1.Study on correlation between serum HCV-RNA level and Anti-HCV with liver fibrosis indexes in patients with chronic hepatitis C

Objective To investigate the correlation between serum HCV-RNA level and anti-HCV with liver fibrosis indexes in the patients with chronic hepatitis C.Methods The serum samples were collected in 96 cases of chronic hepatitis C.The HCV-RNA level was determined by real time fluorescence quantification PCR and Anti-HCV was detected by ELISA.LN,PCⅢ and CⅣlevels were detected by the electrochemical iuminescence analyzer and HA was examined by radioimmunoassay(RIA).The correla-tion between the HCV-RNA and Anti-HCV levels with the liver fibrosis indexes was analyzed with the statistic software.Results Among these 96 cases of Anti-HCV positive chronic hepatitis C,52 cases (54.2%)were HCV-RNA positive.With increasing of the S/CO value of Anti-HCV,the detection rate of HCV-RNA was increased,which were 9.5%,37.1% and 92.5%.The differ-ences in HA,LN,PCⅢ and CⅣ levels between the high viral load group and the low viral load group had no statistical significance (P >0.05).Conclusion The positive detection rate of HCV-RNA is related with the S/CO value of Anti-HCV,the higher the S/CO value,the higher the positive detection rate of HCV-RNA.But the HCV-RNA level has no correlation with the liver fibrosis in-dexes.
2.Research on effect of clopidogrel on platelet VASP phosphorylation level after PCI operation

Objective To use the flow cytometry to detect the platelet vasodilator-stimulated phosphoprotein(VASP)phospho-rylation level and to evaluate the clopidogrel curative effect after PCI.Methods 17 cases in the control group without any drug in-tervention and 26 cases of acute coronary syndrome(ACS)after PCI operation with clopidogrel were selected.Platelet VASP phos-phorylation levels at being selecting and on 7 d after anti-platelet therapy were detected by the flow cytometry and the platelet reac-tivity index (PRI)was calculated.Results The PRI after anti-platelet therapy in the ACS group was decreased significantly,the difference between before treatment and after treatment had statistical significance (P <0.05 ).Conclusion The platelet VASP phosphorylation level detected by the flow cytometry can specifically evaluate the effect of clopidogrel.
3.Efficacy of locking plate fixation with anti-osteoporosis drug treatment of older patients with proximal humeral fractures

Objective To investigate the effect of the locking plate fixation with anti-osteoporosis drug in the treatment of older patients with proximal humeral fractures. Methods Sixty-two patients with proximal humeral fractures who were treated in Yuzhou Mining Company Hospital of Kailuan Group of Hebei Province from Feb. 2011 to Oct. 2013 were divided into observation group(n=30)and control group(n=32). Patients in both groups were given locking plate fixation,and patients in the observer group were also received anti-osteoporosis treatment. The information including hospitalization periods,efficacy and bone mineral density of the two groups was recorded. Results The hospitalization periods of the observation group and control group were (6. 9 ± 1. 0)d,(9. 5 ± 1. 1)d,and the difference was statistically significant(t=8. 426,P﹤0. 05). The efficacy in observer group was excellent in 17 cases,good in 10 cases,fair in 2 cases and poor in 1 case,and the efficacy of the control group was excellent in 14 cases,good in 10 cases,fair in 5 cases and poor in 3 cases. The difference was statistically significant(χ2 =4. 617,P ﹤0. 05 ). After the anti-osteoporosis treatment,the bone mineral density of the observer group and control group were( 0. 79 ± 0. 13)g/cm2 and(0. 66 ± 0. 05)g/cm, the difference was statistically significant( t =10. 251,P ﹤0. 05 ). Conclusion The effect of locking plate fixation with anti-osteoporosis drug is better with shorter hospital stay,safe and reliable,and it is worthy of clinical application.
4.Estimation of the postmortem interval by determining nuclear DNA degradation from bone marrows and brains in rats

