BACKGROUNDThe bacterial composition of periapical lesions in deciduous teeth has not been well documented. This study was designed to explore the bacterial compositions, especially the dominant bacteria in periapical lesions using 16S rRNA sequencing.

METHODSTissue samples were collected from 11 periapical lesions in deciduous teeth with primary endodontic infections. DNA was extracted from each sample and analyzed using 16S rRNA cloning and sequencing for the identification of bacteria.

RESULTSAll DNA samples were positive for 16S rRNA gene PCR. One hundred and fifty-one phylotypes from 810 clones were identified to eight phyla, and each sample contained an average of 25.9 phylotypes. In addition, 59 phylotypes were detected in more than two samples, and Fusobacterium (F.) nucleatum (8/11), Dialister (D.) invisus (8/11), Campylobacter (C.) gracilis (7/11), Escherichia (E.) coli DH1 (6/11), Aggregatibacter (A.) segnis (6/11), and Streptococcus (S.) mitis (6/11) were the most prevalent species. Furthermore, 45 as-yet-uncultivated phylotypes were also identified.

CONCLUSIONSChronic periapical lesions in deciduous teeth contained polymicrobial infections. F. nucleatum, D. invisus, C. gracilis, E. coli DH1, A. segnis, and S. mitis were the most prevalent species detected by 16S rRNA sequencing.

OBJECTIVETo evaluate the effects of Shengli capsules on the sexual ability of normal and castrated male rats.

METHODSShengli capsules were given by intragastric administration to 100 experimental male rats at different doses of 0.35, 0.70 and 1.40 g / kg. Data were collected and analyzed, including capture latency period, times of capture, sexual endurance and times of ejaculation, to assess the effects of Shengli capsules on the sexual ability of the rats. The Castrated Animal Impotence Model was employed to determine the erectile latency period and the function parameters of the preputial gland, seminal vesicle and prostate, so as to test the effects of Shengli on the development of the rats'sexual organs.

RESULTSShengli was proved to be effective in shortening copulation latency in the dose groups of 0.35, 0.70 and 1.40 g / kg (P < 0.01), increasing significantly the frequency of capture in the high- and low-dose groups of 0.35 and 1.40 g / kg (P < 0.05), reducing the latency period to erection in the low-dose group of 0.35 g / kg, and blocking the shrink of the seminal vesicle and prostate in the medium-dose group of 0.70 g / kg.

CONCLUSIONShengli is significantly effective in enhancing the sexual ability of male rats: it can boost libido, increase erection frequency and improve sexual performance. However, further studies have yet to be done on its action mechanisms.

Objective To evaluate the complications and factors associated with titanium plates in mandibular reconstruction with different methods.Methods A retrospective analysis of 660 patients who received primary reconstruction of the mandible was conducted.The characters of the mandibular defect and methods of reconstruction were recorded.The complications and factors associated with the reconstruction were analyzed.Results The majority of mandibular defect was in a shape of L (n =324).The total complication rate was 14.7％ (97/660),which included screw loosening 5.8％ (38/660),plate fracture 3.3％ (22/660),plate exposure 3.8％ (25/660),infection 7.4％ (49/660) and malunion or ununion 2.9％ (19/660).The mandibular defects reconstructed by reconstructive plates had a higher complication rate than that by bone grafts.The complication rate was 10.8％ (63/586)in cases reconstructed by bone grafts with miniplates.The complications were associated with radiation therapy and diabetes.Radiation therapy had a significant effect on plate exposure.Conclusions Reconstruction with bone grafts can minimize the rate of plate related complication.The complications were associated with radiation therapy and diabetes.

