1.Clinical research of small doses penehyclidine hydrochloride in painless colonoscopy

Objective To discuss the application value of small doses penehyclidine hydrochloride in painless colonoscopy examination. Methods From July 2012 to January 2013, 90 patients who planed to have painless colonoscopy were randomly divided into 3 groups with each group 30 cases. Before anesthesia, giving small doses of penehyclidine hydrochloride to small-dose group, giving conventional dose of penehyclidine hydrochloride to routine-dose group, and giving atropine intravenous administration to atropine group. Observe the smoothly de-gree, and check the heart rate, mean arterial pressure and postoperative adverse reactions of three groups. Results There was no obvious difference among the three groups in trems of smooth degree and oxyhemoglobin saturation. In small-dose group, heart rate and mean arterial pressure before and after the examination were better than that of routine-dose group and atropine group, and there were obviously lower inci-dence of postoperative adverse reaction compared with the routine-dose group and atropine group. Conclusion Giving small-dose of penehy-clidine hydrochloride before anesthesia of painless colonoscopy examination can significantly improve the effect of examination, and it can re-duce the incidence of postoperative adverse reactions.
2.Expression of pro-inflammatory cytokine mRNA after spinal cord injury and effect of the methylprednisolone in rats

ObjectiveTo investigate the expression of pro inflammatory cytokine mRNA including interleukin 1β (IL-1β), interleukin 6 (IL-6) and tumor necrosis factor-α(TNF-α) after spinal cord injury (SCI), and the effect of methylprednisolone (MP) on the expression of the cytokine and neurological outcome after SCI in rats.MethodsA moderate SCI model was made with SD rats according to modified Allen's method. 56 SD rats were divided randomly into the sham group (n=4), SCI group (n=24), saline group (n=14) and MP group (n=14, MP 30 mg/kg injected throuh tail vein 30 min after injury). Samples of injured spinal cord were taken at 1 h, 4 h, 12 h, 24 h, 72 h and 7 day after SCI. The mRNA expressions of IL-1β,IL-6 and TNF-α in the spinal cord of rats were measured by semi-quantitative RT-PCR at 1 h, 4 h, 12 h, 24 h, 72 h and 7 d post-injury, and 4 h post-operation in the control group. The effect of MP (30 mg/kg, iv, within 30 minutes post-injury) on the expression of cytokine at 4 h post-injury was tested with RT-PCR. The neurological outcome of rats was evaluated with BBB scores.ResultsThe expression of the cytokine mRNA was little in the sham group and significantly increased in injured groups with IL-1β at 12 h, TNF-αat 4 h reaching peak after injury,and higher than that in sham group until 72 h (P<0.05). While, the expressions of IL-1β and TNF -α mRNA were inhibited obviously in MP group at 4 h after injury, but MP had no effect on the neurological outcomes.ConclusionMP can inhibit inflammatory reaction obviously.

Objective To observe the level changes of serum interleukin-18(IL-18) and tumor necrosis factor-?(TNF-?) in alcohol liver disease of rats.Methods The dose of 56% alcohol [5~9 g/(kg?d)] was administeredvia gastrolavage once daily for 12 weeks in ALD model rats.The control rats were grven the same volume of saline.The rats were killed at the end of 4,8,12W.The pathological changes of liver were observed under light microscope after HE staining,and the levels of IL-18 and TNF-? in serum was determined with ELISA.Results The tissues of model rats showed various changes of chronic alcohol liver disease at the end of 4,8,12W,such as: fatty degeneration,inflammatory changes and fibrosis.The levels of ALT and AST in models were obviously higher than those of the controls(P

BACKGROUND: Hepatocyte transplantation can improve biochemical parameters and survival of animals with acute liver failure. However, cell sources, immunological rejection, distribution and histomorphological alternation of transplanted hepatocytes are important issues for its wide application in clinic. OBJECTIVE: To observe early-stage histomorphological and ultrastructural changes of SV40LT antigen gene transfected hepatocytes after transplanted into rat spleen. DESIGN, TIME AND SETTING: Randomized controlled animal trial was performed at Laboratory of National Hepatobiliary and Enteric Surgery Research Center, Ministry of Health from March to December 2001. MATERIALS: Sixty male Wistar rats were selected for hepatocyte transplantation. METHODS: Sixty rats were randomly divided into 4 groups (n=15). Primary cell group and primary cell plus cyclosporine A group were intrasplenically transplanted with primarily cultured hepatocytes; SV40LT antigen gene group and SV40LT antigen gene plus cyclosporine A group were intrasplenic transplanted with SV40LT antigen gene transfected hepatocytes. Twenty-four hours before and 14 days after transplantation, the rats in primary cell group and SV40LTor antigen gene group were injected with 0.5 mL normal saline through tail vein daily, while the other groups were injected with cyclosporine A (10 mg/kg per day). MAIN OUTCOME MEASURES: The spleen of one rat was harvested every day postoperatively for light microscopic and electron microscopic examinations to observe survival rate, histomorphological and ultrastructural features of transplanted hepatocytes for 14 days. RESULTS: Compared with primary cell group and SV40LT antigen gene group, the survival rate of transplanted hepatocytes in the other groups was significantly increased (P 0.05), while the survival rate in SV40LT antigen gene transfected hepatocytes was significantly higher than primarily cultured hepatocytes 8-14 days postoperatively (P