Objective To detect nuclear DNA degradation of bone marrows and brains in rat cadavers at different temperatures,and develop a new parameter for estimating early postmortem interval(PMI).Methods The brain and bone marrow were taken out for every 4h,during 0~40h after death at 10℃ and 20℃,respectively.And the single cell gel electrophoresis(SCGE) was carried out to detect the nuclear DNA degradation.Linear regression analysis was used to assay the relationship of the comet parameter HeadDNA%,Tail Length(TL) and Olive TailMoment(TM) with PMI.Results Different decline degrees of comet HeadDNA% were found in both brain cells and bone marrow cells after death,the decline of HeadDNA% in brain cells at 20℃ was faster.Compared with degradation in marrow cells,the linear relation between degradation of brain cells and PMI was better.Conclusion with that of comet parameters TL and TM,the perfect linear relationship between HeadDNA% and PMI was also observed.Conclusion Brain tissues are more suitable for PMI estimation by detecting degradation of DNA with SCGE.The HeadDNA% is more valuable for PMI estimation than TL and TM.
5.Expressions of EphA2 and EphrinA1 in bladder carcinomas and their clinical significance

Objective To investigate the expressions of EphA2 and EphrinA1 in bladder carcinomas and their clinical significance. Method The expressions of EphA2 and EphrinA1 were detected on bladder carcinomas (75 cases ) by immunohistochemistry S-P method. Results ( 1 ) The positive rates of expression of EphA2 and EphrinA1 were 61.3%(46/75) and 46.7%(35/75) respectively. (2) The expression of EphA2 was related to tumor size, lymphatic metastasis and TNM staging(P< 0.05),but not to gender,age and pathology type (P > 0.05 ). ( 3 )The expression of EphrinA1 was related to tumor size and lymphatic metastasis (P < 0.05 ), but not to gender, age, pathology type and TNM staging (P > 0.05 ). (4)There was significantly positive correlation between the expression of EphA2 and EphrinA1 (P < 0.05).Conclusions EphA2 and EphrinA1 are related to the occurrence and development of bladder carcinoma.The combined detection of EphA2 and EphrinA1 can help to predict the clinical and pathologic characteristics of bladder carcinoma. It is an important index to assist treatment, they are predictive factors for bladder carcinoma.
6.The changes of respiratory muscle strength by traditional Chinese medicine combined with cholinesterase inhibitors in myasthenia gravis patients

Objective To observe the changes of respiratory musc]e strength by traditional Chinese medicine combined with cholinesterase inhibitors in myasthenia gravis (MG) patients.Methods Thirty-four cholinesterase inhibitor-resistant patients,of them 14 were MG patients with stage Ⅰ ,and 20 were stage Ⅱ ,were treated with bromide dimethylcarbamate ( 360-480 mg/d ).Traditional Chinese potion were administered in those without effectiveness,and the dosages of bromide dimethylcarbamate decreased with Traditional Chinese potion lasting for 4-6 months.Vital capacity ( VC ),maximal voluntary ventilation ( MVV ),maximal inspiratory pressure ( PIM ),maximal expiratory pressure ( PEM ),respiratory centre driving pressure ( P0.1 ),residual volume ( RV )were measured before and after treatment.Results The amelioration of VC,MVV,PIM,PEM,P0.1 ,RV,respiratory muscle strength and other indicators of 34 MG patients were not obviously after treatment with cholinesterase inhibitor alone ( P > 0.05 ).After treatment with traditional Chinese medicine combined with cholinesterase inhibitors,VC,MVV,PIM,PEM ( before treatment:76.66% ± 18.59%,68.03 % ± 10.45 %,43.25 % ± 18.16%,21.75 % ±14.44% ) increased significantly in all 34 MG patients( after treatment:86.91% ± 14.87% ,75.11% ± 11.17%,52.66% ±20.32% ,28.56% ± 10.06% ) ( P < 0.05).RV decreased from 164.94% ± 67.97% to 143.16% ±79.21% (P <0.01 ),and respiratory muscle strength,endurance and other indicators significantly improved (P <0.01).PIM(65.80% ±28.03% to 52.66% ±20.32%),and PEM (37.03% ±20.57% to 28.56% ±10.06%)improved more significantly in group stage than in group stage (P <0.01 ).Respiratory muscle endurance in stage Ⅰpatients ( 108.71% ± 17.56% ) improved significantly than stage Ⅱ patients (96.01% ± 14.12% ,P < 0.01 ).Conclusions Traditional Chinese medicine combined with cholinesterase inhibitors could effectively improve the lung function and respiratory muscle strength in patients with resistance of the cholinesterase inhibitors.The improvement of lung function,respiratory muscle strength were more obviously in stage Ⅰ patients than in stage Ⅱ patients.Respiratory muscle strength and endurance were improved greater in stage Ⅱ than in stage Ⅰ patients.
7.Research and discussion on criteria quantization of scholarship of clinical postgraduates

