CD4 + CD25 + Tr or CD4 + CD25 - Tr in the presence of oxLDL for 48 h.The phenotype of macrophages was determined by flow cytometry.NO production was assessed by Griess reaction an iNOS mRNA was isolated by RT-PCR.ELISA were used to measure the production of cytokine/chemokine like MCP-1,MMP-9,TNF-ct,TGF-β and IL-10 in macrophages response to oxLDL.Results Our data showed that with oxLDL challenge,the Tr modulated macrophages have decreased NO production and iNOS expression,decreased HLA-DR and CD86 expression,and down-regulated proinflammatory cytokine/chemokine production.Conclusion Tr can inhibit the proinflammatory properties of macrophages and steer macrophage differentiation toward an anti-inflammatory cytokine producing phenotype.
4.Preparation of superparamagnetic iron oxide nanoparticles and its acute toxicity to mice

Objective:To prepare superparamagnetic iron oxide(SPIO) nanoparticles and to observe its acute toxicity on mice,so as to pave a way for further study on its long-term toxicity and on its role as a carrier in magnetic resonance gene imaging.Methods: The SPIO nanoparticle was obtained by means of co-precipitation,and its physical and chemical parameters were determined by transmission electron microscope,atomic force microscope,and 1.5 T super conduct MR,etc.According to the administration pathway and doses of SPIO,90 mice were divided into oral administration(with a total dose of 2 104.8 mg/kg and a volume of 40 ml/kg,n=30),intravenous injection(a total dose of 438.5 mg/kg and a volume of 25 ml/kg,n=30) and intraperitoneal injection(with a total dose 1 578.6 mg/kg and a volume of 30 ml/kg,n=30) groups.Another 10 mice in each group receiving the same dose of normal saline via the same pathway served as the controls(n=10).The general condition,the major serologic parameters,and the pathological changes of major organs were observed 14 d after administration in each group.Results: We have successfully prepared SPIO,and its core component was Fe3O4 crystal,with a size of 20-35 nm,a T2 relaxivity of 0.155?106 mol-1?sec-1,a specific saturated magnetization of 68.395 68 emu/g,and a retentivity of 21.463 74 Gs.There was no death of mice during the observation.There was no significant difference in serological parameters between mice of different groups and between each experiment group and their corresponding control group.No edema,degeneration and necrosis were seen in the liver,spleen,kidney,heart,and lungs by H-E staining and marrow by Wright staining;only a few blue particles were observed in the liver and spleen in the administration groups by Prussian blue staining,none observed in the control groups.Conclusion: SPIO prepared in the present study meets the requirement of MR imaging,with no acute toxicity to mice,and warrants further study for future MR gene imaging.
5.Effects of Acorustatarinowii Schott on chronic restraint stress mice and its mechanisms

Objective To evaluate the effects of Acorustatarinowii Schott(ATS)on chronic restraint stress mice and its possible mechanisms. Methods Chronic stressed mice underwent a repeated 8h/d constraint in a specific behavior blocker ,however ,only CSA mice received once gavages of 4.5 g/(kg · d)ATS decoction for con-secutive 28 days. Morris water maze(MWM)task was conducted for evaluating learning and memory of the mice. ELISA was used to examine the levels of plasma corticosteroid. Results ATS dramatically ameliorated cognitive impairments and decreased serum corticosteroid level in the stressed mice(P<0.01). Conclusion ATS improves cognitive deficits provoked by chronic stress in mice ,which may attribute to decreasing plasma corticosteroid levels.
6.Effects of L-glutamine-supplemented parenteral nutritional support on immunity and inflammatory response in post-surgical patients with lung cancer

Objective To investigate the effects of L-glutamine-supplemented parenteral nutritional sup- port on nutritional status, immunity, and inflammatory response in patients with lung cancer following radical opera- tion. Methods Forty-two malnourished patients with lung malignancies who had undergone radical operations were randomly divided into PN plus group (n =22) and PN group (n = 20). From day 1 to day 5 after operations, parenteral nutrients with the non-proteinic calorfic value of 90 kJ · kg-1 d-1 and nitrogen 0.25 g · kg-1 d-1 were given. From day 2, liquid diets were provided to both groups, and L-glutamine particles (30 g) were provided to PN plus group. Peripheral blood specimens were collected before operation, on day 1, and on day 7 after the oper- ations to determine the serum levels of albumin, prealbumin, transferrin, T lymphocyte subgroup 3 (CD3), CD4, CD8, CD4/CD8, immunoglobulin A (IgA), IgG, IgM, and C-reactive protein. Results The levels of the serum transferrin [(2.29±0.34) vs. (2.15±0.42) g/L], prealbumin [ (0. 27±0.76) vs. (0.23±0. 84) g/L], CD4 (30.9%±2.2% vs. 28.9%±2. 3% ), CD4/CD8 (1.17±0.19 vs. 1.10±0. 81), IgG [(12. 94 2.08) vs. (11. 19±1. 95) g/L] were all significantly higher in the PN plus group than those in PN group on day 7 (P <0. 05) . The level of serum C-reactive protein was significantly lower in PN plus group than that in PN group on day 7 [(31.6±16.7) vs. (36.5±13.3) mg/L, P<0.05]. Conclusion Postoperative L-glutamine-supplemented parenteral nutrition support can improve the nutritional status and immune function and reduce the acute inflammatory response in patients with lung cancer.
7.Investigation of the antireflux effect of esophagogastrostomy combined with modified Nissen fundoplication after esophageal middle cancer resection

