OBRA - 90 and the situation of drug use review in America are introduced and analyzed,and inadequate insurance in drug use in China is pointed out.RESULTS & CONCLUSION :The drug use review system in USA is quite successful in ensuring the safety of drug use in public, and we can learn something from it to improve the quality of medication in China.

4.Establishment and identification of the rat models of chondrocyte apoptosis induced by tumor necrosis factor-alpha

BACKGROUND:Tumor necrosis factor-α(TNF-α), a main cytokine inducing chondrocyte apoptosis of osteoarthritis, plays a regulatory role in the process of osteoarthritis. OBJECTIVE:To compare the rat models of chondrocyte apoptosis induced by different mass concentrations of TNF-α, thus providing theoretical basis for further study on the regulation of drugs on chondrocyte apoptosis. METHODS:Chondrocytes were isolated from the knee cartilage of 4-week-old Sprague-Dawley rats of clean grade by mechanical l col agenase digestion and were then incubated with different mass concentrations of TNF-αto induce apoptosis. The morphology of chondrocytes was observed under inverted phase contrast microscope, cel s were identified using immunohistochemical staining of type II col agen, as wel as the cel viability and apoptosis were detected by MTT and DAPI, respectively. RESULTS AND CONCLUSION:(1) In vitro, the cytoplasm of chondrocytes was stained brown-yel ow by using immunohistochemical staining of type II col agen. (2) At 48 hours, the apoptosis rate of chondrocytes induced by 10, 20 and 30μg/L TNF-αwas significantly higher than that of the 0μg/L TNF-α(P<0.01), and the apoptosis rate of chondrocytes induced by 40μg/L TNF-αwas significantly higher than that of the 10μg/L TNF-α(P<0.01). (3) The viability of chondrocytes induced by 10, 20 and 40μg/L TNF-αwas significantly lower than that of the 0μg/L TNF-α(P<0.01). In detail, the viability of chondrocytes induced by 20μg/L TNF-αwas lower than that of the 10μg/L TNF-α(P<0.05);the viability of chondrocytes induced by 40μg/L TNF-αwas significantly lower than that of the 10 and 20μg/L TNF-α(P<0.01, P<0.05). (4) These results suggest that 20μg/L TNF-αcan successful y induce the chondrocyte apoptosis model.
5.Construction of single promoter double expression vector pIRES-p14ARF-p53 and its inhibitory effect on proliferation of osteogenic sarcoma cells

BACKGROUND: Previous researches indicate that ADp14ARF transfecting positive tumor cell line of p53 can inhibit the proliferation; in addition, the inhibitory effect is superior to transfection negative tumor cell line of p53. Whether simultaneous transfection of p14ARF and p53 genes can increase expression and accumulation of p53 and accelerate apoptosis of tumor cells needs further studies.OBJECTIVE: To construct double plasmid expression vector plRES-p14ARF-p53 by using gene engineering so as to observe the inhibitory effect on proliferation of osteogenic sarcoma cells.DESIGN: Randomized controlled observation.SETTING: Department of Orthopaedics, Union Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology.MATERIALS: The experiment was carried out in the Public Laboratory Platform, Immune Researching Room, Basic Medical College, Tongji Medical College, Huazhong University of Science and Technology from January 2005 to October 2006. Human osteogenic sarcoma MG-63 cells were provided by Cell Laboratory, Immune Researching Room, Tongji Medical College, Huazhong University of Science and Technology. plRES-p53 plasmid and plRES vector containing p53total-length gene order were provided by Wuhan Jingsai Biology Company.METHODS: Based on gene engineering, p14DNA (0.5 kb) was amplified from cultured L02 cells of normal human hepatic cells into plRES vector. Recombinant plasmid plRES-p14ARF-p53 was determined with polymerase chain reaction (PCR) and restriction enzyme and transfected into human osteogenic sarcoma MG-63 cells through mediation of liposome to screen positive clones. Otherwise, cells were divided into three groups, including blank control group (MG-63cells), blank vector control group (stably transfecting plRES-neo cells) and p14ARF-p53 group (stably transfecting plRES-p14ARF-p53 cells). ① DNA content and cycle of tumor cells were measured by using flow cytometry before and after transfection. ② Reverse transcription polymerase chain reaction (RT-PCR) and Western blot were used to detect quantitative and semi-quantitative expression of p53 and p14ARF protein in tumor cells after stable transfection. ③Thiazole blue chromatometry and growth curve were used to observe proliferation.MAIN OUTCOME MEASURES: ① DNA content and cycle of osteogenic sarcoma cells; ② expressions of p53 and p14ARF protein in tumor cells; ③ proliferation.RESULTS: Double plasmid expression vector plRES-p14ARF-p53 was constructed successfully. ① DNA content and cycle of osteogenic sarcoma cells: Flow cytometry demonstrated that tumor cells mainly stayed in G1 phase after transfection. ② Protein expression: RT-PCR and Western blot indicated that p14ARF and p53 gene independently expressed in target cell mRNA and protein, respectively. ③ Cell growth: At 24, 48, 72 and 96 hours after MG-63 transfection, inhibitory rates of tumor cells were 33.43%, 69.37%, 66.19% and 75.26%, respectively, which was significant difference as compared with blank vector control group (P < 0.01).CONCLUSION: Wild p53 and p14ARF can synergistically inhibit the proliferation and accelerate the apoptosis of osteogenic sarcoma cells.
6.Manipulation parameter optimization in Liu's back tuina therapy for kids' cough variant asthma in remission stage

