3.Clinical and imaging features and infarct pattern analysis in patients with middle cerebral artery stenosis and occlusion

ObjectiveTo evaluate the clinical and imaging features and infarct pattern in patients with middle cerebral artery(MCA) stenosis and occlusion,to clarify its infarct pathogenesis. Methods35 MCA moderate and severe stenosis or occlusion patients diagnosed by TCD were studied;the clinical and imaging features and infarct pattern were retrospectively analyzed. Results( 1 ) Clinical features :The clinical manifestations of 35 patients was hemiplegia symptoms,in which 16 cases(45.7% ) involved to the face,upper and lower limb,11 cases(31.4% ) involved to the upper and lower limb,3 cases(8.6% ) involved to the facial and upper limb,2 cases(5.7% ) involved only to the facial,2 cases(5.7% ) involved to the facial and upper limb,1 cases(2.9% ) involved only to the upper limb. (2) MRA imaging characteristics and infarct pattern: 29 cases ( 82.9% ) of patients with stenosis or occlusion showed moderate severe stenosis,6 cases( 17.1% ) showed MCA occlusion. 30 cases( 85.7% ) showed ischemic infarction,in which perforating artery infarction was the most common infarct pattern of middle cerebral artery stenosis or occlusive patients( 53.3% ) ,followed by water-shed infarct ( 26. 7) %, pial infarct was ielatively small (20. 0% ). ConclusionThe hemiplegia was the most common clinical symptoms in patients with MCA stenosis and occlusion, and mainly involves to the face,upper and lower limb;The perforating artery infarction was the most common infarct pattern,and its infarct mechanism was an artery-to-artery embolism or hemodynamic abnormalities.

Objective Glycodelin-A is one of glycoproteins secreted from endometrial epithelial cells , and it plays an im-portant role in embryo implantation , inhibition of reject reaction and pregnancy maintenance .Glycodelin-A will become an important potential marker in evaluating the endometrial receptivity and predicting the pregnancy outcome .The aim of this study was to investigate the correlation of glycodelin-A detected on the day of HCG and endometrial receptivity and the effects of glycodelin -A on the pregnancy outcome in the intrauterine insemination (IUI) period. Methods One hundred and seven woman patients with bilateral unobstruct-ed tubes and without endometrial lesions , endometrial polyps or intrauterine adhesions from Oct 2012 to Feb 2014 in our hospital were recruited in this study .Serum glycodelin-A in women receiving IUI on the day of HCG was measured by ELISA .Serum estradiol (E2), progesterone (P), and corpus luteum (LH) levels were measured by immunochemiluminometric assays .The condition of the endometrium was examined by transvaginalultrasonography . Results The serum glycodelin-A level on the day of HCG was higher in pregnant group [(1.47 ±0.38)ng/mL] than that in nonpregnant group ([0.62 ±0.13]ng/mL).The serum glycodelin-A level on the day of HCG was higher in endometrial thickness ≥7 mm group ([1.53 ±0.49]ng/mL) than endometrial thickness <7 mm group ([0.51 ±0.17]ng/mL). Conclusion The serum glycodelin-A level on the day of HCG may reflect endometrial receptivity to a cer-tain extent , which might have prognosis value for pregnancy following IUI .

The two verities of Crgptococcus neoforinans were first observed as well growing with brown color changes on Guizotia abyssinica seed agar(GASA)and caffeic acid commeal agar(CACA.Then,C.neoformans var.neoformans was found in 18/26 pigeon droppings by both two media.C.neoformans var.gattii was not isolated by the two media in 76 Eucalyptus camaldulensis samples.However,an,overgrowth of filamentous fungus was more frequently seen on GASA.Our results suggest that CACA be capable of selevtively isolating C.neoformans with the advantages of less interference fron the overgrowth of filamentous fungi.
7.Analysis of ephedrine and its metabolites in rat urine by HPLC-ESI-ITMSn.

AIMTo estabilish a rapid and sensitive LC-ESI-ITMSn method for the identification of ephedrine and its main metabolites in rat urine.
METHODSAfter optimizing the detection condition of LC-ESI-ITMSn chromatography and mass spectrometry by using a standard ephedrine, the ionization and cleavage rules of ephedrine in ESI-MS and ESI-MSn modes were summarized, and then serving as the basis for the metabolite analysis of ephedrine in rat urine. Rat urine samples of 0-48 h were collected after ig 10 mg x kg(-1) ephedrine, then the samples were purified through C18 solid-phase extraction cartridge. The purified samples were analyzed by LC-ESI-ITMSn.
RESULTSThe structures of ephedrine metabolites were elucidated according to the changes of the molecular weights of the metabolites (deltaM) and their cleavage pattern in ESI-ITMSn. As a result, three phase I metabolites and the parent drug ephedrine were identified existing in rat urine, but no phase II metabolites were found.
CONCLUSIONThe LC-ESI-ITMSn method is rapid and highly sensitive and sepecific, it is suitable for the identification of ephedrine and its metabolites in rat urine.

8.Quality control of shuanghuanglian freeze-dried powder for injection based on its HPLC-ELSD fingerprints and biological profiles.

This paper is aimed to provide the methods of quality control and bioassay of traditional Chinese medicine injections including bioassay method. Shuanghuanglian freeze-dried powder for injection (SFPI) was chosen as study object. HPLC-ELSD fingerprints of SFPI had been established and the samples were differentiated by similarity calculation. Meanwhile, biological profiles of SFPI on Escherichia coli had been established by microcalorimetry. The similarity values were calculated using the correlation coefficient, based on quantitative thermo-kinetic parameters (T2m, Tj, I%). The results indicated that HPLC-ELSD fingerprints, which showed content changes of chemical components, could not monitor minimal variation of different samples, especially that of biological pollutants, while biological profiles could sensitively detect antibiotic activity alterations of the samples, which were kept under specific conditions. In conclusion, characterized by two-dimension, microcalorimetry could supply thermograms as biological profiles characterized to describe the bioactivity of drugs. This study could clearly demonstrate that the correlative detection was proposed as an efficient strategy for quality control of SFPI, based on HPLC-ELSD fingerprints and biological profiles, which could detect quality fluctuation of samples early and quickly and predict the potential adverse drug events (ADE) for ensuring clinical safety.

9.HPLC-ESI/MS analysis of stachydrine and its metabolites in rat urine.

AIMTo identify the main metabolites of stachydrine in rat.
METHODSThe ionization, cleavage and chromatographic characteristics of stachydrine were studied by using high-performance liquid chromatography-electrospray ionization ion trap tandem mass spectrometry (HPLC-ESI/MS) for the first time. These characteristics of stachydrine were used as the basis for the analyses of metabolites in rat urine. The 0 - 24 h urine samples of rats after ig 25 mg x kg(-1) stachydrine were collected and purified by using C10 solid-phase extraction cartridge, and then analyzed by HPLC-ESI/MS to identify stachydrine and its metabolites.
RESULTSThe parent drug (stachydrine), 6 phase I metabolites (N-demethyl, dehydrogenation, ring-oxidation) and 2 phase II metabolites (glycine conjugates of 2 ring-oxidation products) were identified existing in rat urine.
CONCLUSIONThe presented method was proved to be sensitive, rapid, high selective and specific for the identification of stachydrine and its metabolites in rat urine.
