1.Microanatomy and histological study of the fascicular group to deltoid muscle in axillary nerve at the level of quadrilateral zone

Objective To observe and study the distribution and histological feature of the fascicular groups to deltoid muscle in axillary nerve at the level of quadrilateral zone. Methods 12 adult human cadavers (24 upper limbs)embalmed by formalin were used,, and microsurgical longitudinal contradissection of the axillary nerves by tracing their terminal branches were processed. At the level of quadrilateral zone,distrabution of the fscicular groups to deltoid muscle in the never trunk were recorded and their diameters were measured;Another 6 upper extremities (3 right and 3 left) of fresh-frozen human cadavers were obtained.Axillary nerves and their fascicular groups were exposured, and the segments at the level of qusdrilateral zone gotten, then histological cross section was done, acetycholinesterase (ACHE) stained with the method of Karnovsky Roots and myelin stained with the method of Loyezs. Defferent never fibres were distinguished under microscope and the fiber number was counted with IAS . Results At the level of quadrilateral zone,fasciculars were divided into two groups.The fascicular group of anterior branch to deltoid muscle was in the lateral of the nerve trunk . Its cross sectional area was (2.449?1.327)mm2 ,occupied 55.4%?9.3%of the axillary nerve trunk ;Its most fibers were motor fibers;The number of fibers is (2112?631), occupied 45.6 %?1.1%of all fibers of axillary nerve . Conclusions In the treatment of root avulsion of brachial plexus injury, selective suture donor nerve with the lateral fascicular group should be processed to repair the function of shoulder abduction,fibers loss would be reduced and rate of functional restoration improved .
2.Clinical and endoscopic analysis of hepatogenic ulcer hemorrhage

Objective To summarize the clinical symptoms and endoscopic features of hepatogenic ulcer hemorrhage so as to strengthen the recognition of this disease.Methods The clinical data of 42 patients with hepatogenic ulcer hemorrhage from January 2000 to May 2005 were analyzed retrospectively.Results The clinical symptoms were multiple,including melena in 42 cases(100%),hematemesis in 30 cases(71.4%),abdominal distention in 35 cases(83.3%),abdominal pain in 9 cases(21.4%) and acid regurgitation in 18 cases(42.9%).Emergency endoscopy could find active hemorrhage of stomach or duodenum and ulcer lesions with blood scab;The ulcer shape was round or ellipse.The stage under endoscopy all was A_1,17 patients combined with portal hypertensive gastropathy.35 patients was homeostasis after medical conservative treatment,the effective rate was 83.3%,7 patients needed surgical operation.Conclusion Hepatogenic ulcer hemorrhage is not rare,it is an important factor of upper gastrointestinal bleeding in liver cirrhosis.Emergency endoscopy can make a correct diagnosis in time and guide to choose the best treatment planning.

4.Operative treatment of complex talar body fracture through approach of internal or external malleolus osteotomy and Absorbable screw for fixation

[Objective]To introduce an operative treatment of complex talar body fracture and discuss the therapeutic effect in the near future.[Method]From March 1999 to January 2003,8 patients of complex talar body fracture in our department were treated.There were 5 males and 3 females with an average age of 28.9 years from 19 years to 46 years.The injuries were caused by fall in 3 cases,traffic accident in 3 cases,crash in 1 case,and on left 5 cases,right 3 cases.Two cases were combined with malleolus fracture and 1 case with open fracture and dislocation.All patients were treated with operation through approach of internal or external malleolus osteotomy and useing Absorbable screw for fixation.[Result]All the patients were followed up 1 to 3 years,average 1.3 year.By the assessment of clinical effects dependingon the complaints,functions and radiological results of the ankle and subtalar joint,4 patients were rated as being excellent,2 patients good,1 fair and 1 poor,the total rate of being excellent and good was 75%.No postoperative avascular necrosis of the talar was seen on radiographies.[Conclusion]Operation through approach of internal or external malleolus osteotomy and Absorbable scrow for fixation is a good method for treatment of complex talar body fracture.
5.EFFECT OF L-THP ON CONTRACTION OF ISOLATION GUINEA PIG TRACHEA INDUCED BY SEVERAL AGONISTS

