1.The first-line treatment regimens of peripheral T-cell lymphomas

Peripheral T-cell lymphomas (PTCLs) are a kind of non-Hodgkin's lymphoma (NHL),which arise from heterogeneous mature T-lymphocyte and include a variety of different subtypes.Compared with B-cell lymphoma,PTCLs are characterized by a lower incidence,resistance to conventional chemotherapy,widespread dissemination,a higher recurrence rate and unfavorable prognosis.At present,the standard first-line treatment regimens have not yet been developed for PTCLs.Though,there are many studies and trials about new drugs and novel intensive regimens,which improve the clinical curative effects and prognosis of PTCL patients significantly.In this paper,the progress of first-line treatment regimens of patients with different PTCL subtypes was reviewed.
2.Role of platelet activation and changes of platelet parameters in the pathogenesia of cerebral infarction

Conclusions Patients with cerebral infarction show evidence of enhanced platelet activation and increasment of MPV and MAR,which may relate to the pathological process of cerebral infarction. Moreover MPV and MAR can reflect the clinical severity of cerebral infarction better than PLT. Anti-platelet aggregation treatment should be further taken in patients with cerebral infarction.
3.Effects of FS-1272 Gutta on Anti-oxidative System in Lens of Rats with Cataract Induced by D-galactose

[Objective] To discuss mechanism of FS-1272 eye drops on cataract of rats induced by D-galactose.[Method]By using the model of cataract of rats induced by D-galactose to determine the content of SOD,CAT,GSH-px,MDA in lens.[Result]FS-1272 eye drops can increase the activities of SOD,CAT,GSH-px and decrease the content of MDA.[Conclusion]FS-1272 eye drops have the effect of delaying the generation and development of cataract by alleviating the extent of oxidative injury of lens.

Objective To compare the blood-saving effect when acute hypervolemic hemodilution (AHH) was performed with hydroxyethyl starch (HES) 130/0.4 dissolved in electrolyte injection (HES-E) and HES 130/0.4 in sodium chloride injection (HES-NaCl).Methods Thirty patients of both sexes,aged 18-60 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,with body mass index of 18-25 kg/m2,hemoglobin (Hb) >100 g/L,hematocrit (Hct) > 35%,scheduled for elective abdominal operations under general anesthesia,were randomly divided into HES-E group and HES-NaCl group using a random number table,with 15 patients in each group.AHH was performed after induction of anesthesia.In HES-E and HES-NaCl groups,HES-E and HES-NaCl 15 ml/kg were intravenously infused over 30 min,respectively,and the infusion was conpleted before skin incision.Immediately after onset of AHH (T1),at 2 h after the end of AHH (T2),and at the end of operation (T3),arterial blood samples were collected for blood gas analysis and blood routine test,and pH value,base excess,HCO3-,K+,Na+,Cl-,Ca2+,Hb and Hct were recorded.Venous blood samples were collected at T1 and T2 for measurement of blood coagulation parameters including prothrombin time,activated partial thromboplastin time and fibrinogen and thrombelastography parameters.The volume of liquid intake and output and requirement for allogeneic blood transfusion were recorded,and the blood volume expansion rate was calculated.Results Compared with group HES-NaCl,no significant changes were found in the total volume of liquid infused,requirement for allogeneic blood transfusion,blood volume expansion rate,blood coagulation parameters at each time point,Hb and Hct (P>0.05),pH value,base excess,HCO3 and K+ were significantly increased,and Na+ and Cl-were significantly decreased in group HES-E (P<0.01).Conclusion There is no significant difference in the blood-saving effect between AHH with HES-E and HES-NaCl clinically,but HES-E can maintain homeostasis better.
6.Effects of Platelet-activating Factor on Endothelial Ceil Monolayer Permeability under Hydrostatic Perfusion in Vitro

