Hepatocelluar adenoma is a rare benign tumor, the incidence of which has increased since the introduction of oral contraceptives. We recently experienced a case of hepatocellular adenoma with ovarian Mucinous cystadenoma in a 36 year old female patient who had no history of oral contraceptives usage. The radiologic and pathologic differential diagnosis is discussed in this patient.

2.Clinical and Radiologic Results of Endovascular Coil Embolization for Cerebral Aneurysm in Young Patients.

PURPOSE: This study is an overview of the clinical and radiologic outcomes of endovascular coil embolization for cerebral aneurysm in patients aged 40 years and younger. MATERIALS AND METHODS: Between January 2008 and June 2011, 218 aneurysms were treated using endovascular coil embolization and followed up with cerebral angiography at least six months later. Twenty-one aneurysms occurred in patients aged 40 years and younger, while 197 occurred in 179 patients older than 40 years. The clinical and radiologic results were retrospectively analyzed and compared between the two groups using the modified Rankin scale scores and the modified Raymond scale. RESULTS: Follow-up cerebral angiography revealed two (9.5%) major and two (9.5%) minor recanalized aneurysms in patients aged 40 years and younger, and six (3.0%) major and 35 (17.7%) minor recanalized aneurysms in patients older than 40 years. However, the differences between the groups were not statistically significant. The preoperative and postoperative mean modified Rankin scale scores at time for follow-up angiography were 1.14 and 0.19 respectively for patients aged 40 years and younger, and 1.30 and 0.30 respectively for patients older than 40 years. CONCLUSION: The younger patients had clinically favorable outcomes with tolerable angiographic follow-up results. More regular and long-term imaging follow-up is required for younger patients due to their longer life expectancy.

3.A Case of Esophageal Stricture Due to Metastatic Breast Cancer Diagnosed by Using Endoscopic Ultrasound Guided Fine Needle Biopsy.

Metastatic cancer to the esophagus is rare and the breasts are the most common primary tumors that metastasize to the esophagus. Since metastatic breast cancer is located in the submucosal layer, diagnosis by general forceps biopsy is difficult. Hence, various techniques including endoscopic ultrasound guided fine needle aspiration, endoscopic ultrasound guided fine needle biopsy, unroofing technique, and submucosal tunneling method are used for diagnosis. Moreover, the patient's medical history should be inspected carefully and previous histological findings of cancer should also be evaluated. Herein, the authors report a case of metastatic breast cancer along with literature reviews. Endoscopy was performed in patient who had undergone breast cancer surgery 13 years previously. Histological examination from a midesophageal stricture was normal. Endoscopic ultrasound was performed and uneven hypoechoic masses were found in the third layer of the esophageal wall. The esophageal stricture was finally diagnosed as metastatic breast cancer by endoscopic ultrasound-guided fine needle biopsy.

4.Nasality Changes With Age in Normal Korean-Speaking Adults

OBJECTIVES: This study was performed to investigate the effects of aging on nasality and the influence of age-related changes in nasal cavity volume and nasal patency on nasality. METHODS: A total of 180 healthy Korean-speaking adult volunteers, who had no nasal or voice-related complaints, were enrolled in this study. Nasometry, acoustic rhinometry, and rhinomanometry were performed to obtain the nasalance score, nasal cavity volume, and nasal resistance, respectively. Changes in these parameters with age were analyzed. RESULTS: Nasal cavity volume increased significantly, and nasal resistance decreased significantly, with age. The nasalance scores for the nasal passage and oronasal passage decreased significantly with age, while there were no age-related changes in nasalance scores for the oral passage. CONCLUSION: Nasalance scores for the passages containing nasal consonants decreased with age although significant increases were observed in nasal cavity volume and nasal patency with age. Therefore, the age-related decreases in nasalance scores may result from factors other than changes in the nasal cavity.

Intracranial mycotic aneurysms are secondary to infectious endocarditis, meningoencephalitis, cavernous sinus thrombophlebitis, sinusitis, and bacteremia. Usually mycotic aneurysms are arising on distal cerebral vessels, but rarely on internal carotid artery. We experienced mycotic aneurysm of ICA followed by meningoencephalitis and thrombophlebitis in both cavernous sinuses. We observed occurrence of mycotic aneurysm by sequential image, and good result after endovascular treatment. So we present our case.