Objective: To determine the optimal manipulation parameters in Liu's back tuina therapy for remission-stage cough variant asthma (CVA) in kids. Methods: A 5-factor 3-level L18(37) orthogonal design was adopted. The experimental factors and levels were selected: A, the number of times of Rou-kneading Feishu (BL 13); B, the frequency of Rou-kneading Feishu (BL 13); C, the number of times of Tui-pushing in a '介'-like pattern; D, the frequency of Tui-pushing in a '介'-like pattern; E, the intensity of red color after Ca-scrubbing Feishu (BL 13) with salt. A1, A2 and A3 stood for 10 times, 20 times and 30 times, respectively; B1, B2 and B3 stood for 200 times/min, 240 times/min and 280 times/min, respectively; C1, C2 and C3 stood for 50 times, 75 times and 100 times, respectively; D1, D2 and D3 stood for 150 times/min, 175 times/min and 200 times/min, respectively; E1, E2 and E3 stood for pinkish, bright red and dark red, respectively. A total of 108 kids with CVA in remission stage were divided into 18 groups using the random number table method, with 6 cases in each group. Changes in the cough score and fractional exhaled nitric oxide (FENO) level were observed after 15-day treatment. Results: The cough score and FENO level changed significantly in every group after treatment (all P<0.01). By the effect in improving cough score, the factors were listed as D, B, A, C and E in descending order; the most optimal manipulation parameter combination was group 17 (A3B2C1D3E1); the second optimal parameter combinations were group 7 (A3B1C2D1E3), group 13 (A2B1C2D3E1) and group 14 (A2B2C3D1E2). Regarding the effect in down-regulating the level of FENO, the factors were listed as C, A, E, B and D in descending order; the most optimal manipulation parameter combination was group 13 (A2B1C2D3E1); the second optimal parameter combinations were group 18 (A3B3C2D1E2), group 16 (A3B1C3D2E3) and group 14 (A2B2C3D1E2).Conclusion: Two most effective manipulation parameter combinations were selected out of 18 different parameter combinations in Liu's back tuina therapy for remission-stage CVA in kids. Doctors can choose an appropriate manipulation based on the symptom features of the disease.
7.Analysis of Sleep Characteristics of Adult Partial Epilepsy Patients

