1.Analysis of related factors of postoperative delirium in elderly patients

Objective To explore related influencing factors for postoperative delirium in elderly patients, to provide a reference for the prevention and treatment. Methods Fifty-four patients with delirium after surgery were used as observa?tion group, and a total of 150 subjects with no delirium after surgery during the same period were selected as the control group. Data of age, gender, malnutrition, disorders of water and electrolyte metabolism, postoperative mechanical ventilation, postoperative hypoxemia, severe infection and postoperative pain degree, and the combination of basic diseases were com?pared and analysed between two groups. The binary logistic regression analysis was used to analyse the influencing factors of postoperative delirium. The outcome and prognosis were observed and analyzed in observation group. Results The average age was significantly higher in observation group than that of control group (P<0.05). The percentages of postoperative hy?poxemia and severe infection were significantly higher in observation group than those of control group ( P<0.05). Patients with higher age, postoperative hypoxemia and severe infection were risk factors for postoperative delirium. In observation group,1 case died of lung infection, 1 case died of multiple organ failure, the remaining 52 patients were improved and dis?charged from hospital after three months. Conclusion For patients with higher age, postoperative hypoxemia and severe in?fection are the risk factors for occurrence of postoperative delirium. More attention should be paid to clinical preoperative and postoperative periods.

3.Biocompatibility of new absorbable material:nano-composite of poly (L-lactide) and surface grafted hydroxyapatite

Objective To make a comprehensive evaluation of nano-composite of poly(L-lactide) and surface grafted hydroxyapatite(PLLA/PLLA-gHA) as a new material.Methods According to the evaluated critera of medical implanted materials biology and animal trial recommended in GB/T 16886 and IS0 10993 criterion,the new material was carried out on acute systemic toxicity test,haemolysis test,muscular implantation test and subcutaneous injection test.The extract liquid of new material was injected into mice by vena caudalis to test common station,toxic reaction of it at different time,the results were used to evaluate the acute systemic toxicity.Fresh anticoagulant cony blood was mixed with extract liquid of new material with density of 100 g?L-1 to measure each absorbance with spectrophotometer and work out the corresponding rate of haemolysis.The red punctuation and hydropsia of rabbits were observed at different time by subcuntaneous injecting extract liquid into the back of rabbits.PLLA/PLLA-gHA composite plates were implanted into the sacrospinal muscle of rabbits.Cony venous blood was extracted to detect indicatrix of hematology at diferrent time.The material and surrounded tissues were taken out from animals at the 14th,30th,60th,90th,180th,360th day to examine anatomic and pathological changes.Results Rabbits with PLLA/PLLA-gHA composite had good general condition.There was no any acute systemic toxicity in vivo.Data of AST and Scr had no significant difference between experimental group and control group.The hemolysis rate of extrac liquid was 1.22%,which was under the standard criteria(5%).No red punctuation and light hydropsia were observed at different time in the subcutaneous injection test.The inflammation cytochange of PLLA/PLLA-gHA composite group was similar with that of control group in early days,which was met with the general regularity of inflammatory outcome.The fibrosis membrane surrounding the PLLA/PLLA-gHA composite became thinner gradually with the elongation of implantation time.The fibrosis membrane grew into the material at the 360th day.The degree of the fibrosis membrane was below class Ⅰ.Conclusion The new absorbable type PLLA/PLLA-gHA composite has excellent biocompatibility and security.

5.Clinical Distribution and Drug Resistance of Pseudomonas aeruginosa Isolates

OBJECTIVE To investigate the clinical distribution and drug resistance of clinical Pseudomonas aerugilosa isolates,and offer reasonable experimental data for clinical therapy.METHODS P.aerugilosa was identified by ATB Expression system,and its drug resistance was determined by Kirby-bauer and ATB Expression method.RESULTS The main departments in which frequently P.aerugilosa infection accurred were Intensive Care Unit(41.2%) and Respiration Departments(19.3%).The common site of P.aerugilosa infection was respiratory tract(68.2%).The sensitive rate of P.aerugilosa to polymyxin E and cefoperazone/sulbactam was the highest(95.1% and 91.4%),while to meropenem and imipenem was 77.5% and 70.6%.The highest resistant rate of P.aerugilosa to trimethoprim/sulfamethoxazole,ampicillin/sulbactam was 97.1% and 95.1%.The resistunce to ciprofloxacin,ticarcillin and piperacillin,were 64.9%,63.3% and 56.3%.CONCLUSIONS P.aeruginosa is major pathoge in our hospital.It is important to select antibiotics correctly according to the results of susceptibility tests.
6.Study of physiology of visual cortex activated by rotating grating with functional MRI

