2.Effect of Occupational Therapy on the Function of Upper Limbs and Complications in Stroke Patients

ObjectiveTo observe the clinical effect of occupational therapy (OT) on the function of upper limbs and complications in stroke patients with hemiplegia.Methods99 stroke patients were randomly divided into the OT group (52 cases treated with OT and routine rehabilitation) and control group (47 cases treated without OT). Fugl-Meyer Assessment (FMA) and evaluation of hand function were performed before and two months after treatment. The complications of patients in two groups were also compared.ResultsAfter treatment, the functions of upper limbs and hand of patients in the OT group were superior to those in the control group ( P<0.05). The numbers of cases with complications in the OT group were significantly less than that in the control group ( P <0.01).ConclusionOT can improve the functions of upper limbs and hand and abate complications in stroke patients.

These bioactive substances also could be produced by Paenibacillus polymyxa making it show excellent prospect in biological control of plant diseases, treatment of mankind and animals. This article summarizes research advances in Paenibacillus polymyxa and their bioactive substances.
4.Treatment of atlantoaxial instability with atlantoaxial pedicle and lateral mass screw WANG

Objective To study the effect of atlantoaxial pedicle and lateral mass screw in treat-ment of atlantoaxial instability. Methods A retrospective analysis was done on 11 eases of aflantoaxi-al instability treated with atlantoaxial pedicle and lateralmass screw from June 2006 to December 2007. Results The postoperative radiographs verified good position of all screws, with satisfactory atlantoaxial reduction. Follow-up for 3-21 months (average 12 months) showed no spinal cord and vertebral artery in-jury or interfixation failure. Conclusion Atlantoaxial pediele and lateral mass screw technique is a convenient method with solid fixation for treatment of atlantoaxial instability.

7.Pleomorphic Hyanilizing Angiectatic Tumor of Soft Parts:One Case Report with Literature Review

Objective To improve the clinic,CT and pathologic knowledge of pleomorphic hyanilizing angiectatic tumor of soft parts(PHAT).Methods Clinical,CT and pathologic characters of PHAT in one case at the right nuchal region were detailedly analysed with literature review.Results The clinical symptoms of PHAT were slight and unspecified.The lesion appeared as a orbicular-ovate tumour with sublobe and clear edge,the density of tumor was uneven on plain scan and uneven enhanced after injected contrast agent,and a part of the tumour was enhanced close to the CT value of blood vessel.The characters of pathology were the presence of clusters of dilated thin-walled vessels,with prominent hyalinization of vessel wall,perivascular and intercellular hyalinization,and pantomorphia neoplastic cell distributed between these dilated vessels.Conclusion PHAT is a low potential malignant soft tissue tumor,it has some specific CT and pathologic characters.
8.Influence of vitrectomy for corneal thickness in diaebetic retinopathy eyes with Pentacam

BackgroundThe research of corneal thickness after pars plana vitrectomy in DM patient plays an important role not only theoretically but clinically.Objective Present study was to evaluate the change in corneal thickness after pars plana vitrectomy.Methods A prospective coherent study was designed.Seventy-five eyes of 70 consecutive diabetic retinopathy(DR) patients were collected in Tianjin Medical University Eye Center.Pentacam was used to assess the central and periphery corneal thickness by the same investigator preoperatively and 7 days,1 and 3 months postoperatively,respectively.The thickness values from five corneal zone were obtained,including cornea vertex,the thinnest point of the cornea,periphery cornea near the scleral incision of 4 mm away to vertex of cornea on vertical and 240°,120°,60° meridian ( right eye) or 300°,120°,60° meidian ( left eye ).These results were compared and analyzed with ANOVA of repeated measurement data.Subgroup analysis was performed to evaluate the influence of different corneal positions,the use of conventional 20g or 23g transconjunctival sutureless vitrectomy(TSV) groups,surgery duration,gas or fluid endotamponde on corneal thickness.This study was approved by Ethic Committee of this hospital.Written informed consent was obtained from the subjects before any relative medical procedure.Results The mean corneal thickness was ( 639.9 ± 103.1 ),( 689.5 ± 119.3 ),( 666.5 ±113.7),( 650.8 ± 108.6 ) μm before operation,postperative 7 days,1 and 3 months respectively.As covariates appearing in the model,the corneal thickness change rates were revised as the parameters as following: diabeitc duration =13.0 and age =57.2.The revised corneal thickness was significant different among various time points( F=210.928,P=0.000) and different corneal zones(F=24.843,P=0.000) with the size order in turn P4>P3>P1>P2>P5.The corneal thickness change rates were less in 23g TSV group compared with conventional 20-g group (F =53.843,P =0.000) and BSS tamponade group compared with gas tamponade group ( F =5.288,P =0.022).But no significant difference was found in the revised corneal thickness among surgery duration < 1 hour group,1-2 hour group and >2 hour group( F=1.233,P =0.293).ConclusionsVitrectomy is a safe procedure on the ground of cornea,but TSV and fluid endotamponade appear to be more beneficial to the protection of cornea.Pentacam could offer the reliable data in not only central cornea but also periphery cornea.
9.Efficacy of closed-loop coadministration of propofol and remifentanil guided by Narcotrend index in laparoscopic cholecystectomy

Objective To evaluate the efficacy of closed-loop coadministration of propofol and remifentanil guided by Narcotrend index (NI) in laparoscopic cholecystectomy.Methods Sixty American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients of both sexes,aged 20-64 yr,with body mass index of 18-25 kg/m2,scheduled for elective laparoscopic cholecystectomy,were randomized into 2 groups (n =30 each):program regulation group (group P) and artificial regulation group (group A).After the initial target effect-site concentration of propofol was selected,the target effect-site concentration of remifentanil was determined according to the formula.In group A,the target effect-site concentrations of propofol (2-4 μg/ml) and remifentanil (3-4 ng/ml) were adjusted artificially according to anesthesiologists' experience every 5 min to maintain NI value at 26-46.Induction time,anesthesia induction and mean maintenance doses and the initial,highest and lowest target concentrations of propofol and remifentanil,mean NI value,percentage of time with NI between 26 and 46,emergence time,and development of fluctuation in heart rate or mean arterial pressure > 20% of the baseline value and intraoperative awareness were recorded.Results No intraoperative awareness was found in the two groups.Compared with group A,the induction time was significantly shortened,the induction dose and initial target concentration of remifentanil were increased,the mean maintenance dose and lowest target concentration of propofol and remifentanil were decreased,the percentage of time with NI between 26 and 46 was increased,and the emergence time was shortened (P<0.05 or 0.01),and no significant change was found in the induction dose and initial target concentration of propofol,the highest target concentrations of propofol and remifentanil,mean NI value,or incidence of fluctuation in heart rate or mean arterial pressure > 20% of the baseline value in group P (P> 0.05).Conclusion For laparoscopic cholecystectomy,NI-guided closed-loop coadministration of propofol and remifentanil produces safe and effective anesthesia,and the efficacy of precise administration is superior to that of artificially regulated target-controlled infusion.
