1.Preparation and controlled release of berberine magnetic gelatin drug microsphere

BACKGROUND: Research about magnetic drug microsphere has developed rapidly at home or abroad; additionally, previous studies have been focused on glucan, chitosan, and polyester as coating materials; however, gel material has been less reported,in particular gel-coating berberine microsphere has been not reported yet.OBJECTIVE: To prepare berberine magnetic gelatin drug microsphere, and to evaluate drug releasing, loading, and cross-linking using dynamic dialysis and light electron microscope.DESIGN, TIME AND SETTING: Preparation and property of microsphere were performed in Guizhou Yikang Pharmacy Co., Ltd.And Guizhou University from February 2004 to May 2006.MATERIALS: Gel, berberine, glutaraldehyde, liquid paraffin, and Fe_3O_4 were used in this study.METHODS: Magnetic microspheres of berberine drug with gelatin and Fe_3O_4 were prepared by a reverse phase suspension cold-condensation method according to biocompatibility of gel, solidification of glutaraldehyde, and disperse medium of liquid paraffin.MAIN OUTCOME MEASURES: Structure, morphology, drug-loading rate, entrapment efficiency, and in vitro releasing of drug microsphere.RESULTS: The diameter of the microsphere was under 20 urn. Scanning electron microscope demonstrated that Fe_3O_4 and berberine were compounded by gel into a round shape, and the mixture was expressed on micrometer. Ultraviolet absorption and infrared analysis suggested that berberine magnetic gelatin drug microsphere still had structure and property of raw drug. The drug-loading rate of berberine microsphere was 7.2%, the content of Fe_3O_4 was 20.8%, and the entrapment efficiency was 62.6%.the sustained-releasing ability of microsphere was well, and the releasing content counted for 83.5% of total drug within 7 hours.CONCLUSION: The berberine magnetic gelatin drug microsphere which was prepared by reverse phase suspension cold-condensation method characterizes by sample preparation, small diameter, regular morphology, and great sustained-releasing ability.
2.Tripterygium glycosides combined with Xiangdan injection in HSPN efficacy and influence of the clotting mechanism

Objective To observe effects of tripterygium glycosides combined with Xiangdan injection in henoch schonlein purpura nephritis (HSPN)and influence of the clotting mechanism,providing reference for clinical diagnosis and treatment.Methods 76 cases of children with allergic purpura nephritis were randomly divided into control group(n=38)and observation group(n=38)according to the random number table.Control group was given Xiangdan injection treatment.Observation group was given Xiangdan injection treatment combined with tripterygium glycoside.12 weeks was considered as a course of treatment. Results After treatment,the total efficiency rate in treatment group(94.74%)was significantly higher than control group(68.42%),which was statistically different(P <0.05).Before treatment,prothrombin time(PT),fibrinogen(FIB),activated partial thromboplastin time(APPT),platelet(PLT)levels were not significantly different between two groups;FIB and PLT levels in the control group were significantly lower than before treatment(P<0.05 ),PT and APPT had no statistically significant difference before and after treatment.After treatment in observation group,FIB and PLT levels were significantly lower than before treatment(P<0.05 ),PT level was significantly higher compared with before treatment(P<0.05 ),APTT level had no difference before and after treatment.After treatment,PT in treatment group was significantly higher than the control group(P<0.05 ),PLT in treatment group were significantly lower than the control group (P <0.05 ),FIB and APTT had no statistical difference.There was no adverse reaction in two groups after treatment.Conclusion Tripterygium glycosides combined with Xiangdan injection treatment for HSPN has significant effect,which affect the children clotting mechanism to improve the PT,FIB,PLT levels.It is safe and valuable in clinical research.
3.MRI appearances of postpartum posterior reversible encephalopathy syndrome

Objective To investigate the MRI appearances of postpartum posterior reversible encephalopathy syndrome (PRES). Methods The clinical and radiological data of 8 patients with postpartum PRES were analyzed retrospectively, 5 cases were 1 week after postpartum,2 cases were 2 weeks afterpostpartum, 1 case was 4 weeks after postpartum. All cases underwent MRI examination, checking sequence included T,WI,T2WI,FLAIR,DWI,SWI, and 4 of them underwent MR venography (MRV). Results MRI showed basal bilaterally symmetrical distributions of lesions, mainly located in subcortex and cortex of cerebral hemisphere and predominantly in the occipital lobe,the lesions appeared as hyperintensity on FLAIR and T2WI,isointensity or mildly hypointensity on T1WI,hyperintensity on DWI. MRI in 8 patients demonstrated multiple lesions located in bilateral parietal lobe (8 cases),bilateral occipital lobe (8 cases), bilateral frontal lobe ( 4 cases ), bilateral temporal lobe (6 cases ), bilateral tempura-occipital lobe ( 8 cases),splenium of corpus callosum(l case), bilateral basal ganglia (l case), bilateral oval center( 1 case), bilateral cerebellar hemisphere (1 case), left frontal lobe hemorrhage (l case). Five patients' follow-up scan showed decreased or disappeared abnormal signals. Conclusion MRI examination scan identify the location, quality and degree, meanwhile, analyze the pathological foundation of postpartum PRES, which conduces to guiding treatment and preventing complication.
4.Application of combination of various endoscopic techniques in treatment of bile duct stones

Bile duct stones are a common disease in clinical practice in China,and due to a high incidence rate of complications and complex causes,its clinical diagnosis and treatment has become a difficult problem which needs to solved urgently.With the development of science and technology and the enhanced knowledge of this disease,many treatment methods for this disease have emerged.This article summarizes the successful experience in the treatment of bile duct stones in the Center for Integrated Traditional Chinese and Western Medicine Therapy for Biliary and Pancreatic Diseases in our hospital.With reference to the type and distribution characteristics of bile duct stones,a combination of various endoscopic techniques,such as laparoscopy,choledochoscopy,and duodenoscopy,can achieve the goal of minimally invasive,diverse,and individualized diagnosis and treatment and provide feasible thoughts and strategies for the treatment of bile duct stones.
5.Correlation between traditional Chinese medicine syndromes and omics: a literature review.

