1.CLINICAL ANALYSIS OF SEVERE ACUTE RESPIRATORY SYNDROME

Objective To explore the clinical features of severe acute respiratory syndrome (SARS). Methods Clinical data of 70 patients with SARS were retrospectively analyzed. Results The age of the patients ranged from 10 to 74 years old (mean age 33.81?12.73 years). Severe type of SARS was diagnosed in 38 patients and 32 as common type of SARS. The most common symptoms included fever(98.57%), malaise(80.00%), cough(74.28%), shortness of breath (55.71%), headache(38.57%). The fever lasted for 7.20?4.57d. Based on the symptoms and chest X-ray examination, the disease was classified into early stage, progressive stage, and recovery stage with a mean duration of 3.19?2.32d, 8.18?4.36d, and 8.82?7.01d, respectively. It was found that the incidence of severe type of SARS was higher with the increase of age. Compared with common type of SARS, the fever in patients with severe type of SARS was more difficult to control (the mean highest fever 38.74?0.97℃, 38.02?0.14℃, respectively). The duration of fever after treatment (4.12?3.19d, 2.37?1.75d, respectively) was longer in the illness course (27.78?9.99d, 14.93?5.80d, respectively). The ALT, LDH and HBDH in patients with severe type of SARS were higher than those in patients with common type of SARS (ALT:78.95%,50.00%, respectively; LDH, HBDH:42.11%, 18.75%, respectively). Advanced age, pre-existing chronic disease, and elevation of LDH and HBDH were the factors of poor prognosis. Combination therapy was the first choice for the treatment of SARS. Empirical use of glucocorticoid was useful in preventing progression of the disease. Conclusion Although severe type of SARS had a high mortality, combination therapy could be very effective in the treatment of the majority of patients.
2.Risk factors for development of early acute renal failure after liver transplantation in patients with normal renal function

Objective To determine the risk factors for development of early acute renal failure (ARF) after orthotopic liver transplantation (OLT) in patients with normal renal function.Methods Sixty ASA Ⅱ or Ⅲ patients aged 28-64 yr weighing 35-88 kg undergoing OLT were studied.Their preoperative serum Cr and BUN were within normal range.Early ARF was defined as serum Cr≥132 μmol/L and/or BUN≥18 mmol/L within 24 h after operation.The patients were divided into 2 groups: ARF group and non-ARF group.Arterial blood samples and urine specimens were collected before induction of anesthesia for determination of blood β2-micreglobulin(β2-MG) and urinary β2-MG and N-acetyl-β-D-glucurenidnse (NAG). Factors including preoperative liver function,preoperative blood and urinary β2-MG,the amount of urine output and bank blood infused during operation,MAP during anhepatic and neohepatic phase,the amount of vnsoactive drugs and diuretics used during operation,hypotension and arrbythmia during operation were recorded.The risk factors were identified by logistic regression analysis.Results Logistic analysis indicated that serum β2-MG higher than normal value before operation and persistent hypotensien during operation were closely correlated with development of early ARF after OLT.Conclusion Serum β2-MG higher than the normal value before operation.and persistent hypotension during operation are the risk factors for early ARF after OLT.
3.Feasibility of transjugular liver biopsy in minipigs

Objective To evaluate the feasibility and safety of transjugular liver biopsy( TJLB) by using the LABS 200 liver access and biopsy set ( Cook Inc, USA) .Methods Five minipigs were operated though TJLB puncture under the imaging guidance.The liver biopsies were analyzed by histological examination.Results Technical success of TJLB was achieved in all the 5 minipigs.No procedure-related complications occurred, and sufficient amount of specimen for histological examination was obtained in all cases.Conclusions Our preliminary results indicate that transjugular liver biopsy with the use of Cook LABS 200 liver access and biopsy set is clinically safe and feasible, and provide technical support for its clinical application.
4.Network target: a starting point for traditional Chinese medicine network pharmacology.

Understanding the interactions between numerous chemical compounds of herbs or herbal formulae and complex biological systems related with diseases or traditional Chinese medicine (TCM) syndromes is one of great dilemmas in current studies on TCM. To address such a difficult issue, we propose a novel concept and methodology of "Network Target" based on our previous works and from the perspective of network pharmacology as well as systems biology. The network target treats a disease-specific biomolecular network as a target to help design and predict the best possible treatments. Focused on mapping disease phenotypes and herbal compounds into biomolecular networks and then calculating, analyzing and predicting the mechanism of their mutual interactions, the network target approaches will facilitate discovery of effective compounds and their combinations, elucidation of mechanistic relationships between herbal formulae and diseases or TCM syndromes, and development of rational drug designs for TCM. In this paper, our recent progresses on the methodology of Network Target and its applications in herbal medicine are reviewed to provide reference for the coming TCM network pharmacology.

