1.The effect of TLR4 expression in platelets and activation of platelets on the pathogenesis of thrombocytopenia induced by lipopolysaccharide in mice

Objective To determine the role of the platelet activation and the expression of platelet Toll-like receptor 4 (TLR4) in thrombocytopenia induced by lipopolysaccharide (LPS) in mice.Methods ICR mice were randomly (random number) divided into control group,model 3,6,12,24,48,72h groups and neutrocytopenia syndrome (NEP) group.The blood samples of mice in model groups were detected 3,6,12,24,48,and 72 hours after intravenous injection of LPS respectively.Anti- neutrophil monoclonal antibody was administered in the mice of NEP group 24 h before injection of LPS,and blood samples were collected 24 h after injection of LPS.The determination of platelet count (PC),mean platelet volume (MPV) and platelet distribution width (PDW) was carried out with full automatic hemocyte analyzer.The plasma tumor necrosis factor- α (TNF- α),macrophage colony stimulating factor (M-CSF) and soluble CD40 ligand (sCD40L) levels were measured by using ELISA.The rate of platelet TLR4 expression was detected by flow cytometry.Comparison between groups was carried out by using ANOVA statistical methods.Results PC was reduced by 30% 3 h after intravenous injection of LPS,and reached the lowest level within 24 h (P <0.01 ).MPV and PDW were increased compared with healthy controls (P <0.01 ).Plasma TNF - α,M - CSF and sCD40L were significantly increased after LPS stimulation (P <0.01 ),and reached the peak during 6 ~ 24 h after LPS administration.PC had correlation with plasma levels of M - CSF ( r =- 0.746) and sCD40L ( r =- 0.573 ).The platelet TLR4 expression was significantly increased 6 h after LPS stimulation.The platelet TLR4 expression in NEP group was significantly lower than that in 24 h model group ( P < 0.01 ).Conclusions The excessive activation platelet represented by increase in sCD40L,MPV,PDW and high levels of M-CSF in macrophages along with platelet TLR4 expression participate in the pathogenesis of thrombocytopenia.The up - regulation of the platelet TLR4 expression is dependent on neutrophils in case of thrombocytopenia induced by LPS.
2.The training needs analysis and improvement strategies of general practitioners' communica-tion skills

Objective To point out the weak link of general medical education and explore the training mode and evaluation mechanism for communication. Methods Totally 100 general practi-tioners from four Shanghai hospitals took a questionnaire survey to know the present situation and the training needs of doctor-patient communication skills. Results Among all the subjects, there were 73%who thought strengthening the doctor communication ability was the key to improving the doctor-patient relationship, 71%who chose doctor-patient communication practice course as the training con-tent, 78%who tended to personalized analysis and guide, 57%who accepted experienced GP in class. Conclusion For GPS communication skills training requirements, curriculum integration, clinical practice teaching should be strengthened to set up the corresponding training evaluation system.

Wuhan Pu Ai Hospital implemented the pharmaceutical direct supply mode since October 2006 as an experiment.The pilot proved the mode as successful in reducing drug costs proportion,promoting rational use of drugs,and raising business volume of the hospital.Authors also recommend to further efforts in drug supply price supervision mechanism,government subsidy mechanism,direct supply catalogue and drug supply mode among others.Other recommendations include innovation in directly supply mode,hospital internal reform,packaged performance appraisal and incentive mechanism for medical workers,and building a ceiling mechanism for total medical expense control.
4.Study on in vitro culture of atrial myoeytes in suckling guinea pig

Objective To explore the condition and method of primary cultures of atrial myoeytes in sukling guinea pig .Methods The atrial myoeytes of sukling guinea pig with 1‐2 d old were taken and digested once by 0 .06% trypsin ,then repeatedly digested by 0 .08% type Ⅱ collagenase for four times .And the cells after former twice ,latter twice and former and latter four times of diges‐tion were adopted for conducting respective culture .The influence of three kinds of culture method on atrial myocytes production , survival rate ,pulsating rate and purification rate were observed .Results Three kinds of culture method all obtained the considera‐ble number of atrial myocytes and a higher survival rate ,pulsation rate and purification rate ,particularly the cell culture method af‐ter latter twice digestion .Conclusion Once digestion by 0 .06% trypsin and 0 .08% type Ⅱ collagenase for four times can isolate a great quantity and good condition of atrial myocytes .
5.Study on CXCR1/CXCR2 antagonist G31P anti-inflammatory reaction mediated by neutrophils

