1.The influence of hepatic ischemia-reperfusion injury to the hepatic cell cycle G2/M-related gene Gadd45a and Egr-1 expression

ObjectiveTo explore the possible mechanism that Gadd45a and Egr-1together participate in the DNA damage repair,by analyzing the influence of hepatic ischemia-reperfusion injury (IRI) on the rnRNA and protein expression of hepatic cell cycle-related genes.Method Methods Forty-two SD rats were divided into normal control group,ischemia 30 min group and reperfusion 30 min,1h,2 h,3 h and 24 h groups.The hepatic specimens of rats at the corresponding time points after success of modeling were obtained. Chip experiment was used to screen out the key genes Gadd45a,Egr-1and GAPDH for target genes.We used Northern blot and RT-PCR to confirm the chip's results. The expression of related proteins was detected by using immunofluorescence and immunohistochemistry.Results Northern blot detection results showed that the RT-PCR products and the target genes for testing were concordant.The expression spectral chip signal and RT-PCR detection revealed that the expression of target genes had a 4-fold increase at early reperfusion period (1h) (up-regulation of Egr-1,Gadd45a and HSP70-1a by 9.39,8.28 and10.8 respectively.At the later stage of reperfusion (24 h),the expression of Egr-1,Gadd45a and Hspala was reduced to the normal levels, and the chip screening results were consistent with the RT-PCR test results.Fluorescence immunohistochemistry demonstrated that in the reperfusion1h and 2 h groups,the expression of Egr-1in nuclei of liver cells was positive. Immunohistochemistry indicated that the Gadd45 expression in the nucleus and cytoplasm was gradually increased with increasing reperfusion time.ConclusionGadd45a and Egr-1are the key genes of the nuclear factor (NF)-kB regulatory pathways,and involved in the regulation of cell cycle arrest at G2/M phase and repair of damaged DNA in rat hepatic IRI.

3.Application of elastic stable intramedullary nailing fixation for treatment of long bone fractures in children

Elastic stable intramedullary nailing (ESIN) is a well-accepted fixation method of long bone fractures in children.The advantages of ESIN include primary bone union with avoidance of growth plate injury and minimum invasive surgery.The good results of ESIN are obtained when surgeons have a good knowledge of the principle of the correction of the fracture and its stability, especially in the understanding of the unique pathophysiology characteristics of pediatric fractures.This review discussed the biomechanical mechanisms of ESIN, and the principles,indications and contraindications of ESIN in the treatment of pediatric long bone fractures.The article also comprehensively introduced the application of ESIN in femur,tibia,fibula,humerus, ulna, radius,clavicle and metacarpal bone fractures.The technique details of nailing, preoperative planning, positioning of the patient, sizing of the nail, entry points for the nail, reduction maneuvers for safe passage of nails through the fracture site, and postoperative management were also introduced in this review.
4.Treatment of recurrent small cell lung cancer

Small-cell lung cancer (SCLG) relapses in the majority of patients,even though most patients respond to first-line therapy.Subsequent therapy can provide significant palliation and prolongation of survival for many patients. At present,topotecan is considered the standard second-line chemotherapy.Recently,amrubicin has also shown more favorable antitumor activity,and is the most promising at present.Unfortunately,targeted agents have failed to demonstrate effectiveness for SCLC.
5.Effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection

Objective:To evaluate the effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection.Methods:38 post-operation patients with serious intra-abdominal infection were divided into two groups randomly.One as treatment group(n=19)and the other as control group(n=19).Albumin,blood glucose,fasting insulin concentration were measured At 72 hours after the operation and 144 hours.Insulin resistance index was calculated using the homeostasis model assessment(HOMA).Finally,the results of two groups were analyzed.Results:The albumin concentration of Indomethacin group at 72 hours was significantly higher than that after 72 hours,Insulin resistance of 144 hours was lower than the control group(P0.05).Conclusion:Postoperative abdominal infection using indomethacin suppository can significantly reduce insulin resistance,and significantly reduced the decomposition of albumin.
6.Neoadjuvant chemotherapy followed by radical surgery for the treatment of locally bulky cervical carcinoma