Objective To discuss the effects of applying comprehensive evaluation of quantitative index to choose scholarship of clinical postgraduates.Methods Totally 259 clinical postgraduates of Beijing Friendship Hospital affiliated to Capital Medical University,who took part in the choosing of scholarship during 2008-2011 were enrolled.During the graduating year,comprehensive evaluation of quantitative index was adopted to choose scholarship through appraisal on postgraduates.Results were described as percentage and were analyzed by SPSS 15.0.Results Seventy-seven postgraduates won the scholarship (during 2008-2011),among which there were 20 doctors (26.0%),57 masters (74.0%).Thirteen persons won the first scholarship,26 persons the second scholarship,38 persons the third scholarship.Distribution of the majors of the winner was appropriate.Conclusions Comprehensive evaluation of quantitative index is effective in choosing scholarship for clinical postgraduates and it is beneficial to guide clinical postgraduates to improve their comprehensive quality.
8.Identification of Loganin Metabolites in Rats by UPLC-Q-TOF-MS/MS

Objective To identify the metabolites of loganin in rat urine through using ultra high performance liquid chromatography combined with triple TOF mass spectrometry (UPLC-Q-TOF/MS/MS) after oral administration of loganin;To summarize the metabolic pathway of loganin. Methods Rats were given a gavage with loganin. The urine samples were prepared by use of solid phase extraction (SPE). The chromatographic separation was performed on an UPLC C18 column with gradient elution program of acetonitrile and 0.1% formic acid aqueous as mobile phase. Dynamic background subtraction (DBS) technology was employed to trigger information dependent acquisition (IDA) to obtain the characteristic fragment ions of metabolites. Results MetabolitePilot software including several data processing methods were employed to speculate the structures of liganin metabolites and three metabolites were identified. Then, the metabolic pathway of loganin in rats was summarized. Conclusion The analytical method was simple, credible and highly-sensible, which is useful for screening and identifying trace constituents in vivo.
9.Study on Anticoagulant Activity in Vivo of Active Component F2-2 from Eupolyphaga seu Steleophaga

Prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), plasma fibrino-gen (FIB), platelet aggregation rate and blood clots-fibrinolytic dynamic figure were taken as indexes in the evalua-tion of anticoagulant activity in vivo of active component F2-2 from Eupolyphaga seu Steleophaga. After 5 days of hypodermic injection of adrenaline, the rat model of acute blood stasis was established. Indexes were determined af-ter the model rats were treated with an intragastric administration of F2-2 for 9 days. The results showed that com-pared with the model group, PT/APTT was prolonged, FIB content was decreased, platelet aggregation rate and the largest of blood coagulation were declined after 9 days of intragastric administration in the model group. However, there was no difference on TT. It was concluded that the anticoagulant component F2-2 separated from E. seu Steleophaga showed favorable anticoagulant activity in vivo. However, its mechanism remained unknown and request-ed further researches.
10.Value of three-dimensional volume-rendering CT pulmonary contusion volume measurement in prediction of ARDS development

Objective To investigate the value of three-dimensional CT volume rendering in predicting ARDS following pulmonary contusion and identifying high-risk patients.Methods Seventy-one cases of pulmonary contusion (AIS > 2 points) confirmed by chest CT during an emergency admission between July 2010 and June 2011 were enrolled.Using computer-generated three-dimensional reconstruction,contusion volume was measured and expressed as a percentage of total lung volume.The admission data,such as blood gas analysis results,systolic arterial pressure,hematocrit,AIS,ISS,and injury distribution,were prospectively collected.Independent predictive factors of ARDS following pulmonary contusion was identified using logistic regression analysis and further estimation on accuracy and value of the predictors were performed.Influence of contusion volume percentage on clinical outcomes was detected.Results Of all,mean contusion volume percentage was (22.07 ± 14.50)% (range,5.60%-61.00%),which was not strongly correlated to the admission PaO2/FiO2 ratio (R2 =0.059).ARDS and infection were diagnosed in 31 cases and 25 cases respectively.PaO2/FiO2 ratio and contusion volume percentage were independent predictive factors of ARDS after pulmonary contusion.The best cut-off of contusion percentage in predicting ARDS development was 21.5% with a specificity of 80.0%,sensitivity of 71.0%,positive predictive value of 73.3%,and negative predictive value of 78.1%.Conclusion Three-dimensional CT volume rendering technique allows quantification of pulmonary contusion and identification of patients at high risk of ARDS,to whom further treatment may be directed.