Objective To Study on CXCR1/CXCR2 antagonist G31P anti-inflammatory reaction mediated by neutrophils.Methods Detect whether G31P can block chemotaxis of neutrophils induced by human IL-8 and inhibit the release of IL-8 by epithelia of segmental bronchus;establish HEK293 cell line transfected by pcDNA3.0-CXCR1 ,2,4 and detect the chemotaxis of IL-8 for HEK293 ;establish the experi-mental model of pneumonia induced by the P.aeruginosa,take count of the nucleated cells in the bronchoal-veolar lavage fluid(BALF),analyze myeloperoxidase(MPO) of lung tissue and observe the histopathology changing of it.Results G31P can inhibit the chemotaxis for neutrophils and transfected HEK293 cell line,inhibit the A549 releasing of inflammatory mediators;the proportion of neutrophils declines in G31P treat-ment group,pathology examination appears clear discrepancy.Conclusion G31P can block the chemotaxis of chemotactic factor with ELR+ CXC to neutrophils,block the combination of chemotactic factor with its re-ceptor CXCR2,block the CXCR2 on the surface of alveolar epithelia and vascular endothelial cells.Accordingly,neutrophils recruiting to topoinflammation can be prevented.
6.Mesenchymal stem cells reduce the irradiation induced lung injury

Objective To evaluate the role of mesenchymal stem cells (MSCs) derived from mouse bone and embryo dorsal aorta(DA) area in the treatment of irradiation induced lung injury of mouse model. Methods The mice were divided into four groups as normal control group, irradiation group,bone MSCs treatment group and DA MSCs treatment group. Immunohistochemical Analysis of lung tissue was observed after 9 months of treatment. Results Fibrosis and alveolar infiltration were scored in each group. The score for fibrosis and alveolar is 0. 17 in normal control group, 2 in irradiation group, 1 in bone MSCs treat group and 1.38 in DA MSCs treat group. Conclusion The extent of irradiation Induced Lung Injury could be reduced thorough the treatment of MSCs derived from mouse bone and embryos dorsal aorta ( DA ) area.
7.Effects of high expression of Csk-binding protein on morphology and biological function of Jurkat cells

BACKGROUND:The linkage and synergistic effect of adaptor proteins can effectively regulate signal transduction of T cel s, which can form a limit or amplification cascade to realize the complex immune function of T cel s. C-terminal Src kinase (Csk)-binding protein (Cbp) is an adaptor protein, which mainly exert the negative feedback regulation of Src kinase activity. This negative feedback effect depends on Y317 of Cbp, which may be involved in the SH2 domain of Csk. OBJECTIVE:To explore the effects of high expression of Cbp on ultrastructure and related biological function of Jurkat cel s. METHODS:The virus particles were constructed with expressing enhanced green fluorescent protein (EGFP) only and Cbp-EGFP fusion protein to transfect Jurkat cel s. There were untransfected group (Jurkat group), negative control group (transfected with expression of EGFP virus only), and Cbp group (transfected with Cbp-EGFP virus). RESULTS AND CONCLUSION:Confocal microscope showed that cel transfection efficiency was more than 95%and Cbp was located on the cel membrane. Optical microscope showed after transfection with Cbp-EGFP virus, more Jurkat cel s shrunk, with poor size uniformity. Apoptosis detection showed that after transfection with Cbp-EGFP virus, the number of apoptotic and necrotic cel s was greatly increased. Cbp mRNA expression was increased, Csk expression was decreased obviously and lymphocyte-specific protein tyrosine kinase expression was increased. So, in Jurkat cel s, the high expression of Cbp can decrease the uniformity of cel s and increase the necrosis cel s, thus inhibiting the signal transduction.
8.Effect of propofol on secretory function of adrenal medullary chromaffin cells of rats

Objective To investigate the effect of propofol on the secretory function of adrenal medullary chromaffin cells of rats.Methods The rat adrenal pheochromocytoma cells cultured in vitro were seeded in 24-well plates at a density of 1×1010 cells/ml and divided into 5 groups (n=12 each) using a random number table:control group (group C),different concentrations of propofol groups (P1 and P2 groups),and different concentrations of etomidate groups (E1 and E2 groups).Propofol was added with the final concentrations of 30 and 100 μmol/L in P1 and P2 groups,respectively.Etomidate was added with the final concentrations of 4 and 40 μmol/L in EI and E2 groups,respectively.The cells were then incubated for 10 min in an incubator at 37 ℃.At the end of incubation with drugs,6 wells in each group were selected,and physiological salt solution 200 μl was added;another 6 wells in each group were selected,and high K+ physiological salt solution was added.[3 H] PSS and [3 H] K+-PSS were measured by [3 H] norepinephrine release assays.Results Compared with group C,[3 H] K+-PSS and [3 H] K+-PSS/PSS were significantly decreased in P1,P2 and E2 groups (P<0.05),and no significant changes were found in [3H] K+-PSS and [3H] K+-PSS/PSS in group E1 (P>0.05).Compared with group P1,[3 H] K+-PSS and [3 H] K+-PSS/ PSS were significantly decreased in group P2 (P<0.05).Conclusion Propofol can inhibit the secretory function of adrenal medullary chromaffin cells of rats in a concentration-dependent manner.
9.Meta - analysis of clinical randomized controlled trials comparing ReZOOM with ReSTOR multifocal intraocular lenses in cataract surgery

