Chinese Pharmacological Bulletin 1987;0(03):-
A bolus injection of endotoxin and platelet activating factor ( PAF ) via external jugular vein in rats resulted in severe pulmonary edema, accompanied by significant increase in extravascular lung water volume, bronchoalveolar lavage fluid ( BALF ) protein concentration, BALF cell counts and polymorphonuclearcyte percentage. PAF receptor antagonist SRI63-441 showed partial protective effects on endotoxin and PAF induced lung edema and BALF cytological changes.

The concentration of 1.8 ng/ml platelet activating factor (PAF) caused the elevation of pulmonary vascular permeability and a steady increase of lung weight gain (LWG) in isolated perfused rat lung. The LWG, fluid filtration coefficient and extravasation amount of T-1824 labelled albumin were lower of 89. 2%, 70. 9% and 75. 5%, respectively, in PAF perfusion without Ca2+ group than those in PAF perfusion with Ca2+ group, within 35 min. Mean pulmonary capillary pressure and pulmonary arterial pressure decreased from + 0.725 ? 0.389 kPa and +1.187 ? 0.320 kPa to -0.708 ? 0.315 kPa and - 0.025 ? 0.279 kPa, respectively. Calcium free perfusate without PAF slightly increased vascular permeability in isolated rat lungs. The results suggest that PAF-induced rat lung injury depends on the existence of extracellular calcium ion.

Sixty two patients with small bowel bleeding underwent double balloon endoscopy (DBE).Clinical features were compared between patients with positive DBE findings (50 cases) and those with negative findings (12 cases). Results revealed that the causes of melena were discovered in 75.0%(21/28) patients via oral approach, while the lesions were found in 81.8% (18/22) patients with hematochezia via anal approach ( P < 0.01 ). Duration of bleeding was longer and number of bleeding episodes was higher in patients with positive DBE findings than those with negative findings (P < 0.05 or P < 0.01 ).The time interval between the latest bleeding and DBE was significantly shorter in patients with positive DBE findings (P < 0.05).Positive diagnostic rate was significantly higher in patients with three or more bleeding episodes than those less than three times (92.5% vs.11.1%,P < 0.01 ).DBE performed within seven days or less increased the positive diagnostic rate significantly than that after seven days (97.1% vs.59.3%,P<0.01).

Intravenous administration of E. coli endotoxin (2mg/kg) caused a persistent increase in platelet activating factor (PAF) contents in arterial blood and bronchoalveolar fluid (BALF). Meanwhile phospholipase A2(PLA2) activity in serum and BALF was elevated and mean arterial blood pressure declined. The total cell number, expecially polymorphonuclear leukocytes and lymphocytes, and protein concentration in BALF were markedly increased 6 h after endotoxin injection. Lung index and extravascular lung water content of endotoxin-treated animals were significantly highter than those of controls. Pretreatment with PAF receptor antagonist SRI 63-441 blocked the increase in PLA, activity and attenuated endotoxin-induced hypotension and acute lung injury. The results suggest that PAF mediates endotoxin-induced lung injury, and leukocyte activation by PAF and the subsequent release of oxygen metabolites and lysoenzymes are important intermediate mechanisms leading to high permeability pulmonary edema.

Chinese Journal of Clinical Oncology 2015;(6):336-340
Objective:To investigate the influence of chemotherapy on the phenotype of secreted protein, acidic and rich in cysteine (SPARC) in gastric cancer (GC). Methods:Immunohistochemistry was used to analyze SPARC expression in 132 GC patients. Among these patients, 54 with preoperative chemotherapy and 78 without preoperative chemotherapy were selected to analyze the effect of chemotherapy on SPARC phenotype by comparing the postoperative specimens of the two cohorts. Results:SPARC expression was higher in GC lesions than in the desmoplastic stroma surrounding the tumor cells and noncancerous tissues. High SPARC expression was related to invasion depth, lymph node metastasis, and TNM staging. SPARC expression was lower in patients with preoperative chemotherapy than in controls ( P<0.05). Gross type, histology, depth of invasion, lymph node metastasis, TNM staging, and SPARC phenotype correlated with the overall survival of the patients with preoperative chemotherapy. Further multivariate analysis suggested that lymph node metastasis, histology, and SPARC phenotype after chemotherapy were independent prognostic indicators of GC. Conclusion:SPARC expression was associated with invasion depth, lymph node metastasis, TNM staging and GC prognosis. Preoperative chemotherapy may change the phenotype of SPARC in GC patients.

Objective To explore the correlation between ultrasonographic findings and relevant technical factors in traumatic lipohemarthrosis. Methods Fifty-two patients with traumatic lipohemarthrosis underwent ultrasonic examination in supine position (26 in unbent, 16 in external rotary position, 10 in unbent and 45° rotation) before operation, and 22 of them underwent dynamic examination. Findings of ultrasography were compared with manifestations of MRI and CT regarded as gold standard. Results Fifty-two patients with traumatic lipohemarthrosis were diagnosed correctly. The best effect of ultrasonic examination of lipohemarthrosis was observed on suprapatellar bursa. Superior border of patella were showed well in supine straight position, then turned to the level of patella, and the structure below patella were showed badly. The best posture was in supine genuflex position with 45° rotation when the joint effusion presented, by which thicker layer liquids could be displayed than in straight position. Three layer liquids could not be displayed synchronously in mild external rotary rest position of lower extremity, and the combination of findings at internal and external side were favorable to the diagnosis. Liquids bed changed from one layer to two layers and then to three layers, from fuzzy to clear gradually in dynamic examination. Conclusion Ultrasonic manifestations of traumatic lipohemarthrosis are closely correlated to the position, posture and arresting time during examination.