Objective To investigate the antireflux effect of esophagogastrostomy combined with modified Nissen fundoplication after esophageal middle cancer resection.Methods Eighty-two patients with esophageal middle cancer were divided into 2 groups by random digits table method,41 cases underwent esophagogastrostomy with a stapler only (control group),41 cases underwent esophagogastrostomy with stapler combined with modified Nissen undoplication (observation group).Three months after surgery,the patients of the 2 groups completed the research of EORTC QLQ-C30 questionnaire,and examined with esophageal manometry and gastroscopy.Results There was no statistical difference in incidence of postoperative complication between the 2 groups (P > 0.05),death was not found during peroperative period.The scores of heart burn and regurgitation in observation group [(13.2 ± 6.1) and (16.9 ± 3.9) scores] were significantly lower that those in control group [(25.6 ± 7.2) and (26.6 ± 4.2) scores],there were statistical differences (P < 0.05),but there was no statistical difference in score of dysphagia between the 2 groups (P > 0.05).The resting pressure of anastomotic site in observation group [(3.5 ± 2.3) mmHg,1 mmHg =0.133 kPa] was significantly higher than that in the stomach [(2.7 ± 2.1) mmHg],there was statistical difference (P< 0.05).The DeMeester score was (54 ± 32) scores,control group was (141 t 84) scores,there was statistical difference (P < 0.05).The incidence of reflux esophagitis in observation group was 48.8%(20/41),in control group was 75.6%(31/41),there was statistical difference (P < 0.05).Conclusions Esophagogastrostomy combined with modified Nissen fundoplication after esophageal middle cancer resection significantly increases the pressure at the anastomotic site,thus reduces the extent of gastroesophageal reflux,which leads to the reduction of the extent of reflux esophagitis and the improvement of the quality of life.
8.Construction and screening of cDNA library of osteosarcoma cells.

OBJECTIVETo construct a cDNA library of osteosarcoma SAOS-2 cells and screen the proteins interacting with the bone morphogenetic protein-2 (BMP-2) to explore the role of BMP-2 in the development of prostate cancer.
METHODSThe total RNA was extracted from SAOS-2 cells, from which poly(A)(+)RNA was purified to generate the cDNA using RT-PCR with primer SMARTIII and primer CDSIIIoligo(dT). The library was constructed and screened by cotransformation of the double-stranded cDNA (dscDNA) and linearized pGADT7-Rec with the bait plasmid to the yeast AH109. The proteins interacting with BMP-2 were screened by AH109 mating with the bait strain Y187, and the interaction was confirmed using far-Western blot analysis.
RESULTSThe cDNA library of human osteosarcoma SAOS-2 cells was constructed, which was characterized by high diversity and sufficient capacity. The dscDNA size range was between 250-5000 bp, with a cotransformation efficiency of 4.3 x 10(5) and recombination efficiency of 1.9 x 10(6), and 4.3 x 10(5) clones were screened. The mating efficiency was 32% and 1.0 x 10(6) clones were screened by the bait of BMP-2, and 4 positive clones was obtained and sequenced.
CONCLUSIONThe diversity and capacity of the cDNA library meet the needs for screening genes related to prostate cancer.

9.Callus induction of Cinnamonum camphora and formation of borneol.

OBJECTIVETo optimize the condition of callus of Cinnamonum camphora induced.
METHODGC and plant tissue culture method were applied in the study.
RESULTThe effect of callus induced and the growth of callus were different in MS medium with different proportion of hormone. The ration of callus induced was the highest and the growth of callus was the most prosperous in the MS medium with 4 mg x L(-1) 2,4-D and 0.2 mg x L(-1) 6-BA. It is found that callus induced by young leaf contained borneol, but callus induced by young stem not.
CONCLUSIONThe optimization of callus of C. camphora induced is using the MS medium with 4 mg x L(-1) 2,4-D + 0.2 mg x L(-1) 6-BA. Callus induced by young leaf can generate borneol.

10.Clinical practice of video-assisted thoracoscopic surgery in children.

OBJECTIVE To estimate the value of video-assisted thoracoscopic surgery (VATS) in diagnosis and treatment of children with chest diseases. METHODS From May 1997 to October 2001, forty-one children (25 boys and 16 girls) with chest diseases received VATS under general anesthesia in our hospital. Their average age was 6.9 years (range 9 days to 16 years) and their median body weight was 22.5 kg (2.8-54.0 kg). Operative procedures included fibrinopurulent empyema with debridement and decortication in 15 children, biopsy and(or) resection of mediastinal tumor in 11, bullectomy and cystectomy of the lung in 6, lobectomy with huge cyst of the lung or sequestration in 5, clearance of hemothorax in 2, and exploration, and repair of diaphragmatic hernia in 2. RESULTS The mean operative time was 74 minutes (range 30 to 220 minutes). The lost blood volume was 33 ml (range 10 to 150 ml). The mean duration of chest drainage and hospital stay after surgery was 2.4 days and 7.0 days (range 4 to 15 days) respectively. One infant born after 9 days with congenital diaphragmatocele died of respiratory failure due to left pulmonary hypoplasia 10 days after operation. Postoperative morbidity was 7.3% (3 patients). Forty patients were followed up for an average of 15.6 months, 38 patients lived and developed normally, and 2 received chemotherapy. CONCLUSION Video-assisted thoracoscopy is a safe and effective diagnostic and therapeutic procedure for children with chest disease, and this approach has an important place in pediatric thoracic surgery.
