1.Clinical study of phacoemulsification in small pupil cataract after filtering operation of glaucoma

AIM: To investigate the clinical effects of phacoemulsification and surgical techeniques in small pupil cataract after filtering operation of glaucoma.
METHODS: Thirty-six cases ( 36 eyes ) of small pupil cataract after filtering operation of glaucoma were underwent phacoemulsification combined with foldable intraocular lens implantation. Postoperative visual acuity, intraocular pressure filtration bleb and complications were observed. The follow-up time was 3mo.
RESULTS: The preoperative uncorrected visual acuity (UCVA) or best-corrected visual acuity ( BCVA) ≤0. 3 were in all patients. Postoperativerly, the UCVA or BCVA≥0. 3 after 1wk and 3mo were achieved in 23 eyes (63.89%) and 34 eyes (94. 44%). BCVA was <0. 3 in 2 eyes ( 5. 56%) including optic nerve atrophy of one eye and diabetic retinopathy of one eye. All patients were kept the level of normal intraocular pressure and completed filtering bleb, there were no serious complications.
CONCLUSION: Phacoemulsification in small pupil cataract after filtering operation of glaucoma was complex, and reasonable application of operation skills can still achieve better clinical results. The main factor of affecting the visual acuity is pre-existing retinal optic neuropathy.
2.Clinical observation on cervical chiropractic for cervical spondylosis of vertebral artery type

Objective:To observe the curative effect of cervical chiropractic for cervical spondylosis of vertebral artery type (CSA) and to explore its possible mechanism.Methods:Sixty CSA patients were randomly divided into two groups.Thirty patients in the treatment group were treated with chiropractic manipulation for cervical vertebrae,once every other day,7 treatments in total.The other 30 patients in the control group received oral administration of flunarizine hydrochloride,10 mg per night,for 14 d.Cervical vertigo and functional assessment scale and transcranial cerebral Doppler (TCD) were measured before treatment and 7 days after treatment.Results:The total effective rate was 96.7% in the treatment group and 83.3% in the control group.The curative rate was 66.7% in the treatment group and 20% in the control group.The differences between the two groups in the total effective rate and the curative rate were statistically significant (both P<0.05).The scores of cervical vertigo symptom and functional assessment after treatment in both groups were significantly higher than those before treatment (both P<0.01);the difference between the two groups was statistically significant (P<0.05).After treatment,the maximum systolic velocity (Vs),the maximum diastolic velocity (Vd),the mean velocity (Vm),the pulsatility index (PI) and the vascular resistance index (RI) in both groups were significantly improved compared with those before the treatment (all P<0.01);there were significant differences between the two groups (all P<0.05).Conclusion:Cervical chiropractic is an effective method for CSA,and its curative effect is better than that of flunarizine hydrochloride alone.Its mechanism may relate to correcting cervical instability.

4.Application of nasointestinal decompression intubation in small bowel obstruction

Objective To evaluate the nasointestinal decompression intubation in acute small intestinal obstruction. Methods Ten patients with acute small bowel obstruction received nasointestinal decompression intubation under x-ray guidance. The nasointestinal decompression tube passing over a guidewire was inserted into small intestine near Tres ligament or further down distally with assistance of patients adopting in multi-physical positions. Results The intubation of nasointestinal decompression tubes into small intestine was technically successful in all patients with average procedural time of 16 min.(10 ~ 35 min). After placement of the tube, all patients obtained various degrees of symptoms relief including abdominal pain, distention, vomiting, etc. Four patients with simple adhesive obstruction recovered completely and the tube was removed 2 weeks later. Three patients were refered to surgical operation, and 3 others gave up for further treatment. There were no complications such as bleeding or perforation related to intubation. Conclusion Nasointestinal decompression intubation under guidance of X-ray is rather simple, less time consuming, especially with high efficiency for preoperative gastrointestinal decompression and treating simple adhesive bowel obstruction; ought to be recommended. 5.The oral microbiome community variations associated with normal, potentially malignant disorders and malignant lesions of the oral cavity

The human oral microbiome has been known to show strong association with various oral diseases
including oral cancer. This study attempts to characterize the community variations between
normal, oral potentially malignant disorders (OPMD) and cancer associated microbiota using 16S
rDNA sequencing. Swab samples were collected from three groups (normal, OPMD and oral
cancer) with nine subjects from each group. Bacteria genomic DNA was isolated in which full
length 16S rDNA were amplified and used for cloned library sequencing. 16S rDNA sequences
were processed and analysed with MOTHUR. A core oral microbiome was identified consisting
of Firmicutes, Proteobacteria, Fusobacteria, Bacteroidetes and Actinobacteria at the phylum level
while Streptococcus, Veillonella, Gemella, Granulicatella, Neisseria, Haemophilus, Selenomonas,
Fusobacterium, Leptotrichia, Prevotella, Porphyromonas and Lachnoanaerobaculum were detected at
the genus level. Firmicutes and Streptococcus were the predominant phylum and genus respectively.
Potential oral microbiome memberships unique to normal, OPMD and oral cancer oral cavities were
also identified. Analysis of Molecular Variance (AMOVA) showed a significant difference between
the normal and the cancer associated oral microbiota but not between the OPMD and the other two
groups. However, 2D NMDS showed an overlapping of the OPMD associated oral microbiome
between the normal and cancer groups. These findings indicated that oral microbes could be potential
biomarkers to distinguish between normal, OPMD and cancer subjects.

