1.Clinical analysis of cardiac reserve function and outcome of preeclampsia

Objective To investigate cardiac reserve function and pregnant results of preeclampsia.Methods Pregnant women chosen by randomized table and hospitalized in the department of obstetrics of the first affiliated hospital of Chongqing MedicaI University were involved in this study from January to December,2008.Sixty-nine cases of normal pregnant women were divided into 3 groups:normal group 1(20cases),normal group 2(26 cases)and normal group 3(23 cases).The normal group number 1 to number 3 were paired to gestational hypertension(20 cases),mild preeclampsia(26 cases)and severe preeclampsia (23 cases),respectively.The digital technique of heart sound signal processing WaS used to measure cardiac reserve function parameters including the heart rate(HR),the ratio of the amplitude of the first heart sound to the second heart sound(S1/S2)and the ratio of diastolic to systolic duration(D/S)of pregnant women.The pregnant results were also recorded.Results (1)The ratio of S1/S2 in the group of severe preeclampsia 4.5±3.2 was significantly bigher than the group of gestational hypertension 2.2±1.1 and normal group 32.2±1.1(P<0.01).The ratio of D/S of the group of severe preeclampsia 1.1±0.3 Was significantly lower than the group of gestational hypertension 1.4±0.3 and normal group 31.4±0.2(P<0.01).(2)All cases of the normal group number 1 to number 3 and the group of gestational hypertension were found the ratio of D/S≥1.00,whereas 1 case(1/26,4%)of the group of mild preeclampsia and 4 cases(4/23.17%)of the group of severe preeclampsia were found the ratio of D/S<1.00.(3)The two cases of the pregnant wornen(including one woman died of cardiac arrest)with S1/S2>5.00 and D/S<1.00,who were in the group of severe preeclampsia,were sent to Intensive Care Unit after delivery.There were 4 cases(4/26,15%)and 6 cages(6/23,26%)of 1 minute Apgar score less than 7,7 cases(7/26,27%)and 6 cases(6/23,26%)sent to neonatal Intensive Care Unit,2 cases(2/26,8%)and 4 cases(4/23,17%)of neonatal death in the group of mild preeclampsia and in the group of severe preeclampsia,respectively.All newborns in the normal group number 1 to number 3 and the group of gestational hypertension were normal.Conclusions The cardiac reserve function of a pregnant woman with preeelampsia is decreased with the severity of preeclampsia and the parturient and neonatal results get worse.The distal technique of heart sound signal processing is a safe,harmless,simple and effective tool to measure cardiac reserve funotion of preeclampsia,and is worthy of widely used clinically.
2.Effect of Qiling Decoction combined HAART on expression levels of Treg cells and Th17 in HIV/AIDS patients.

OBJECTIVETo explore the effect of Qiling Decoction (QD) combined highly active antiretroviral treatment (HAART) on expression levels of peripheral blood Th17 and Treg cells in HIV/AIDS patients.
METHODSTotally 55 HIV/AIDS patients were randomly assigned to the treatment group (28 cases) and the combination group (27 cases). Besides, 21 HIV negative patients were recruited as the healthy control group. Those in the treatment group received HARRT alone, while those in the combination group received HAART combined QD. The observation lasted for 24 weeks. Meanwhile, according to peripheral blood CD4+ T cell counts before treatment, HIV/AIDS patients were assigned to three subgroups. For patients in subgroup 1, 1 cells/microL < CD4+ T cell counts < or = 100 cells/microL; For patients in subgroup 2, 101 cells/microL < CD4+ T cell counts < or = 200 cells/lL; For patients in subgroup 3, 201 cells/microL < CD4+ T cell counts < or = 350 cells/microL. Expression of peripheral blood Th17 and Treg cells, and number of CD4+ T cell counts were detected using flow cytometry (FCM)in HIV/AIDS patients at the pre-treatment baseline, week 4, 12, and 24, as well as those in the healthy control group.
RESULTSCompared with the healthy control group, CD4+ T cell counts and the baseline expression level of Th17 cells in the peripheral blood of HIV/AIDS patients significantly decreased, the expression level of Treg cells significantly increased P < 0.01). Compared with before treatment in the same group, CD4+ T cell counts all increased at week 4, 12, and 24 in the two treatment groups, showing statistical difference (P < 0.05, P < 0.01). There was no statistical difference in the effective rate at various CD4+ T cell levels between the two groups (P > 0.05). There was no statistical difference in expression levels of Th17 and Treg cells between the combination group and the treatment group at any time point (all P >0.05). The Th17/Treg ration significantly increased in the combination group after 24 weeks of treatment, showing statistical difference when compared with the treatment group (U = 2.135, P = 0.038).
CONCLUSIONQD could improve the immune balance of Th17/Treg cells, which might be one of its mechanisms for improving HIV/AIDS patients' immunity.

