1.Transobturator tension-free mesh for female anterior pelvic floor reconstruction

Objective To investigate the efficacy and safety of anterior pelvic floor reconstruction with transobturator tension-free vaginal mesh in the treatment of moderate to severe or recurrent anterior vaginal wall prolapse. MethodsAnterior pelvic floor reconstruction with transobturator tension-free vaginal polypropylene mesh were performed on 36 women with stage 3 to stage 4 or recurrent anterior vaginal wall prolapse.Those with stress urinary incontinence(SUI) or potential SUI were also treated with transobturator inside-out tension-free vaginal tape(TOT).The surgical efficacy was determined by comparison of POP-Q classification system pre-and post-operation.Complications of the procedure were statistically analyzed. Results The mean time of operation for anterior pelvic floor reconstruction with transobturator tension-free vaginal polypropylene mesh was 52 min,mean intraoperative blood loss was 55 mL,and mean hospital stay was 3.5 d.No operative complications occurred.All the anterior vaginal wall prolapse was corrected.No recurrence was observed after the follow up for 2 to 24 months,with the effective rate of 100%.The grade of urinary incontinence was improved remarkably,with the effective rate of 100%.The erosion after the procedure with vaginal polypropylene mesh happened in 2 cases,with the incidence of 5.56%. Conclusion Anterior pelvic floor reconstruction with transobturator tension-free vaginal polypropylene mesh is a minimally-invasive procedure for the treatment of moderate to severe or recurrent anterior vaginal wall prolapse.TOT should also be performed for those with SUI or potential SUI.Though erosion of mesh may take place post-operation,this procedure is safe and has a favourable short-term effect.The long-term effect leaves for further investigations.

4.Treatment of Lumbar Intervertebral Disk Herniation with Electric Acupuncture

To make objective evaluation on treatment of lumbar intervertebral disc herniation with electric acupuncture. Method: All 200 cases were randomly allocated into an observation group (100 cases) and a control group (100 cases). Cases in the observation group were treated with electric acupuncture. Cases in the control group were treated with oral administration of Mobic. Result: The total effective rate in the observation group was 86.5%,and the total effective rate of the control group was 75.0%. There was a significant difference between the two groups (P<0.01). Conclusion: Electric acupuncture is a safe and remarkably effective way for lumbar intervertebral disc herniation.
6.Micro satellite instability in the development of gastric cancer

Objective To detect the micro satellite instability (MSI) from the precancerous and cancerous lesions (GC) and its effect on carcinogenesis.Methods Silver staining single strand conformation polymorphism polymerize chain reaction (PCR SSCP) was used to screen MSI markers at 5 loci in formalin fixed,paraffin embedded tissues of GC (n=30), dysplasia (n=30), intestinal metaplasia (IM) ( n =40) and corresponding normal gastric tissues.Results The abnormal shifting of the single strand DNA was identified in 7(23 3%)out of GC, in 9 (30%) out of dysplasia and in 8(20%) out of IM samples respectively. GC with MSI was associated with distal location of the tumors ( P =0 044). MSI was not detected in low grade IM tissues.Conclusion MSI may be of an early event during gastric carcinogenesis. It contributes probably to the acquisition of a transformed cell phenotype and to the development of gastric cancer.
7.Application of magnetic resonance imaging in diagnosis and treatment of pelvic neoplasms

Objective To study the value of magnetic resonance imaging(MRI) in the diagnosis and treatment of pelvic neoplasms. Methods The patients who were suspected of malignant pelvic neoplasms were performed MRI.The surgical approach was determined according to the MRI results.The MRI results were compared with operation findings and pathological reports. Results The MRI results were consistent with operation findings and pathological reports in 22 cases highly suspected of pelvic neoplasms and 15 cases with abnormal endometrial echo.Therapeutic regimen was designed for 5 cases with uterine cervix cancer according to MRI stages,and favourable surgical curative effects were obtained. Conclusion MRI provides a reliable guideline for surgical approach and may improve the outcomes.
8.Trends of demographic and primary disease in new end-stage renal disease dialysis patients of Zhejiang province from 2008 to 2013

