Objective To study the surgical techniques and effects of laparoscopy-assisted sigmoid colon vaginoplasty.MethodsClinical data of 27 patients,who underwent laparoscopy-assisted sigmoid colon vaginoplasty at Beijing Anzhen Hospital between June 2006 and June 2007,were retrospectively analyzed.A 15-cm segment of pedicled sigmoid colon was isolated using an ultrasound knife.The distal end of the segment was pulled into the vaginal space in the cul-de-sac of Douglas under a laparoscopic vision as the neovagina.The continuity of the intestinal tract(end-to-end bowel anastomosis) was restored using a circular mechanical suture through the rectum.ResultsThe surgery was successfully completed in all the cases,no intra-operative complication occurred.The mean blood loss and operation time was 82 ml(50-180 ml) and 168 min(120-246 ml) respectively.One patient developed incomplete intestinal obstruction 16 days after the operation,and was cured by conservative treatment.Follow-up was available in 21 patients for 14-20 months.Five patients had no sexual partner during the follow-up,while the other 16 patients were satisfied with their sexual lives after the surgeries.ConclusionLaparoscopic vaginal reconstruction using a sigmoid colon segment is satisfying for cosmetic,functional,and anatomic results.

ObjectiveTo evaluate the role of multi-slice CT(MSCT) reconstruction in the diagnosis of intracranial foreign bodies.MethodsMSCT scans were performed in 16 patients with intracranial foreign bodies. The volumetric data were reconstructed in the workstation. 5 patients were detected by CT angiography (CTA) examination. Imaging results were then compared with findings found during surgery.Results11 foreign bodies located in one brain lobe, 3 passed two brain lobes, 2 located in ventricle. Among them, 6 cases were closed to parietal including 2 cases closed to cerebral arterial circle. 14 patients were operated and extirpated 18 foreign bodies. Compared with surgical findings, the orientative accuracy of reconstruction imaging was 100%, qualitative accuracy was 85.7%.ConclusionMSCT reconstruction may display the intracranial foreign bodies from multi-angles. It is propitious to know the characteristics, location and whole shape of the foreign bodies for the surgeon before the operation, accordingly to make a best operation project.

This paper introuduces two ways of WEIDA LMW-1000 Magnetic Resonance Imaging System's communication with Windows 98 computer. It has a practical value to the improvement of the system's operation interface.

Objective: To determine the optimal manipulation parameters in Liu's back tuina therapy for remission-stage cough variant asthma (CVA) in kids. Methods: A 5-factor 3-level L18(37) orthogonal design was adopted. The experimental factors and levels were selected: A, the number of times of Rou-kneading Feishu (BL 13); B, the frequency of Rou-kneading Feishu (BL 13); C, the number of times of Tui-pushing in a '介'-like pattern; D, the frequency of Tui-pushing in a '介'-like pattern; E, the intensity of red color after Ca-scrubbing Feishu (BL 13) with salt. A1, A2 and A3 stood for 10 times, 20 times and 30 times, respectively; B1, B2 and B3 stood for 200 times/min, 240 times/min and 280 times/min, respectively; C1, C2 and C3 stood for 50 times, 75 times and 100 times, respectively; D1, D2 and D3 stood for 150 times/min, 175 times/min and 200 times/min, respectively; E1, E2 and E3 stood for pinkish, bright red and dark red, respectively. A total of 108 kids with CVA in remission stage were divided into 18 groups using the random number table method, with 6 cases in each group. Changes in the cough score and fractional exhaled nitric oxide (FENO) level were observed after 15-day treatment. Results: The cough score and FENO level changed significantly in every group after treatment (all P<0.01). By the effect in improving cough score, the factors were listed as D, B, A, C and E in descending order; the most optimal manipulation parameter combination was group 17 (A3B2C1D3E1); the second optimal parameter combinations were group 7 (A3B1C2D1E3), group 13 (A2B1C2D3E1) and group 14 (A2B2C3D1E2). Regarding the effect in down-regulating the level of FENO, the factors were listed as C, A, E, B and D in descending order; the most optimal manipulation parameter combination was group 13 (A2B1C2D3E1); the second optimal parameter combinations were group 18 (A3B3C2D1E2), group 16 (A3B1C3D2E3) and group 14 (A2B2C3D1E2).Conclusion: Two most effective manipulation parameter combinations were selected out of 18 different parameter combinations in Liu's back tuina therapy for remission-stage CVA in kids. Doctors can choose an appropriate manipulation based on the symptom features of the disease.