AIM: To systematic review the effectiveness of refractive multifocal intraocular lens ( MIOL ) ReZOOM vs diffractive MIOL ReSTOR in the treatment of cataract.
METHODS: Randomized controlled trials comparing refractive MIOL ReZOOM with diffractive MIOL ReSTOR were identified by searching CENTRAL, MEDLINE, EMbase, WANFANG MED ONLINE, CMJD, SinoMed, and CNKI. Related journals also were hand-searched. Methodological quality of randomized controlled trials ( RCTs ) was evaluated by simple evaluate method that recommended by the Cochrane Collaboration. Data extracted by two reviewers with designed extraction form. RevMan software ( release 5. 2 ) was used for data management and analysis.
RESULTS:A total of 7 trials (846 eyes) were included for systematic review. Subgroup analyses were used according to different model comparison of ReSTOR MIOL. The results showed a significant difference in the mean of the best distance corrected intermediate visual acuity ( BDCIVA) in the ReZOOM MIOL group with WMD=-0.11, 95% CI (-0.16, -0.06) (P<0.0001). It showed a significantly difference in the mean of the uncorrected near visual acuity ( UCNVA ) , complete spectacle independent rate, halo rate and glare rate in the ReSTOR MIOL group with WMD= 0. 09, 95% CI (0. 05, 0. 14) (P<0-00001), WMD= 2. 62, 95%CI (1. 76, 3. 91) (P<0. 00001), WMD=1. 35, 95% CI (1. 15, 1. 60) (P=0. 0004) and WMD=1-29, 95% CI (1. 09, 1. 53) (P= 0. 003). There was no significant difference between the two groups in the mean of the uncorrected distance visual acuity ( UCDVA ) , the uncorrected intermediate visual acuity ( UCIVA) , the best corrected distance visual acuity ( BCDVA ) and the best distance corrected near visual acuity ( BDCNVA ) with WMD-0.03, 95% CI (-0.06, 0.01) (P=0.15), WMD= -0.04, 95% CI (-0. 09, 0. 01) (P=0. 10), WMD= -0. 01, 95%CI (-0. 04, 0-02) (P=0. 55) and WMD= 0. 06, 95% CI (-0. 06, 0. 17) (P=0. 32).
CONCLUSION: Patients implanted with ReZOOM MIOL can provide better BDCIVA; patients implanted with ReSTOR MIOL show better UCNVA, are less likely to appear light halo, glare and other visual adverse reactions; correction in spectacles cases, patients implanted with ReZOOM or ReSTOR MIOL have considerable performances in the far and near visual acuity.
10.Study on relation of ovary-body surface correlativity with acupoints.

OBJECTIVETo study the specific regularity of acupoints to treat genital system diseases.
METHODSA model of inflammatory ovary was developed by injection of 0.08 mL, 10% mustard oil into the ovary of the rat. And the reactive points of plasma extravasarion points on the skin were detected after injection of 0.25 mL evens blue into the tail vein. The relation among the regions of plasma extravasation points, dorsal ganglion of governing ovaries and the acupoints to treat the ovarian diseases was analyzed.
RESULTSThe exudative points mainly distributed at Guanyuan (CV 4)-"Zigong" (EX-CA 1) point-area, "Shenshu" (BL 23)-"Mingmen" (GV 4) point-area, "Changqiang" (GV 1)-"Huiyin" (CV 1) point-area, "Sanyinjiao" (SF 6)-"Zusanli" (ST 36) point-area, "Zhongwan" (CV 12) point-area, "Gansho" (BL 18) area, with the most in the "Guanyuan" (CV 4)-"Zigong" (EX-CA 1) point-area and next at "Shenshu" (BL 23)-"Mingmen" (GV 4) point-area.
CONCLUSIONThe reactive point-area of plasma extravasation resulted from inflammation of ovary are identical to the ovary a corr.sponding dorsal ganglion. Acupuncture at the corresponding acupoints of the reactive point-areas has specific regulative action on ovarian functions.