The antagonistic effects of L-THP, Ver and Pap to contraction of isolated guinea pig trachea preparations induced by BaCl2 10-3mol/L ,KC1 3?10-2mol/L, 5-HT 2?10-5mol/L, Ach 2?10-5mol/L and His 2? 10-5mol/L were studied.The antagonistic effects of L-THP were different from Ver and Pap. The IC50 value of L-THP for contraction induced by BaCl2, KC1, 5-HT and His was 3 .5? 10-5mol/L , 4.5 ? 10-5mol/L , 2.5?10-3 mol/L and 8.9? 10-5mol/L respectively. It was showed that sensibility of several agonists to inhibition by L-THP was correlated with dependence of extracellular Ca2+. The potencies of L-THP on anti-KCl and anti-5-HT actions were similar. The results suggested that LTHP possesses anti-calcium and anti-5-HT action on guinea pig trachea.
6.Three-dimensional imaging reconstruction of the ulnar nerve functional fascicular groups

Objective To reconstruct on the basis of histological study the computerized 3D ima ges of the fascicular groups of ulnar nerve,which can be used to guide the microsurg ical repair of injuries of brachial plexus and ulnar nerve and to provide morpholog ical evidence for the functional rec overy of intrinsic muscles of hand after root avulsion of brachial plexus.Methods The successive AchE-staining histo logical cross-sections were made.T he photos of the sections were transformed into digital information and sto red in the computer.Different functional fas-cicular groups were distinguished a nd marked on each section.The topographic database was analyzed by image pro-cessing software,with the support o f OpenGL three dimensional graphic l ibrary.3D images of the fascicular g roups of ulnar nerve were reconstructed usin g the contour-based surface constru ction method in the program written w ith VC language.Results The 3D images of the individual external surface of the reconstructive fa scicular groups were displayed and the human-computer in teraction was realized by performin g the arbitrary rotation and the rand om cross-section.By selecting the int erested object,the spatial structu re of any fascicular group could be di splayed in the same space simultaneously.Compare d with the result of microanatomy,th e 3D shape could truly reflect the str ucture,adjacent relationship and arrangem ent of fascicular groups in the ulnar nerve trunk.Conclusions Since the re-constructive 3D images can truly reflect the arrangement and distribution of the functional fascicular grou ps,they will have positive effects on understanding the internal structure of ulnar nerve,designing operative procedur e and adjusting operative method for surgeons.[
7.Seperatable Nylon Ring Ligation Treatment Under Endoscope for Esophageal Varices Hemorrhage

Objectives To explore the effect and advantages of seperatable nylon ring ligation under endoscope for esophageal varices hemorrhage.Methods 24 cases of esophageal varices hemorrhage accepted ligation treatment under endoscope guide by using Olympus separatable nylon ring ligation apparatus. The effect was recorded. Results The average ligation points on one case were 10 2. The emergency hemostatic rate hemorrhage from esophageal varices repture was 100%. The disappearance rate of esophageal varices was 73 7%. No severe complication was occurred during operation.Conclusions Seperatable nylon ring ligation treatment under endoscope is a new safe and effective method for esophageal varices hemorrhage. This method is simple and cheap.so it has valuable for spreading use in clinic.
8.Microanatomic study on arrangement and distribution of the fascicular groups in ulnar nerve trunk to the intrinsic muscles

Objective To observe the arrangement and distribution of the fascicular groups in the ulnar nerve trunk to the intrinsic muscles so as to provide anatomical evidence for selective repair in the clinic.Methods 12 cadaveric upper extremities recently embalmed with formalin were obtained. The ulnar nerve and its branches were exposed. Microanatomy and longitudinal contradissection of each functional fascicular group were performed by tracing their terminal branches, and the arrangement and distribution were observed and recorded. The fascicular groups to the intrinsic muscles of hand in the nerve trunk were observed to see how they crossed and mixed with other fascicular groups. The diameter of the nerve trunk and the fascicles to the intrinsic muscles were measured, and the area and the distribution range of the fascicles calculated. Results The shape and the diameter of the ulnar nerve trunk varied at different positions. The fibers of functional fascicular groups had the definite differentiation at distal while interfused and diffused at proximal. The fascicular groups to the intrinsic muscles of hand were the largest and their fibers relatively concentrated (the distribution range occupied 40% ~ 80% of the ulnar nerve), and had the definite localization at the forearm and distal to the 3/8 segment of upper arm. Conclusion Since the fascicles to the intrinsic muscles of the hand are the most important in the ulnar nerve, selective repair by using epineural and perineural techniques at the forearm and upper arm will reduce the loss of nerve fibers and will contribute to the functional recovery.
9.High-frequency ultrasound in the assessment of tendon injury and repair: study protocol for a self-controlled animal trial