Objective To observe the status of occuh hepatitis B virus infection in chronic viral hepatitis patients with non-A to E hepatitis virus infection and explore the diagnostic value of fluorescence quantitative polymerase chain reaction (FQ-PCR) technique for occult hepatitis B virus infection. Methods The amount of HBV-DNA in serum and liver tissue from 57 patients with non-A to E hepatitis virus infection who were diagnosed as chronic viral hepatitis by Menghini method liver biopsy were detected by using FQ-PCR technique,then the relation between the viral load of HBV DNA in liver tissue and hepatic inflammatory activity were analyzed.Results Thirteen (22.81% ),22 (38.60%) patients were positive for HBV DNA in serum and liver tissue,respectively.The positive rate and the level of HBV DNA quantity in liver tissue were significantly higher than those in serum;HBV DNA was found positive in both serum and liver tissue in 13 cases,negative in both serum and liver tissue in 35,positive in liver tissue but negative in serum in 9,and in none of the cases HBV DNA was positive in serum but negative in liver tissue ( P<0.01 ).The logarithmic value of HBV DNA from 13 patients in liver tissue and in serum was respectively:(6.62±1.21 ) copies/g vs.(4.03±1.06) copies/ml,P < 0.01.The hepatic lesions of all HBV DNA positive patients were active pathologic changes,but the level of HBV DNA in liver tissue was not significantly correlated with the grade of hepatic inflammation activity ( P0.05 ).Conclusion Occult HBV infection is the etiology of part of the chronic viral hepatitis patients with non-A-E hepatitis virus infection.Missed diagnosis will occur if diagnosis of hepatitis B is only hased on detection of serum HBV markers. It is useful for improvement of the diagnostic level of HBV infection via detection of HBV DNA quantitatively in serum especially in liver tissue of chronic viral hepatitis with non-A-E hepatitis virus infection by using FQ-PCR technique The chronic viral hepatitis patients occult HBV infection should be also given effective anti-viral therapy.

Objective To demonstrate various operative approaches to the removal of the tumors involving the anterior skull base and explore the effectiveness of the pericraniai flap in reconstructing the anterior skull base. Methods The clinical data of 47 patients with anterior skull-based tumors were retrospectively reviewed and analyzed: subffontal approach, extended subfrontal approach and combined craniofacial approaches were performed in 7, 8 and 32 patients, respectively; pericranial flap and galeo-pericranial flap were employed in the reconstructing the anterior skull base. Results Tumors were radically resected in 33, subtotally resected in 12 and partly resected in 2 patients with 21.2% overall complication rate. Conclusion The 3 approaches are highly effective in the removal of anterior skull-based tumors. The pericranial flap or galeo-pericranial flap was reliably and effectively used in the reconstructing anterior skull base with minimally complication rate and excellent endurance of postoperative radiotherapy.

BACKGROUNDThe aim of this study was to investigate the potential relationship between the dynamic expression of Toll-like receptor 2 and 4 (TLR2/4) in peripheral blood mononuclear cells as well as changes in serum concentration of inflammatory factors and acute lung injury (ALI) in patients after orthotopic liver transplantation (OLT).

METHODSThe peripheral blood samples of 27 patients (23 men and 4 women with ASA III to IV) who received OLT were collected for measurement of TLR2/4 at T1 (after induction of anesthesia), T2 (25 minutes after anhepatic phase), T3 (3 hours after graft reperfusion) and T4 (24 hours after graft reperfusion). The expression of TLR2/4 in mononuclear cells was measured by flow cytometry. The serum concentrations of tumor necrosis factor (TNF)-alpha, interleukin (IL)-1beta and IL-8 were measured by enzyme-linked immunosorbent assay (ELISA). Twenty-seven patients were assigned to ALI group (n = 9) and non-ALI group (n = 18) according to the diagnostic criteria of ALI. The expression of TLR2/4 in the ALI group or non-ALI group was analyzed.

