1.Effect of Occupational Therapy on Unilateral Spatial Neglect after Stroke

Objective To observe the effect of occupational therapy(OT) on unilateral spatial neglect(USN) in stroke patients.Methods 108 stroke patients with USN were randomly divided into OT group(54 cases) with OT and other rehabilitation therapy and control group(54 cases) without OT.They were assessed with line bisection test,number cancellation test,copying drawing test and clock-drawing before and 2 months after treatment.Results The incidence of USN of both groups were significantly reduced after treatment(P<0.01),while that of the OT group were lower than of the control group(P<0.01).Conclusion OT can significantly improve the USN after stroke.
2.Effect of Occupational Therapy on the Function of Upper Limbs and Complications in Stroke Patients

ObjectiveTo observe the clinical effect of occupational therapy (OT) on the function of upper limbs and complications in stroke patients with hemiplegia.Methods99 stroke patients were randomly divided into the OT group (52 cases treated with OT and routine rehabilitation) and control group (47 cases treated without OT). Fugl-Meyer Assessment (FMA) and evaluation of hand function were performed before and two months after treatment. The complications of patients in two groups were also compared.ResultsAfter treatment, the functions of upper limbs and hand of patients in the OT group were superior to those in the control group ( P<0.05). The numbers of cases with complications in the OT group were significantly less than that in the control group ( P <0.01).ConclusionOT can improve the functions of upper limbs and hand and abate complications in stroke patients.
3.Effect of Occupational Therapy on Hemianesthesia and Hand Function after Stroke

Objective To explore the effect of occupational therapy (OT) on hemianesthesia and hand function after stroke. Methods 50 stroke patients with hemianesthesia were divided into OT group (n=25) and control group (n=25). The OT group accepted OT combined with routine therapy and the control group accepted routine therapy only. They were assessed with upper extremity sense of Fugl-Meyer Assessment (FMA) and evaluation of hand function before and after treatment. Results The scores of FMA and hand function improved significantly in both groups 2 months after treatment, and improved more in the OT group than in the control group (P<0.05). Conclusion OT can improve the sense and the hand function in stroke patients with hemianesthesia
4.Effect of Occupational Therapy on Post Stroke Depression

Objective To explore the effect of occupational therapy (OT) on the depressive mood and activity of daily living (ADL) in patients with post stroke depression (PSD). Methods 42 patients with PSD were divided into OT group (n=21) and control group (n=21).Both 2 groups received routine therapy, and OT group received ADL training, selective recreation and sport and appreciation of music in addition.They were evaluated by modified Bathel index (MBI) and Hamilton Self-rating Scale for Depression (HAMD) before and after treatment.Results The scores of MBI increased and the scores of HAMD decreased significantly after 4 weeks in both 2 groups (P<0.001), especially in the OT group (P<0.01). Conclusion OT can improve not only ADL, but also the depressive mood in patients with PSD.

7.Optimization of Xylanase Production by Paecilomyces thermophila in Solid State Fermentation

A new thermophilic fungus J18 isolated from the soil samples was identified as Paecilomyces thermophila. This strain produced effectively xylanase utilizing several lignocellulosic materials in the solid-state fermentation (SSF) , and wheat straw was the best carbon source. The results of single-factor-experiment showed that the wheat straw of particle size 0. 3 mm ~ 0.45 mm, initial moisture content of 83% , initial pH of 7. 0 and cultivation temperature of 50℃were the optimal conditions for xylanase production. Under the optimized conditions, it produced 18 580 U/g dry substrate after 8 days of cultivation. Therefore, xylanase production by Paecilomyces thermophila in SSF possess great potential for commercial applications.
8.Analysis of new effector T cells named TH17 cells in peripheral blood of patients with rhenmatoid arthritis

Objective To investigate the distribution of TH17 cells in peripheral blood of patients with rheumatoid arthritis(RA). Methods Intracelluar flow cytometirc detection of TH17 cells in peripheral blood was establised using PHA or PMA + Ion as stimulators in vitro. Thirty-five active and 30 stable RA pa-tients and sex and age paired healthy controls were enrolled in this study. Results The stimulating effect of PMA + Ion was better than that of PHA alone. Under PMA + Ion stimulation, the percentage of TH17 cells in peripheral blood from RA patients and healthy controls was increased significantly(P<0.05). With or with-out PMA + Ion stimulation, such percentage in active group was higher than that of stable group and both of them were higher than that of heathy controls(P<0.05). The distribution profile of TH1 cells was similar to that of TH17 cells. Conclusion There is a distribution abnormality of TH17 cells in peripheral blood of RA patients, which could be used as a new marker for the assessment of immunological condition in RA.
9.Use of the SmartPrep technique with concentration of contrast medium control in CT angiography of vertebral and carotid artery