BACKGROUND: Tendon injury is common in clinic, which is mainly treated by surgical anastomosis. Postoperative tendon healing is usually assessed through surgeons' experience due to high cost and application restrictions of MRI examination. Thus there is still a lack of a convenient and objective imaging support. With the advancement and widespread application of high-frequency ultrasound, the diagnosis rate of tendon injury has been improved remarkably; thereafter, high-frequency ultrasound used for assessing tendon injury and repair has become an issuehas become an issue of concern.OBJECTIVE: To clarify the ultrasonic imaging features of tendon repair through high-frequency ultrasound scancombined with histological examination.METHODS: This was a single-central, preoperative and self-controlled animal experiment and finished in the Central People'sHospital of Siping, China. 130 adult male Highbrow chickens were selected and were then randomized into 13 groups (n=10per group). One side of each chicken hind foot was randomly selected as experimental limb to undergo achillotomy followedby repair using the modified Kessler method (groups 2-13) or no treatment (group 1); the contralateral limb served as control.Moreover, passive flexion-extension functional training targeting the experimental limbs was performed in the groups 8-13beginning at the 1st day after surgical anastomosis, several times a day. The high-frequency ultrasound and hematoxylineosinstaining were conducted before and after chillotomy (group 1), and at 3 (groups 2 and 8), 7 (groups 3 and 9), 14 (groups4 and 10), 21 (groups 5 and 11), 35 (groups 6 and 12) and 42 (groups 7 and 13) days after surgical anastomosis, respectively.RESULTS AND CONCLUSION: The primary measurement outcomes were the repair and healing of the injured tendonas assessed by high-frequency ultrasound; the secondary outcomes were the pathological manifestations of the injuredtendon detected by hematoxylin-eosin staining. Our findings will provide preclinical proof for high-frequency ultrasounduse in the assessment of tendon injury, repair and healing as well as for the rehabilitation therapy that promotes functionrecovery in the future.
10.Clinical Observation of Recombinant Human Brain Natriuretic Peptide on Acute Decompensated Heart Failure

OBJECTIVE:To observe therapeutic efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in the treatment of acute decompensated heart failure (ADHF). METHODS:129 ADHF patients admitted in our hospital during Jul. 2013-Jul. 2015 were randomly divided into observation group(69 cases)and control group(60 cases). Control group was giv-en routine treatment. Observation group was additionally given rhBNP 1.5 μg/(kg·d)vein shock(d1),and then rhBNP 1 mg+5%Glucose injection 100 ml by 0.007 5 μg/(kg·min),ivgtt(d2-4). Treatment courses of 2 groups lasted for 7 d. Therapeutic efficacy and heart function indexes [left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVDd)] were observed in 2 groups as well as NT-proBNP level,medication fluid intake and urine volume 24 h after medication. The occurrence of ADR was observed. RESULTS:3 cases dropped out and 66 cases were included finally in doservation group. Total effective rate of AD-HF therapy in observation group (96.97%) was significantly higher than in control group (86.67%),with statistical significance (P<0.05). There was no statistical significance in LVEF,LVDd and NT-proBNP between 2 groups before treatment(P>0.05). Af-ter treatment,LVEF of 2 groups were increased significantly while LVDd were decreased significantly;LVEF of observation group [(42.3±3.6)%] was significantly higher than in control group[(37.9±3.3)%],while LVDd of observation group [(55.5± 3.6)%] was significantly lower than in control group[(61.3 ± 3.4)%]. NT-proBNP levels of 2 groups were decreased significant-ly,and the observation group [(1 389.5±29.6)pg/ml] was significantly lower than the control group [(2 778.0±26.8)pg/ml]. 24 h after medication,urine volume of 2 groups were significantly higher than medication fluid intake,and urine volume of observation group [(1 781.4 ± 89.7)ml] was significantly higher than that of control group [(1 372.6 ± 78.3)ml],with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between observation group (3.03%) and control group (5.00%),without statistical significance(P>0.05). CONCLUSIONS:rhBNP is significantly effective for ADHF,promotes heart function recovery and reduces renal function injury with good safety.
