1.Clinical performance of fiber post restorations in elderly patients with residual root and crown

Objective To investigate the clinical performance of fiber post restorations in elderly patients with residual root and crown and to analyze the factors affecting the clinical success rate.Methods A total of 66 elderly patients requiring post restoration were selected.They were randomly divided into two groups:the observation group (n=33,restored with fiber post-resin cores) and the control group (n=33,restored with cast metal post-cores).All patients were rechecked 1 year and 2years after the restoration.The clinical efficacy and the failure types were recorded.The differences in clinical performance between the 2 groups were compared by using Fisher's exact test.Results The differences in the gingival index and periodontal index in the 2 groups between the experimental teeth and the control teeth were zero.There were no significant differences in dental morphology,root canal treatment failure,tooth color matching and marginal coloration between the 2 groups (all P>0.05).1 case with post debonding and 1 case with fiber post fracture were found in the observation group and 1 case with marginal coloration was found in control group.There was no significant difference in clinical success rate between the 2 groups (93.94% vs.96.97%,P>0.05).Conclusions The clinical success rate in fiber post-resin core restoration is the same as in cast metal post-cores restoration in elderly patients.
3.Application and analysis of competition based training mode for first aid skills in residency program

Objective:To explore the model of first aid skills training based on competition.Method:The second-year residents who participated in the competition in 2021 ( n = 142) were selected. According to whether they attended BLS training in the first year, they were divided into group A ( n = 88) who attended BLS training and group B ( n = 54) who did not. Chi-square test, T test, Wilcoxon rank sum test and Logistic regression were used in our study. Results:There was no significant difference in gender and specialty between both groups (female, 62.5% vs. 68.5%, P= 0.466; TCM, 50% vs. 53.7%, P= 0.668), but the real resuscitation experience of group A was more than that of group B (40.9% vs. 9.3%, P= 0.000). The qualified rate of CPR and the compression score in group A was higher than that in group B [(81.8% vs. 61.1%, P = 0.006; (30±5) vs. (25 ±10), P= 0.001], including compression frequency, depth, rebound and compression/respiration rate (73.9% vs. 55.6%, P= 0.024; 88.6% vs. 70.4%, P= 0.006; 96.6% vs. 87%, P= 0.031). In the theoretical examination, the correct rates of electrocardiogram [(53.63±2.9)% vs. (50.44±2.57)%] and first aid medication [(57.38±3.55)%, P = 0.001] in the two groups were significantly lower than the qualified rate. After adjusting other factors, Logistic regression analysis showed that the CPR qualification rate in group A was 2.769 times higher than that in group B ( P= 0.015, 95% CI 1.215~6.311) Conclusions:The first aid skills training mode based on competition can objectively reflect first aid skills level of residents. We found that the quality of CPR skills was not related to gender, specialty and real CPR experiences but experience of BLS training was an independent influencing factor. And how to identify and manage arrhythmias and how to choose different emergency drugs are urgent emergency skills for residents to improve.
4.Susceptibility weighted imaging in evaluation of the fetal vertebra and vertebral anomalies

Objective To explore the advantages of susceptibility weighted imaging (SWI) in depiction of normal fetal vertebra and vertebral anomalies.Methods This prospective study was approved by our institutional review board, and written informed consent was obtained from every participant, Fifty-eight pregnant women (gestation age 22 to 39 weeks, average 29 ± 3 weeks) who were suspected of carrying babies with vertebral anomalies by ultrasound screening underwent 1.5 T fetal spine MRI[including half-fourier acquisition single-shot turbo spin-echo(HASTE),true fast imaging with steady-state(True FISP) and SWI sequences]. MR images were reviewed for their quality by two radiologists independently. The image scores in HASTE, True FISP and SWI were compared by using Kruskal-Wallis test and Mann-Whitney U test. Three segments (cervical, thoracic and lumbosacral segments, respectively) of 15 fetuses were, at random, collected to compare among HASTE,True FISP and SWI and then evaluated by ANOVA analysis.The diagnostic accuracy of the three sequences among 32 cases with follow-up results was calculated respectively and compared by using Chi-square test. Results There was statistical differences among three sequences(χ2=50.685,P<0.05).The scores of SWI was higher than that of True FISP, and the scores of True FISP was higher than that of HASTE in the evaluation of the fetal vertebra(P all<0.05).The differences among cervical,thoracic and lumbosacral segments on True FISP and HASTE showed significant difference statistically (P all<0.05), also the image quality of cervical segment could not meet the requirement of clinical diagnosis. The image quality of SWI was favorable clinically and no statistical difference among three segments was found(P>0.05).A total of 32 fetal vertebral anomalies were identified by follow-up after birth including hemivertebra (n=14), fusion of vertebrae (n=1), butterfly vertebra (n=1), multiple vertebral malformations(n=9),spinal bifida(n=5),caudal regression syndrome(n=2).The diagnostic accuracy of SWI, True FISP and HASTE was 93.75% (30/32), 56.25% (18/32) and 37.50% (12/32) respectively.The diagnostic accuracy of SWI was the best compared to that of True-FISP and HASTE(χ2=10.083,20.017;P<0.01). Conclusion SWI proved to be the optimal technique in depiction of fetal vertebra and vertebral anomalies than True FISP and HASTE,especially in depiction of cervical vertebra.
5.Age-related study of brachial plexus MRI contrast to noise ratio and diffusion tensor imaging values in normal adults

