1.Prevention and treatment of severe complications of thoracoscopy

Objective Study the prevention and early diagnosis of severe complications of thoracoscopy. Methods 400 patients,(18~81)years,ASA Ⅰ-Ⅲ,underwent thoracoscoy,including 320 pulmonary bubble resections and 80 thorax,mediastinum or lung tumor biopsy or resections.All patients were anesthetized with one-lung ventilation using a double-lumen tube and monitored continuously in circulatory and respiratory functions with a multi-runctional monitor. Results Severe intraoperative complications occurred in 8 patients,including 5 reexpansion pulmonary edema,1 complete occlusion of double-lumen tube due to massive pleural effusion,1 massive bleeding and 1 cardiac arrest due to bilateral pneumothorax.All complications were successfully treated and the patients were cured and discharged. Conclusions Thoracoscopy must be well prepared.Applying one lung anesthesia with double-lumen tube,careful observation during operation,early recognition and treatment of complications may contribute to assuring the safety of this procedure.
2.Experimental study of pulmonary surfactant administration to the rabbits' lung grafts

Objective To investigate the protective effects of pulmonary surfactant (PS) in lung transplantation.Methods Forty-eight rabbits were divided into control group (group C),PS1 group,and PS2 group. Rabbit lungs were harvested and preserved at 4 ℃ for 6 h before transplantation. The PS1 group received 100 mg /kg of intratracheal PS before reperfusion,while the PS2 group received the same dosage PS before preservation. Using the hetero-heart-lung transplantation model,pulmonary graft function and airway compliance was assessed every 30 min for 3 h when the model was transformed into self-working state. After finishing the observation,the tissue specimens were taken for analysis of wet/dry ratio (W/D),the contents of malondialdehyde (MDA) and myeloperioxidase (MPO). All values were compared to the control group.Results The two groups treated with PS showed better airway compliance and higher PaO 2 and lower PaCO 2 than group C ( P
3.Effect of outpatient nursing intervention in patients with chronic rhinosinusitis

Objective To investigate the effect of outpatient nursing intervention in patients with chronic rhinosinusitis(CRS)on the aspects of clinical efficacy, quality of life and nursing satisfaction after medical therapy. Methods The clinical efficacy according to the judgment standard for clinical effect was valued. Medical outcome study short-form 36-items health survey (SF-36) was used to assess the quali?ty of life of the subjects. Satisfaction Questionnaire was used to evaluate the satisfaction scale of patients. Eighty subjects with CRS were randomly divided into the control group (40 cases) and the trial group (40 cases). The trial group was given nursing intervention on basis of usual nursing. In three months after treat?ment, the data and statistics analysis was purely descriptive by SPSS 20.0. Results Clinical efficacy:in the group, nineteen subjects were cured,fourteen subjects were improved and inefficacy occurred in seven subjects. The effective rate was 77.5%(31/40). In the trial group, twenty-six subjects were cured,twelve subjects were improved and inefficacy occurred in two subjects. The effective rate was 95.0%(38/40). There were differences in general data between the two groups (χ2=5.165,P=0.023). Quality of life:the average score of the control group was 58.10±5.04, while the average score of the trial group was 72.22±5.63, the average score of quality of life in the trial group was significantly higher than that in the control group(t=3.08,P=0.025). Nursing satisfaction:there were significant differences in nursing satisfaction between the control group and the trial group (u=-7.048,P=0.000). Conclusions Outpatient nursing intervention can improve the effect of patients with chronic nasal sinusitis after medical therapy, improve the quality of life and improve nursing satisfaction.
5.Sensitization test of the digital coralline hydroxyapatite artificial bone scaffold at the maximal dosage

