1.Influence of topical Iotemax on intraocular pressure following excimer laser corneal refractive surgery

Background Excimer laser corneal refractive surgery is widely used because it reduces the dependency of refractive error eye to glasses.However,the adverse effects of anti-inflammatory drugs is a problem after operation.Lotemax eyedrops is a kind of steroidal anti-inflammatory drug,with a good effect on the operative eye.But its influence on intraocular pressure(IOP) after long-term topical administration is less studied.Objective This study was to investigate the influence of lotemax eyedrops on IOP after long-term topical administration in myopic eyes received excimer laser corneal refractive surgery.Methods An one-month retrospective case serial study was adopted.The case history data of 1552 eyes from 1552 Patients who received excimer laser corneal refractive surgery were collected in Tangshan Eye Hospital from 2011 January to 2012 January,including 321 eyes with laser in situ keratomileusis(LASIK),608 eyes with sub-Bowman keratomileusis (SBK) and 623 eyes with femtosecond laser for flap excimer laser in situ keratomileusis(FS-LASIK).Lotemax eyedrops was topically administered after operation 4 times per day initial and then decreased 1 time weekly for 1 month.IOP was measured in before,1 week,2 weeks,3 weeks,4 weeks after operation with non-contact tonometer.The increasing range of IOP was divided into <5 mmHg,5-15 mmHg and > 15 mmHg after operation.Optical head parameters were measured by Heidelberg retina tomography when IOP increasing after operation.Results Within 1 month,elevated IOP was found in 47 of 1552 eyes after administration of lotemax eyedrops,including 3.12% (10/321) in the LASIK group,3.29% (20/608) in the SBK group and 2.73% (17/623) in the FS-LASIK group,without significant difference among them(x2 =1.95,P>0.05).The IOP elevated 1 week after operation and remained normal after that till drug withdrawal in all eyes.There were no significant differences in the disk area (1.65 ±0.44) mm2 vs.(1.66 ±0.44) mm2,disk volume (0.40 ±0.09)mm3 vs.(0.39±0.08) mm3 and mean nerve fiber layer thickness (0.28 ± 0.08) mm vs.(0.29 ± 0.07) mm in IOP elevation duration compared with 1 month after IOP recovery(t =0.34,0.81,0.48,P>0.05).Conclusions Topical administration of 0.5% lotemax eyedrops after excimer laser corneal refractive surgery leads to reversible high IOP.It is very important to monitor the IOP during use of the drug.
2.Anatomic characteristics of the vessels in the spermatic cord of the varicocele patient: A laparoscopic study.

OBJECTIVETo study the numbers and locations of spermatic veins, testicular arteries, and lymphatic vessels in the spermatic cord of the varicocele patient under the laparoscope.
METHODSFifty-seven varicocele patients received laparoscopic ligation of spermatic veins, during which we recorded the numbers and observed the locations of spermatic veins, testicular arteries, and spermatic lymphatic vessels.
RESULTSDuring the surgery, we identified 3.3 ± 1.2 spermatic veins, 1.4 ± 0.9 testicular arteries, and 4.3 ± 1.1 spermatic lymphatic vessels. No statistically significant differences were observed between the two side in the numbers of the spermatic veins, testicular arteries and spermatic lymphatic vessels (P > 0.05). The testicular arteries were seen on the exterior of the spermatic veins and winding around them, while the spermatic lymphatic vessels mostly between the veins.
CONCLUSIONThe spermatic veins, testicular arteries, and lymphatic vessels in the spermatic cord of the varicocele patient have their specific anatomic characteristics. Laparoscopic identification of these vessels may contribute to the surgical treatment of varicocele.

3.Clinical analysis and surgery of traumatic implantation cysts of the iris

Objective To explore the clinical characteristics, surgical treatment and related factors of traumatic implantation cyst of the iris. Design Retrospective cases series. Participants Thirty-six cases of traumatic implantation cyst of the iris. Methods Thir- ty-six cases of traumatic implantation cyst of the iris were reviewed. Main Outcome Measures Age, surgical history, history of disease, surgical method of patients. Results All cases of traumatic implantation cyst of the iris were secondary to perforating injury. 10 cases had been undertaken cataract surgery. The histories of disease in 6 months~1 year and 1 year~10 years were 30.56% respectively. Sur- gical exicision was taken in all cases. There were 2 recurrence cases. Conclusion Traumatic implantation cyst of the iris is almost see- ondary to perforating injury. Surgical excision is an effective strategy to treat this disease.

