1.Pulmonary inflammatory pseudotumor:clinical characteristics and surgical treatment

Purpose: To investigate the pathogenesis and clinical treatment of pulmonary inflammatory pseudotumor (PIP). Methods:To analyze the pathogenic characteristics and X-ray findings of 18 cases of PIP. Results:The rate of incidence in PIP is increasing in recent years .70 percent of the patients have infection of the respiratory system and is related to the abuse of large doses of antibiotics, there is little specificity in X-ray or CT so it is easy to confuse with malignant tumor of the lung .In pathologic sections of PIP, no malignant changes were seen, and only inflammatory cells, lymphocytes and plasm cells could be seen by light microscope. Conclusions:The antibiotic must be used properly in infection of the respiratory system and surgical treatment can be done in some asses to exclude tumor of the lung in diagnosis.
2.Study on relationship between changes of serum hs-CRP level and stroke subtype according to TOAST criteria in patients with acute cerebral infarction

Objective To investigate the changes of the serum high sensitive C-reactive protein (hs-CRP)level in patients with acute cerebral infarction (ACI) and study the relationship between hs-CRP level and ischemic stroke subtypes according to the trail of org10172 in acute stroke treatment (TOAST) criteria.Methods The serum hs-CRP was measured in 152 ACI patients by immunonephelometrical method. All ACI patients were classified into 5 major ischemic stroke subtypes based on TOAST criteria. And then the relationship between hs-CRP level and their TOAST subtypes were analysed.Results The percentage of each ischemic stroke TOAST subtypes of 152 patients was as following: stroke of undetermined etiology 41.45%, small-vessel occlusion 34.87%, cardioembolism 5.26%, large-artery atherosclerosis 15.79%, and stroke of other determined etiology 2.6%. Among 5 ischemic stroke subtypes, cardioembolism patients were related to the highest positive rate of hs-CRP(87.50%) and the highest hs-CRP level [(11.60?7.85)mg/L]. The hs-CRP level in large-artery atherosclerosis or stroke of other determined etiology were (10.77?4.27) mg/L or (6.45?3.25) mg/L respectively, Stroke of undetermined etiology and small-vessel occlusion subtypes were associated with the lowest positive rate of hs-CRP(38.10%, 37.74%) and the lowest hs-CRP level [(4.09?5.65)mg/L,(3.99?0.56)mg/L]. 3.Current status of age-related eye diseases in elderly population and their visual function and visual-related quality of life

Objective To explore the prevalence of age-related eye diseases (AREDs) among people aged 70 years or above in Hangzhou and evaluate the impact of AREDs on visual function (VF) and visual-related quality of life (QOL) in elders.Methods This study involved a total 2 111 elderly people (≥70 years).All participants received visual acuity and comprehensive eye examinations and complete VF and QoL questionnaires.Results The main cause of visual impairment was AREDs.And the causes were age-related cataract (79.82%),AMD (45.64%),glaucoma (10.95%) and diabetic retinopathy (DR,7.30%).VF and QoL scores declined gradually with age.And the scores declined more rapidly with declined visual acuity among the elders.VF and QoL scores in patients with age-related cataract,AMD,glaucoma and DR were successively lower.After adjusting for age,gender and visual acuity,the elders with AREDs had lower scores across all domains of VF and QoL.Scores for subscales of VF and QoL domains were significantly lower among those with DR and glaucoma compared with those with age-related cataract and AMD.Conclusion Age,presenting vision and AREDs are associated with VF and QoL in this elderly population.And senile people with glaucoma and DR have severe declines in VF and QoL,independent of presenting visual acuity.
4.Detection of Early Cervical Cancer Patient’s Peripheral Blood CK19 mRNA before Surgery and its Clinical Significance

