1.Study on Optimum Conditions of Isolation and Purification of Specific Toxin Fractions Produced by Exserohilum turcicum

Five fractions,Ⅰ(R_f0.06)、Ⅱ(R_f0.21)、Ⅲ(R_f0.45)、Ⅳ(R_f0.60)、Ⅴ(R_f0.75)have been obtained after HT-toxin from race 1 of Exserohilum turcicum 99-2 isolated by TLC. In all of these fractions,only fractions Ⅱ had specific toxicity to the corn leaves with Ht1 gene. Then fractions Ⅱ-1、Ⅱ-2、Ⅱ-3 were isolated from fractions Ⅱ by HPLC purification,and the bioassay result showed only fraction Ⅱ-3 was toxigentic to corn leaves with Ht1 gene but non-toxigentic to corn leaves without Ht1 gene. Fractions Ⅱ-1、Ⅱ-2 and Ⅱ-3 were scanned by UV-Vis spectrophotometer. It was shown that the fractions Ⅱ-1 and Ⅱ-2 had analogous spectrum,and especially the fraction Ⅱ-3 had a special peak at 300nm.
2.Microsurgical anatomic study of subtemporal approach using a minimal access

Objective To investigate the surgical exposure of the subtemporal approach and explore clinical indications using a minimal-access.Methods Ten adult cadaveric heads fixed with formalin were used in this study.The holes with 3.0 cm × 2.5 cm of zygomatic arch vertically were operated on each head with subtemporal approach.During the anatomical procedures,measured the maximal exposure lengths of tentorial margin,posterior cerebral artery,and anterior border of brain stem,vertical distances between highest structure in the field of view and posterior clinoid process,the shortest distances form the zygomatic arch 1/3 to tentorial edge,sulcus lateralis mesencephali and anterior clinoid process; After tentorium of cerebellum was cut,measured the straight distance form the internal carotid artery to the optic nerve and form the posterior communicating artery to the tentorial edge.Results Oculomotor,trochlear nerve,tentorial edge,superior cerebllar artery.,P1-P2 segment of posterior cerebral artery,ventrolateral surface of mesencephalon and pon higher than root of trigeminal nerve,anterior and posterior clinoid process,posterior communicating artery and anterior choroidal artery,and superior portion of pituitary stalk could be observed via subtemporal approach using a minimal-access.Conclusion 1.The subtemporal approach using a minimal-access can protect the superficial temporal artery and the facial nerve branches especially in the process of the flap formation.It does little damage to the temporallis,reduces the invalid exposure of brain tissue,farthest lowers the damage to the scalp,skull and adjacent tissue ; 2.The subtemporal approach using a minimal-access can obtain the exposure rang.It can see the upper pons,petroclival region,tentorial notch area,ventrolateral brain stem.

Objective To assess the prediction value of Pmax and Pd on idiopathic paroxysmal atrial fibrillation(IPAF) and the possible mechanism of amiodrone preventing the recurrence of IPAF.Methods Pmax and Pd were measured in 60 patients with IPAF and 60 healthy control subjects by 12-lead surface electrocardiography. 30 of patients with IPAF were assigned to treatment with amiodrone for 6 months. 5.Clinical study of three-dimensional conformal radiotherapy combined with XELOX program chemotherapy in the treatment of postoperative local recurrence of rectal cancer

Objective To investigate the clinical efficacy of three-dimensional conformal radiotherapy combined with XELOX program chemotherapy in the treatment of postoperative local recurrence of rectal cancer.Methods One hundred and twenty-nine patients with postoperative local recurrence of rectal cancer were divided by random digits table method into treatment group (65 cases) and control group (64 cases).The patients in treatment group were treated with three-dimensional confonmal radiotherapy combined with XELOX program chemotherapy.The patients in control group were treated with XELOX program chemotherapy.The short-term effect,symptom control rate,long-term efficacy and adverse reaction were observed.Results The efficiency rate in treatment group [ 90.8% (59/65) ] was significantly higher than that in control group [ 73.4%(47/64) ] (P < 0.05 ).The improvement rates of pain,hematochezia and incomplete obstruction between treatment group and control group had significantly statistical differences [95.2%(40/42) vs.75.0%(27/36),92.9%(26/28) vs.79.5%(31/39),94.7%(18/19) vs.75.0%(9/12)](P <0.05).The 1,2,3-year survival rates between treatment group and control group had significantly statistical differences [ 67.7% ( 42/62 ) vs.43.5 % ( 27/62 ),53.2% ( 33/62 ) vs.30.6% ( 19/62 ),38.7% ( 24/62 )vs.14.5% (9/62)] (P < 0.01 ).The adverse reaction in two groups were mainly bone marrow suppression,gastrointestinal reactions,hand-fcot syndrome and radiation cystitis.The incidence rate of adverse reaction in treatment group was slightly higher than that in control group,but there was no significant difference between two groups (P > 0.05).Conclusion Three-dimensional conformal radiotherapy combined with XELOX program chemotherapy in patients with postoperative local recurrence of rectal cancer can improve the short-term effect,symptom control rate and long-term survival rate.
6.A Clinical Study of Related Acupoint Pain Threshold Detection in Cholecystitis Patients

