1.The Application of insulin pump in diabetic patients

Objective To evaluate the effect of insulin pumps in diabetic patients. Methods Perioporative 37 diabetic patients in our hospital were randomly divided into two groups;19 patients were given continuous insulin pump subcutaneously(CS Ⅱ), 18 multiple subcutaneous injection(MS Ⅱ), comparing the level of blood sugar control, pre-operative time, control of blood sugar levels, wound healing, and so on, postoperative enteral nutrition. Results Two groups of blood glucose levels before treatment had no significant difference (P > 0.5), after treatment the de-creased level of blood sugar and blood sugar level of stability in the CS Ⅱ group better than the MS Ⅱ group, the differ-ence was signifieant(P <0.01). CS Ⅱ significantly shortened the pre-operative preparation time,before and after op-eration and postoperative enteral nutrition ,control of blood sugar stable ,the wound healed well and shortened hospital stay. Conclusion Insulin pump is an effective manner in the perioperative treatment of diabetes for smoothing opera-tion, reducing the burden on patients and complications.
2.Diagnosis and surgical management of lumbar degenerative juxtafacet joint cysts

[Objective]To discuss the diagnosis and surgical management of lumbar degenerative juxtafacet joint cysts. [Methods]From June 2004 to June 2007,seven patients were diagnosed as degenerative juxtafacet joint cysts and were treated by surgery. The authors made a retrospective analysis on the clinical data of these 7 patients. Among these 7 patients,there were one as ligamentum flavum cyst and six as facet joint cysts. Five lesions were located at the L4、5 level and two were located at the L5S1 level. According to the preoperative lateral films and CT scan,segment instability were found in two patients. Fenestration decompression,resection of cyst and 1/3 medial articular process were performed on five patient without preoperative segment instability,while additional posterior fixation and posterolateral fusion were done to two patients with preoperative segment instability.The mean length of follow up were 14 months (range: 9~24 months ). The VAS score and ODI score were used for clinical outcome assessment.[Results]The pathological results confirmed the preoperative radiological diagnosis in all patients. Improvement were found in all patients,including completely recovery in five patients and significant relief of pain in other two patients.[Conclusion]Lumbar degenerative juxtafacet joint cysts has been proved to be a cause of low back pain. Preoperative CT scan and MRI are helpful in the diagnosis and the clinical outcome of surgery is satisfactory.
3.Detection of autoantibodies in 60 patients with primary biliary cirrhosis.

Objective To study the types of autoantibodies in patients with primary biliary cirrhosis.Methods There were 60 patients with primary biliary cirrhosis from January 1995 to December 2004 in People's Hospital. We analyzed those patients' autoantibodies results and clinical data.Results There were 75% patients with anti-mitochondrial antibody(45/60),and antinuclear antibodies were detected in 60%(36/60)PBC patients,with the following hierarchy of specificities:23%(14/60)speckled,20%(12/60)multiple nuclear dots,16%(10/60)nuclear membranous,6%(6/60)anti-centromere,1.6%(1/60)homogeneous,20%(12/60)anti-SSA,10%(6/60)anti-SSB and 1.6%(1/60)anti-RNP. Several patients showed multiple specificities. Comparing PBC patients with or without AMA,no statistically significant difference was found on ages,biochemical and immunological parameters.Conclusion AMA-negative PBC patients share the same clinical features with AMA-passive PBC. Except for AMA,other antibodies may present in PBC patients. Multiple nuclear dots and nuclear member antinuclear antibodies may be helpful for diagnosing PBC patients without AMA.
4.Effect of Monoammonium Glycyrrhizinate on Ascorbic Acid and Lead Complex

Stability constant of ascorbic acid a nd lea d complex was measured by pH potentiometry at different tempcraturcs and concentrations of monoammonium glycyrrhizinate (MG ).At a concentration of 5.0?10-4mol/L and at biolugical condition, lgk1 = 8.72 and lgk2 =6.91. The result suggestod that the stability constant can be imptoved at decreased temperatures and increased concentrations of MG.
6.Study on the Inclusion Compound of Baicalin with Hydroxypropyl-?-Cyclodextrin

Objective: To prepare the inclusion compund of baicalin-hydroxypropyl-?-cyclodextrin (HP-?-CD) and identify it. Methods: The inclusion compound of baicalin-HP-?-CD was confirmed by freeze-drying method. Results: The inclusion compound of baicalin-HP-?-CD was confirmed by IR absorption spectroscopy and differential scanning calorimetry (DSC). The results indicated that the inclusion compound of baicalin-HP-?-CD was formed. The content analysis of the inclusion compound showed that the molecular ratio of baicalin to HP-?-CD was 1∶1. Conclusion: The solubility of baicalin is increased from 0.112mg/ml to 2.743mg/ml. when the drug is included by HP-?-CD.
7.Preparation and evaluation of 99Tcm-(HYNIC-Lys3 -bombesin) (tricine) (TPPTS) for imaging the Balb/c nude mice bearing human pancreatic cancer