Objective:To investigate the correlation of age with diffusion tensor imaging (DTI) values as fractional anisotropy (FA), apparent diffusion coefficient (ADC) and contrast to noise ratio (CNR) of three-dimensional nerve-sheath signal increased with inked rest-tissue rapid acquisition of relaxation imaging (3D SHINKEI) of the brachial plexus in normal adults.Methods:A total of 54 adult healthy volunteers and 6 patients with Guillain-Barre syndrome were prospectively enrolled from October 2018 to April 2019. Healthy volunteers were divided into 3 groups according to age as 21-40 years old group ( n=20), 41-60 years old group ( n=20), and ≥61 years old group ( n=14). All of them underwent MRI examination of the brachial plexus, including DTI and 3D SHINKEI sequences. The average FA and ADC values of the brachial plexus were measured and calculated through the fusion of DTI and 3D SHINKEI by 2 physicians independently. The contrast to noise ratio (CNR) of brachial plexus nerve was measured in 3D SHINKEI sequence images. Intraclass correlation efficient (ICC) was used to analyze the consistency between the two physicians. A simple linear regression model and Pearson correlation analysis were used to detect the correlation between FA, ADC, CNR and age. One-way analysis of variance was used to compare the differences of FA, ADC and CNR in different age groups. The FA and ADC values in different genders were compared by independent sample t-test. Results:Inter-observer agreements of the 2 physicians were good for FA and ADC values with ICC values of 0.811 and 0.901, respectively. For different groups, FA values were 0.397±0.023, 0.368±0.023, and 0.334±0.018 and ADC values were (1.376±0.072) × 10 -3 mm 2/s, (1.466±0.086) × 10 -3 mm 2/s, (1.486±0.080) × 10 -3 mm 2/s, for 21-40, 41-60, and ≥61 years old groups, respectively with statistical significant difference ( F=25.311, P<0.001; F=9.948, P<0.001). The CNR of the brachial plexus were 510.583±192.846, 502.581±128.821, and 426.782±113.648 for 21-40, 41-60, and ≥61 years old group without statistical difference ( F=1.429, P=0.249). The FA value of brachial plexus was highly negatively correlated with age ( r=-0.745, P<0.001), while the ADC value was moderately positively correlated with age ( r=0.596, P<0.001). The CNR of 3D SHINKEI sequence was negatively correlated with age ( r=-0.292, P=0.033). There was no statistically significant difference in brachial plexus FA and ADC values between male and female subjects ( t=1.496, P=0.141; t=-1.557, P=0.126). The FA value of Guillain-Barre syndrome patients was lower than that of healthy volanteers in the same age group ( t=6.129, P<0.001), and the ADC value had no statistical diference ( t=-1.335, P=0.186). Conclusion:The values of FA, ADC and CNR of brachial plexus in normal adults change with age. Among them, FA value is more significant.
6.Isolation and genome-wide evolutionary analysis of a type Ⅲ WU polyomavirus strain

Objective:To isolate and culture WU polyomavirus (WUPyV), and to analyze the genome-wide evolutionary patterns, homology and population dynamics.Methods:Real-time quantitative PCR was used to detect the nasopharyngeal aspirate samples of hospitalized children with respiratory tract infection in Beijing Friendship Hospital during 2020 to 2022. Primary human airway epithelial cells cultured at the air-liquid interface were used to isolate and culture WUPyV. Whole genome sequence of the isolated strain was obtained by Sanger sequencing. For phylogenetic and evolutionary dynamics analysis, the whole genome was compared with the published whole genome sequences in GenBank database.Results:The detection rate of WUPyV was 4.7% (31/659) during 2020 to 2022, and a clinical strain BJ0593 of WUPyV type Ⅲc was successfully isolated. The homology of the whole genome and gene fragments of WUPyV was high. The average evolutionary rate of VP2 gene was about 1.256×10 -4 substitution/site every year, and the population dynamics of WUPyV tended to be flat in the last decade. Conclusions:This study successfully isolated a clinical WUPyV type Ⅲ strain for the first time, which provided the basis for further investigation on the molecular evolution and pathogenicity of WUPyV.
7.Analysis of Mechanism of Yishen Huashi Granules for Improving Diabetic Nephropathy Based on Metabonomics and Transcriptomics

