1.Pathogenesis, clinical evaluation and treatment of neuropathic cancer pain

Neuropathic cancer pain (NCP) arises from physical or chemical damage to peripheral or central neurons or in the neural conduction system.The mechanisms of NCP include pain directly related to tumor involvement,pain associated with chemotherapy,radiotherapy and surgery,neuropathic syndromes associated with paraneoplastic syndromes,inflammation and other factors.A detailed history and careful physical examination are important means of diagnosis of NCP.The clinical evaluation of NCP should use standardized pain assessment scale.Till now,the treatments of NCP include opioid combined with auxiliary analgesic drugs,interventional treatment and gene treatment.Deciding treatment strategies according to the pathogenesis of NCP,multidisciplinary collaboration,combined therapy with different analgesic drugs and technologies are the therapeutic directions for NCP.
2.Primary giant cell angioblastoma of bone:a clinicopathological study

Purpose To analyze the clinicopathological characteristics of giant cell angioblastoma ( GCAB) presenting primarily in the bone. The differential diagnosis is discussed. Methods The clinical, radiological and histopathological profiles in three cases of bony GCAB were reviewed. Results The tumor occurred in two adult females and one boy. The lesion involved the second vertebrae, left distal femur and 3rd to 5th left metacarpal bones in one case each. Clinically, all three patients presented with bone pain. Two patients were accompanied with restricted mobility. Radiological examination showed bone destruction with involvement of adjacent soft tissue in two cases. Histologically, the tumor was composed of irregular angiomatoid nodules which showed striking plexiform architecture. On high power, the nodules were composed of small vessels surrounded by proliferative oval-to-spindled pericytes which showed concentric aggregation around the vascular channels. The hallmark of the lesion was the presence of multinucleated osteoclast-like giant cells scat-tered within the nodules. By immunohistochemistry, the endothelial cells were positive for CD31 and CD34, whereas the surrounding pericytes were positive for alpha smooth muscle actin and vimentin. CD68 staining highlighted the osteoclast-like giant cells. Conclu-sions GCAB is a distinctive vascular tumor of intermediate malignancy characterize by local aggressiveness. On rare occasions, GCAB may occur in adult patients and manifest as primary bone lesions. Awareness of its characteristic features will help in the distinction from a variety of disease including granulomatous lesions, plexiform fibrous histiocytoma, tufted hemangioma and Kaposiform heman-gioendothelioma.
3.Strategic thinking of human oocyte cryopreservation

BACKGROUND:Because of the special and complicated biological characteristics of oocytes, the suitable cyropreservation technology for oocytes faces more various chalenges. However, the uneven survival rate and fertilization, damages of developmental potential of the thawed oocyte stil exist. OBJECTIVE:To introduce the progress in basic and application researches of oocyte cryopreservation technology, and to iluminate the technical defects and thoughts and possible research points to overcome these problems. METHODS: A computer-based search of CNKI and PubMed was performed for articles concerning oocyte cryopreservation from January 2004 to October 2014. The search terms were oocyte, cryopreservation, vitrification in Chinese and English, respectively. The old and repeated articles were excluded. Finaly, 41 articles were included in result analysis. RESULTS AND CONCLUSION:The cryopreservation of oocytes by slow freezing and vitrification has been used in clinic. At the same time, the uneven survival and fertilization rate, damaged developmental potential of the thawed oocyte stil puzzle the clinicians. The key point to breakthrough or improvement of oocyte cryopreservation technology is the systematization of the protocol for oocylte cryopreservation, for example, the choice of cryoprotective agents, the development of carriers for oocytes, and the determination of oocyte stage. Furthermore, other related technologies should also be considered, including thein vitro mature technology of oocytes, cryopreservation and transplantation of ovary tissues.
4.Protective Effects of Rosiglitazone Maleate on Renal Ischemia-Reperfusion Injury in Mice and Its Mechanism

OBJECTIVE: To investigate the protective effects of rosiglitazone maleate on renal ischemia-reperfusion injury in mice and its mechanism.METHODS: 30 mice were randomly divided into sham operation group(normal saline),model group(normal saline) and rosiglitazone maleate group(rosiglitazone maleate 5 mg?kg-1?d-1).Three groups received relevant drugs for 10 days.Renal ischemia-reperfusion injury model was induced in model group and rosiglitazone maleate group by clipping the renal pedicle 40 minutes after the last administration.The renal index,the content of blood urea nitrogen(BUN) and creatinine(Cr),the content of malondialdehyde(MDA),the activities of superoxide dismutase(SOD) and glutathione peroxidase(GSH-Px) in renal tissue were detected.Pathological change of renal tissue was observed by HE stain.RESULTS: Compared with model group,the renal index of the mice in rosiglitazone maleate group reduced(P

So far, the major therapeutic drugs of diabetes are western medicine , but side effects of hypoglycemic western med-icine are large and not easy to take for a long time .In recent years , we found the side effects of some kind of hypoglycemic Chinese herbal medicine were small , much easier to be accepted by the patients .Thus, the clinical treatment of diabetes may have new meth-ods.According to the mechanisms of Traditional Chinese Medicine for protection of islet function , this review will discuss them from re-ducing oxidative stress , increasing the number of islet cells and promoting βcells to secrete, relieving insulin resistance , increasing in-sulin sensitivity and so on .All of these aspects reflect the unique role of Traditional Chinese Medicine in the treatment of diabetes and protection of islet function .
6.Efficacy of lipid-lowering and anti-inflammatory effects of statins sequential therapy in coronary ;heart disease patients undergoing elective PCI