3.Research about effect of spray drying conditions on hygroscopicity of spray dry powder of gubi compound's water extract and its mechanism.

OBJECTIVETo investigate moisture content and hygroscopicity of spray dry powder of Gubi compound's water extract obtained at different spray drying conditions and laying a foundation for spray drying process of Chinese herbal compound preparation.
METHODIn the paper, on the basis of single-factor experiments, the author choose inlet temperature, liquid density, feed rate, air flow rate as investigated factors.
RESULTThe experimental absorption rate-time curve and scanning electron microscopy results showed that under different spray drying conditions the spray-dried powders have different morphology and different adsorption process.
CONCLUSIONAt different spray-dried conditions, the morphology and water content of the powder is different, these differences lead to differences in the adsorption process, at the appropriate inlet temperature and feed rate with a higher sample density and lower air flow rate, in the experimental system the optimum conditions is inlet temperature of 150 degrees C, feed density of 1.05 g x mL(-1), feed rate of 20 mL x min(-1) air flow rate of 30 m3 x h(-1).

4.Clinical value of emergent perspective stent implantation for left-side obstructive colonic cancer

Objective To investigate the clinical value of emergent perspective stent implantation for leftside obstructive colonic cancer.Methods The clinical data of 26 patients with obstructive colonic cancer who received emergent perspective stent implantation at the Affiliated Xuzhou Hospital of Southeast University from October 2011 to February 2014 were retrospectively analyzed.A soft and a hard guidewire and a catheter were applied in the operation.The guidewires were put through the hole of the intestinal tumor,and then the metal stent and the conveyor were guided by the guidewires and were pulled through the hole of the intestinal tumor,finally the stent was released and the conveyor was adjusted to ensure that the stent was at the right position.Patients were followed-up through outpatient examination till 4 weeks after tumor resection.Results Of the 26 patients,9 were with rectal cancer,10 with sigmoid colonic cancer,6 with descending colonic cancer,1 with splenic flexure cancer.The median length of stenosis was 4.6 cm (range,2.0-8.0 cm).The surgery of the 26 patients was successful.The mean operation time was 35.2 minutes (range,15.2-72.0 minutes),and the mean time of stent implantation was 5.6 minutes (range,2.6-26.9 minutes).Patients had watery or loose stool for 4-8 times after stent implantation.Ten hours after the operation,all the patients were given liquid diet.The remission rate of clinical symptoms was 100.0% (26/26).No colonic perforation was detected during the operation.Two patients were complicated with slight bleeding,and was alleviated by medication.Twenty-six patients received stage Ⅰ tumor resection procedure within 7-10 days after the symptoms of intestinal obstruction were remised.The success rate of surgery was 100.0% (26/26).No infection and other drainage were detected after tumor resection through follow-up.Conclusion Emergent perspective stent implantation for left-side obstructive colonic cancer is safe and effective.
5.The posterior approach combined with pedicle screw fixation for the treatment of lower cervical fractures and dislocation

