1.Clinical study of super-early operation combined with traditional Chuanxiongqin on treatment of hypertensive intracerebral hemorrhage

Objective To explore an effective treatment for hypertensive intracerebral hemorrhage.Methods By the method of random and control,patients with hypertensive intracerebral hemorrhage were randomly divided into two groups:the treatment group(32 patients)was treated with integrated traditional Chinese and western medicine therapy,including super-early operation,conventional western medicine and Chuanxiongqin injection treatment.The control group(40 patients)was treated with operation and conventional western medicine treatment.The effect was evaluated on 28 th day after treatment.Results The effect of the treatment group was superior to that of the control group(χ~2=4.15,3.26,P<0.05).The treatment group had lower mortality rate(χ~2=8.04,P<0.05)and lower morbidity of complications(pulmonary infection:6/16 cases,χ~2=11.37,P=0.01;upper-congestive hemorrhage:8/16 cases,χ~2=4.10,P=0.04)statistical data indicated that there was significant difference between treatment group and control group.Conclusion Super-early operation with traditional Chinese medicine in treatment of hypertensive intracerebral henorrhage has a better effect than the treatment without traditional Chinese medicine.
2.Resistance mechanism of Mycoplasma hominis to quinolones in Chengdu area

Objective To detect the resistance of Mycoplasma hominis to quinolones in Chengdu area,explore resistance mechanism of topoisomerase gene gyrA, gyrB, parC and parE mutations associated with drug resistance and provide epidemiological data.Methods Mycoplasma hominis was identified by 16SrRNA gene sequencing technique and antibiotic susceptibility test was carried out by broth microdilution method.Resistance genes were amplified by PCR,whereas sequence alignment was analyzed by DNAMAN software and BLAST.Results Resistance rates of Mycoplasma hominis to ciprofloxacin, levofloxacin, moxifloxacin and gatifloxacin were 92.4%(61/66),87.9%(58/66),71.2%(47/66)and 66.7%(44/66),respectively.Totally 45 strains with different susceptibility to quinolones were screened for amplification and sequencing of topoisomerase genes, of which, 31 strains resistant to moxifloxacin and gatifloxacin harbored GyrA S153L amino acid mutation, 68.9%(31/45), 41 strains resistant to ciprofloxacin and levofloxacin harbored ParC S91I amino acid mutation, 91.1%(41/45).In addition, a new amino acid substitution of ParE A463S was found in 2 high-level resistant strains.No amino acid change was found in GyrB.Conclusions Resistance of Mycoplasma hominis to quinolones is closely associated with amino acid changes caused by mutations in gyrA and parC genes.Different quinolones have different targeting roles and high level resistance is associated with multiple gene mutations.
3.Stress status and causes of male nurses in women and children′s hospital of Southwest China

Objective To investigate the pressure of male nurses in women and children′s Hospital and analyze it, so as to provide decision support for nursing administrators. Methods Used general information questionnaire, Perceptual Stress Scale, China Nurses'Pressure Source Scale (CPSS) in Sichuan,Chongqing,Yunnan,Guizhou,Tibet and other places of the maternal and child hospital male nurses, and analyzed the results. Results According to the CPSS scoring standard,63 in 101 subjects(62.4%)male nurses were in the state of stress, and the average score of stress was 26.0 ± 7.4. Binary Logistic regression showed that the main factors of male nurses′pressure were the lack of support from parents and family,low income, less promotion opportunities and non-nursing work. Conclusion The pressure situation of male nurses in maternity and child care hospitalis affected by family, work, society and so on. Managers should timely relieve the stress for male nurses and maintain the stability of male nurses′team.
4.Efficacy of single-anastomosis duodenoileal bypass as revision surgery following sleeve gastrectomy