Objective To explore the difference in pain threshold between related acupoints and the specificity of acupoints in cholecystitis patients.Method Actual measurement was made in 80cases, a normal group of 30 cases (volunteers without psychological and physiological diseases) and a patient group of 50 cases (volunteers with cholecystitis). The probe of an algometer was perpendicularly placed on the selected acupoint, the pressure point was pushed and pressing was stopped immediately after the subject felt pain. The pain thresholds displayed by the measured acupoints were recorded. The pain thresholds of the selected acupoints were compared between the normal and patient groups and thedifferences were analyzed to explore the specificity of acupoints.Result A comparison of the pain thresholds of the Back-Shu points of the Bladder Meridian of Foot-Taiyang between the patient and normal groups showed that there were no significant differences in the thresholds of the left Back-Shu points Geshu (BL17), Ganshu (BL18), Danshu (BL19), Pishu (BL20), Weishu (BL21) and Dachangshu (BL25) of the bladder meridian between the two groups (P>0.05) and there were significant differences in the thresholds of the right Back-Shu points Geshu, Ganshu, Danshu, Pishu, Weishu and Dachangshu of the bladder meridian between the two groups (P<0.01). The pain threshold was significantly lower in thepatient group than in the normal group. A comparison of the pain thresholds of the main points of the Liver Meridian of Foot-Jueyin, the Gallbladder Meridian of Foot-Shaoyang and the stomach meridian between thepatient and normal groups showed that there were significant differences in the thresholds of the right points Liangmen (ST21), Riyue (GB24) and Qimen (LR14) of the liver, gallbladder and stomach meridians (P<0.01) and no significant difference in the threshold of the right point Taichong (LR3) between the two groups (P>0.05). The pain thresholdwas significantly lower in thepatient group than in the normal group. There were no significant differences in thethresholds of the left points Liangmen, Riyue, Qimen and Taichong of the Liver Meridian of Foot-Jueyin, the Gallbladder Meridian of Foot-Shaoyang and the stomach meridian between the patient and normal groups (P>0.05).Conclusion The pain sensitivity of ipsilaterali related acupoints increase and the relative specificity of acupoints exist in cholecystitis patients.
7.Dynamic hip screw, proximal femoral nail antirotation and InterTan nail for intertrochanteric fractures

BACKGROUND: The extramedulary fixation system including dynamic hip screw (DHS) is commonly used in treatment of Intertrochanteric fracture. However, in patients with unstable intertrochanteric fracture, extramedulary fixation system often leads to the failure of fracture fixation. Intramedulary fixation system including both proximal femoral nail antirotation (PFNA) and InterTan nail has been widely used in the treatment of unstable intertrochanteric fractures. OBJECTIVE:To compare the therapeutic effects of extramedulary fixation system containing DHS, PFNA and InterTan nail in the treatment of intertrochanteric fracture. METHODS:Literatures were searched in Wanfang, PubMed, Embase, Medline, the Cochrane library to screen literatures published from January 1990 to November 2014. Relevant studies addressing extramedulary fixation system containing DHS, PFNA and InterTan nail were screened. RESULTS AND CONCLUSION: 346 articles were screened, and 13 of them were in accordance with the inclusion criteria. 1 271 patients with different types of intertrochanteric fracture were assessed in this study. Compared to DHS group, patients treated with PFNA and InterTan nail had shorter operation time and less blood loss. No significant difference in rehabilitation time and Harris score was detected among three kinds of fixation methods. Additionaly, PFNA and InterTan nail had a similar effect. These findings verify that compared with DHS, PFNA and InterTan nail can optimize the surgery, but cannot elevate postoperative outcomes.

9.Prevention and treatment of chylous leakage after Whipple operation

Objective To study the prevention and treatment of chylous leakage after Whipple operation.Methods Clinical data of 381 patients underwent Whipple operation from Jan 2010 to march 2013 were retrospectively analyzed.Results Among 381 patients,23 patients had chylous leakage,the incidence was 6.04%.While in 89 patients,in which intraoperative precautionary physical ligation or mattress suturing of the lymphatic vessels were undertaken,no chyle leakage occurred ; Most of chyle leakage occurred in malignant tumor after radical resection(21 patients),most chylous leakage was found 5-8 days after surgery,with daily volume of 260-450 ml.All patients of chylous leakage were cured by conservative treatment.Conclusions Chylous leakage are common in PD.Intraoperative preventive measures such as lymphatic duct ligation or safe suturing can effectively prevent chyle leakage.Conservative therapy heals most chylous leakage after Whipple operation.
10.Clinical application of laparoscope combined with gastroscope surgery in patients with gastric stromal tumor

Objective To study the clinical application of laparoscope combined with gastroscope surgery in patients with gastric stromal tumor. Methods The clinical data of 50 patients with gastric stromal tumor were retrospectively analyzed. Among them 20 cases were treated with traditional laparotomy (laparotomy group), and 30 cases were treated with laparoscope combined with gastroscope surgery (combination group). The intraoperative bleeding, operation time, postoperative recovery time, postoperative hospitalization time and complications were compared between 2 groups. Results One case in combination group received open surgery (3.33%, 1/30), mainly because of obesity, and tumor rupture occurred in 1 case during operation. All specimens of the 2 groups had no tumor residue. There were no statistical differences in tumor diameter and operation time between 2 groups (P>0.05); the intraoperative bleeding, postoperative recovery time and postoperative hospitalization time in combination group were significantly lower than those in laparotomy group:(26.33 ± 14.21) ml vs. (42.57 ± 15.67) ml, (37.96 ± 8.80) h vs. (60.14 ± 13.41) h and (6.42 ± 2.80) d vs. (12.04 ± 4.69) d, and there were statistical differences (P<0.05). The 2 groups had no death, stomach bleeding, anastomotic stenosis and anastomotic leakage. The laparotomy group had postoperative incision fat liquefaction in 3 cases and incision infection in 2 cases, and the combination group had pulmonary infection in 1 case. All were cured after symptomatic treatment. Conclusions Laparoscope combined with gastroscope surgery in the treatment of gastric stromal tumors has a short operation time, less blood loss, faster postoperative gastrointestinal function recovery, shorter hospitalization time, and no major complications. It is a more ideal and minimally invasive surgery.