Objective To evaluate the feasibility of using the posterior approach with cervical pedicle screw fixation technique for the treatment of lower cervical spine fractures and dislocations.Methods Thirty patients suffered lower cervical fractures and dislocations were retrospectively analyzed,and they were underwent cervical pedicle screw system fixation surgery from January 2010 to December 2012.There were 22 males and 8 females,with an average age of 41 years (range,24-61 years).Eight injuries were located at C4,5,12 at C5,6,and 10 at C6.7.According to the American Spinal Cord Injury Association (ASIA)impairment scale,8 cases were grade A,12 were grade B,5 were grade C,3 were grade D,and 2 were grade E.Results Reduction and fixation of the injured segments were performed via a posterior approach in all 30 patients.One hundred and forty pedicle screws were inserted successfully in all patients on the postoperative radiographic and CT scans.All the patients were followed up for 3-23 months,with the average of 11 months.According to the ASIA impairment scale,eight cases with grade A were still grade A,but the feeling and movement improved obviously.Ten grade B improved to grade C and the rest cases were complete recovery after operations.During the course of the follow up,solid bone union was achieved in all patients,and there were no hardware failures.All the patients' postoperative MRI showed disappearance of the spinal cord compression after reduction in all cases.Conclusion For the patients of lower cervical fractures and dislocation,the first choice of operation should be the posterior approach reduction and fixation with cervical pedicle screw.The correct lengthwise traction method could unblock the locked facet effectively,and prevent the retrusion of disc and the further injury of spinal cord.For the patients of disc disruption,the posterior approach could be used only.The cervical pedicle screw system could provide three-dimensional reduction of the injured cervical segments and the adequate mechanical strength.The posterior approach combined with pedicle screw fixation is feasible and effective for the treatment of lower cervical fractures and dislocation.
6.Mobilization efficiency of granulocyte colony-stimulating factor and stem cell factor to bone marrow mononuclear cells and mechanisms.

The mobilization efficiency of granulocyte colony-stimulating factor (G-CSF) and stem cell factor (SCF) to bone marrow mononuclear cells (MNCs) in mice was observed, and the changes of CXCL12/CXCR4 signal were detected in order to find out the mobilization mechanism of stem cells. Kunming mice were randomly divided into two groups. The mice in treatment group were subjected to subcutaneous injection of G-CSF at a dose of 100 mug/kg and SCF at a dose of 25 mug/kg every day for 5 days, and those in control group were given isodose physiological saline. The MNCs were separated, counted and cultured, and the colony-forming unit-fibroblast (CFU-F) was evaluated. CD34(+)CXCR4(+) MNCs were sorted by flow cytometry. The expression of CXCL12 protein in bone marrow extracellular fluid was detected by ELISA, and that of CXCL12 mRNA in bone marrow was measured by RT-PCR. The results showed that the counts of MNCs in peripheral blood and bone marrow were increased after administration of G-CSF/SCF (P<0.01). The factors had a dramatic effect on the expansion capability of CFU-F (P<0.05). Flow cytometric of bone marrow MNCs surface markers revealed that CD34(+)CXCR4(+) cells accounted for 44.6%+/-8.7% of the total CD34(+) MNCs. Moreover, G-CSF/SCF treatment induced a decrease in bone marrow CXCL12 mRNA that closely mirrored the fall in CXCL12 protein. In this study, it is evidenced that G-CSF/SCF can effectively induce MNCs mobilization by disrupting the balance of CXCL12/CXCR4 signaling pathway in the bone marrow and down-regulating the interaction of CXCL12/CXCR4.
7.Gene therapy with human vascular endothelial growth factor in prevention of restenosis after angioplasty

Objective: To investigate the effect of human vascular endothelial growth factor on restenosis after angioplasty. Methods: A rabbit model of injured carotid artery was established using percutaneous transluminal angioplasty. The pcDNA3/hVEGF165(500 μg,n=12) and pcDNA3 (500 μg,n=12) were separately transfected into injured arterial wall with 30 min incubation. The carotid artery was imaged by arotic angiography at the end of week 2 and week 4. Pathology analysis and Northern blot analysis were performed for harvested injured artery segment. Results: Arotic angiography showed carotid artery diameter narrowness were obviously lessened at week 2 and week 4 in experimental group than that in control group; H-E stains showed lumina narrow ratio were obviously reduced at week 2 and week 4 in experimental group than that in control group[(9.58±1.35)% vs (31.72±1.72)%;(18.09±2.93)% vs (44.05±3.28)%, P<0.01 ]; By Northern blot analysis, the expression of hVEGF165mRNA in experimental group were upregulated than in contol group. Conclusion: pcDNA3/hVEGF165 can be transfected into smooth muscle cell and continue to secret bioactivity protein at least for 4 weeks; it can accelerate reendothelialization and prevent restenosis.
8.Progress in epigenetic study of clear cell renal cell carcinoma