We established a method to study vascular permeability in vitro on perfused endothelial cell monolayer cultured on micropore filter membrane. It can be used to determine filtration coefficient (Kf) and osmotic reflection coefficient (?) to proteins. Hanks balanced salt solution (HBSS) or 5 g/L albumin in HBSS was used to perfuse confluent endothelial monolayer. Control Kf values were 10.1 ?0.75 and 3.6?0.75?l . min-1 . cm-2 . kPa-1 (n = 3, x?sx) respectively for HBSS and albumin HBSS perfusion, suggesting that albumin may decrease endothelial monolayer permeability to water and small molecules. After exposure of endothelial monolayer to 10-8 mol/L platelet-activating factor (PAF) for 30 min, Kf values increased to 193.1% and 133.3% respectively for HBSS and albumin HBSS perfusion. Protein clearance rate (?l. min-1 . cm-2:) and osmotic reflection coefficient of control endothelial monolayer were 8.0?3.22 and 0. 37?0.09 respectively. In PAF treated endothelial monolayer,they were 12.2 ? 2.95ul min-1 cm 2 and 0.18?0.06, revealing increased permeability to albumin. Computer-assisted image processing demonstrated that PAF treatment decreased cell area while increased cell form factor and intercellular space. The results sug-gest that endothelial cells retracted, rounded and it may be an important mechanism in PAF-induced increased vascular permeability.
7.Biothermokinetic studies on four properties of traditional Chinese materia medica——Comparison of different preparation properties of Coptidis Rhizoma by microcalorimetry

Object To probe into the objectivity and authenticity of four properties (cold, heat, warm, and cool) of raw Coptidis Rhizoma (RCR) and their preparations from biophysics and biochemistry. Methods Microcalorimetry was used to obtain the metabolic growth power-time curve of bacteria affected by the total alkaloid in different preparations of RCR and biothermokinetic parameters. Combined with the records of ancient herbal literatures, the influence of properties by different preparations to RCR were synthetically evaluated. Results All the effective total alkaloid in different preparations of RCR can restrain the Escherichia coli growth and metabolism in different level. Drugs, such as Coptidis Rhizoma Stir-fried with ginger juice (CRGJ), Coptidis Rhizoma stir-fried with wine (CRW), and Coptidis Rhizoma stir-fried with Fructus Evodiae (CRFE), with a little Warm Property can decrease the multiplication rate constants of E. coli index and increase heat output in growth metablism remarkably, while the drugs, such as RCR, Coptidis Rhizoma stir-fried with vinegar (CRV), and Coptidis Rhizoma stir-fried with gallbladder juice (CRGBJ), with a little Cold Property can decrease the multiplication rate constants and increase heat output a little. There was a stable difference between them. Conclusion Microcalorimetry is a new and useful mean for the study of the properties of tradition Chinese medicine.
8.The changes of respiratory muscle strength by traditional Chinese medicine combined with cholinesterase inhibitors in myasthenia gravis patients