Objective To investigate the detection of early cervical cancer patient's peripheral blood CK19 mRNA and its clinical significance. Methods 30 early (ⅠA2~ⅡA) cervical cancer patients from the department of gynecological tumors in the third affiliated hospital of Kunming Medical University were selected as experimental group, 15 patients with uterine myoma and 15 healthy volunteers as negative control group,15 advanced (ⅡB~ⅢB) cervical cancer patients as positive control group. Peripheral blood samples before receiving radical resection of experimental group,and the same samples before treatment of control groups were collected. The change of relative expression level and positive rate of peripheral blood CK19 was analyzed using qRT-PCR technique, and its correlation with clinicopathological parameters and prognosis was further analyzed. Results There was significant difference in relative expression level of CK19 mRNA in the peripheral blood between experimental group and control groups ( <0.001) . And the positive rate of CK19 in the peripheral blood from 30 patients with early cervical cancer before operation in the experimental group was 30% (9/30) . Furthermore, CK19 mRNA positive rate before surgery was correlated with lymph node metastasis, muscularis invasion and pathologic risk factors for postoperative ( <0.05) . There were no significant correlation between CK19 mRNA positive rate and other clinical and pathological features such as age,clinical stage (FIGO),tumor cell differentiation,tumor size,pathological type and etc ( > 0.05) . Conclusions CK-19 mRNA is a specific and suitable molecular marker for the detection of circulating tumor cells in early cervical cancer. Detection of peripheral blood CK19 mRNA expression before receiving radical hysterectomy in patients with early cervical cancer can also make up for the disadvantage of other methods such as the imaging, tumor marker detection. Most important of all, it can provide the basis for choosing a proper adjuvant therapy post-operation and estimating the relapse and prognosis.
5.The clinical senses of serum insulin and C-peptide in patients with different subtypes of acute cerebral infarct

Objective To investigate the pattern of fluctuations and clinical significance of fasting serum insulin(FINS) and C-peptide(CP) levels in patients with different subtypes of acute cerebral infarct(ACI) and its relationship with serum lipid. Methods FINS and CP were measured in 152 ACI patients by chemiluminescent immunoassay. All ACI patients were classified into 5 major ischemic stroke subtypes according to the trial of org10172 in acute stroke treatment(TOAST) criteria. And then the relationship between FINS and CP level and serum lipid in different TOAST subtypes were analysed. Results The percentage of each ischemic stroke TOAST subtype was as follow: stroke of undetermined etiology 40.13%, small-vessel occlusion 34.21%, cardioembolism 5.26%, large-artery atherosclerosis 15.79%, and stroke of other determined etiology 4.61%. Among 5 major stroke subtypes, large-artery atherosclerosis patients had the highest levels of FINS and CP. The levels of FINS and CP in small-vessel occlusion were (8.237?5.144) ?U/ml and (1.761?0.975)ng/ml,respectively. Stroke of other determined etiology subtypes were associated with the lowest levels of FINS and CP. Apparently, other factors, such as TC, TG, LDL, SBP, DBP, age and HDL, could also affect the levels of FINS and CP in serum. Conlusions Levels of FINS and CP varied in different subtypes of ACI. There was a significant correlation among insulin resistance, hyperinsulinemia(HIS) and lipid metabolic abnormality in ACI.
6.Effect of angiotensin Ⅱ receptors on collagen synthesis of fibroblasts derived from human hypertrophic scars

BACKGROUND: It has been reported that Angiotensin Ⅱ (Ang Ⅱ) is related to occurrence and development of dermatofibrosis; however, less is explored about the expression and effect of AT1 and AT2 receptors in the fibroblasts of human hypertrophic scar.OBJECTIVE: To observe the expression of Ang Ⅱ type 1 (AT1) and type 2 (AT2) receptors in human hypertrophic scars, and explore their effects on collagen synthesis of fibroblasts.DESIGN, TIME AND SETTING: Randomized control experiment was performed at the Experimental Center, Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA between August 2006 and November 2007. PARTICIPANTS: Samples of hypertrophic scare were taken from 18 patients (10 males and 8 females, 19-47 years Old). Seven specimens of normal skin served as control. All of the specimens collected were divided into two parts, one part for immunohistochemical staining after fixated by 4% paraformaldehyde, the other part for culturing fibroblasts.METHODS: The expression of both AT1 and AT2 receptors in fibroblasts of hypertrophic scare was detected with immunohistochemical staining and radioligand receptor binding assay. Collagen synthesis was examined in cultured fibroblasts of hypertrophic scars by measuring [3H]-proline incorporation into collagenous proteins.MAIN OUTCOME MEASURES: The expression of both AT1 and AT2 receptors in human hypertrophic scars; the [3H]-proline incorporation value in cultured fibroblasts.RESULTS: Positive staining signals of both AT1 and AT2 receptors were found in fibroblasts of hypertrophic scars. Similar results were also observed in cultured fibroblasts of hypertrophic scars, expression level of AT1 and AT2 receptors were (10.69±2.15) fmol/106 cells and (4.9±1.05) fmol/106cells, respectively. In cultured fibreblasts, Ang Ⅱ stimulation significantly increased collagen synthesis, which was inhibited by valsartan, an AT1 receptor blocker, but augmented by PD123319, an AT2 receptor antagonist.CONCLUSION: Both AT1 and AT2 receptors were expressee in the fibreblasts of hypertrophic scars, and Ang Ⅱ regulates collagen synthesis in hypertrophic scar fibroblasts through a negative cross-talk between AT1 and AT2 receptors, which might contribute, at least partly to formation and maturation of human hypertrophic scars.
7.Survey of totally thoracoscopic anatomic segmentectomy for the peripheral stage ⅠA non small cell lung cancer