Background and purpose:Cervical cancer is one of the common disease in women in china.Radical surgery remains the first choice for patients with early-stage cervical cancer.However,the majority of patients with bulky disease have lymph nodes or metastases when diagnosed.Palatinum-based neoadjuvant chemotherapy has been utilized in locally advanced cervical cancer with the aim of improving response and survival.The study was to evaluate the effect of neoadjuvant chemotherapy followed by radical surgery for the patients with locally bulky Ⅰ_(B2) and Ⅱ_A cervical carcinoma.Methods:Between September 2003 and February 2004,51 untreated patients with Ⅰ_(B2) and Ⅱ_A cervical carcinoma were enrolled in study group,and were treated with neoadjuvant chemotherapy with cisplatin,vincristine and bleomylin for one cycle followed by radical hysterectomy and pelvic lymphadenectomy;another 51 patients as control group were treated with radical surgery alone.The tumor size,depth of stromal invasion,lymph node metastasis and involvement of surgical specimens were assessed.Results:The overall clinical response rate was 41.2%.There were no complete and progressive ones.The blood loss during operation was 116.82?45.50ml,it was significantly lower than that in control group(P
7.Clinical Efficacy of BCG-polysaccharide Nucleic Acid Cominbed with Isotretinoin in the Treatment of Pa-tients with Plane Warts

Objective:To study the clinical efficacy and safety of BCG-polysaccharide nucleic acid combined with isotretinoin soft capsules in the treatment of plane warts. Methods:Seventy patients with plane warts were randomly divided into the observation group and the control group(n=35). The control group were treated with isotretinoin soft capsules, limp 10mg, bid, 4 weeks to 10mg, qd. The observation group were given BCG-polysaccharide nucleic acid 1ml, im, every otherday, on the basis of the contrl group. After 8 weeks of treatment, T cell populations and immunoglobulin levels were detected, and the efficacy and adverse reactions were recorded. Results:After the treatment, T cell population level, IgG and IgA of the two groups were significantly improved(P<0. 01), and the improvement in the observation group was significantly better than that in the control group (P<0. 05). The onset time of the observa-tion group was shorter than that of the control group ( P<0. 01). The total effective rate of the observation group was higher than, that of the control group(P<0. 05). The occurrence of adverse reactions in the two groups were no significant(P>0. 05). Conclusion:The clinical efficacy of BCG-polysaccharide nucleic acid combined with isotretinoin soft capsules in the treatment of plane warts is sig-nificant with promising security.
8.G3BP: a promising target for cancer therapy.

G3BP (Ras-GTPase-activating protein SH3 domain binding protein), a protein which binds to RasGAP SH3 domain, belongs to RNA-binding protein family, implicating in the downstream of Ras signaling. G3BP harbors the activities of endoribonuclease and DNA helicase, and can induce stress granules formation. G3BP plays a general role in the signal pathways of cell proliferation, differentiation, apoptosis and RNA metabolism. It has been shown to be over-expressed in a number of human malignancies and has a close relationship with tumor invasion and metastasis. Given that it has been implicated in several pathways that are known to be involved in cancer biology, G3BP may provide a new target for cancer therapy.
9.Research progress for early diagnosis of hepatocellular carcinoma

Hepatocellular carcinoma is one of the most common malignant tumors.With the concealed characteristics and non-specific clinical expressions,its presence can be frequently missed,allowing progression to advanced stages where prognosis is poor.Therefore,early diagnosis is essential to improve the survival rate of patients with hepatocellular carcinoma.Recent developments in imaging technology and molecular biology are making strides in identifying and finding biomarkers of hepatocellular carcinoma,leading to earlier diagnosis.This article describes the latest developments,in combination with previous diagnostic methods to make a comprehensive examination of strategies for early diagnosis of hepatocellular carcinoma.
10.Research progress of CD+4 CD+25 Foxp3 Treg cells in the hematological malignancies

Thymus derived CD+4; CD+25 regulatory T cells (Treg) are thought to be specific T cells that play an important role in controlling autoimmunity, preventing transplant rejection, restraining anti-infectious immune response, suppressing allogeneic immune respons. More recently, these cells are reported to have the ability of suppressing antitumor immune response. Foxp3 are the most specific protein of Treg. In this review, we will discuss the expression of CD+4 CD+25 Treg in patients with hematological malignancies and its implication for immunotherapy.