Objective To synthesize 99Tcm- (hydrazinonictinamide- [Lys3] -bombesin) (tricine)(trisodium triphenylphosphine-3,3',3"-trisulfonate) ((HYNIC-[Lys3]-BBS) (tricine) (TPPTS)) and evaluate its biodistribution and binding capability with tumor tissue in Balb/c nude mice bearing human pancreatic cancer xenografts. Methods HYNIC was conjugated to the [Lys3] -BBS at pH = 9.0 with SnCl2 as reducing agent and both tricine and TPPTS as coligands for 99Tcm-labeling. 99Tcm-HYNIC-[Lys3]-BBS)(tricine) (TPPTS) was purified by Sep-Pak C18 cartridge and was analysed by HPLC. The radiochemical purity and radiolabeling yield were measured. The stability of 99Tcm-(HYNIC-[Lys3]-BBS) (tricine)(TPPTS) in serum, biodistribution (% ID/g) in the normal mice and imaging of the Balb/c nude mice bearing human pancreatic cancer xenografts in vivo were studied. Results The radiolabeling yield was (90 ±2)% and the radiochemical purity was over 95%. The radiochemical purity after 4 h in serum was over 85%. The distribution in normal mice showed rapid clearance from blood (the uptake was (0.07 ±0.01) %ID/g at 2 h postinjection). 99Tcm-(HYNIC-[Lys3]-BBS) (tricine) (TPPTS) was excreted mainly via the kidney with little radioactivity accumulation in the liver and gastrointestinal tract (the uptake of liver, stomach, intestine was (0.27 ±0.03), (0.06 ±0.03), (0.04 ±0.00) %ID/g at 2 h postinjection). Marked uptake of radioactivity was found in tumor tissue of the Balb/c nude mice bearing human pancreatic cancer with maximum T/NT ratio of 3.71 ± 0.57 at 2 h postinjection. Conclusions 99Tcm-(HYNIC-[Lys3]-BBS)(tricine) (TPPTS) can be easily prepared with high radiolabeling yield and radiochemical purity. The stability in serum and good biodistribution charateristics make it useful for the diagnosis of human pancreatic cancer with over-expression of the gastric-releasing peptide(GRP) receptor.
8.Recombinant human bone morphogenetic protein 2 composite bone in the lumbar spine fusion

BACKGROUND:Many studies have confirmed that recombinant human bone morphogenetic protein 2 plays a very important role in bone formation and fracture healing, but recombinant human bone morphogenetic protein 2 alone implanted is prone to diffusion and degradation, which is unable to play a persistent role in new bone formation. OBJECTIVE:To explore the effect of recombinant human bone morphogenetic protein 2 composite bone in the rabbit lumbar fusion. METHODS:Thirty New Zealand white rabbits were selected to make posterior lumbar intertransverse fusion models, and then were randomly divided into three groups, in which, L5-6 intertransverse implantation of autologous iliac bone, al ogeneic bone and recombinant human bone morphogenetic protein 2 composite bone (recombinant human bone morphogenetic protein 2 and al ogeneic bone complex) was done respectively. At 6 weeks after implantation, gross observation, X-ray examination and histological observation were performed. RESULTS AND CONCLUSION:Fusion rate and percentage of new bone area were higher in the composite bone group than the autologous iliac bone and al ogeneic bone groups (P<0.05);the tensile strength was lower in the al ogeneic bone group than the other two groups (P<0.05), but there was no difference between these two groups except the al ogeneic bone group. X-ray films showed cal us formation in the implanted region of the three groups. In the autologous iliac bone group, a large amount of cartilage tissues formed along with a smal amount of bone trabeculae and a certain amount of woven bones. In the al ogeneic bone group, the implant was covered with a large amount of fibrous tissues, bone island was seen and there was also a smal amount of bone trabeculae and cartilage tissues. In the composite bone group, a great amount of bone trabeculae and cartilage tissues were visible to form woven bone and cortical bone. These findings indicate that the recombinant human bone morphogenetic protein 2 composite bone can obtain good effect in the rabbit lumbar fusion.
10.Treatment of osteochondritis lesion in the knee with reduction and fixation under arthroscopy.

OBJECTIVETo study clinical results of reduction and fixation under arthroscopy for the treatment of osteochondrotical lesion in the knee.
METHODSFrom December 2012 to December 2013, 4 patients with the knee joint osteochondritis lesion and free bone fragments were treated with arthroscopy to detect the stripped cartilage, and then the cartilages were reduced with small incision and fixed by cartilage nail. There were 3 males and 1 female, with age of 15, 15, 20 and 27 years old. The durations of the disease were 1 d, 5 d, 1 month and 1 year. All the patients had swelling and effusion in the knee joint, and the floating patellar test was positive. CT examination showed bone defects and loose bodies. The Lysholm, VAS, and Tegner scale were used to evaluate the knee joint functions before and after operation.
RESULTSAll the patients underwent successful surgery. Postoperative CT showed good location of the cartilage without loss of reduced bone and cartilage. All the patients were followed up, and the duration ranged from 10 to 13 months, with a mean of 12 months. In one patient, arthroscopy examination was conducted for a second time to examine the connection of the fracture part to the surrounding cartilage, and it showed that the internal fixator was not absorbed. The Lysholm, VAS and Tegner scale of all patients were better than those before operation.
CONCLUSIONTreatment of osteochondritis lesion with the fixation of absorbable cartilage nails may reconstruct the integrity of articular surface and recover the stability of joints, and it is an effective treatment method.