RESULTSCompared to the non-ALI group, the volumes of blood loss, ascites, total output and transfused red blood cells were higher in the ALI group, and the anhepatic phase lasted longer (P < 0.05, P < 0.01). The expression of TLR2/4 in mononuclear cells increased significantly at T3 and T4, and serum concentrations of TNF-alpha, IL-1beta and IL-8 increased significantly too. There was no significant difference in Child-Turcotte-Pugh (CTP) scores between the ALI group and non-ALI group (P > 0.05). The expression of TLR2/4 in mononuclear cells increased significantly at T3 and T4 in the ALI group (P < 0.05, P < 0.01). A positive correlation was noted between the expression of TLR4 in mononuclear cells and the serum concentrations of TNF-alpha, IL-1beta (P = 0.041, P = 0.046) in the ALI group. In the non-ALI group, statistical results showed that the expression level of TLR2/4 in mononuclear cells was not significantly different during the peri-operative period of OLT (besides TLR4 expression at T4). Compared to the non-ALI group, the increasing amplitude of TLR2/4 expression in mononuclear cells was more significant in the ALI group. The patients whose TLR2/4 expression in mononuclear cells exceeded that at T1 by one time were more likely to suffer from ALI (P = 0.013), with a relative risk of 16.

CONCLUSIONThe expression level of TLR2/4 in mononuclear cells increases significantly in the peri-operative period of OLT, and it may be a high risk factor for occurrence of postoperative ALI.

BACKGROUNDRecent studies have revealed that lung cell apoptosis plays an important role in pathogenesis of cigarette-induced chronic obstructive pulmonary disease (COPD). Tumor necrosis factor alpha (TNF-alpha) is one of the most important cytokines which are involved in COPD. This study aimed at investigating the influence of its inhibitor, recombinant human necrosis factor-alpha receptor II:IgG Fc fusion protein (rhTNFR:Fc) on alveolar septal cell apoptosis in passive smoking rats.

METHODSForty-eight rats were randomly divided into a normal control group, a passive smoking group, an rhTNFR:Fc intervention group and a sham intervention group. The passive smoking rats were treated by exposure to cigarette smoking daily for 80 days. After smoking for one month the rhTNFR:Fc intervention group was treated with rhTNFR:Fc by subcutaneous injection, the sham intervention group injected subcutaneously with a neutral preparation (normal saline 0.1 ml, manicol 0.8 ml, cane sugar 0.2 mg, Tris 0.024 mg as a control. Lung function was determined and the levels of TNF-alpha in serum and broncho-alveolar lavage fluid (BALF) were measured with enzyme-linked immunosorbnent assay (ELISA). Lung tissue sections stained by hematoxylin and eosin (HE) were observed for study of morphological alternations. Mean linear intercept (MLI) and mean alveolar numbers (MAN) were measured and the terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) method was carried out to determine the percentage of positive cells and distribution of apoptotic cells.

RESULTSIncreased MLI and decreased MAN were found in the passive smoking group compared with both the normal control group and the rhTNFR:Fc intervention group (P < 0.05). Forced expiratory volume in 0.3 second (FEV 0.3)/forced vital capacity (FVC) and peak expiratory flow (PEF) were lower in the passive smoking group than that in the normal control group (P < 0.05). Compared with the sham intervention group, FEV 0.3/FVC and PEF increased in the rhTNFR:Fc intervention group (P < 0.05). The levels of TNF-alpha in serum were higher in the passive smoking group than that in the normal control group (P < 0.05) and rhTNFR:Fc intervention group (P < 0.05). Significant differences were found between the levels of TNF-alpha in the serum of the rhTNFR:Fc intervention group and sham intervention group (P < 0.05). The levels of TNF-alpha in BALF were higher in the passive smoking group than that in the normal control group (P < 0.05), but no significant differences of TNF-alpha levels in BALF were found between the passive smoking group and rhTNFR:Fc intervention group. The number of TUNEL positive cells in alveolar septa was significantly increased in the passive smoking group as compared with the normal control group and the rhTNFR:Fc intervention group (P < 0.05).

CONCLUSIONThis study provides preliminary evidence that rhTNFR:Fc may interfere with TNF-alpha and reduce alveolar septal apoptosis in smoking rats.

OBJECTIVETo describe the prevalence of anemia and yearly trends (1993-2003) among women who came to the hospitals or maternal and child health units for premarital examinations in 6 counties of Jiangsu and Zhejiang provinces.

