Chinese Journal of Practical Nursing 2021;37(3):174-180
Objective:To establish nursing quality indicator system for freestanding hemodialysis facilities and identify the weight values of indicators. Methods:With the structure-process-outcome quality model as the framework, the literature analysis method, field survey and semi-structure interview were used to form the indicators item pool. Through Delphi method, experts were consulted for two rounds. The indicator weight was calculated by AHP.Results:The indicator system of this study included 3 primary indicators, 15 secondary indicators and 46 tertiary indicators. Among the primary indicators, the process quality indicator had the largest weight (0.493), followed by the outcome quality indicator (0.310), and the structure quality indicator had the smallest weight (0.196).The process quality were the main evaluation contents. According to the indicator weight, the main manpower allocation (0.131) in secondary indicators had the highest weight, and the proportion of nurses with different hemodialysis work experiences (0.108) in the tertiary indicators had the highest weight. Conclusion:The indicator system of this study is scientific and integrated, which can be used as a nursing quality evaluation tool to verify its practicability in clinical nursing practice.

Objective To guarantee the quality of measurenents with the radon-thoron discriminative detectors of our laboratory. Methods LD-P radon-thoron discriminative detector participated in the international intercomparison for integrating radon/thoron detectors organized by National Institute of Radiological Science (NIRS,Japan).Detectors were sent to NIRS for exposure.Radon intercomparison was conducted with radon chamber providing three levels of exposure:low, medium and high levels.Thoron intercomparison was carried out at thoron chamber, which also provided three levels of exposure:low, medium and high levels.Detectors were posted back to our laboratory for etching and analysis after exposure. Then the measured values were submitted to NIRS.Finally the reference values were informed of us.Results The relative percent difference (RPD) between the measured value and the reference value for radon was-13.8%,-14.4% and-17.1% at low, medium and high levels respectively, and that of thoron were-14.4%,8.9% and-3.2% at three levels respectively. Conclusions Both radon and thoron measurement of our detectors rank asCategory Ⅰ in the 4th international intercomparisons for integrating radon/thoron detectors with the NIRS radon/thoron chambers.

OBJECTIVE To study the rate and the risk factors of hospital infection after hemopoietic stem cell transplantation and improve treatment strategy.METHODS The clinical data of 21 cases of hemopoietic stem cell transplantation were analyzed respectively in our hospital.RESULTS Hospital infection was found in 15 cases, the infection rate was 71.4%.Fifteen kinds of bacteria and 4 kinds of fungi were observed. The risk factors were aggressive operation, the abuse of glucocorticoid and antibiotics.CONCLUSIONS The patients have a high rate of hospital infection after hemopoietic stem cell transplantation and a poor prognosis because of hospital infection. To decrease the rate of hospital infection after hemopoietic stem cell transplantation, the whole environmental protection should be carried out except decreased aggressive operation and the correct use of glucocorticoid and antibiotics.

Clinical Medicine of China 2008;24(8):755-756
Objective To retrospectively study the methods and effects of interventional therapy in primary hepatic carcinoma complicated with arteriovenous fistula. Methods Interventional chemotherapy and embolization was applied for treatment of 46 cases with primary hepatic eereinoma complicated with arteriovenous fistula. Postoperative clinical changes were observed. Results All 46 cases were successfully treated. No severe complication was found. Total effective rate was 95.65%. 1-year survival rate was 75.09%. Conclusion Interventional treatment is safe and effective in treating primary hepatocellular carcinoma complicated with hepatic arteriovenous fistula.

Objective To study the ways of auto swivel reposition under Barium meal for treating stomach volvulus and its clincial value. Methods The patients with stomach volvulus confirmed by gastroenterography underwent reposition according to the volvulus and the effect was observed. The cause was examined. Results 100 cases underwent treatment of auto swivel reposition under Barium meal, among whom 98 cases were successfully repositioned and 51 cases were known for the cause. Conclusion Auto swivel reposition under Barium meal for treating stomach volvulus and its clincial value is simple and non-invasive and acceptable, being of great value.

Objective To establish an improved model of exercise-induced glycometabolism in type II diabetic rats, and to provide a theoretical reference for the establishment of exercise prescription for type II diabetes. Methods Forty-five 8-week old SPF male Wistar rats were used in this study.Of which 32 were fed with high-fat diet for 7 weeks, and intraperitoneal injection of 30 mg/kg STZ was given to establish the rat model of type II diabetes. The normal rats and successful model rats were divided into four groups:The normal control group (C group),normal exercise group (CE group),diabetic group (DM group) and diabetic exercise group (DME group). The exercise group was assigned by the Ploug training protocol,6 days/week,60 min/day,for a total of 8 weeks. After the high fat diet fed for 7 weeks, blood sample was taken from the tail vein,FBG and serum insulin were detected after baseline and 8 weeks exercise, and blood sample was collected from the tail vein to determine the FBG.Serum insulin (FINS) was detected by orbital blood sampling at the end of 8 weeks of exercise, and HOMA-IR was calculated. Results 1.After 7 weeks of high fat diet, compared with the groups C and CE,the levels of FBG,FINS and HOMA-IR were significantly higher in the DM and DME groups.2.After 8 weeks of exercise intervention, compared with the groups C and CE,FINS was significantly lower in the groups DM and DME,but the FBG and HOMA-IR were higher.Compared with the DM group, the level of FINS was significantly higher in the DME group, and the levels of FBG and HOMA-IR were significantly lower. The body weights of DM and DME groups were significantly lower than those of the groups Cand CE,the body weight had no significant difference between the DME and DM groups, and similar result was between the groups CE and C.Conclusions 1.The rat model of type II diabetes is successfully established with high fat diet for 7 weeks plus STZ injection(30 mg/mL).2.Aerobic exercise 60 min/day for a total of 8 weeks can improve the glycometabolism in type 2 diabetic rats,to be an ideal animal model for study of the mechanism of prevention and amelioration of type II diabetes.

Objective To investigate the levels and distribution of radon concentrations in groundwater in some representative cities in China, and estimate the effective dose from inhaled radon released from domestic groundwater. Methods A total of 12 cities in 7 provinces (municipalities) were selected, including Beijing,Inner Mongolia,Ningxia,Shaanxi,Henan,Liaoning and Heilongjiang. In total,73 water samples from groundwater supply were taken.Radon concentrations in water samples were determined by using a continuous radon monitor with air-water exchanger. Results The average radon content in groundwater for drinking was 11.8 Bq/L in 12 cities in 7 provinces, ranging from 1.0 to 63.8 Bq/L.The radon concentrations in 37% water samples exceeded 1 1.1 Bq/L,the safe limit recommended for drinking water by the United States Environmental Protection Agency (EPA).The radon contents in all of the water samples was lower than the reference level 100 Bq/L recommended by World Health Organization (WHO).The average annual effective dose arising from inhaled radon released from groundwater was 29 μSv (2.4 to 160 μSv).Conclusions Generally, the effective dose from inhaled radon released from groundwater is negligibly low. However,in some areas dominated by granite bedrock, the dose contribution from radon released from groundwater to residents should be routinely monitored.