5.Laparoscopic vs open appendectomy in obese patients

Objective To evaluate laparoscopic appendectomy(LA) and open appendectomy(OA)in obese patients.Methods From January 2008 to November 2010,153 obese patients with appendicitis were operated on,92 cases were treated with initial LA and 61 cases with upfront OA.The operative time,intraoperative bleeding volume,intestinal recovery period,the rate of using acesodyne,major postoperative complications,the duration and hospital cost were studied.Results In LA group,4 cases were converted to open surgery.All the variables in LA group were better than those in OA group except the hospital cost and the differences were statistically significant (the operative time:t =14.0,P < 0.001 ;intraoperative bleeding volume:t =19.7,P < 0.00 1 ;intestinal recovery period:t =12.3,P < 0.001 ;the rate of using acesodyne:t =21.01,P < 0.001 ;main postoperative complications:x2 =40.138,P < 0.001 ;the hospital stay:t=17.3,P<0.001) except the in-hospital cost(t=1.434,P>0.05).Conclusions Compared with OA,LA is a better choice for obese patients with appendicitis because of its advantages of minimal injury,early recovery,less complications and short hospital stay.
6.Ifosfamide and vinorelbine combined chemotherapy in the treatment of advanced non-small cell lung cancer

Objective To evaluate therapeutic effects and toxicity of advanced non-small cell lung cancer (NSCLC)treated by combining chemotherapy on ifosfamide(IFO)and vinorelbine(NVB).Methods 107 cases pa- tients with advanced NSCLC were enrolled.IFO was given in a dosage of 1.5g/m~2 on day 1 to 4.and NVB in a dosage of 25mg/m~2 on day 1 and 8.It was repeated every three or four weeks,up to two to four cycles.Results Two patients had complete response and 40 patients had partial response.The overall response rate was 47.7% ,the median survival time 10.3 months,1-year and 2-year survival rate was 42% and 12.3%,respectively.The main toxicity was bone marrow suppression.Conclusion The regimen is effective,sale and tolerable in advanced non- small cell lung cancer therapy.
7. Research on color differences between enamel and dentin of natural teeth

Objective To investigate the color differences of enamel and dentin of natural teeth, so as to lay a foundation for reducing the color differences of natural dentin and core of all ceramic restorations. Methods X-rite Color i7 Spectrophotometer was used to examine the chroma value and chromatic difference (δE) of enamel and dentin of natural teeth (thickness 0. 9 mm, 0. 6 mm and 0. 3 mm, n = 30). Results The L* values (lightness) of enamel and dentin increased with the decrease of slice thickness, and the a* values (red-green) and b* values (yellow-blue) decreased with the decrease of slice thickness (P<0. 05). The absolute L*, a* and b* values of the dentin were significantly greater than those of enamel of same thickness (P<0. 05). There were great color differences between enamel and dentin of different thicknesses, with the mean δE values within a range of 5. 303-13. 109. The color difference gradually increased with the thickness increase of enamel and dentin. The δE value of the enamel and dentin of the same thickness was 5. 115 (0. 9 mm), 5. 855 (0. 6 mm), and 6. 053 (0. 3 mm), respectively. Conclusion There is noticeable color difference between the enamel and dentin of natural teeth, and the difference increases with the increase of their thickness difference. The dentin has a more yellowish color and less lightness compared with the enamel. So each laminate of all ceramic restorations needs a shade selection for all ceramic restorations, and more attention should be paid on the color matching of natural dentin and core ceramic laminate.
8.The expression of basic fibroblast growth factor 1 during human embryonic yolk sac hematopoiesis.

