Objective To discuss the characteristics and management of pregnancy on cesarean section scar. Methods Five women with pregnancy on cesarean section scar were reviewed from Apr. 2000 to Mar. 2003. Results All of the five patients had cesarean section before and suffered from painless,irregular vaginal bleeding after 6～8 weeks of amenorrhea. The pregnancy could be misdiagnosed as invasive mole or cervical pregnancy. All had better outcome by uterine suction under ultrasound monitoring without massive bleeding or hysterectomy. Conclusions Women who had cesarean section before are in the risk of pregnancy on the old scar. Clear diagnosis should be made before any treatment. Ultrasound monitoring,local MTX injection and packing can be utilized to prevent perforation during suction. Patients should be monitored continuously after operation with serum hCG level and ultrasound. The principle prevention of pregnancy on cesarean section scar is to reduce the cesarean section rate and effective contraception.

Objective To discuss the effect and related mechanism on epithelial?mesenchymal transition(EMT)process and invasive ability of the trophoblasts JEG?3 by targeted silencing of Notch?1 gene. Methods Notch?1?siRNA interference fragment was designed and synthesized to sta?bly transfect JEG?3 cells. Changes in the invasive ability of trophoblasts were detected by Transwell chamber invasion assay. The expressions of Notch?1 ,E?cadherin ,vimentin and EMT transcriptive regulators slug ,snail ,twist were analyzed in transfected trophoblasts by Western blot. Results The relative expression of Notch?1 protein in JEG?3 and the invasive ability of JEG?3 were significantly decreased after transfection with Notch?1?siRNA,the expression of the epithelial indictor E?cadherin was significantly increased while the mesenchymal indicator vimentin was de?creased with the reduction of EMT transcriptive regulators slug,snail and twist. Conclusion The invasion of trophoblasts was affected by targeted si?lencing of Notch?1 gene and Notch?1 can reduce the invasion of trophoblasts through influencing EMT process.

BACKGROUND: It is proved that nerve regeneration induced by terminolateral neurorrhaphy(TLN) is not as active as that induced by end-to-end suture. Exogenous epidermal growth factor(EGF) increases the opportunity of neuron survival in vitro and promotes nerve regeneration. Whether it can increase nerve regeneration after terminolateral neurorrhaphy deserves further study.OBJECTIVE: To evaluate the effect of exogenous EGF in promoting nerve regeneration after terminolateral neurorrhaphy.DESIGN: A randomized controlled trial.SETTING: Orthopedic Institute of Chinese PLA General Hospital.PARTICIPANTS: The trial was conducted in the Orthopedic Institute of Chinese PLA General Hospital from September 2001 to February 2002. A total of 32 male Wistar rats, weighting 200- 250 g, were randomized to control group and EGF group with 16 rats in each group.METHODS: The right peroneal nerve was transected and an epineural window of 1 mm was created on the neighboring tibial nerve. The distal end of the transected peroneal nerve was sutured to the windowed tibial nerve by means of end-to-side attachment. Each rat in EGF group received injection of 0. 1 mL/d EGF diluted with normal saline at 2 g/L for two weeks while each in control group received injection of normal saline (0. 1 mL/d) at the distal site of the transected peroneal nerve for two weeks. Histological, morphological and electrophysiological examinations were performed 4 and 8 weeks after operation.MAIN OUTCOME MEASURES: The regeneration rate of myelinated nerve, motor nerve conduction velocity and ultrastructural changes of the two groups.rate of myelinated nerve fibers: 4 and 8 weeks after operation, it was better in EGF group[ (52.42 ± 1.45)% and(61.41 ± 1.54)% ] than that in control nerve conduction velocity: 4 and 8 weeks after operation it was obviously greater in EGF group[ (30. 33 ±0. 88)m/s and(34. 36 ± 1.09)m/s] than that in conObservation of ultrastructure: The number of myelinated nerve fibers, and the thickness and maturation degree of myelin sheath were significantly better than those in control group.CONCLUSION: Exogenous EGF can promote nerve regeneration, increase nerve conduction velocity after terminolateral neurorrhaphy.