BACKGROUND:We have successfuly prepared digital coraline hydroxyapatite artificial bone scaffold in previous experiments, and it has been confirmed that it has the necessary physical and chemical properties of bone tissue engineering scaffolds. OBJECTIVE: To evaluate the sensitization of digital coraline hydroxyapatite artificial bone scaffold. METHODS:A total of 32 guinea pigs were randomly divided into saline group (negative control group, n=8), 5% formaldehyde group (positive control group,n=8), experimental A group (the mass ratio of 3:1,n=8), and experimental B group (the mass ratio of 4:1,n=8). Sensitization test at the maximal dosage was performed according toBiological Evaluation of Medical Devices-Part 10: Tests for Irritation and Delayed-Type Hypersensitivity, including intracutaneous induction, local induction, and provocation. Patch was removed after 24 and 48 hours, and the skin response was classified according to Magnusson and Kligman criteria. Patch was removed after 48 hours, and the skin was performed with biopsy, stained with hematoxylin-eosin, and observed under optical microscope. RESULTS AND CONCLUSION: Sensitization response was not tested in the negative control group, experimental A group and experimental B group at 24 and 48 hours after patch removal; however, moderate erythema was observed in the positive control group. Optical microscope demonstrated that spongiosis, edema, diffuse or perivascular mononuclear infiltration was not observed, and only a smal number of basicytes were seen in the experimental A and B groups. These findings indicate that the digital coral hydroxyapatite artificial bone scaffolds, with the mass ratio of 3:1 and 4:1, are biologicaly safe for sensitization.
6.Standard decompressive craniectomy with large bone flap resection and removal of necrotic brain tissues for malignant brain swelling after brain injury

Objective To observe the curative effects of malignant brain swelling after brain injury by using the operations of standard large Craniotomy decompression and removal of necrotic brain tissues. Methods 80 cases of malignant brain swelling after severe brain injury were observed. The standard decompression surgery of large era-niotomy routine were used in 40 cases,while the operations of standard large craniotomy decompression and removal of necrotic brain tissues were used in the other 40 cases. The curative effects of the results after 15 days and six mon-ths were analyzed. Results The incidence of intracranial hypertension (37.5%, 15/40 ) and brain tissue incisional hernia (32.5% ,13/40)of the group using standard large craniotomy decompression and removal of nee-rotic brain tissues treatment was significantly lower than that of simple standard large craniotomy decompression treat-ment group [15.0% (6/40) vs 10.0% (4/40)] after 15 days (P<0.05 ). The prognosis excellent rate ( good + re-sidual) of the group using standard large Craniotomy decompression and removal of necrotic brain tissues treatment was significantly higher (60.0%) than that of simple standard large craniotomy decompression treatment group (40.0%) after six months (P<0.05 ). Conclusion The operations of standard large craniotomy decompression and removal of necrotic brain tissues can effectively reduce the intracranial hypertension of malignant brain swelling after trauma injury,and reduce mortality and morbidity of severe brain injury.

AIM:To investigate the mechanism responsible for albumin microbubbles adherence to activated leukocytes. METHODS: In vitro studies were performed in which activated or nonactivated leukocytes were incubated with albumin microbubbles and observed under microscopy. 10.The role of Clq receptor and anti-Clq antibody in the pathogenesis of systemic lupus erythematosus

Objective To investigate the expression of Clq complement receptor(ClqRp and gClqR)in the peripheral blood mononuclear cells(PBMC)of systemic lupus erythematosus(SLE), and the corre|ation between serum levels of complement lq(Clq)and anti-Clq autoantibodies(ClqAb)with ClqRp and gClqR is analyzed. The probable mechanism of ClqRp and gClqR in the development of SLE is explored. Methods The peripherial blood monouclear cells of 58 SLE patients and 30 healthy subjects were collected for the detection of the expression of ClqRp and gClqR by flow cytometry. The serum levels of Clq and ClqAb were detected by single radial immunodiffusion and enzyme-linked immunosorbent assay(ELISA)re- spectively. The correlation between ClqRp and gClqR and other disease activity parameters, such as Clq, ClqAb, SLEDAI score, anti-dsDNA antibody, C3, C4 levels were analyzed.