Objective To assess the inlfuence and safety of early atorvastatin sequential therapy in coronary heart disease (CHD) patients underdoing elective percutaneous coronary intervention on selected indicators of inflammation and serum lipids. Methods A total of 88 CHD patients who got ready to receive the elective percutaneous coronary intervention (PCI) were divided in two groups at random:The sequential dose group was called group A (atorvatatin 80mg as loading dose ,40 mg/d for 1 month after PCI and 20 mg/d subsequently, n=43), and the ordinary dose group was called group B ( atorvastatin 20 mg/d, n=45). During the follow-up, blood samples were taken at baseline, 3 days,1 month, 3 months and 6 months for myeloperoxidase (MPO), matrix metalloproteinase-9(MMP-9), serum lipids, serum alanine aminotransferase (ALT), glutamyl endopeptidase (GGT) and creatine kinase (CK) levels. Main adverse cardiac events and adverse effects were also analyzed. Results Compared with the baseline, the level of low-density lipoprotein-cholesterol (LDL-C) and total cholesterol (TC) was signiifcantly decreased in both two groups after treatment (P < 0.05). The LDL-C and TC in both groups were all decreased after treatment, and the degree of LDL-C and TC decreased at 3 days had signiifcant differences between the two groups (TC:26.6%vs. 14.0%;LDL-C:36.9%vs. 19.4%, both P<0.05). The degree of LDL-C decreased at 1 month between the two groups also had signiifcant differences (45.0%vs. 26.9%;P<0.05). The changes in LDL-C and TC level were also signiifcantly different between the two groups at 3 and 6 months (P<0.05). While TG and HDL-C remained unchanged. MMP-9 and MPO were both reduced post medication compared to pre-treatment levels (P<0.05). MPO was decreased signiifcantly in group A after treatment (F=18.455, P=0.000). And the same differences in group B were also signiifcant (P < 0.05). MPO post medication between the 2 groups were not significantly different (P > 0.05). Reduction in MMP-9 also showed signiifcant in both groups after treatment (group A:F=46.911, P=0.00;group B:F=19.156, P=0.00). The adverse effects had no signiifcant differences between the 2 groups (P>0.05). Conclusions The atorvastatin sequential theapy in CHD patients undergoing elective percutaneous coronary intervention could decrease serum lipids signiifcantly. Pretreatment with atorvastatin for patients undergoing PCI could inhibit inlfammation. The MACE and adverse effects were similar between the two groups.
7.Attachment representation in adolescent patients with somatoform disorders

ObjectiveTo examine the attachment style,the anxious and avoidant related attachment in adolescent patients with somatoform disorders.MethodsAll of the 66 patients who were diagnosed as somatoform disorder and 80 controls completed the assessments of common information,experiences in close relationships scales.All the data was analyzed by SPSS13.0.ResultsCompared with control group,somatofonn disorder group had higher ratio of insecure attachment,and there were statistically significant differences between them (29.5%vs 81%,x2 =40.584,P<0.01 ).The scores of avoidant and anxious related attachment in somatoform group were significant higher than those in control group ( separately 70.51 ± 15.08 vs 62.58 ± 14.18,t =4.449,P <0.0l ;64.25 ± 18.91 vs 51.49 ± 14.13,t =3.210,P < 0.01 ).ConclusionThere is a higher incidence of insecure attachment and higher level of anxious and avoidant related attachment in adolescent patients with somatoform disorder.The most common style is dismissing attachment.
8.Exploration on pre-practice training model for medical undergraduates

Objective To explore pre-service training model and its efficacy for medical undergraduate interns.Methods Anonymous questionnaire survey was conducted for 50 medical undergraduates by teaching management staff.Results Totally 62% students thought that pre-practice training was necessary,62% students commented that the arrangement of this pre-practice training was reasonable and feasible,more than 95% students supported the content and form of this training and more than 95% students believed that pre-practice training can culture the sense of organizational discipline,foster favorable doctor-patient relationship and promote clinical knowledge and skill acquisition.In general,medical undergraduates had high degree of recognition after systematic training.Conclusion A set of standardized pre-practice training programs should be summarized and constantly be refined and improved so as to ensure the training quality.
9.Management of Antimicrobial Drugs During Perioperative Period:Application of Information Technology

OBJECTIVE In order to improve the traditional management mode,we try to explore the application of information technology for management of antimicrobial drugs during perioperative period. METHODS Through analysis of our research program case,the differences between the traditional management mode and the information-based management mode were compared by the method of reviewing literature data and analyzing hierarchy process of administration. RESULTS There were obvious limitations in the traditional management mode,but the information-based management mode exhibitsed evident advantages,because it focused on node control and process management. CONCLUSIONS The information-based management mode can establish a long-term management mechanism of antimicrobial drugs during perioperative period,and also can individualizedly implement precise management goal.It is an innovation of the traditional management mode.
10.Questionnaire on Awareness and Consumption Situation of Health Care Products in Hospitalized Patients

Objective:To investigate the consumption situation and cognition of health care products in hospitalized patients, ana-lyze the problems in purchasing and use of health care products to provide evidence for the correct selection and rational use of health care products. Methods:The investigation and analysis was conducted with an anonymous questionnaire on awareness and consumption situation of health care products in hospitalized patients. Results:The survey received 129 valid questionnaires, and more than 1/3 re-spondents did not know the differences between health care products and medicines, and 27. 91% of them treated diseases with health care products, 57. 36% of them purchased health care products per month, and 96. 12% of them thought the efficacy of health care products were inconsistent with the advertising efficacy. Conclusion:Patients cannot distinguish health care products from medicines, and lack of trust in the health care products market. Corresponding measures should be performed to help patients purchase and use of health care products and safeguard their legitimate rights and interests.