This study was designed to investigate the impact of ultra-filtration extract mixture from Astragals mongholicus (UEMAM) o radiosensitivity of H22 ascitic tumor in mice to 12C6+ ions radiation. The H22 ascitic tumor model was established in mice by intraperitoneal injection of 0.2 mL H22 ascitic cells. The animals were subsequently divided into 4 groups randomly, treated with normal saline, UEMAM, heavy ion beam radiotherapy and UEMAM plus heavy ion beam radiotherapy, respectively. The body weights, abdomen circumference of the mice were measured and the mouse behavior was monitored every day; survival time was recorded to evaluate life extension effect; flow cytometry technique was used to detect H22 cell apoptosis and cell cycle; protein levels of p53, Bax, Bcl-2 and cleaved Caspase-3 were analyzed by Western blot; the single cell gel electrophoresis was used to detect the level of deoxyribonucleic acid damage (DNA damage). The results suggest that UEMAM significantly increased survival time, and decreased body weights and abdomen circumference over the saline control group. The treatment increased cell apoptosis, cycle arrest and DNA damage compared to the saline control group. UEMAM significantly enhanced the therapeutic effect of heavy ion beam radiation in survival time, and decreased body weights and abdomen circumference in the tumor-baring mice. The combination increased cell apoptosis, cycle arrest and DNA damage compared to the radiotherapy group. The results of Western blot suggest that the treatment significantly enhanced p53-induced apoptotic signals. The experiment discovered that UEMAM could improve radiosensitivity of H22 ascitic tumor through activation of p53-mediated apoptotic signal pathway.

OBJECTIVETo investigate the correct method of bone resection and posterior capsular soft tissue releasing in total knee arthroplasty (TKA) for the patients with rheumatoid arthritis with stiff knee in flexion.

METHODSFrom November 2009 to January 2012,15 patients with rheumatoid arthritis with stiff knee in flexion underwent primary TKA and releasing of the posterior soft tissues. There were 7 males and 8 females,aged 22 to 75 years old (58.7 years old on average). The preoperative range of movement(ROM) was (3.2 ± 1.7)°. According to Knee Society score (KSS) criterion, the preoperative clinical score was 23.3 ± 12.5 and functional score was 35.2 ± 9.8. Based on the correct osteotomy, effective releasing of posterior structures was used for different degrees of flexion contracture during the TKA procedure.

RESULTSAll the patients were followed up, and the average duration was 2.3 years (1.6 to 3 years). At the latest follow-up,the KSS clinical score was 81.7 ± 6.5 and functional score was 82.8 ± 9.3. The flexion and extension ROM of the knee joint was (103.5 ± 13.1). Three knees remained 50 flexion contracture deformity, but the function of the affect knees was good.

CONCLUSIONThe effective releasing of the soft tissue of posterior capsule is a major management for correction of the flexion contracture in TKA. The correct releasing of posterior structure can not only achieve fundamental gap of TKA but also effectively avoid bone over-resection.

Objective To isolate HIV-specific T cell clone and to expand them in vitro through the activation-induced expression of CD137 molecule.Methods The peripheral blood mononuclear cells were isolated from HIV-infected patients and HIV Gag specific CD3+CD8+CD137+T cell subset were sorted to 96-well plate in 1 cell/well by multicolor flowcytometry and single cell sorting.After 14 days in vitro culture with feeder cells and cytokines, the numbers and phenotypes of the cultured HIV-specific T cells were calcu-lated and identified.Results The CD137 expression was low on rested T cells but up regulated by the stim-ulation with Gag peptide pool.The CD8+CD137+T cells could secret IFN-γ.The number of CD8 T cells reached to 106 after 14 days in culture and expanded to 107-108 cells after 28 days of culture in vitro 100%of the cells remained activated upon Gag stimulation.Conclusion In stead of using IFN-γ, CD137 could be utilized as a novel molecule to isolate and expand HIV specific T cells in vitro.The expanded antigen spe-cific T cell clones could maintain good activation status.

