1.Radiofrequency Thermocoagulation Combined with Ozone Intradiscal and Extradiscal Injection in the Treatment of Failed Back Surgery Syndrome:Report of 58 Cases

Objective To explore the clinical effects and safety of radiofrequency thermocoagulation combined with ozone injection in the treatment of failed back surgery syndrome (FBSS). Methods A total of 58 patients suffered from FBSS were treated with radiofrequency thermocoagulation and ozone injection from October 2006 to December 2012.The changes of lumbar and leg functions were observed .The evaluation of clinical efficacy was made by visual analogue scale ( VAS) and Oswestry disability index (ODI) at one week, one month, three months, six months, and twelve months postoperatively , respectively. Results The operation was successfully completed in all the 58 patients , without serious complications .All the cases were followed for one year .The ODI was 20.3 ±3.1 at the last follow-up, which was significantly lower than that before operation (79.8 ±2.6, t=96.871, P=0.000).The VAS of back pain was (7.6 ±0.4) points preoperatively and (3.1 ±0.2) points at the last follow-up, which also showed statistical significance (t=45.206, P=0.000).According to the modified MacNab criteria, clinical outcomes showed excellent in 18 cases and good in 23 cases, with an excellent and good rate of operation being 70.7% (41/58). Conclusion Treatment of FBSS by using radiofrequency thermocoagulation and ozone injection is an effective option .
2.Effect of Kidney nourishing decoction in the treatment of reproduce patients in serum hormone

Objective To investigate the effects of Kidney nourishing decoction in the treatment of reproduce patients in serum hormone .Methods 120 cases with reproduce patients from June 2014 to June 2016 in hangzhou traditional chinese medical hospital were selected,and divided into control group and experiment group, 60 cases in each group.The control group was given artificial cycle treatment, the experiment group was treated with Kidney nourishing decoction, two groups were treated for three months.Serum sex hormone and AMH content, clinical effect and pregnancy rate were compared before and after treatment.Results Compared with before treatment, levels of FSH, LH, E2 and FSH/LH in two groups decreased, AMH content increased after treatment,the thickness of endometrium and ovarian antral follicle number increased,symptom scores decreased after treatment (P <0.05);Compared with the control group, levels of FSH, LH, E2 and FSH/LH in the experiment group were lower, AMH content were higher, the thickness of endometrium and ovarian antral follicle number in the experiment group were higher,and the symptom scores were lower (P<0.05).The total effective rate of the study group was 90.00% higher than the total effective rate of the control group 73.33% (P<0.05).There was no significant difference in adverse reaction rate between the two groups, the pregnancy rate in the experiment group 30.00% was higher than the control group 11.67% ( P <0.05 ) .Conclusion Kidney nourishing decoction can reduce the sex hormone and increase the content of AMH , and increase the pregnancy rate in the treatment of reproduce patients.

5.An analysis of ocurrence and prognosis related factors of renal dysfunction in patients with multiple myeloma.

Objective To analyse the occurrence and prognosis related factors of renal dysfunction in patients with multiple myeloma(MM).Methods During Janaury 1991 and Janaury 2007 seventy-four cases of MM in Department of Hematology,Gernal Hospital of Tianjin Medical University were enrolled in this study.The occurrence,the risk factors and prognosis were analyzed.Results The incidence of renal dysfunction(RD)in MM was 56.8%.Age,hypertention,hemoglobin,serum ALB,GLO,calcium,phosphonium,?2-MG,the percentage of plasma and immature plasma cells in bone marrow,Durie-Salmon stage and type of MM were associated with the incidence to RD with MM by monovariam analysis.Hypertention,serum ALB and ?2-MG were associated with the incidence of RD with MM by multivariant analysis.ALB was the protected factor and the other two were risk factors.The renal function improved rapidly in the patients who received CR or blood transfusion for severe and moderate anemia.Compared with patients of normal renal funtion,RD patients survived shorter and had higher early death rate.Conclusion Hypertention and high tumor burden are risk factors of renal dysfunction in MM.Effective chemothemapy and supportive treatment helped renal function recovery.
6.Clinical analysis of 35 cases of adult henoch schonlein purpura

Objective To examine the epidemiological and clinical features of the adults with Henoch Schonlein Purpura (HSP),and improve the diagnosis and therapeutic efficacy of HSP.Methods Thirty five adults with HSP from the hematological department of Tianjin Medical University General Hospital between 1993.9 to 2005.12 were enrolled in this study.Epidemiological,clinical and laboratory data,therapeutic methods and efficacy were investigated.Results Most patients with HSP in our department were male and adolescent.HSP occurred more commonly in the winter.The important predisposing factors were infection and having specific food.Purpuric lesions were the most common presenting signs.Multiple organ involvement was more common.Purpura was present in all case,lower extremities were more commonly involved.Joint involvement was observed in 15 cases(42.9%),abdominal involvement in 20 cases(57.1%),renal involvement in 20 cases(57.1%).Conclusion The adult patients with HSP had more tendenecy to have severe renal damage and multiple organ involvement.
7.Clinical analysis of autoimmunic hemocytopenia complicated with thrombosis.