Objective To explore the sleep characteristics of adult partial epilepsy patients. Methods 45 patients newly diagnosed as adult partial epilepsy and 40 cases of healthy controls were monitored with sleep encephalogram (EEG), the sleep parameters were analyzed.Epworth Sleepiness Scale (ESS) was used to evaluate the daytime sleepiness next day. The sleep parameters and ESS score were compared between 2 groups, and the correlation between sleep parameters and ESS score in the patients were analyzed. Results The ESS score was higher in the patients than in the controls (P<0.05); Compared with the controls, the total sleep time decreased (P<0.05), the sleep efficiency reduced (P<0.05), the percentage of non-rapid eye movement sleep (NREM) stages 1 and 2 and the wakefulness stage increased (P<0.05), the percentage of NREM stages 3 and 4 decreased (P<0.05) in the patients; There was no significant difference in total recording time, sleep latency, REM latency, and arousal times between the patients and the controls (P>0.05). The ESS score was higher in the patients who attacked 3 times a month or more (group A) than less than 3 (group B). Compared with group B, the total sleep time decreased (P<0.05), the sleep efficiency reduced (P<0.05), the percentage of NREM stages 1 and 2 and the wakefulness stage increased (P<0.05), the percentage of NREM stages 3 and 4, and REM decreased (P<0.05) in group A. There was no significant difference in sleep latency, REM latency and arousal times between 2 groups (P>0.05). The ESS score of the patients was negatively correlated with sleep efficiency (r=-0.49, P<0.05) and the percentage of NREM stages 3 and 4 (r=-0.54, P<0.05), and was not correlated with other sleep parameters (P>0.05). Conclusion The sleep architecture of patients with adult partial epilepsy is abnormal, and their daytime sleepiness is more than healthy persons. Epileptic seizure can aggravate sleep architecture abnormality and daytime sleepiness. Abnormal sleep parameters are related with ESS score.
8.Changes of gastric emptying after acute myocardial infarction in awake rats

Objective To investigate the changes of gastric emptying in awake rats after acute myocardial infarction(AMI). Methods The male Wistar rats were submitted to thoracotomy followed by or not by occlusion of the left coronary artery. After 24 h, AMI(n=12)and sham groups (n=9)were fed in gavage(1 ml)with ~(99m)-Tc-DTPA labeled test meal. Half gastric emptying time (GET1/2) and 50min residual rate were measured by single photon emission computed tomography. The rats of AMI group were sacrificed after 50 min. Plasma level of motilin was measured by radioimmunoassay in AMI and sham group respectively(n=9). AIM: To evaluate the results of preservative free sodium hyaluronate and basic fibroblast growth factor(bFGF) eye drops in the treatment of dry eyes after eye operation.·METHODS: One hundred and thirty two patients of dryeyes underwent 149 different eye surgeries. The Schirmer test Ⅰ and tear break up time (BUT) were performed prior to surgery and post operatively after 2 weeks and after 4 weeks when the patients were using Oflaxacine, Tobramycin-dexamethasone eye drops. In addition, preservative free sodium hyaluronate and bFGF eye drops were given post operatively. The antibiotic-cortisone eye drops were stopped after the requirted period depending on the type of eye surgery done, and preservative free sodium hyaluronate and bFGF eye drops were stopped after 4 weeks.Then, after 5 weeks Schirmer test Ⅰ and BUT were done.·RESULTS: The Schirmer test Ⅰ and BUT results prior to surgery and after eye surgery showed a statistically significant improvement of the dry eye condition.·CONCLUSION: Usage of preservative free sodium hysluronate and bFGF eye drops help definitely in the management of dry eye after ophthalmic surgery.
10.Identification of Active and Passive Hands Motor Cortex Area with Functional MR Imaging in Hemipegic Patients after Stroke

ObjectiveTo identify the finding of functional MRI(fMRI) during active and passive fingers moving in hemipegic patients after stroke.Methods23 stroke patients with hemipegia were asked to actively moving their fingers and then passively moving with others' help.MSEPI technique was performed with planar parallel AC-PC.During each scan series,they were asked to perform repetitive finger-thumb opposition movement in the same period of active and rest.All the scanning data was transferred to a workstation and dealt with statistical method.Some patients examined fMRI again after rehabilitation.ResultsfMRI showed normal in the healthy side,with the decrease or displacement in the affected side of motor cortex area.Motor cortex area in the affected side would be recovered after rehabilitation.ConclusionfMRI can be used to evaluate the difference between active and passive moving motor cortex,and help to judge the prognosis of the patients with motor functional disorder.