The epigenetic changes of clear cell renal cell carcinoma(ccRCC )are considered to be the main molecular mechanisms of its pathogenesis ,including DNA methylation ,histone modification ,microRNA change ,and so on.DNA methyltransferases(DNMTs)catalyze the occurrence of DNA methylation.DNA methylation changes are mani-fested in the overall low methylation of the genome and the high methylation of specific sites ,which were involved in the de-velopment of ccRCC by affecting the expression of tumor suppressor genes.Due to histone-modified enzyme involvement ,histone modification is shown as possible genetic reversal.MicroRNA plays an important role in the abnormal expression of ccRCC genes.With the studies of epigenetic mechanism and molecular pathology ,it is important to explore the mechanisms and to seek effective early diagnosis ,treatment and prognosis intervention of ccRCC.
9.An analysis on the epidemic characteristics and influencing factors of dyslipidemia among adults in Wenzhou

Objective To learn the prevalence situation of dyslipidemia among adult residents in the southern coastal area, Wenzhou,Zhejiang.Methods Adult residents were chosen by using multi -stage sampling method from 11 counties in Wenzhou in 2013.Questionnaire survey,medical examination,and biochemical detection triglycerides were conducted among the residents.Chi -square test,and multiple logistic regression analysis was used to identify the risk factors related to dyslipidemia.Results The overall prevalence of dyslipidemia was 44.99%(the standardized prevalence rate was 42. 93%),and prevalence rate in women (43.91%)was higher than that in men (45.80%),but the urban(14.26%)and rural(13.83)areas prevalence was not significant.The awareness rate of dyslipidemia was only 21.73%.The prevalence of isolated low HDL -C,high LDL -C,hypercholesterolemia,hypertriglyceridemia,mixed hyperlipidemia was 13.95%, 10.45%,19.34%,12.98%,4.69%,respectively.Multivariate logistic regression analysis showed that gender,age, nationality,coronary heart disease,dyslipidemia,family history,aquatic products,milk,pickled products edible frequency,living and working pressure,body mass index,central obesity,hypertension,diabetes were related the prevalence of dyslipidemia,but the risk factors of different lipid fractions were different.Conclusion The prevalence of dyslipidemia has been in a high level among adult residents in Wenzhou,but the awareness rate of dyslipidemia was low. Measures should be Strengthened to prevent dyslipidemia .
10.A clinical study of cardiac reserve mobilizing condition for pregnant women.

This study was conducted on the basis of informed consent of subjects and was approved by the ethical review committee concerned. 527 pregnant women voluntarily participated in this project for the investigation of cardiac reserve mobilizing condition. Using the digital technique of heart sound signal processing, we measured the heart rate (HR), the ratio of the first heart sound to the second heart sound (S1/S2)and the ratio of diastolic to systolic duration (D/S) during pregnancy. There was significant difference of HR and S1/S2 between the group of non-pregnant (G1), the group of 28-36 pregnant weeks (G2), and the group of 37-42 pregnant weeks (G3) (HR, S1/S2: G1 vs G2, G1 vs G3: P < 0.01). HR of the pregnant women increased with the increase of pregnant weeks. D/S decreased with the increase of pregnant weeks. There was significant difference of D/S between G1, G2, and G3 (G1 vs. G2: P < 0.01; G1 vs. G3: P < 0.01). There was also significant difference of D/S between G2 and G3 (G2 vs. G3: P < 0.05). Everybody in the non-pregnant women group was found to have D/S > or = 1.30; 64.33% of pregnant women were found to have 1.00 < D/S < 1.30, whereas 3.05% of pregnant women were found to have D/S < 1.00. These data revealed that the heart burden of the pregnant woman increased with the increase of pregnant weeks and the mobilization of cardiac reserve.