Objective To evaluate the safety and feasibility of totally thoracoscopic anatomic pulmonary segmentectomy (TTAS) for the treatment of the peripheral stage ⅠA non small cell lung cancer(NSCLC).Methods The study involved 50 consecutive patients undergoing totally thoracoscopic anatomic segmentectomy (TTAS) from September 2010 to November 2012 in the First People's Hospital affiliatied to Nanjing Medical University.The diameter of the tumors were less than 2 cm [(mean diameter(1.35 ±0.48) cm].All lymph node sampling of N1 and N2 were neglive,All patients received symtematic lymph node dissection.The pulmonary vessels were individually ligated,and the bronchi were closed using an endoscopic stapler.The intersegmental plane was identified using the demarcation between the resected(inflated) and preserved(collapsed) lungs.Staplers were used for intersegmental dissection.Results The mean operative time and intraoperative bleeding were (191.5 ± 50.4) min and (49.2 ± 54.6) ml respectively.The chest tube drainage duration was (3 ± 1) days.The number of stapler cartridges used for intersegmental division was 3.9 ±0.8.The mean number of lymph nodes and nodal stations dissected were 12.6 ± 2.8 and 6.0 ± 1.5 respectively.No mortality and complications were observed 30 days after the surgery.Further,no local recurrence or metastases were observed during follow-up.Conclusion Totally thoracoscopic anatomic segmentectomy(TTAS) is a feasible and safe technique.With systematic lymph node dissection,TTAS can be a reasonable therapeutic option for stage ⅠA NSCLC.
8.THE EXPERIMENTAL STUDY OF ROLLING PATENCY TEST

Immediate patency assessment of a completed microvascular anastomosis is considered an essential step in a microsurgical procedure. The most frequently used patency test is the milking patency test, which is too traumatic for use in clinical microsurgery. An atraumatic patency test remains to be devised. According to the principle of mechanics that rolling friction is smaller than sliding friction, we designed the so-called rolling patency test. The femoral arteries of Wistar rats were used in the experimental study to make a comparison between the two patency tests. The result showed that intact endothelial cells covered more than 90% of the inner wall of the artery in the rolling patency test, but less than 25% in the milking patency test. The study suggests that the rolling patency test is a reliable and atraumatic immediate patency test for use in clinical microsurgery.
9.Effects of Low Load Exercise on Gait and Balance in Patients with Parkinson's Disease

Objective To explore the effects of low load exercise on the gait and balance in patients with Parkinson's disease. Methods 12 inpatients with Parkinson's disease from May to August, 2015 accepted low load exercise on Power Rehabilitation System 14 times in 2 weeks, with the medication as before. They were assessed with 3D gait analysis and Berg Balance Scale before and after treatment. Results The step length, stride length and walking speed improved after treatment (P<0.05), and improved more in the seriously affected limbs. There was no difference in cadence, stride time, step time, single support, double support, opposite foot off and opposite foot contact after treatment (P>0.05). The scores of Berg Balance Scale improved after treatment (P<0.05). Conclusion The low load exercise can improve the gait and balance in patients with Parkinson's disease.

Objective:To study the role of adhesion molecules in the pathogenesis of polymyositis. Methods:The abnormal expression of adhesion molecules on T cells in peripheral blood and muscle fibers from patients with myositis was analyzed by two colour immunofluoresence and RT PCR methods respectively. Results:The expression of adhesion molecules including lymphocyte function associated antigen 1(LFA 1 ),very late antigen 4(VLA 4) on T cells in peripheral blood and intercellular adhesion molecule l(ICAM 1) on muscle fibers from patients with myositis was markedly higher than that in the healthy control group. Conclusion: These findings suggested that adhesion molecules may be responsible for the migration of T cells and destraction of muscle fibers.