Objective To study the optimal enhancement phase and exact triggering scan time on spiral CT angiography of vertebral and carotid artery,and to improve the rate of 3D-CT display in vertebral and carotid artery.Methods One hundred and thirty-three cases of CT angiography of vertebral and carotid artery(87 vertebral and 46 carotid artery)were examined with SmartPrep software,which controled the scan at real time after 100 ml contrast medium(350 mg I/ml Omnipaque)were quickly injected through foot back vein.One hundred and thirteen cases of control group were examined by triggeing off the scanning with a delay time of 18—22 s after blous injection through foot back vein.The vertebral and carotid arteries were demonstrated with 3 dimensional reconstruction.Results The successful rate in SmartPrep group was 100% with the enhancement situated in best(n=130)or good(n=3)scanning phase.The threshold concentration values of target vessel were kept on 100 HU in the whole scan period[(18?4)s].However, the triggering time of enhancement scanning was not equal(15—43s)and the deviation was up to 28 s.The average concentration of target vessel was obviously higher in SmartPrep group[(161.4?2.0)HU]than in the control group[(133.3?2.2)HU](t=-9.456,P
10.Analysis of clinical factors related to genotype B and C chronic hepatitis B infection in Zhenjiang area

Objective To compare the differences of clinical characteristics between genotype B and C chronic hepatitis B(CHB)patients and to summarize clinical factors related to genotype C hepa- titis B virus(HBV)infection.Methods Seventy eight CHB patients who were diagnosed with genotype B or C infection by liver puncture biopsy and genotyping were enrolled.Their serum HBV DNA levels were detected.Severe hepatitis,liver cirrhosis,hepatocellular carcinoma and HBeAg positive rate were analyzed to determine the pathologic inflammation and fibrosis degree of liver tissue.Chi square test and Logistic multiple regression analysis were employed for the statistical analysis.Results The serum albumin and pre-protein were lower in genotype C CHB patients than that in genotype B.The alanine aminotrans- ferase,total bilirubin and prothrombin time were higher in genotype C CHB patients than that in genotype B.The rates of genotype C patients increased significantly with the grade of liver necroin- flammation progressing from GO to G4(1.8%,11.1%,20.4%,33.3%,33.3%) and the stage of liver fibrosis progressing from SO to S4(5.6%,5.6%,14.8%,33.3%,40.7%),but the rates of genotype B patients did not change significantly with the grade of liver necroinflammation(16.7%, 25.0%,25.0%,20.8%,12.5%)and stage of liver fibrosis progressing(16.7%,29.2%%,20.8%, 16.7%,16.7%).There was statistical significance in grades of liver necroinflammation(X~2= 11.49,P=0.022)and stages of liver fibrosis(X~2=13.56,P=0.006)between genotype B and gen- otype C patients.The rates of genotype C CHB patients were higher than,similar with and lower than the rates of genotype B patients of HBV DNA level above 1.0?10~6 copy/mL,between 5.0?10~2-1.0?10~6 copy/mL and under 5.0?10~2 copy/mL,respectively(51.8% vs 12.5%,35.2% vs 45.8% and 13.0% vs 41.7%).There was statistical significance of HBV loads between genotype B and genotype C patients(X~2=13.25,P=0.001).HBeAg positive rate in genotype C patients was significantly higher than that in genotype B patients(61.1% vs 25.0%,X~2=8.67,P=0.003).The rates of decompensated cirrhosis,compensated cirrhosis and no-cirrhosis in genotype C patients were higher than,similiar with and lower than the rates in genotype B patients,respectively(40.7% vs 4.2%,22.2% vs 20.8% and 37.0% vs 75.0%).There was statistical significance of the rate of cirrhosis between genotype B and genotype C patients (X~2=12.47,P=0.002).Conclusions The degree of liver necroinflammation and fibrosis,the HBeAg positive rate and the incidence of cirrhosis are all related with genotype C HBV infection.