Objective To probe the clinical features and possible mechanisms of autoimmunic hemocytopenia complicated with thrombosis.Methods From 2007 to 2008,the clinical and laboratory data of seven patients with autoimmunic hemocytopenia complicated with thrombosis in the hematology department General Hospital tianjin medical university were analysed.Results One of the patients was with Evans syndrome,and another six patients with immuno-related pancytopenia.Three were complicated with lower extremity Veins embolism;two with cerebral vessel and pulmonary artery embolisms respectively;one with splenic artery and coronary artery embolisms respectively.Among them,three were complicated embolism of two parts;six patients had higher D-Dimer level.They were treated with glucocorticoids,cyclosporin A,and immunoglobulin for immunic hemocytopenia and with anti-coagulation therapy(aspirin、heprin and warfarin) for thrombosis.Seven patients' hemogram,signs and symptoms improved gradually since they received immunosuppression and anticoagulation therapy.The symptoms of thrombosis disappeared two weeks later.The degree and extent of thrombosis also alleviated;at the same time,the function of blood coagulation recovered partly or completely.Conclusion Thrombosis is a rare but severe complication of autoimmunic hemocytopenia.Promptly anti-coagulation and immunosuppressive therapies can get ideal therapeutic effects.

Idiopathic cytopenia of uncertain significance is a new term which describes a cytopenia in one or more myeloid lineages that is constant,does not meet the minimal criteria of an MDS and cannot be explained with any other hematologic or non-hematologic diseases.The proposal,diagnosis and therapy of ICUS are reviewed in this paper.
9.CD~+_5B lymphocytes in the bone morrow of autoimmunic hemocytopenia patients and its clinical significance

Objective To investigate the quantitative abnormality of CD~+_5B lymphocytes in the bone morrow of the patients with autoimmunic hemocytopenia and its clinical significance.Methods The patients were referred to the Institute of Hematology & Blood Diseases Hospital,Chinese Academy of Medical Sciences & Peking Union Medical College.Quantities of CD~+_5B lymphocytes in the bone morrow of 14 patients with autoimmune hemolytic anemia(AIHA)or Evans syndrome,22 immunorelated pancytopenia(IRP)patients and 10 normal controls were assayed by FACS.The correlation between their clinical and laboratory parameters with CD~+_5B lymphocytes was analyzed.Results The qutantity of CD~+_5B lymphocytes were significantly higher in AIHA and Evans and IRP patients than in normal controls;there was no significant difference between AIHA、Evans syndrome and IRP patients.The qutantity of CD~+_5B lymphocytes in the bone morrow of all cytopenic patients showed negative correlation with C_3 complement.In AIHA、Evans syndrome patients,the quantity of CD~+_5 B lymphocytes in their bone morrow showed positive correlation with IBIL.In Evans syndrome patients,the quantity of CD~+_5B lymphocytes in their bone morrow showed positive correlation with PAIgG and PAIgM.The qutantity of CD~+_5B lymphocytes in the bone marrow of all cytopenic patients showed negative correlation with treatment response,but no correlation with the yields of CFU-E、 BFU-E、CFU-F and CFU-GM cultured from their bone marrow mononuclear cells in vitro.Conclusion CD~+_5B lymphocytes in the bone marrow of the patients with autoimmunic hemocytopenia significantly increase and are related to the disease severity and clinical response.It is suggested that CD~+_5B lymphocytes might have important roles in the pathogenesis of autoimmunic cytopenia.
10.Clinical analysis of 72 cases of immunorelated pancytopenia with positive BMMNC-Coombs tests

Objective To study the clinical and laboratory features of the patients with immunorelated pancytopenia(IRP).Methods The risk factors,manifestations,blood cell counts,bone marrow phenotypes,autoantibodies and immunosuppressive therapy response of 72 patients with IRP were analyzed.Then they were followed up for(6 ~36)months,to see their long-term outcome and the prognostic factors.Results The infection,anaphylaxis and pregnancy were highly suspected to be the risk factors of IRP.Most of these patients were with pancytopenia72.2%(52/72);91.7%(66/72)of them were anemic with large or normal MCV;75.0%(54/72)of them were with leukopenia;18.1%(13/72)of them had fever.Thrombocytopenia was common,but serious bleeding rare.68.1% of these cases were with normal or decreased bone marrow cellularities and increased normoblasts.They were all found to have positive results of bone marrow mononuclear cell Coombs test,negative results of routine hemolysis tests and no evidence of malignant clonal hematopoiesis.42.9%(18/42)of these patients had C3 decreased,and 19.0%(8/42)C4 decreased.Immunosuppressive therapy was administered to 72 IRP patients.The response rate at 36 months was 87.5%(28/32).Conclusion IRP is a blood disease induced by acquired autoantibodies mediated bone marrow inhibition or destruction.It manifests pancytopenia with relative higher percentages of reticulocytes and neutrophils,and responds well to immunosuppressive therapy.