OBJECTIVETo explore the effect of basic fibroblast growth factor 1 (bFGF1) during embryonic development on hematopoiesis, to study the expression of FGF1, vascular endothelial growth factor receptor (KDR), CD133, CD34 and transcription factors Ihh, SCL, GATA-1, GATA-2 and PU. 1 in the yolk sac, and to learn about the role and relationships of FGF1, hematopoietic cells and transcription factors during embryonic hematopoiesis.
METHODS10 microm sections and total RNA were prepared from 107 human embryos aged 3-12 weeks. Immunohischemical SP staining and RT-PCR were performed.
RESULTSThe yolk sac blood islands of human 3 approximately 12 weeks embryos consisted of peripheral vascular endothelial cells and central hematopoietic cells. The expression of FGF1 was firstly found in visceral mesoderm around periphery of yolk sac blood island at day 16, while was little inside it. KDR was not or lowly expressed and CD34 and CD133 were not expressed then. The expression increased, gray value decreased and staining enhanced at day 21. Strong staining of CD34+, CD133+ and KDR+ cells were found in blood island and mesoderm at day 30, their gray values changed from 156 +/- 16, 173 +/- 18 and 160 +/- 14 to 53 +/- 7, 52 +/- 6 and 69 +/- 8 respectively. FGF1 expression was strong positive, the gray value declined dramatically from 161 +/- 13 to 40 +/- 5. Some positive cells formed vessel-like structure along the periphery of blood island. Moderate expression of CD34+, CD133+, KDR+ cells increased at day 45, the cells aggregated into mass in blood island and FGF1+ cells did the same in blood island, while little in mesoderm. Its gray valve was increased. After 7 weeks, CD133+, KDR+, CD34+ cells significantly decreased their gray values increased, the staining became week. FGF1 was weakly expressed in yolk sac and its gray value increased to 179 +/- 22. RT-PCR showed Ihh, SCL, GATA-1 and GATA-2 were expressed at different time in yolk sac. PU. 1 were not expressed at day 16, and then expressed.
CONCLUSIONSThe hematopoietic properties of yolk sac may be dependent on signaling through FGF receptors and FGF1 plays an important role in hematopoietic stem cell homeostasis. The FGF pathway regulates primitive hematopoiesis by modulating transcription factors such as Gata1 expression level and activity.

9.Clinical Study on Shugan Hewei Formula for Treatment of Cancer-related Depression

Objective To observe the clinical efficacy of Shugan Hewei Formula for cancer-related depression. Methods Totally 92 cancer-related depression patients were randomly divided into two groups, with 46 patients in each group. All patients already finished the treatment such as radiotherapy and chemotherapy, and were in stable stage. The Fluoxetine Hydrochloride Dispersible Tablets was given to the patients in the control group, 20 mg each time,once a day,orally;Shugan Hewei Formula was given to those in the treatment group,a dose a day,twice a day, orally. The treatment for both groups lasted for 6 weeks. Before and after treatment, HAMD scores and KPS scores were observed; the change of cerebral neurotransmitters levels in the brain of patients were detected; IL-6 and TNF-α in serum were tested by ELISA. Results HAMD scores in the two groups decreased markedly after treatment (P<0.05). HAMD scores in treatment group was less than that in control group after treatment (P<0.05). KPS scores increased in treatment group after treatment, and were better than the control group (P<0.05). The contents of NE, 5-HT increased, IL-6 and TNF-αdecreased in both groups after treatment (P<0.05). The contents of NE and 5-HT in the treatment group were higher than the control group after treatment, while the contents of IL-6 and TNF-α were lower than the control group (P<0.05, P<0.01). No adverse reactions and side effects were observed in the treatment group. 5 cases of mild nausea and 6 cases of abnormal liver function were found in the control group. Conclusion Shugan Hewei Formula can increase the contents of NE and 5-HT in the brain of cancer-related depression patients, decreased cytokines levels, and improve patients' depressive state.
10.Distribution and clinical significance of HBV genotypes in patients with HBV infection in 30 regions of China.

OBJECTIVETo explore the distribution and clinical significance of HBV genotypes in patients with HBV infection in China.
METHODSerum samples were collected from 2922 patients with HBV infection. HBV genotyping was performed with type-specific primers polymerase chain reaction, and the virological and biochemical markers were detected, which differences in the genotypes distribution between various regions and liver function and virological markers between various HBV genotyping were analyzed.
RESULTThe genotype B, C, B + C, D of 2922 patients with HBV infection accounted for 15.9%, 83.5%, 0.41%, 0.21% respectively. In Northern China, genotype C was most prevalent, accounting for 90% of all cases, while it was less common in Southern China; genotype C was present in Zhejiang and Jiangsu provinces, but genotype B was comparatively more common in Guangdong, Hunan, Hubei, and Jiangxi provinces. B, C genotype HBV infection patients in the sex difference was not statistically significant; B genotypes compared with C genotype HBV infection patients, the average age of is less (P < 0.001); HBeAg positive rate of C genotype HBV infection patients are higher than that of B genotype (P = 0.023); Viral load of genotype C HBV infection patients is higher than that of genotype B (P = 0.038); Cholinesterase and Albumin levels of genotype C HBV infection patients are lower than that of genotype B (P values were 0.016, <0.001).
CONCLUSIONThere were HBV genotype B, C, B + C and D in Chinese patients with HBV infection, with genotype B and C being the major ones. Mainly in northern regions of genotype C, C genotype significantly reduced the southern region, some of the southern region dominated by B genotype. Genotype C HBV infection patients are older, and their HBeAg-positive rate is higher, and their liver damage is more severe, but their viral load is less.