ObjectiveIn this study, based on ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS/MS) and high-throughput transcriptome sequencing technology(RNA-seq), we investigated the mechanism of Yishen Huashi granules in regulating serum metabolites and renal messenger ribonucleic acid(mRNA) expression to improve diabetic kidney disease(DKD). MethodSD rats were randomly divided into normal group , model group and Yishen Huashi granules group, with 8 rats in each group. The rat model of DKD was established by intraperitoneal injection of streptozotocin. Yishen Huashi granules group was given 5.54 g·kg-1·d-1 of Yishen Huashi granules by gavage, and the normal group and the model group were given the same amount of normal saline for 6 weeks. During the experiment, the body weight and blood glucose of rats were monitored, and the rats were anesthetized 24 hours after the last administration, blood was collected from the inferior vena cava, serum was separated, and renal function, blood lipid, and inflammatory indicators were detected. Kidney tissue of rats was fixed in neutral paraformaldehyde, and stained with hematoxylin-eosin(HE), Masson and periodic acid-Schiff(PAS) to observe the renal pathological changes. UHPLC-MS/MS and RNA-seq were used to identify the changes of serum metabolism and the differences of renal mRNA expression, and real time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to detect the differential mRNA and protein expression in renal tissue to explore the common expression mechanism. ResultCompared with the normal group, rats in the model group showed a decrease in body weight, a significant increase in blood glucose, urinary microalbumin to urinary creatinine ratio(UACR), blood urea nitrogen(BUN), cystatin-C(Cys-C), β2-microglobulin(β2-MG), interleukin-6(IL-6), triglyceride(TG) and total cholesterol(TC), and a significant decrease in total superoxide dismutase(T-SOD)(P<0.01). After the intervention of Yishen Huashi granules, all the indexes were improved to different degrees in rats(P<0.05, P<0.01). Compared with the normal group, the model group showed renal mesangial stromal hyperplasia, fibrous tissue hyperplasia and tubular vacuolar degeneration. Compared with the model group, the renal pathology of rats in Yishen Huashi granules group was improved to a certain extent. A total of 14 target metabolites and 96 target mRNAs were identified, the target metabolites were mainly enriched in 20 metabolic pathways, including sphingolipid metabolism, glycerophospholipid metabolism, and the biosynthesis of phenylalanine, tyrosine and tryptophan. The target mRNAs were enriched to obtain a total of 21 differential mRNAs involved in the TOP20 pathways closely related to glycolipid metabolism. A total of 6 pathways, glycerophospholipid metabolism, arachidonic acid metabolism, purine metabolism, primary bile acid biosynthesis, ascorbic acid and uronic acid metabolism, and galactose metabolism, were enriched by serum differential metabolites and renal differential mRNAs, among them, there were 7 differential metabolites such as phosphatidylethanolamine(PE) and 7 differential mRNAs such as recombinant adenylate cyclase 3(ADCY3). Seven differential metabolites had high predictive accuracy as verified by receiver operating characteristic(ROC) curve, and the results of Real-time PCR and Western blot were highly consistent with the sequencing results. ConclusionYishen Huashi granules can reduce UACR, BUN and other biochemical indexes, correct the disorder of glucose and lipid metabolism, and improve renal function of DKD rats. And its mechanism may be related to the regulation of the level of PE and other blood metabolites, and expression of Phospho1 and other mRNAs in the kidney, of which six pathways, including glycerophospholipid metabolism, may play an important role.
8.Automated Pre-delineation of CTV in Patients with Cervical Cancer Using Dense V-Net.

We use a dense and fully connected convolutional network with good feature learning in small samples, to automatically pre-deline CTV of cervical cancer patients based on CT images and evaluate the effect. The CT data of stage IB and IIA postoperative cervical cancer with similar delineation scope were selected to be used to evaluate the pre-sketching accuracy from three aspects:sketching similarity, sketching offset and sketching volume difference. It has been proved that the 8 most representative parameters are superior to those with single network and reported internationally before. Dense V-Net can accurately predict CTV pre-delineation of cervical cancer patients, which can be used clinically after simple modification by doctors.