Objective To research the blood supply changes of uterine and fibroid before and after UAE for uterine fibroids.Methods Sixteen patients with symptomatic uterine myoma were treated by Seldinger's bilateral uterine arteries embolization.The same level dynamic enhanced CT scans were undertaken 7 days before and after the procedure with another one 3 months later.The CT values of uterine and fibroid were measured and analyzed. Results 16 cases finished the same level dynamic CT enhancement scans successfully. The dynamic enhanced curves and the CT values showed no significant difference in uterine but had significant difference in fibroids. The fibroids were in a state of ischemia after the procedure.Conclusions Selective uterine arterial embolization is a new safety and effective method for treatment of uterine myoma.

7.Efficacy Analysis of Gujing Tang Combined with Paroxetine of Treatment of Premature Ejaculation for 60 Cases

[Objective] A randomized control ed trial of traditional Chinese medicine recipe Gujing Tang treatment of premature ejaculation was studied. [Methods] The subjects: 60 patients with premature ejaculation form department of andrologic outpatient, the patients were randomly divided into two groups, Integrated TCM-WM Groups and paroxetine Groups. 30 cases in each group. The Integrated TCM-WM Groups were given Gujing Tang , with paroxetine 10mg, oral, Qd at the same time; The paroxetine groups were given paroxetine 10mg, oral, Qd. Both groups were treated for 3 months. Ejaculation status was evaluated at the timepoint:before treatment, 3 months after treatment,and fol ow-up evaluation of six months after treatment. Evalu-ate parameter including Intravaginal ejaculation latency(IELT) and score of sexual function and Chinese patients with premature ejaculation evaluation -5 (CIPE-5). [Results] After 3 months, both groups were significantly improved, and Integrated TCM-WM Groups improved more significantly. After 6 months of fol ow-up, IELT, motion times and CIPE-5 score of Integrated TCM-WM Groups decreased slightly, but stil had significant difference com-pared to before treatment, and this parameter of Paroxetine groups was returned to the level of baseline, and had no significant difference compared with baseline. [Conclusion] Traditional Chinese medicine Recipe Gujing Tang combined with paroxetine, could not only have rapid onset, enhance patient confidence, improve the psychological state, but also improve the long-term efficacy and avoid recurrent disease after treatment, therefore can achieve ef-fective treatment of premature ejaculation.

9.Analysis and Comparison of MRI Diagnosis of Spinal Cavernous Hemangioma with Its Clinical and Pathology Features

Objective To study the connection between MRI characteristics of spinal cavernous hemangioma(SCH)and its clinical and pathological features.Methods 20 cases of SCH confirmed by pathology were retrospectively analysed.The characteristics of MRI signal intensity were analysed in comparison with the clinical manifestations and pathology.Results Among the 20 cases,5 lesions located in the cervical segment of spinal cord,12 lesions in the thoracic segment,2 in the lumbar segment and 1 in the thoracic-lumbar segement.SCH could be devided into four patterns:intramedullary lesion,epidural lesion,intradural-extramedulary lesion and vertebral lesion.The typical MRI characteristics of SCH were mixed signal intersity on T_1 or T_2 weighted images,low signal intensity ring could be seen around the lesions and more pronounced on T_2 images.Spinal cord compression syndrome was the main clinical manifestation.Cavernous hemangiomas were red or purple mulberg-like lesions which were composed of dilated,thin-walled vessel with a simple endothelial lining and a variably thin fibrous adventitia under the microscope.Conclusion MRI is the most reliable method in diagnosing SCH,MRI characteristics of signal intensity of SCH are correspond with their clinical and pathological features.
10.Clinical study of azo steen,montelukast sodium combined with levocetirizine in treatment of chronic urticaria

Objective To explore the clinical efficacy and value of azo steen,montelukast sodium combined with levocetirizine in the treatment of chronic urticaria. Methods 150 cases with chronic urticaria admitted in department of dermatology of Hubei Zhongshan Hospital were divided into A,B and C group,each group had 50 cases. Patients in group A were received montelukast sodium and levocetirizine treatment,which in group B were received montelukast sodium and azo steen treatment,group C were received above three kinds of drug treatment. Total effective rate,adverse reaction and safety in three groups were observed and compared. Results Total effective rate of group C was significantly higher than A and B group(P<0.05), and the safety of group C were better than A and B group(P<0.05 ),too. The occurrence of adverse reaction in group C was significantly less than that in group A and B (P <0.05 ).Conclusion Azo steen,montelukast sodium combined with levocetirizine has good efficacy in treatment of chronic urticaria. It can relieve itching wheals and other symptoms in a short time,with less adverse reaction.