Objective To research the physiology of visual cortex activated by rotating grating with functional-MRI(fMRI), and to identify the components of the activation. Methods Functional MRI was performed in 9 healthy volunteers by using GRE-EPI sequences on a 1.5 T MR scanner. In the block designing, rotating grating, static grating, and luminance were plotted as task states, while static grating, luminance, and darkness were set as control states. The stimuli tasks included six steps. Imaging processing and statistical analysis was carried out off-line using SPM99 in single-subject method. Results Some respective areas of visual cortex were activated by the various stimuli information supplied by rotating grating. The strong activation in the middle of occipital lobe located at primary vision area was related to the stimuli of white luminance. Its average maximum points were at 13,-98,-2 and 11, -100,-4. The bilateral activations of Brodmann 19th area located at MT area were related to visual motion perception. Its average maximum points were at 46,-72,-2 and -44,-74, 0. The mild activation in the middle of occipital lobe was related to form perception. Its average maximum points were at -12,-98,-6 and -16,-96, (-6.) Conclusion The plotting of control state is important in bock design. The effective visual information of rotating grating includes components of luminance, visual motion perception, and form perception. FMRI has potential as a tool for studying the basic physiology of visual cortex.

8.Initial therapy indications of acute primary immune thrombocytopenia in children

Objective To explore the initial therapy indications of acute immune thrombocytopenia (ITP) in children based on the classification treatment.Methods Three hundred and eighty newly diagnosed ITP cases were enrolled in this study from Jan.1st 2012 to Apr.30th 2013 in Children's Hospital Affiliated to Shanghai Jiaotong University.In total 380 patients,there were 214 male cases (56.31%) and 166 female cases (43.68%).The cases were divided into observation group and therapy group according to the initial platelet count which was ≤ 30 × 109/L or the bleeding over moderate volume or with active bleeding.Platelet values were observed in the observation group weekly,adrenal cortical hormone and immunoglobulin treatment were adopted in the therapy group,cases were followed up to Aug.30th 2013,9 months on the average.Results Three hundred and five cases showed overall response (80.26%) and 75 cases showed no response(19.74%).One hundred and seventy-eight cases were divided into observation group (46.84%),in which 133 cases (74.72%) showed complete response or response.Two hundred and two cases were included in therapy group (53.16%),in which 167 cases (82.67%) were with complete response or response.There was no statistical difference between the 2 groups in curative effect (Z =-0.54,P =0.59).Forty-five cases in observation group were no response and accepted therapy,35 of them (77.78%) had response.There were equal efficiency in the initial therapy group and the subsequent therapy group (x2 =3.60,P =0.06).There was no difference between the age of onset,sex and season in 2 groups.Cases aged from 1 month to 1 year seemed to have a high incidence because of vaccination,and in the cases aged from 3 to 14 years the onset was related to infection.The children over 3 years old had higher risk factors in self-healing and the curative effect.There was no severe bleeding or adverse effect or dead cases in this study.Conclusions It is feasible to take platelet count ≤30 × 109/L as the threshold for initial therapy indications.Almost half of the cases could avoid overtreatment and pretherapy observation will not reduce the initial cure effect; no severe internal bleeding was observed in all the cases.
9.Perioperative nutritional support in patients with liver transplantation

Objective: To investigate the effective methods of nutritional support, to improve the endurance for the operation and to promote postoperative recovery in patients undergoing liver transplantation. Methods: The perioperative nutritional condition and nutritional-support methods were reviewed in 33 patients with liver transplantation. In the first 3 postoperative days, parenteral nutrition (PN) was used in combination with infusion of human albumin and plasma. From the 4th to 5th day, enteral nutrition (EN) was used in combination with PN. Finally, the complete oral intake of food was applied. Results: Of 33 patients, there were 30 patients whose serum levels of total protein (TP) and albumin (ALB) were elevated, and glutamic-pyruvic transminase (GPT), total-bilirubin (T-BIL) and combined-bilirubin (D-BIL) were descended after nutrition support. Conclusion: The operative endurance and postoperative recovery were improved effectively by proper nutritional support in liver transplantation patients.
10.Research on the physical components stimulated by rotating grating using functional magnetic resonance imaging

AIM: Using functional magnetic resonance imaging, to inspect v sual cortex physical reactions stimulated by rotating grating, and to its components. METHODS: On 1.5T MR scanner, GRE-EPI imaging sequence was carried on 9 h discover ealthy volunteers, the visual cortex response data were processed after delinearation by SPM99.RESULTS: Different components of rotating grating excited different areas of the visual cortex. Dramatic response in the central part of the occipital lobe, which was related to white light stimuli, located at primary visual cortex. Response area in bilateral Broadaman 19 areas was related to vision-motion function. And weak response area in the central part of the occipital lobe was related to shape perception.CONCLUSION: Rotating grating conclude plenty of visual information, and it excites different areas of the optic center as a stimuli. fMRI is a valuable equipment to study the physiology of visual cortex.