Objective To investigate the routine methods of α-amylase (AMY) test in scrum which meets the requirements of ISO 15189.Methods Fifty human serum samples with different concentrations of AMY (40- 750 U/L) were collected from March to December in 2008,to form the patients' frozen serum group.Four AMY measurement systems including Roche,Wako,MINDRAY and MAKER were used.The frozen standard materials with concentrations of ( 70.1 ± 3.7 ) U/L and ( 418.3 ± 22.1 ) U/L and the patients' frozen serum group were measured simultaneously by using IFCC reference method and 4 AMY measurement systcms based on 7170A automatic biochemistry analyzer.Thc linear regression analysis was made between the measurement results of each system and IFCC reference method.The equivalence,agreement and trueness were also evaluated by using the file EP9-A2 method.Bland-Altman GraphicalAnalysis and the improved Bland-Altman Graphical-Analysis of MVS1.80 software.Results Judging by the standards of IFCC reference method,the measurement results of 4 measurement systems were obviously different. ( 1 ) When measuring standard materials the results were 66.4,70.6,69.4 and 49.2 U/L respectively and 394.0,456.4,406.7,302.4 U/L respectively.The measurement results of MINDRAY were in agreement with that of IFCC reference method.( 2 ) When mcasuring the patients' scrum group by 4 measurement systems and IFCC reference method,the slopes of the linear regression equations were 0.934,1.070,0.930 and 0.731.respectively.And the intercepts were 0.886,6.249,5.388 and 3.574,respectively.According to the EP9-A2 method,the measurement results of Roche Wako,MINDRAY were equivalent to that of IFCC reference method.According to Bland-Altman Graphical-Analysis, the measurement results of Roche and MINDRAY were in agreement with that of IFCC reference method.The average biases of each measurement system were - 6.11％ ( Average bias ± 2s were 2.81％ and -9.40％ ),1.99％ ( Average bias ± 2s were 10.35％ and - 6.36％ ),- 2.70％ (Average bias ± 2s were 2.37％ and -7.77％ ) and -34.72％ ( Average hias ±2s were -24.20％ and -45.24％ ),respectively.According to the improved Bland-Altman Graphical-Analysis, the measurement results of MINDRAY are correct.The average biases of each measurement system were - 5.92％ (Average bias ± 2s were -2.81％ and -9.03％),2.10％ (Average bias ±2s were 10.74％ and -6.53％),-2.64％ ( Average bias ± 2s were2.24％ and -7.51％ ) and - 29.51％ ( Average bias ± 2s were 21.82％ and - 37.21％ ),respectively.Conclusions ( 1 ) The measurement results of different measurement systems do not necessarily have crreet results though they have claimed to have traceability.(2) The trueness of measurement results using the same system may not come to the same conclusion when evaluated by different methods.So laboratories should select and establish a procedure to evaluate trueness of routine methods and adopt those meeting the trueness requirements of ISO 15189.

OBJECTIVE: This paper highlights the therapeutic effect of tragus cartilage and temporalis fascia graft on repairing bone and skin defect in external auditory canal. METHOD: Forty-one surgical patients were recuited, including 39 cases of external auditory canal cholesteatoma and 2 cases of osteoma. External auditory canal bone wall and skin defect and mastoid air cells exposure were found during operation. We followed up these cases periodically (in 3 weeks, 3 months, 6 months and 1 year later respectively) after repairing the patients' external auditory canal bone wall and skin with tragus cartilage and temporalis fascia graft. RESULT: Twenty-nine cases healed perfectly while other 12 cases grew small granulation in 3 weeks. After shaving granulations and tamping Tela Iodoformum, wounds healed in the following week. The epithelization of the external auditory canal in our patients was complete after 3 months and no stenosis were found. Natural morphology of external auditory canal was maintained without mastoid air cells exposure after 6 months to 1 year of time. CONCLUSION It is not only convenient to acquire the tragus cartilage and temporalis fascia, but also easy to repair the defect of external auditory canal bone wall and skin. It can reconstruct the wall of external auditory canal, obliterate mastoid cells and perfectly maintain the normal form of external auditory canal via the cartilage.

Objective To explore the clinical characteristics, surgical treatment and related factors of traumatic implantation cyst of the iris. Design Retrospective cases series. Participants Thirty-six cases of traumatic implantation cyst of the iris. Methods Thir- ty-six cases of traumatic implantation cyst of the iris were reviewed. Main Outcome Measures Age, surgical history, history of disease, surgical method of patients. Results All cases of traumatic implantation cyst of the iris were secondary to perforating injury. 10 cases had been undertaken cataract surgery. The histories of disease in 6 months～1 year and 1 year～10 years were 30.56% respectively. Sur- gical exicision was taken in all cases. There were 2 recurrence cases. Conclusion Traumatic implantation cyst of the iris is almost see- ondary to perforating injury. Surgical excision is an effective strategy to treat this disease.