As the post-genome era is approaching, omics has become a hot topic in the research field of life sciences and is also widely used in traditional Chinese medicine (TCM) research. For discussing the correlation between syndromes and omics, the authors explored the application of genomics, proteomics and metabonomics in the study of syndrome classification from the macroscopic view, after a systematic and normative study of the literature. The authors drew the conclusion that research into the correlation between syndromes and omics has great clinical significance in terms of the scientific application and quantification of TCM treatment based on syndrome differentiation.
6.EEG AND CLINICAL MANIFESTATION OF NERVOUS SYSTEM IMPAIRMENT BY PULMONARY TREMATODIASIS

The clinical manifestation and EEG of 20 cas-es of the patients with damage to nervous systemby pulmonary trematoaiasis are analyzed in the ar-ticle. Those cases made up 30. 3% of the totalnumber of the patients suffered form pulmonarytrematodiasis treated during the same period of thetime. Their clinical signs and symptoms, apartfrom those of Lungs, of skin, and of muscule,were headache, hemiplegia, aphasia, epilepsy,haemorrage in the subarchnoid space and impair-ment of multiple cranial nerves. besides, therewas also a case with damage to the spinal cord. Cases with abnormal EEG constituted 95. 4%of the patients with brain damage. The degree ofthe EEG abnomality varied from case to case ac-cording to the severity and extent of the lesions inpatient brains. Generally speaking, the abnormali-ty of EEG in the cases with cerebral impairmentwas more severe than that in the cases with dam-age only of meningeal. In the cases with circum-scribed granuloma formation the abnormal EEG al-so showed localization. Just for those reasons, authors believe that acontineous check up of EEG is more helpful in pre-dicting the involvement of lesion in the brain, theseverity of the damage and even the outcome of thedisease.

8.Long-term follow-up and analysis of high tibialosteotomy in the treatment of varus knee osteoarthritis

[Objective]To evaluate long-term therapeutic effect of high tibial osteotomy HTO in the treatment of varus knee osteoarthritis.[Method]Eighteen males(twenty four knees)and eighty four females(ninety one knees)were accept HTO,patients were from thirty-five to sixty-five years,with average age 52.32(35~65)years.[Result]Ninety one knees were followed up from 10 years and 2 months to 21 years and 9 months,average 15 years and 1 month.HSS criterionwas used as evaluation standard,the ratio of excellent and good post operation 0.5 year,1 year,2,5,10,15 years were 98%,98%,98%,96%,78%,57%.[Conclusion]The therapeutic effect of HTO is good in the first 15 years,but it obviously degrade after 15 years,and HOA can work against time for prolongation of period before the possible later total knee replacement.
9.A survey of correlated ratio in maxillary anterior teeth smiling

Objective To survey aesthetics cognition for smiling beauty in young people smile. Methods Smile model was established. The middle incisor width/length ratio, lateral incisor and middle incisor width ratio, difference of lateral incisor and the middle incisor gingival level, gingiva exposure, buccal corridor width and smile line radian were used as variable values to change respectively. A total of 200 young people were selected to evaluate results. The differences in index of ideal value and acceptable range between different dender groups were calculated. Results The ideal value of middle incisor width/length ratio was 0.75, and acceptable range was 0.65-0.85. The ideal value of lateral incisor and middle incisor width ratio was 0.618, and acceptable range was 0.518-0.718. The ideal value of lateral incisor and the middle incisor gingival level difference was-0.5 mm, and the acceptable range was-1-0 mm. The ideal value of gingiva exposure was 0 mm, and the acceptable range was 0-2 mm. The ideal value of buccal corridor was 0.09, and the acceptable range was 0.05-0.21. Coordinate smile was ideal smile line (value=1), and the acceptable range was 0.5-1. There were no statistically significant differences in smile esthetics of six ideal value indicators and acceptable ranges between different gender groups. Conclusion The ideal values and acceptable ranges of six indexes of quantitative criteria can be used for clinical treatment.
10.Application of endoscopic pancreatic duct exploration in precise pancreatectomy for intraductal papillary mucinous neoplasms

The diagnostic rate of intraductal papillary mucinous neoplasms (IPMNs) has been increased as the improvement of imaging and endoscopic techniques.All main duct type and mixed variant IPMNs should be resected as long as the patient is a good surgical candidate with reasonable life expectancy.Evaluation of resection margin ( especially circumferential margins) is very important to prevent the recurrence of IPMNs,and intraoperative pancreatoscopy plays an important role in determining the appropriate size for pancreatic resection.In this article,the experience in the treatment of 1 patient with IPMNs by precise pancreatectomy guided by endoscopy-assisted intraoperative pancreatic duct inspection was introducted.
