Objective To explore the pathogenic mechanism associated with insulin resistance(IR) and adiponetin(APN) abnormality in carotid atherosclerosis(CAS) patients complicated with nonalcoholic fatty liver disease(NAFLD),and to study their relationships with traditional Chinese medical(TCM) syndrome types.Methods Seventy-four CAS patients complicated with NAFLD(group A) and 25 CAS patients(group B) were enrolled into the study.Fasting blood glucose(FBG),fasting serum insulin(FI),and serum APN levels were measured.IR and APN were evaluated by the homeostasis assessment(HOMA) method.HOMA-IR and APN in group A were compared with those in group B,and bivariate correlation analysis was made between the two indexes in group A.The relationship of HOMA-IR and APN with TCM syndrome types was analyzed in group A.Results (1) HOMA-IR was obviously increased,and APN was obviously decreased in group A(P
2.Observation on curative effect of ozone combined with nifuratel nystatin vaginal soft capsules on the treatment of vaginitis

Objective To explore the clinical effect of ozone combined with nifuratel nystatin vaginal soft capsules on the treatment of vaginitis, and to provide the basis for clinical application. Methods 204 patients with vaginitis in our hospital from January 2013 to December 2016 were divided into the observation group (114 cases) and the control group (90 cases) according to the different treatment methods. The control group was treated with nifuratel nystatin vaginal soft capsules, and the observation group was treated with ozone combined with nifuratel nystatin vaginal soft capsules. The clinical efficacy, symptom relief time and adverse reaction were compared between the two groups. Results After treatment, the total effective rate in the observation was 97.37%, which was significantly higher than 85.56% in the control group(P<0.05). The rapid remission rate of the clinical symptoms in the observation group was significantly higher than that in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The effect is significant that ozone combined with nifuratel nystatin vaginal soft capsules is used on the treatment of vaginitis, which can quickly relieve symptoms, and will not significantly increase the adverse reactions, is worthy of application.
3.Impacts of the injection with flurphen mixture at Shenshu (BL 23) on hemodynamics and analgesia in patients with extracorporeal shock wave lithotripsy.

OBJECTIVETo compare the differences in pain reaction, hemodynamics and clinical efficacy between extracorporeal shock wave lithotripsy (ESWL) after injection with flurphen mixture (mixture of droperidol and fentanyl citrate) at Shenshu (BL 23) and simple ESWL in the patients.
METHODSSixty-four cases of urinary calculi with ESWL were randomized into an observation group and a control group, 32 cases in each one. In the observation group, 15 to 20 min before ESWL, flurphen mixture (droperidol injection 1.25 mg and fentanyl citrate injection 0.05 mg were diluted to 6 mL with 0.9% sodium chloride solution 4.5 mL) was injected at bilateral Shenshu (BL 23). In the control group, no any adjuvant therapy and medication were used before ESWL. The changes in blood pressure and heart rate, visual analogue scale (VAS) score, lithotripsy frequency till calculi complete removal and the rate of calculi complete removal after the first lithotripsy were observed in the two groups.
RESULTSIn the control group, blood pressure and heart rate were higher during lithotripsy than those before lithotripsy (both P<0.05). In the observation group, the differences in blood pressure and heart rate were not significant statistically as compared with those before lithotripsy (both P>0.05). The blood pressure and heart rate during lithotripsy in the observation group were apparently lower than those in the control group (both P<0.05). VAS scores during lithotripsy in the observation group were lower apparently than those in the control group (both P<0.05). The lithotripsy frequency in the observation group was less than that in the control group. The rate of calculi complete removal in 1 week after the first lithotripsy in the observation group was higher than that in the control group [75.0% (24/32) vs 50.0% (16/32), P<0.05].
CONCLUSIONThe flurphen mixture at Shenshu (BL 23) significantly alleviates pain reaction in patients undergoing ESWL, avoids the fluctuation of hemodynamics and improves the clinical effect of lithotripsy.

4.Quality Evaluation of Traditional Chinese Medicine by 1H-NMR-based Metabolomics

Traditional Chinese medicine (TCM) is an important part of modern medicine. Application of modern ana-lytical techniques to identify the authenticity of TCM and evaluate its quality is an important and critical content in the process of the modernization and internationalization of TCM. This paper described the development and charac-teristics of a 1H-NMR-based metabolomics technology, and reviewed its application in the species identification, quality evaluation of Daodi Y aocai (i.e., genuine medicinal materials), processing theory and the best harvest time of TCM. Besides, based on previous work, further discussion was given on technical methods and key question of the 1H-NMR metabolomics method. This paper provided a methodological reference for the species identification and quality evaluation of TCM and other herbal medicines.
5.Application of strong ion gap and other indicators in ICU