Objective:To evaluate the short-term efficacy of single-anastomosis duodenoileal bypass as revision surgery following sleeve gastrectomy.Methods:The clinical data of 51 patients who underwent single-anastomosis duodenoileal bypass as revision surgery after sleeve gastrectomy in our center from Jan 2020 to Jan 2023 were retrospectively analyzed.Results:A total of 51 patients underwent single-anastomosis duodenoileal bypass at 47 (34, 60) months after sleeve gastrectomy. All patients completed single-anastomosis duodenoileal bypass surgery by laparoscopic approach without conversion to laparotomy. The operative time was (122.8±37.4) min, the intraoperative bleeding volumes was (24.2±13.5) ml, and the length of hospital stay was 6 (5, 6) d. One year after surgery, the patient's weight decreased from (98.5±16.2) kg to (69.8±8.2) kg, BMI decreased from (34.8±3.9) kg/m 2 to (24.7±2.1) kg/m 2, excess weight loss increased from 44.0%± 11.7% to 97.1%±18.1%, systolic blood pressure decreased from (147.6±17.2) mmHg to (121.7±7.5) mmHg, diastolic blood pressure decreased from (84.2±11.5) mmHg to (76.0±12.1) mmHg, fasting blood glucose decreased from (7.1±1.8) mmol/L to (4.6±0.4) mmol/L, triglycerides decreased from (1.7±1.1) mmol/L to (0.7±0.2) mmol/L, cholesterol decreased from (5.9±0.4 ) mmol/L to (3.9±0.4) mmol/L, LDL decreased from (3.7±0.5) mmol/L to (2.3±0.2) mmol/L, HDL increased from (1.1±0.3) mmol/L to (1.5±0.7) mmol/L, and uric acid decreased from (347.2±90.1) μmol/L to (270.9±59.6) μmol/L, the difference were statistically significant ( F=55.9, 114.1, 158.7, 54.3, 3.1, 59.3, 24.2, 91.5, 180.3, 8.2, 9.8, all P<0.05). Conclusion:As revision surgery of sleeve gastrectomy, single-anastomosis duodenoileal bypass is feasible and effective, significantly reducing weight and obesity complications.
5.Analysis of the Development Status of Neuropsychiatric Therapeutic Drug Monitoring in Terms of the External Quality Evaluation

OBJECTIVE: To analyze the development status of neuropsychotropic therapeutic drug monitoring (TDM), and to provide reference for improving the quality of TDM in neuropsychiatry department. METHODS: The laboratory data of neuropsychotropic TDM external quality assessment (EQA) were collected from Website of Clinical Laboratory Center of Ministry of Health (http://www.clinet.com.cn) during 2008-2017, and then analyzed statistically in respects of TDM project, the number of involved laboratory, monitoring method, etc. RESULTS: By the end of 2017, the Clinical Laboratory Center of the Ministry of Health had conducted 5 neuropsychiatric TDM projects involving 4 drugs, including 3 items of serum TDM (carbamazepine, valproic acid, phenytoin), lithium ion monitoring (lithium carbonate) in biochemical routine and newly added pharmacokinetic laboratory biological sample monitoring (carbamazepine) in 2014. From 2008 to 2017, the number of involved laboratories increased from 209 to 603, with carbamazepine (serum, plasma) and valproic acid showing good annual growth trend, exceeding 200 and phenytoin 107, but only 27 in lithium carbonate until 2017. Among all monitoring methods of involved laboratories, the number of laboratories which adopted fluorescence polarization immunoassay (FPIA) decreased year by year. By the end of 2017, there were fewer than 10 laboratories. The number of laboratories using acridinium direct chemiluminescence, enzyme-multiplied immunoassay technique (EMIT), and high performance liquid chromatography (HPLC) had steadily increased. By the end of 2017, acridinium direct chemiluminescence was the most used in laboratory, followed by EMIT and HPLC. CONCLUSIONS: Due to the increasing number of participating laboratories, it can be seen that the consciousness of quality control of laboratory personnel is increasing day by day; but the complexity of TDM monitoring methods limits the types of drugs used in external quality evaluation. It is suggested to formulate guidelines, quality control standards and operation rules as soon as possible, so as to ensure the accuracy of test results.