Objective To observe the changes of respiratory musc]e strength by traditional Chinese medicine combined with cholinesterase inhibitors in myasthenia gravis (MG) patients.Methods Thirty-four cholinesterase inhibitor-resistant patients,of them 14 were MG patients with stage Ⅰ ,and 20 were stage Ⅱ ,were treated with bromide dimethylcarbamate ( 360-480 mg/d ).Traditional Chinese potion were administered in those without effectiveness,and the dosages of bromide dimethylcarbamate decreased with Traditional Chinese potion lasting for 4-6 months.Vital capacity ( VC ),maximal voluntary ventilation ( MVV ),maximal inspiratory pressure ( PIM ),maximal expiratory pressure ( PEM ),respiratory centre driving pressure ( P0.1 ),residual volume ( RV )were measured before and after treatment.Results The amelioration of VC,MVV,PIM,PEM,P0.1 ,RV,respiratory muscle strength and other indicators of 34 MG patients were not obviously after treatment with cholinesterase inhibitor alone ( P > 0.05 ).After treatment with traditional Chinese medicine combined with cholinesterase inhibitors,VC,MVV,PIM,PEM ( before treatment:76.66% ± 18.59%,68.03 % ± 10.45 %,43.25 % ± 18.16%,21.75 % ±14.44% ) increased significantly in all 34 MG patients( after treatment:86.91% ± 14.87% ,75.11% ± 11.17%,52.66% ±20.32% ,28.56% ± 10.06% ) ( P < 0.05).RV decreased from 164.94% ± 67.97% to 143.16% ±79.21% (P <0.01 ),and respiratory muscle strength,endurance and other indicators significantly improved (P <0.01).PIM(65.80% ±28.03% to 52.66% ±20.32%),and PEM (37.03% ±20.57% to 28.56% ±10.06%)improved more significantly in group stage than in group stage (P <0.01 ).Respiratory muscle endurance in stage Ⅰpatients ( 108.71% ± 17.56% ) improved significantly than stage Ⅱ patients (96.01% ± 14.12% ,P < 0.01 ).Conclusions Traditional Chinese medicine combined with cholinesterase inhibitors could effectively improve the lung function and respiratory muscle strength in patients with resistance of the cholinesterase inhibitors.The improvement of lung function,respiratory muscle strength were more obviously in stage Ⅰ patients than in stage Ⅱ patients.Respiratory muscle strength and endurance were improved greater in stage Ⅱ than in stage Ⅰ patients.
9.A CBCT study of alveolar bone structure around incisors of the patients with verical facial type of skeletal Class Ⅱ

Objective:To investigate the structure of alveolar bone around incisors in patients with verical facial type of skeletal ClassⅡ by CBCT.Methods:60 skeletal Class Ⅱ patients were divided into low angle,average angle and high angle cases(n =20)by FMA.CBCT scanning was made and the thickness of the labial and lingual alveolar bone around the incisors in each subject was meas-ured.The measurements were analyzed using ANOVA.Results:The total width of alveolar bone at the root apical level of maxillary and mandibular incisors,the lingual thickness of alveolar bone at the root midpoint of maxillary incisors and labial thickness of alveolar bone at the root midpoint of mandibular incisors in the high angle cases were thinner than that in the average and low angle cases(P <0.05),but there was no significant difference in labial thickness of alveolar bone at the crest among 3 groups.Conclusion:The alveo-lar bone thickness around incisors of high angle patients is thinner,more attention should be paid for the alveolar bone absorpation and tooth root exposure in these patients.
10.Value of three-dimensional volume-rendering CT pulmonary contusion volume measurement in prediction of ARDS development

Objective To investigate the value of three-dimensional CT volume rendering in predicting ARDS following pulmonary contusion and identifying high-risk patients.Methods Seventy-one cases of pulmonary contusion (AIS > 2 points) confirmed by chest CT during an emergency admission between July 2010 and June 2011 were enrolled.Using computer-generated three-dimensional reconstruction,contusion volume was measured and expressed as a percentage of total lung volume.The admission data,such as blood gas analysis results,systolic arterial pressure,hematocrit,AIS,ISS,and injury distribution,were prospectively collected.Independent predictive factors of ARDS following pulmonary contusion was identified using logistic regression analysis and further estimation on accuracy and value of the predictors were performed.Influence of contusion volume percentage on clinical outcomes was detected.Results Of all,mean contusion volume percentage was (22.07 ± 14.50)% (range,5.60%-61.00%),which was not strongly correlated to the admission PaO2/FiO2 ratio (R2 =0.059).ARDS and infection were diagnosed in 31 cases and 25 cases respectively.PaO2/FiO2 ratio and contusion volume percentage were independent predictive factors of ARDS after pulmonary contusion.The best cut-off of contusion percentage in predicting ARDS development was 21.5% with a specificity of 80.0%,sensitivity of 71.0%,positive predictive value of 73.3%,and negative predictive value of 78.1%.Conclusion Three-dimensional CT volume rendering technique allows quantification of pulmonary contusion and identification of patients at high risk of ARDS,to whom further treatment may be directed.