Objective: To investigate the anticancer action of taurine and its potential mechanisms. Methods: 101 weanling female SD rats were randomly divided into four groups. They were:(1)control group (CL):fed a basic diet;(2)high fat group(HF):fed a high fat diet (10% lard);(3)taurine group (Tau):fed a taurine diet (5% taurine);(4)taurine and high fat group(TH):fed a mixed diet (5%taurine and 10% lard). 10mg DMBA were given to each rat by gavage at age of 6 weeks. Then the rats were fed assigned diet for 26 weeks. By 5 weeks after DMBA administration the rats were palpated once weekly and the time, size, location of each tumor were recorded. Objective To explore the operation procudure and notice of establishing a rat model of abdominal heterotopic heart transplantation by single operator,and summarize the detailed skill and experience for beginners.Methods 68 pairs male SD(recipients)/Wistar(donors) rats,preoperative anesthesia for recipients and donors at the same time,8/0 line was preparing for blocking recipients vena cava and abdominal aortic,blocking the branch vessel.Left and right superior vena ligature respectively with hilar,cutting the ascending aorta,perfusion cardioplegia,free pulmonary artery,across transverse sinus of pericardium for pulmonary artery cutting,atrial wash,free donor heart.Choosed appropriate incision of receptor abdominal blood vessels,mattress suture at 6 and 12 points,a single suture for vent gases,continuous suture in artery,single needle in vein,reperfusion after exhaust gas.Recording operation time,HE staining,IL-1β/CD3 immunohistochemistry slice,flow cytometry analysis of CD4+T/CD8+T lymphocytes subsets in the peripheral blood.Results Sixty-eight cases were treated by single operater,the operation time was(58.8±4.5)minutes,artery suture time was(7.6±2.2)minutes,vein suture time was(13.5±4.2)minutes,the total donor heart ischemic time was(31.8±4.5)minutes,the success rate was 88.2％.The rejection reaction was stronger on the third and fifth day after surgery,with high expression of IL-1β/CD3 in cardiac allografts.The CD4+T/CD8+T lymphocytes subsets increased in the peripheral blood at first day after heart transplantation.Conclusion By fully preparation and skillfully operation,establishing a rat model of abdominal heterotopic heart transplantation by single operator has a stable success rate.

Objective:To investigate the iodine content distribution of drinking water in Jiangsu Province.Methods:In 2017, the iodine content in drinking water of residents in townships (streets, hereinafter referred to as townships) of Jiangsu Province was investigated. Administrative villages (neighborhood committees, hereinafter referred to as administrative villages) of Jiangsu Province were selected as units for the investigation of townships with the median water iodine greater than 10 μg/L. Delimitation criteria: the median iodine content of water < 40 μg/L was iodine deficiency area, 40-100 μg/L was suitable iodine area, and > 100 μg/L was high iodine area.Results:A total of 1 285 townships were surveyed in 101 counties (cities, districts) in 13 cities, and the median water iodine in townships was 7.28 μg/L. Among them, there were 875 townships with median water iodine < 10 μg/L, accounting for 68.09%; 315 townships 10-< 40 μg/L, accounting for 24.51%; 56 townships 40-100 μg/L, accounting for 4.36%; 39 townships > 100 μg/L, accounting for 3.04%. The water iodine survey at the administrative village level was conducted in 5 864 administrative villages in 410 townships in 6 cities. Among them, 769 administrative villages with median water iodine < 10 μg/L, accounting for 13.11%; 3 558 administrative villages 10 - < 40 μg/L, accounting for 60.68%; 613 administrative villages 40 - 100 μg/L, accounting for 10.45%; and 924 administrative villages > 100 μg/L, accounting for 15.76%.Conclusion:According to the water iodine, there are iodine deficiency, suitable iodine and high iodine areas in Jiangsu Province, mainly in iodine deficiency areas.