5.Effect of dexmedetomidine on quality of recovery from anesthesia in patients undergoing modified electroconvulsive therapy with propofol anesthesia

Objective To evaluate the effect of dexmedetomidine on the quality of recovery from anesthesia in the patients undergoing modified electroconvulsive therapy (MECT) with propofol anesthesia.Methods One hundred and ten patients of both sexes,aged 18-50 yr,weighing 45-80 kg,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective MECT with general anesthesia,were randomly assigned into 2 groups (n =55 each) using a random number table:control group (group C) and dexmedetomidine group (group D).Dexmedetomidine was infused intravenously in a dose of 0.5 μg/kg (in normal saline 10 ml) over 10 min in group D,while normal saline 10 ml was infused intravenously over 10 min in group C.Propofol 1.5 mg/kg and succinylcholine 0.5 mg/kg were injected intravenously,and MECT was performed in the two groups.The emergence time was recorded.The development of cardiovascular events,nausea and vomiting,respiratory depression,headache,somnolence and agitation during recovery from anesthesia was recorded.Results Compared with group C,the incidence of nausea and vomiting,headache and agitation during recovery from anesthesia was significantly decreased (P<0.05),and no significant changes were found in the emergence time,and incidence of hypertension,tachycardia,respiratory depression and somnolence during recovery from anesthesia in group D (P> 0.05).Conclusion Dexmedetomidine (intravenously infused in a dose of 0.5 μg/kg over 10 min before anesthesia) can raise the quality of recovery from anesthesia in the patients undergoing MECT with propofol anesthesia.
6.A clinical study of piezosurgery osteotomy in lateral sinus floor augmentation

Objective:To evaluate the clinical effect and safety of piezoelectric osteotomy in lateral sinus floor augmentation.Meth-ods:53 implants were placed in 30 sites after lateral sinus floor augmentation(LSFA)in 29 patients by piezoelectric osteotomy.47 implants were placed simultaneously with LSFA,6 were delayed.6-8 months after implantation,the final fixed prostheses were per-formed.The patients were followed-up at 3,6,and 12th months after restoration.Results:The mean residual bone height(RBH) was 4.46 mm before operation.The mean height of sinus elevation was 7.34 mm after operation.No membrane perforation occurred. Peri-implantitis,peri-implant mucositis and implant mobility were not found during follow-up.X-ray examination showed good os-seointegration in the interface between the implants and bones.The implant survival rate and the operation success rate were 100%. Conclusion:The application of piezosurgery is safe and reliable in lateral sinus floor augmentation.
7.Preparation of ~(99)Tc~m-ASON-EGF and its pharmacokinetic features in rabbits

Objective To prepare and explore the pharmacokinetic parameters for ~ 99 Tc~ m -ASON-EGF in healthy rabbits. Methods ~ 99 Tc~ m -ASON-EGF was prepared according to previous methods and its changes of concentration in blood were measured by radioactivity counts per minute. The experimental data were dealt with by 3p97 software and its true compartment model was estimated by AIC value, R~ 2 value, the 1/c and F test. Subsequently, its half-life of distribution (T_ 1/2 ?), half-life of elimination (T_ 1/2 ?), central compartment volume of distribution (Vc), total apparent volume of distribution (Vd) and total rate of clearance (CL) were calculated by the software. Finally, the binding rate of plasma protein was determined by trichloroacetic acid precipitation. Results The best model of ~ 99 Tc~ m -ASON-EGF in vivo was two-compartment model and its T_ 1/2 ?, T_ 1/2 ?, Vc, Vd and CL were 5.28 min, 89.23 min, 67.8 ml, 915.6 ml and 7.1 ml/min respectively. After being incubated with fresh plasma for 1.5 h, its binding rate was 10.69%. Conclusion Its process of transportation in healthy rabbits is fitted to two-compartment model and the pharmacokinetic properties are desirable.
8.Observation of the morphological changes of corneal nerve in experimental herpes simplex keratitis in rabbit eyes