METHODSData were obtained from the records of the premarital examinations in perinatal health care surveillance system that had been established since 1992 in these areas. We reviewed hemoglobin levels of those women who were enrolled in the perinatal health care surveillance system from 1993 to 2003. Anemia was defined according to the WHO (2001) criterion. We calculated the prevalence of anemia and analyzed the yearly trends based on the data of hemoglobin concentration.

RESULTSIn the period of 1993-2003, there were 82 995 anemia cases identified among 193,434 women with an overall anemia rate as 42.9%. The rates of anemia were high (65.5%) in 1993 but low (25.8%) in 2003. 99.7% of the anemic women whose hemoglobin concentration were between 80-119.9 g/L. Time trend analysis indicated a significant decline on anemia rate while monthly analysis showed that the prevalence rates were high (48.2%) in September and low in March (39.5%). The results also showed that the prevalence rates of anemia were relatively higher in farmers and workers in rural enterprises, and lower in Han ethnicity than minorities. The higher prevalence rates of anemia were presented among the women with less education, lower body mass index, or at older age.

CONCLUSIONFor those premarital women in 6 counties of Jiangsu and Zhejiang provinces, the overall anemic rate presented a significant downward trend between 1993 and 2003 while the prevalence of anemia remained high, especially for the women with less education, lower body mass index or older ages.

Model informed precision dosing for warfarin is to provide individualized dosing by integrating information related to patient characteristics, disease status and pharmacokinetics /pharmacodynamics of warfarin, through mathematical modeling and simulation techniques based on the quantitative pharmacology. Compared with empirical dosing, it can improve the safety, effectiveness, economy, and adherence of pharmacotherapy of warfarin. This consensus report describes the commonly used modeling and simulation techniques for warfarin, their application in developing and adjusting dosing regimens, medication adherence and economy. Moreover, this consensus also elaborates the detailed procedures for the implementation in the warfarin pharmacy service pathway to facilitate the development and application of model informed precision dosing for warfarin.

BACKGROUND: Acetabulum anterior column shape is complex, and closed to femoral artery, and femoral nerve. Fixed screws easily went into the acetabulum. Currently, there was less data on quantitative anatomy data of pelvic acetabulum fracture with anterior column plate internal fixation. OBJECTIVE: To compare the pelvis gender differences by analyzing digital anatomical features of normal adult acetabulum anterior column section through digital three-dimensional reconstruction and measurement. METHODS: Totally 30 normal adults (half male and half female) received pelvic CT scans, and data were obtained. Using Materialise Mimics Innovation Suite 16.0 software, the boundary of the pelvis was used as reference line for sectioning. The obturator groove, iliopubic eminence, anterior inferior iliac spine, and the anterior superior iliac spine were used as a reference mark. In 15 males and 15 females (30 sides), the corresponding boundary line from obturator groove to the anterior superior iliac spine of acetabular anterior column was sliced into 5 mm-thick sections. The tangent line was vertical to the boundary line. The tangent plane was vertical to the upper plane of the anterior column. The angle and length of each section 5, 10, 15 mm points from the boundary line to the acetabulum, and the perpendicular distance from anterior and posterior edges of the acetabulum to anterior inferior iliac spine, iliopubic eminence and the pubic tubercle. RESULTS AND CONCLUSION: (1) Acetabular anterior column from the obturator groove to the anterior superior iliac spine section was not significantly different (P > 0.05). (2) No matter in males or females, the tangent angle of the fifth layer section was minimum, and the tangent length of the sixth layer section was longest. The length and angle of the second slice at 5 mm point were not significantly different between males and females. However, above indexes in others were significantly larger in females than in males (P < 0.05). (3) Results indicated that in different fracture ranges and different fixation plate positions, the angle and length of pedicle screws are dynamic, so we only selected in accordance with above range, but cannot fix in a certain value. The design of the most accurate and effective placement angle and length should be aimed at nailing design parameters for each individual patient. The use of Mimics software can be used for three-dimensional reconstruction of CT data of adult acetabular anterior column fracture, and can measure the number of indexes, and provide a theoretical reference for the clinical diagnosis and treatment of acetabular anterior column fracture.