Helicobacter pylori is a causative agent of type B gastritis and plays a central role in the pathogenesis of gastroduodenal ulcer and gastric cancer. To elucidate the host-parasite relationship of the H. pylori infection on the basis of molecular biology, we tried to evaluate the genomic diversity of H. pylori. An ordered overlapping bacterial artificial chromosome (BAC) library of a Korean isolate, H, pylori 51 was constructed to set up a genomic map. A circular physical map was constructed by aligning ApaI, Notl and SfiI-digested chromosomal DNA. When the physical map of H. pylori 51 was compared to that of unrelated strain, H. pylori 26695, completely different restriction patterns were shown. Fifteen known genes were mapped on the chromosome of H. pylori 51 and the genetic map was compared with those of strain 26695 and J99, of which the entire genomic sequences were reported. There were some variability in the gene location as well as gene order among three strains. For further analysis on the genomic diversity of H. pylori, when comparing the genomic structure of 150 H. pylori Korean isolates with one another, genomic macrodiversity of H. pylori was characterized by several features: whether or not susceptible to restriction digestion of the chromsome, variation in chromosomal restriction fingerprint and/or high frequency of gene rearrangement. We also examined the extent of allelic variation in nucleotide or deduced amino acid sequences at the individual gene level. fucT, cagA and vacA were confirmed to carry regions of high variation in nucleotide sequence among strains. The plasticity zone and strain-specific genes of H. pylori 51 were analyzed and compared with the former two genomic sequences. It should be noted that the H. pylori 51-specific sequences were dispersed on the chromosome, not congregated in the plasticity zone unlike J99- or 26695-specific genes, suggesting the high frequency of gene rearrangement in H. pylori genome. The genomc of H. pylori 51 shows differences in the overall genomic organization, gene order, and even in the nucleotide sequences among the H. pylori strains, which are far greater than the differences reported on the genomic. diversity of H. pylori.

7.Higher infiltration by Th17 cells compared with regulatory T cells is associated with severe acute T-cell-mediated graft rejection.

The aim of this study was to evaluate whether the Th17 and Treg cell infiltration into allograft tissue is associated with the severity of allograft dysfunction and tissue injury in acute T cell-mediated rejection (ATCMR). Seventy-one allograft tissues with biopsy-proven ATCMR were included. The biopsy specimens were immunostained for FOXP3 and IL-17. The allograft function was assessed at biopsy by measuring serum creatinine (Scr) concentration, and by applying the modified diet in renal disease (MDRD) formula, which provides the estimated glomerular filtration rate (eGFR). The severity of allograft tissue injury was assessed by calculating tissue injury scores using the Banff classification. The average numbers of infiltrating Treg and Th17 cells were 11.6 +/- 12.2 cells/mm2 and 5.6 +/- 8.0 cells/mm2, respectively. The average Treg/Th17 ratio was 5.6 +/- 8.2. The Treg/Th17 ratio was significantly associated with allograft function (Scr and MDRD eGFR) and with the severity of interstitial injury and tubular injury (P < 0.05, all parameters). In separate analyses of the number of infiltrating Treg and Th17 cells, Th17 cell infiltration was significantly associated with allograft function and the severity of tissue injury. By contrast, Treg cell infiltration was not significantly associated with allograft dysfunction or the severity of tissue injury. The results of this study show that higher infiltration of Th17 cell compared with Treg cell is significantly associated with the severity of allograft dysfunction and tissue injury.

Purpose:
We aimed to develop a novel method for orthotopic colon cancer model, using tissue adhesive in place of conventional surgical method.
Materials and Methods:
RFP HCT 116 cell line were used to establish the colon cancer model. Fresh tumor tissue harvested from a subcutaneous injection was grafted into twenty nude mice, divided into group A (suture method) and group B (tissue adhesive method). For the group A, we fixed the tissue on the serosa layer of proximal colon by 8-0 surgical suture. For the group B, tissue adhesive (10 μL) was used to fix the tumor. The mortality, tumor implantation success, tumor metastasis, primary tumor size, and operation time were compared between the two groups. Dissected tumor tissue was analyzed for the histology and immunohistochemistry. Also, we performed tumor marker analysis.
Results:
We observed 30% increase in graft success and 20% decrease in mortality, by using tissue adhesive method, respectively. The median colon tumor size was significantly increased by 4 mm and operation time was shortened by 6.5 minutes. The H&E showed similar tumor structure between the two groups. The immunohistochemistry staining for cancer antigen 19-9, carcinoembryonic antigen, cytokeratin 20, and Ki-67 showed comparable intensities in both groups. Real-time quantitative reverse transcription analysis showed eight out of nine tumor markers are unchanged in the tissue adhesive group. Western blot indicated the tissue adhesive group expressed less p-JNK (apototic marker) and more p-MEK/p-p38 (proliferation marker) levels.
Conclusion
We concluded the tissue adhesive method is a quick and safe way to generate orthotopic, colon cancer model.