Objective To investigate the clinical application value of strong ion gap(SIG)generated by the third generation equa-tion following pH and the anion gap(AG)in critically ill patients.Methods On the basis of the determination results of blood gas and blood biochemistry,the equation in the Stewart-Figge methodology was adopted to calculate SIG.Results (1)There were sta-tistically significant differences in K+ ,Na+ ,HCO3 - ,AG,pH,PO4 3 - ,ALB,SIG and Cr between the critical illness groups and the control group(P <0.05).(2)The SIG mean value ratio the death group to non-death group was 3.04,the difference was the big-gest,P <0.05,AG and Cr took the second place.Conclusion SIG as a new indicator in combination with other biochemical and blood gas indicators,such as the HCO3 - ,AG,pH,PO4 3 - ,Cr and lactic acid can reflect the situation of the acid-base disorders in critically ill patients and plays a strong indicative role in predicting the disease development trends.

The framework of ancient Chinese medical literature clinical terminology classification standardization is one of the foundations of TCM language system. Its research will further promote and perfect TCM information standard system. We have adhered to the connection among ancient TCM classification framework, modern TCM and western medicine classification framework. Exploration on framework construction of ancient Chinese medical literature clinical terminology classification standardization was based on previous work of Chinese medicine clinical terminology classification and code standard, reference frame structure of SNOMED CT, research findings of classification standard framework of Chinese medicine clinical terms, andQian Jin Fang.
7.EUS for diagnosis of 9 esophageal tuberculosis patients:analysis and literature review

Objective To improve diagnosis on esophageal tuberculosis by the endoscopic ultrasonography (EUS).Methods A retrospective analysis of 9 cases of esophageal tuberculosis patients diagnosed by EUS or EUS guided ifne needle aspiration (EUS-FNA) who were treated from 2011 to 2016 and literature reviewed.Results The main presenting symptoms were dysphagia (6 cases), burning pain after sternum (3 cases), low-grade fever and night sweat (2 cases), cough and hemoptysis (1 case). The middle esophagus was involved in 7 cases, upper and lower third of the esophagus were affected in 1 cases. The endoscopic appearances included ulcer (1 case), mass lesion (8 cases). The diagnoses were conifrmed by biopsies with pathology in 5 cases. The layers of the esophageal wall were unclear or disappeared in some cases, and in some patients low echo occupying lesions inside or outside the esophageal wall were showed by EUS. The internal echo of the lesions was heterogeneous, and strong echo spots could be observed. In most cases, enlarged mediastinal lymph nodes were observed. EUS-FNA was used to take biopsy specimens in 2 cases: biopsy specimens suggested tuberculosis; and anti-TB treatment has good effect as well.Conclusion Dysphagia is the major clinical symptom of esophageal tuberculosis, always inlfuences the middle part of the esophagus. The commonest abnormalities identiifed by endoscopy are ulceration and mass lesion. The diagnostic rate of the disease can be improved by endoscopic examination with biopsies, EUS and EUSFNA data.
8.The expression and significance of SDF-1/CXCR4 biological axis in laryngeal squamous cell carcinoma and lymph node metastasize.

OBJECTIVE:
To study the correlation between the stromal cell-derived factor (SDF-1) and the receptor fusin (CXCR4) in carcinoma of larynx, and investigate some mechanisms of SDF-1/CXCR4 during the development, invasion and lymph node metastasis of laryngocarcinoma.
METHOD:
Detecting the expression of SDF-1 and CXCR4 by immunohistochemical method (SP) in laryngocarcinoma, paraneoplastic tissues, normal laryngeal mucosa and cervical lymph node. Using Kruskal-Wallis H test, chi2 test, Spearman rank correlation analysis and so on to do statistical analysis.
RESULT:
The positive expression rate of SDF-1 and CXCR4 in laryngocarcinoma was obviously higher than in paraneoplastic tissues and normal laryngeal mucosa tissues (P < 0.01). And the expression of two proteins was correlated with lymph node metastasis (P < 0.01), clinical stage (P < 0.01) and pathological grading of tumor (P < 0.05). The positive expression rate of SDF-1 and CXCR4 protein in metastasis lymph node tissue was higher than that in non metastasis lymph node tissue (P < 0.01). The expression of SDF-1 is correlated positively with the expression of CXCR4 in laryngocarcinoma.
CONCLUSION
SDF-1 and CXCR4 protein are highly expressed in laryngocarcinoma and in metastasis lymph node tissue. And they are correlated with lymph node metastasis, clinical stage and pathological grading of the tumor. According to the results, the two proteins may relate to infiltration and metastasis of laryngeal squamous cell carcinoma and play a role of synergistic action in the development and invasion of carcinoma of larynx.