Objective To analyze the changes of primary disease and demography of patients with end-stage renal disease (ESRD) who started dialysis between 2008 and 2013 in Zhejiang province, in order to provide statistical support for the improvement of dialysis quality.Methods The study retrospectively reviewed all the incident ESRD patients on dialysis registered in Zhejiang Dialysis Quality Control Center from 2008 to 2013.Their demographics and primary cause were investigated.Stratified analysis based on dialysis modality, duration and age were conducted.Results A total of 26 310 incident ESRD patients who started dialysis between 2008 and 2013 in Zhejiang were selected, among which 14 886 cases were male and 11 424 cases were female (male: female: 1.30: 1).The study gave priority to patients aged 45 and above (73.9%), with an average age of (55.7± 16.1) years.The top primary disease was chronic glomerulonephritis (51.3%), followed by diabetic nephropathy (17.3%) and hypertensive nephrosclerosis (6.4%).From 2008 to 2013, the incidence of dialysis was 46.3, 60.1, 71.2, 85.8, 104.1, and 119.9 per million population respectively.The average age of patients was increased year by year, from 52.4, 53.3, 54.8, 56.0, 56.9, to 57.6 years.The share of peritoneal dialysis, as well as the promotion of diabetic nephropathy in primary disease was on the rise.With the increase of age, the proportion of primary diseases such as diabetic nephropathy and hypertension nephrnsclerosis was increased.Conclusions In Zhejiang province, incident dialysis patients increase annually.More males than females are on dialysis.The average age and proportion of peritoneal dialysis are rising.The leading cause is chronic glomerulonephritis, but the incidence of diabetic nephropathy is on the increase, which requires attention.
9.Research on the physical components stimulated by rotating grating using functional magnetic resonance imaging

AIM: Using functional magnetic resonance imaging, to inspect v sual cortex physical reactions stimulated by rotating grating, and to its components. METHODS: On 1.5T MR scanner, GRE-EPI imaging sequence was carried on 9 h discover ealthy volunteers, the visual cortex response data were processed after delinearation by SPM99.RESULTS: Different components of rotating grating excited different areas of the visual cortex. Dramatic response in the central part of the occipital lobe, which was related to white light stimuli, located at primary visual cortex. Response area in bilateral Broadaman 19 areas was related to vision-motion function. And weak response area in the central part of the occipital lobe was related to shape perception.CONCLUSION: Rotating grating conclude plenty of visual information, and it excites different areas of the optic center as a stimuli. fMRI is a valuable equipment to study the physiology of visual cortex.
10.Clinical significance of serum proteomic profiling before and after cervical cancer surgery

Objective To investigate the value of serum proteomic profiling in cervical cancer detected pre-surgery and post-surgery. Methods Magnetic bead and matrix-assisted laser desorption/ionization-time of flight-mass spectrometry (MALDI-TOF-MS) were used to detect the serum samples from 54 cases with cervical cancer before and after surgery and 53 serum samples from healthy women. The results of spectra were analyzed by Biomarker Wizard software. Results Significant variation of proteomic profiling between pre-surgery and post-surgery were analyzed. There were 22 proteins with different mass/charge (M/Z) values significantly different (P<0.01) at the M/Z value range from 1500 to 50 000, among of which relative content of proteins with M/Z 3981, 4290, and 28 066 in pre-surgery cervical cancer patients were higher than those in health women [(1.51±1.78)% vs (0.83±0.38)%, (2.70±2.19)% vs (1.72±0.91)%, (1.99±1.70)% vs (0.92±0.95)%; P<0.01], while in the post-surgery patients, relative content of these three proteins significantly decreased to (0.59±0.45)%, (1.01±0.64)%, (0.54±0.37)%, respectively. But the relative content of another three proteins with M/Z 11 487, 11 529, and 11 678 were significantly increased in post-surgery patients [(0.38±1.41)% vs (2.74±3.67)%, (0.16±0.46)% vs (2.00±1.76)%, (1.02±1.67)% vs (7.71±9.46)%; P<0.01]. Conclusion Serum proteomic profiling could screen out the proteins which had significant variation between pre-surgery and post-surgery serum, of which with M/Z 3981, 4290, and 28 066 may be related with tumor burden, while with M/Z 11 487, 11 529, and 11 678 may be response to surgical stress.