Objective To observe the morphological changes of corneal nerve in experimental herpes simplex keratitis (HSK) in rabbit eyes. Methods Inoculation of the scarified cornea with herpes simplex virus-Ⅰ(HSV-Ⅰ)leads to herpetic infection of cornea. The HSK was detected by slit lamp and polymerase chain reaction(PCR), and the changes of corneal nerves in various periods with methylene blue vital staining and ultrastructure of corneal nerve were studied. Results Under light microscope, fewer corneal nerve bundles were observed, scattered with lower density of nerve fiber at center of cornea on the 4th day after the infection. At days 7 and 14, the seriously damaged nerve fiber was intermittent. The neuraxon became shorter with little polarity and the density of nerve fiber was extremely low. At day 30, the density was still low while the nerve fiber was approximately normal. Under electromicroscope, at day 4, the lamellar sheath of nerve fiber in the epitheliums appeared intermittent, and the neuroplasm of endings was partly lysed. During day 7 to day 14,neuroplasm was damaged and became vacuolar. The mitochondria swelled with vacuolar crest, and then were destroyed and lysed. The nerves in stroma were also injured. On day 30, neurilemma sheath was still intermittent with the decrease of microfilament and racuole. Conclusion HSK leads to the damage of nerve and the decrease of nerval density, and the damage of corneal nerve repairs very slowly.
9.Experimental Study on Effects of Endothelin and Xuesaitong Injection on Hepatic, Renal and Myocardial Tissues in Obstructive Jaundice

Objective To study the effects of endothelin (ET) and Xuesaitong injection on hepatic, renal and myocardial tissues after bile duct ligation (BDL) in rabbits. Methods Seventy-two rabbits were randomly divided into three groups: BDL group (24 rabbits), BDL+Xuesaitong injection group (24 rabbits), and sham operation group (24 rabbits). Each group was subdivided into four subgroups of postoperative 3, 6, 9 and 12 d (6 rabbits in each subgroup). Automatic biochemical analysis equipment was used to detect the levels of serum TBIL, ALT, BUN and Crea. The levels of ET in plasma, hepatic, renal and myocardial tissues were measured with radioimmunological method. 10.Impact of continuous nursing based on WeChat Public Platform and Sojump on fluid exchange in patients with peritoneal dialysis

Objective:To explore the effect of continuing care based on the WeChat public platform questionnaire star on the fluid replacement operation of peritoneal dialysis patients.Methods:Convenience sampling method was used to select 210 patients undergoing peritoneal dialysis in the Panyu Central Hospital, Guangzhou from January to June 2019. They were divided into a experimental group and a control group by random number table method, each with 105 cases. The patients of the control group were given routine continuous care for peritoneal dialysis, such as outpatient follow-up, telephone follow-up, home visit, etc., and oral education. On this basis, in the experimental group, the patients first joined the WeChat public platform, implementing network-based health education and questionnaire stars for real-time testing, and multi-dimensional communication with patients. The knowledge of the peritoneal dialysis fluid replacement operation, the fluid replacement operation evaluation score, the catheter outlet replacement drug evaluation score, the fluid replacement operation self-management score and compliance were compared before and after intervention in the two groups. And the incidence of complications related to the peritoneal dialysis fluid exchange operation of the two groups were compared before and after intervention.Results:The experimental group′s knowledge of peritoneal dialysis fluid exchange operation, examination score of fluid exchange operation, and abnormal situation management score of fluid exchange operation management were 55.88±8.97, 92(89, 96), 12(11, 12) after three-month intervention, and higher than the control group [51.88±10.24, 91 (86, 94), 12 (9, 12)], with statistically significant difference ( t value was 3.00, Z value was -2.483, -2.308, P<0.05). After six-month intervention, the relevant knowledge of peritoneal dialysis fluid replacement operation, fluid replacement operation evaluation score, catheter outlet replacement drug evaluation score and fluid replacement operation management score (routine fluid replacement operation, abnormal situation handling) of the experimental group were 57.52±9.41, 93 (90,96), 90 (87,94), 18 (17,18), 12 (11,12), which were higher than the control group [53.61±10.29, 90 (86,94), 90 (87,92), 17 (15,17), 12 (9,12)], with statistically significant difference ( t value was 2.87, Z value was from -7.181 to -2.664, P<0.01). The difference between the groups was statistically significant ( χ2 value was 4.21-7.517, all P<0.05); and the difference between the intervention time points was statistically significant ( χ2 value was 11.805-164.95, all P<0.05). After six-month intervention, the complications of catheter infection, peritonitis, and poor drainage were 9 (8.91%), 5 (4.95%), and 5 (4.95%) in the experimental group, which were significantly lower than those in the control group [21 (20.39%), 14 (13.59%), 15 (14.56%)], with statistically significant differences ( χ2 value was 5.36, 4.51, 5.33, both P<0.05). Conclusions:The continuous care of the WeChat public platform questionnaire star has a significant promotion effect on the improvement of the knowledge, operation ability, compliance and self-management behavior of the peritoneal dialysis patients, and can effectively reduce the complications caused by the peritoneal dialysis operation.