Objective To assess the diagnostic value of MRCP before LC.Methods 944 cases with chronic calculous cholecystitis underwent MRCP before LC from June 2004 to June 2007 in our department.incidence rate of cholecvstolithiasis together with common bile duct stones and incidence rate of anatomic abnormity of bile duct were collected.Results The incidence rate of cholecvstolithiasis together with common bile duct stones were 8.1%(77/944),and the oecurence ofACBDS were 1.2%(11/944).The incidence rate of anatomic abnormity of bile duct were 3.7%(35/944).ConclusionMRCP can not only offer a excellent diagnostic value of ACBDS and anatomic abnormity of bile duct,but also reduce the occurrence of CBDS remainder and iatrogenic bile duct iniuries.
7.Postoperative delirium in elderly patients after gastrointestinal surgery

Objective To study postoperative delirium in elderly patients.Methods We investigate the morbidity of postoperative delirium in 142 elderly patients (≥ 60 years)after gastrointestinal surgery by using Confusion Assessment Method (CAM) and Delirium Rating Scale Revised-98 (DRS-R98) scores.Data were analyzed using Student's t test and Chi-squaretest respectively with SPSS 19.0.Results Of 142 patients,delirium was diagnosed in 36 patients(25.4%),delirium developed in 4,7,17,7,1 patients in posto perative 1,2,3,4-7,7 + days respectively.There were significant difference in hospital stay:17.7 ± 2.6 days (postoperative delirium) and 13.4 ± 2.3 days (no postoperative delirium),t =4.608,P =0.000 1.The postoperative complications (52.8% / 23.6%,x2 =10.710,P =0.001) and ICU admission (22.2%/6.6%,x2 =6.939,P =0.008) significantly increased.Conclusions Postoperative delirium is recognized as one of the most common surgical complications in elderly patients with gastrointestinal surgery leading to other major postoperative complications,and prolonged hospitalization.
8.Isolation and in vitro propagation of aldehyde dehydrogenases positive breast cancer stem cells and investigation on biological property of these cells

Objective To identify a aldehyde dehydrogenases positive (ALDH+) cancer stem cell subpopulation in MCF-7 cells and to investigate the proliferation and differentiation characteristics of these cells in vitro.Methods ALDH+ breast cancer stem cells were isolated from MCF-7 cells by flow cytometry and the biological property of ALDH+ breast cancer stem cells were examined by scarification test,MTT,growth curvature and Transwell migration assay.Results The ratio of CD-/low24 CD+44 cells was about 1.4 % in MCF-7 cells.The ratio of ALDH+ CD-/low24CD+44 cells was about 1.2 %.The growth curvature of ALDH+ breast cancer stem cells was almost the same with that of CD-/low24 CD+44 cells.The distance between cells was obviously shorter in both CD-/low24 CD+44 cells scarification zone and ALDH+ CD-/low24 CD+44 cells scarification zone.The migration ability of CD-/low24CD+44 cells and ALDH+ CD-/low24CD+44 cells was stronger than control group cells.There were migration ability differences between CD-/low24CD+44 cells and ALDH+ CD-/low24CD+44 cells.The results of Transwell experiments were in coincidence with above results.Lots of CD-/low24CD+44 cells and ALDH+ CD-/low24CD+44 cells were through the membrane.In MTT assay,absorbance values were 1.05±0.098,1.56±0.075 and 1.67±0.032.Conclusion CD--/low24CD+44 and ALDH+ CD-/low24CD+44 breast cancer stem cell subpopulation exist in MCF-7 cells.ALDH could potentially be used as a molecular marker to identify breast cancer stem cell subpopulation.

Objective To learn the prevalence situation of dyslipidemia among adult residents in the southern coastal area, Wenzhou,Zhejiang.Methods Adult residents were chosen by using multi -stage sampling method from 11 counties in Wenzhou in 2013.Questionnaire survey,medical examination,and biochemical detection triglycerides were conducted among the residents.Chi -square test,and multiple logistic regression analysis was used to identify the risk factors related to dyslipidemia.Results The overall prevalence of dyslipidemia was 44.99%(the standardized prevalence rate was 42. 93%),and prevalence rate in women (43.91%)was higher than that in men (45.80%),but the urban(14.26%)and rural(13.83)areas prevalence was not significant.The awareness rate of dyslipidemia was only 21.73%.The prevalence of isolated low HDL -C,high LDL -C,hypercholesterolemia,hypertriglyceridemia,mixed hyperlipidemia was 13.95%, 10.45%,19.34%,12.98%,4.69%,respectively.Multivariate logistic regression analysis showed that gender,age, nationality,coronary heart disease,dyslipidemia,family history,aquatic products,milk,pickled products edible frequency,living and working pressure,body mass index,central obesity,hypertension,diabetes were related the prevalence of dyslipidemia,but the risk factors of different lipid fractions were different.Conclusion The prevalence of dyslipidemia has been in a high level among adult residents in Wenzhou,but the awareness rate of dyslipidemia was low. Measures should be Strengthened to prevent dyslipidemia .
